Effect of dental bleaching on the microhardness and surface roughness of sealed composite resins

  • Objectives: The aim of this in vitro study was to evaluate the microhardness and surface roughness of composite resins before and after tooth bleaching procedures. Materials and Methods: Sixty specimens were prepared of each composite resin (Filtek Supreme XT and Opallis), and BisCover LV surface sealant was applied to half of the specimens. Thirty enamel samples were obtained from the buccal and lingual surfaces of human molars for use as the control group. The surface roughness and microhardness were measured before and after bleaching procedures with 35% hydrogen peroxide or 16% carbamide (n = 10). Data were analyzed using 1-way analysis of variance and the Fisher test (α = 0.05). Results: Neither hydrogen peroxide nor carbamide peroxide treatment significantly altered the hardness of the composite resins, regardless of surface sealant application; however, both treatments significantly decreased the hardness of the tooth samples (p < 0.05). The bleaching did not cause any change in surface roughness, with the exception of the unsealed Opallis composite resin and dental enamel, both of which displayed an increase in surface roughness after bleaching with carbamide peroxide (p < 0.05). Conclusions: The microhardness and surface roughness of enamel and Opallis composite resin were influenced by bleaching procedures.

The effect of melatonin on prevention of bisphosphonate-related osteonecrosis of the jaw: an animal study in rats

  • Objectives: Melatonin induces human stem cells, converts pre-osteoblasts to mature osteoblasts, and reduces the duration of this transition. However, melatonin itself prevents activation of osteoclasts. Here, we evaluate the role of melatonin in prevention of bisphosphonate-related osteonecrosis of the jaw. Materials and Methods: In this experimental-interventional study, 30 rats were evaluated in 3 groups. The first and second groups received saline and zoledronic acid, respectively, for 4 weeks and the third group received 4 weeks of zoledronic acid and 3 weeks of melatonin simultaneously. First-right-maxillary-molar extraction was performed for all animals, which were sacrificed after 4 weeks of recovery. The extraction sockets were examined histologically for the presence of osteonecrosis, number of osteoclasts and fibroblasts, severity of inflammation, and vascularization. Data were analyzed by chi-square, one-way ANOVA, Tukey, Kruskal-Wallis and Fisher's exact statistical tests (α=0.05). Results: Osteonecrosis was observed in 20%, 90%, and 70% of the first, second and third groups, respectively (P=0.008). The lowest number of osteoclasts and fibroblasts was seen in the third group. Conclusion: Melatonin may effectively prevent some undesirable side effects of bisphosphonates. However, further studies are required to confirm the results of this study.

Development and Evaluation of a Motivational Interviewing Program for Exercise Improvement in Persons with Physical Disabilities (지체장애인의 운동실천을 위한 동기면담 프로그램 개발 및 평가)

  • Purpose: The aims of this study were to develop a motivational interviewing program for exercise improvement in persons with physical disabilities and to examine the effect of this motivational interviewing intervention. Methods: The study employed a nonequivalent control group pretest and posttest design. A total of 62 persons with physical disabilities (30 in the experimental group, 32 in the control group) were recruited from 2 community rehabilitation centers. The experimental group received 8 sessions of a group motivational interviewing program, scheduled once a week, with each session lasting 60 minutes. Test measures were completed before the intervention, immediately after the end of the intervention, 2 weeks later, and 6 weeks after the end of the intervention. Measures included self-efficacy for exercise, decisional balance for exercise, stage of change for exercise, regularity of exercise, exercise maintenance, and independent living ability. Data were analyzed using the ${\chi}^2-test$, Fisher's exact test, Independent samples t-test, and repeated measures ANOVA, conducted using IBM SPSS Statistics version 18. Results: The experimental group showed a significant increase in self-efficacy for exercise (F=50.98, p<.001), benefit (pros) of exercise (F=24.16, p<.001), and independent living ability (F=50.94, p<.001), and a significant decrease in loss (cons) of exercise (F=26.50, p<.001). There were significant differences between the two groups in stages of change for exercise (p<.001), regularity of exercise (p<.001), and exercise maintenance (${\chi}^2=26.61$, p<.001). Conclusion: The motivational interviewing program has the potential to improve exercise levels in persons with physical disabilities.

Effects of Simulation-based Practice using Standardized Patients for the Care of Women with Postpartum Hemorrhage on Nursing Student's Clinical Performance Competence and Critical thinking Deposition (표준화 환자를 활용한 산후출혈 산모 간호 시뮬레이션 실습이 간호학생의 임상수행능력과 비판적 사고 성향에 미치는 효과)

  • Purpose: This study was conducted to examine the effects of simulation-based practice using standardized patients for the care of woman with postpartum hemorrhage as related to clinical performance competence and critical thinking deposition of nursing students. Methods: A nonequivalent control group pre-post test experimental design was used. Fifty four third year students were recruited, 29 students for the experimental group and 25 students for the control group. The simulation-based practice about nursing care related to postpartum hemorrhage included skill training, team-based practice, and debriefing. It was implemented with the experimental group for about 30 minutes in October 6, 2011. Data were analyzed using frequency, ratio, chi-square, Fisher's exact probability, and t-test using the SPSS/WIN program. Results: The experimental group who had the simulation-based practice showed significantly higher knowledge and skills of clinical performance competence than the control group (t=2.19, p=.003). But the experimental group who had the simulation-based practice did not show significantly higher critical thinking deposition than the control group (t=-0.32, p=.748). Conclusion: The results indicate that a simulation-based practice is an effective teaching method to improve knowledge and skills for clinical performance competence of nursing students. Further study is needed to identify the effect of a simulation-based practice on nursing attitudes.

Development and Effect of a Smartphone Overdependence Prevention Program for University Students Based on Self-Determination Theory (자기결정성 이론 기반 대학생 스마트폰 과의존 예방프로그램 개발 및 효과)

  • Purpose: This study aimed to develop a smartphone overdependence prevention program for college students based on the self-determination theory (SDT) and evaluate its effectiveness. Methods: A non-equivalent control group repeated measures design was used for the study. Participants were 64 university freshmen (experimental group: 29, control group: 35). The developed program consists of eight sessions conducted twice a week. The program was designed to promote autonomy, competence, and relatedness the three elements of the basic psychological needs of self-determination theory. The participants were assessed before the program, immediately after, and 1 and 3 months after the program. Data were collected from April 23 to September 14, 2018 and analyzed by performing a Chi-square test, Fisher's exact test, independent t-test, and repeated measures ANOVA using SPSS/WIN 23.0. Results: This study showed improvement in the basic psychological needs (F=3.90, p=.010) in the experimental group compared to the control group. Specifically, competence (F=2.93, p=.035), relatedness (F=2.89, p=.045), and self-regulatory ability (F=3.11, p=.028) improved significantly. Conclusion: Study findings indicate that the smartphone overdependence prevention program based on the Self-determination theory could be an effective intervention for improving basic psychological needs and self-regulation ability. Therefore, this program could be an efficient strategy for smartphone overdependence prevention in university students.

  • Muzzle flash based on gunnery is the target that has huge energy. So, gunnery target in a long range over xx km is distinguishable in the IR(infrared) images, on the other hand, is not distinguishable in the CCD images. In this paper, we propose the classification method of gunnery targets in a infrared images and in a long range. The energy from gunnery have an effect on varous pixel values in infrared images as a property of infrared image sensor, distance, and atmosphere, etc. For this reason, it is difficult to classify gunnery targets using pixel values in infrared images. In proposed method, we take the profile of pixel values using high performance infrared sensor, and classify gunnery targets using modeling GMM and shape of profile. we experiment on the proposed method with infrared images in the ground and aviation. In experimental result, the proposed method provides about 93% classification rate.

Temporal Change in Vertical Distribution of Woody Vegetation on the Flank of Sakurajima Volcano, Southern Kyushu, Japan

  • This study explained vertical distributions and growth environments for woody vegetation. It had been degenerated by long-term volcanic activity of Sakurajima; vegetation and thicknesses of tephra layers and forest soils were investigated at 5 sites (250-700 m in altitude) with different altitudes localized at the northwestern-northern flanks of Sakurajima in Kagoshima Prefecture. The results in 2015 were compared with the vertical distribution of woody vegetation in 1963, when the volcanic activity of Sakurajima was relatively moderate. Thus, we investigated temporal changes in the vertical distribution of woody vegetation owing to volcanic activity over about 50 years (1963-2015). We indicated altitude decreased, the number of woody vegetation, number of species, sum of cross-sectional area of tree diameter at breast height, Fisher-Williams's diversity index ${\alpha}$, and forest soil thickness increased. However, these values were found to be degenerated when compared to climax forest values, and succession was incomplete. It seems that because the woody vegetation of the flank was affected by volcanic activity for a long time, exposing them to severe growth environments, areas with lower altitudes became distant from the craters of Sakurajima, thereby weakening the effect of volcanic activity in these areas at lower altitudes. a at the same altitudes over about 50 years (1963-2015) decreased by about 31-72%, and the sum of the cross-sectional area in tree diameter at breast heights decreased by about 14-62%. Thus, comparative growth environments for woody vegetation in 2015 were more severe than that of 1963, with respect to tephra layer thickness. In addition, for vegetation succession in the flank of Sakurajima, vegetation restoration should be promoted through the introduction of artificial woody plants covered by symbiotic microorganisms or organic materials.

The Effects of Hypertension Health School Program on Hypertension-related Knowledge, Self-efficacy, Self-care Behavior and Physiological Parameters in Hypertensive Patients (고혈압 건강교실 프로그램이 고혈압 환자의 질병 관련 지식, 자기효능감, 자가간호행위 및 생리적 지수에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of hypertension health school program performed in a public health center located in Y-si. Most interesting were the effects on hypertension-related knowledge, self-efficacy, self-care behavior and physiological parameters for hypertensive patients. Methods: Nonequivalent control group pretest-posttest design was employed. 45 patients with hypertension living in Y-si were assigned into an experimental group (n=23) or a control group (n=22). Experimental group was provided with the 8-weeks hypertension health school program from April 7 to May 20 in 2014. Data were analyzed with SPSS/WIN 21.0 using descriptive statistics, $x^2$-test, Fisher's exact test and t-test. Results: The result indicated a significant difference between the experimental group and control group in the scores of hypertension-related knowledge (t=-10.97, p<.001), self-efficacy (t=-4.56, p<.001), self-care behavior (t=-407, p<.001), physiological parameters including systolic blood pressure (t=2.18, p=.032) and diastolic blood pressure (t=2.74, p=.008) and cholesterol levels (t=5.04, p<.001). Conclusion: The conclusion of this study is that the hypertension self-help group program has a significant effect on the change of hypertension-related knowledge, self-efficacy and self-care behavior for hypertensive patients.

The prophylactic uses of antibiotics for the prevention of surgical site infection and the effects: The 3-year experience in a tertiary hospital (수술 예방적 항생제의 사용 현황 및 관리전후 효과)

  • Background : The objective of this study was to examine the effect of management system for the appropriate prophylactic use of antibiotics in surgical patients at a tertiary hospital from 2007 to 2010. Methods : We collected clinical data of three different surgical procedures(colectomy, heart surgery, hysterectomy) for three months of 2007 and 2010, respectively. The number of total cases was 245(137, 54, 54) in 2007, 240(133, 42, 65) in 2010. We measured the rate of use of inappropriate prophylactic antibiotics, administration within 1 hour prior to the incision and the antibiotics prescription days after surgery. To evaluate the effectiveness of the management system, the results of the two groups(Group1=2007, Group2=2010) were compared by t-test, chi-square test or Fisher's exact test. Result : The rate of Aminoglycoside uses decreased drastically from 11.4% to 0.8%(P<.001). The selection of 3rd/4th Cephalosporin dropped from 11.8% to 5.8%(P=.020). The combination of antibiotics decreased from 27.8% to 11.7%(P<.001). The antibiotic prescription rate on discharge declined from 11.8% to 2.5%(P<.001) and the number of antibiotics prescription days after surgery was shortened from 4.2 days to 2.3 days(P<.001). No significant difference in the rate of administration within 1 hour between two groups was found. Through 3-year management, 5 out of 6 measures were significantly improved(except the administration within 1 hour). The rate of surgical site infection decreased from 2.4% to 1.3%(P=.504). Conclusion : The findings demonstrate that the management system for the prophylactic use of antibiotics in surgical patients was effective in decreasing the rate of surgical site infection during 3 years.

Effect of intracanal medicaments used in endodontic regeneration procedures on microhardness and chemical structure of dentin

  • Objectives: This study was performed to investigate the effects of different intracanal medicaments on chemical structure and microhardness of dentin. Materials and Methods: Fifty human dentin discs were obtained from intact third molars and randomly assigned into two control groups and three treatment groups. The first control group received no treatment. The second control group (no medicament group) was irrigated with sodium hypochlorite (NaOCl), stored in humid environment for four weeks and then irrigated with ethylenediaminetetraacetic acid (EDTA). The three treatment groups were irrigated with NaOCl, treated for four weeks with either 1 g/mL triple antibiotic paste (TAP), 1 mg/mL methylcellulose-based triple antibiotic paste (DTAP), or calcium hydroxide [$Ca(OH)_2$] and finally irrigated with EDTA. After treatment, one half of each dentin disc was subjected to Vickers microhardness (n = 10 per group) and the other half was used to evaluate the chemical structure (phosphate/amide I ratio) of treated dentin utilizing attenuated total reflection Fourier transform infrared spectroscopy (n = 5 per group). One-way ANOVA followed by Fisher's least significant difference were used for statistical analyses. Results: Dentin discs treated with different intracanal medicaments and those treated with NaOCl + EDTA showed significant reduction in microhardness (p < 0.0001) and phosphate/amide I ratio (p < 0.05) compared to no treatment control dentin. Furthermore, dentin discs treated with TAP had significantly lower microhardness (p < 0.0001) and phosphate/amide I ratio (p < 0.0001) compared to all other groups. Conclusions: The use of DTAP or $Ca(OH)_2$ medicaments during endodontic regeneration may cause significantly less microhardness reduction and superficial demineralization of dentin compared to the use of TAP.
