Perioperative Changes of Serum $\beta_2$-microglobulin Concentration in Open Heart Surgery (개심술시 혈청 $\beta_2$-microglobulin 의 변동에 관한 연구)

  • Nowadays, in spite of the remarkable development of the results of open heart surgery, the incidence of postoperative acute renal failure [ARF] has been increased due to the expansion of the candidates and prolonged operation time for complicated cases. It is also well known that ARF after open heart surgery, if once occurred, is a critical complication, therefore early diagnosis and treatment about it are very important for prognosis. Recently a low molecular weight [2 * microglobulin [[2* MG]has been used as a indicator of renal function. Because of the properties of [2 * MG, serum concentration of it is increased in glomerular dysfunction and urine excretion of it is increased in tubular dysfunction. Author studied about the perioperative changes of serum [2* MG and BUN concentration in 25 children and 30 adults for evaluation of significances of [2* MG as a parameter of postoperative renal function in open heart surgery, and the results were obtained as follows. l. In open heart surgery, the serum [2* MG concentration was elevated postoperatively in the all cases from the first postoperative day. 2. There were a significant correlation between the preoperative BUN and [2* MG concentration [P< 0.01]. The correlation factor[r] in child group was 0.8512, and in adults 0.8636. 3. The maximum level of serum [2* MG in the both child and adult groups were noticed in 4th postoperative day as 2.61*0.80mg/ 1 in child group and 3.39*1.47 mg/ 4 in adult group, and there was a significant difference between the two groups statistically[P < 0.05]. 4. The pattern of changes of serum concentration of [2* MG with time was very similar with the changes of BUN, but in a case of ARF [expired with it] the changes of [2* MG was more remarkable. 5. There was a significant differences in the maximum level of [2* MG between the 2 group according to the ECC time [groups of below and above 60 minutes] [P< 0.01].

Analysis of Relationship between Mixed Venous PO2 and Status of Cardiac Performance with Hemodynamic Values after Correction of Cyanotic Congenital Heart Disease (청색심기형 교정술후 혼합정맥혈 산소분압과 심근상태 및 혈류역학치와의 상관관계 분석)

  • We utilized pulmonary artery pressure monitoring system in risky patients for preventing the postoperative pulmonary hypertensive crisis and for sampling the mixed venous blood. And this mixed venous blood oxygen saturation [MVSO2] or partial pressure [MVPO2]tells us many meaningful patients state. We selected 59 cyanotic congenital heart diseased patients, who were operated in our hospital from Nov. 1987 to Oct. 1988, in the Department of Thoracic and Cardiovascular Surgery, Seoul National University Children\ulcorner Hospital, who had pulmonary artery pressure monitoring catheter and who made us know their mixed venous oxygen condition. We found that there was no close relationship between MVPO2 and Cardiac Index [C.I.] during early postoperative period, but on the first and second day after operation the correlation coefficient was increased as r=0.35[p=0.008], r=0.78[p=0.0001]. So we concluded that the correlation between MVPO2 and C.I. was more reliable with time going as hemodynamic stabilization. And we experienced no survivors whose MVPO2 was under 20 torr, but that was not the only factor for death. From these results, we conclude that we can consider the MVPO2 [or MVSO2] representing C.I. after stabilized postoperative condition of the open heart surgery patients, but during early postoperative period, in addition to this MVPO2, we should do also apply other parameter such as urine output, arterial blood pressure, left atrial pressure and pulmonary arterial pressure for exact estimation of the patients status.

The Study on the Urinary Iodine Excretion of Koreans Living in Rural Areas

  • More accurate evaluation of iodine consumption of Koreans can be made by measuring the urinary iodine excretion of people living in representative areas. The data about average iodine excretions by region, sex and age were gathered in order to suggest as a factor the criteria on the progress or prognosis of thyroid disease patients. This study was conducted on 3,000 subjects (2,000 Younggwang-gun residents and 1,000 Muan-gun residents) between July 2004 and August 2005. The data sampling was done based on stratified random sampling and the data were analyzed according to age (the subjects were divided into age groups, five years each) and sex of the subjects. Of the 3,000 subjects, a total of 1,592 people (1,174 in Younggwang-gun and 418 in Muan-gun) participated in this study, which used ISE (iodine ion selective electrode) to measure the concentration of iodine in urine. The 1,592 subjects are composed of 732 males and 860 females. The average urinary iodine excretion was $3.10{\pm}1.75mg/L$ (0.31~15.2 mg/L). The average iodine excretion of males was $3.09{\pm}1.61mg/L$ (0.42~15.2 mg/L) while it was $3.11{\pm}1.86mg/L$ (0.31~12.5 mg/L) among females, which represents no significant difference between males and females. However, the values were significantly higher than those of Europeans and Americans. There were statistically significant differences among the regions. When the data were analyzed according to age, females in their 40s were found to have a little less urinary iodine excretion and males had less and less iodine excretion as they get older. These results are deemed to have a statistically significant difference. This study was conducted on a large number of people (N=1,592) for the first time in Korea. If the data collected through this study can be regarded as the average urinary iodine excretion of Koreans, it is possible to conclude that the average iodine consumptions of Koreans are a lot more than Europeans and Americans. Thus, the effect of much iodine consumption should be studied further.

  • We made a comparative study, A patient who had suffered dizziness and ataxia took Hyungbangsabaek-san(荊防瀉白散) at first and next took Hyungbangdojeock-san(荊防導赤散). He was diagnosed as a Soyangin suffering chest knotting syndrome(結胸證) by Sasang constitutional medicine(四象醫學) and took two kinds of herbs in a row. We observed his phase of dizziness and ataxia with the conditions of stool and urine excavation, pink eye, sweating, and tongue color and tongue coating. When he took Hyungbangdojeock-san these conditions improved more than when he took the other one and his dizziness and ataxia also improved. Hyungbangdojeock-san was more useful when the severity of the disease was shallower and he had more exterior syndromes than Hyungbangsabaek-san.

  • Tyrosinemia type I is an autosomal recessive inborn error of tyrosine metabolism that caused a mutation. Clinical symptoms include progressive liver damage with liver failure, coagulopathy, hypophosphataemic rickets, renal tubular dysfunction and a high risk of hepatocellular carcinoma. If left untreated, the affected infants may die from liver failure within the first year of life. PharmacoloIcal therapy with 2-(2-nitro-4-trifluoromethylbenzoyl)-1,3-cyclohexanedione (NTBC) has offered an effective therapeutic option in addition to dietary restriction of tyrosine and phenylalanine. As prognosis of tyrosinemia type I is improving with early diagnosis and early treatments, it meets the criteria for a condition that would benefit from newborn screening. We report a case of tyrosinemia type I diagnosed by newborn screening and successive biochemical analysis of plasma and urine, treated by dietary restriction and NTBC.

Study of Findings from Health Examinations among University Students (일개 대학의 건강검진 결과에 대한 연구)

  • This study aimed at examining participation rate in taking health examinations. abnormal findings. and recognition and responses for re-examination recommendation among junior students at a university. Data were collected by interviews and self-reports between March and April. 2000. five months after receiving findings of health examinations. Question items in the questionnaire were made by the researchers. Participation rate for the health examination was $22.5\%$ of all registered junior students: $25.4\%$ for men and $16.6\%$ for women. College of medicine ranked the first in the participation rate. Of the examinees. $22.8\%$ showed abnormal findings. Of those students with abnormal findings. 149 students who were registered at the time of data collection became the subjects of the present study. The average age of the subjects was 23.7 years. The proportion of those with very good or. good self-evaluated physical health was $24.1\%$. while the proportion for mental health was $55.1\%$. The most prevalent problem for men was liver problem and for women anemia. More than $92\%$ of the subjects were aware of their abnormal findings. Those who sought advice were $71.8\%$ and their parents were most frequently asked for advice. As for the contents. $33.7\%$ were advised to visit a hospital. Of the 65 students recommended for re-examination. $60.9\%$ with liver problems took re-examination. while $37.2\%$ with urine problems. The multiple responses of the reasons for not following the recommendation for re-examination were 'not a serious problem $(63.9\%)$,' 'having no time $(22.2\%)$,' Students' recognition of the importance of health examination should be raised to increase their participation rate. When abnormal findings were detected. parents need to be informed for achieving adequate follow-up. All the students with abnormal findings need to be consulted by university health personnel to facilitate proper actions.

Clinical Studies on 5 Cases of Acute Viral Hepatitis A Treated by Oriental Medicine (급성 바이러스성 A형 간염의 한방치료 5례에 관한 임상보고)

  • We conducted this study to report the clinical manifestations of 5 acute viral hepatitis type A treated by oriental medical treatment. Five hepatitis A patients who visited OO oriental hospital from May 2008 to July 2009 were treated by oriental medical treatment, including herbal acupuncture, acupuncture and herbal medicine and western medical treatment, including fluid therapy and peroral medicine. Prodromes were similar to influenza and duration from symptom onset to first visit were $5.0\;{\pm}\;1.6$ days. Chief symptoms included jaundice, itching, dark urine, anorexia, fatigue, nausea, vomiting, abdominal discomfort. The mean values of the initial laboratory test were serum total bilirubin(TB) of $3.62\;{\pm}\;2.77\;mg/dL$, aspartate aminotransferase(AST) of $729\;{\pm}\;422\;IU/L$, alanine aminotransferase(ALT) $774\;{\pm}\;754\;IU/L$. The peak mean values of those were $6.30\;{\pm}\;2.13\;mg/dL$, $2177\;{\pm}\;2573\;IU/L$, $2238\;{\pm}\;1682\;IU/L$ respectively. The mean duration of hospitalization were $15\;{\pm}\;3$ days. Follow-up at 3 weeks after discharge showed that all patients recovered without complications. Treatment of oriental medicine has effectiveness on acute viral hepatitis type A. Further study is needed.

Study to classify the Clinical symptoms of the Diabetes Mellitus according to the Sasang constitution (사상체질에 따른 당뇨임상증상 유형 연구)

  • A Study was researched in oder to classify the clinical symptoms of the diabetes mellitus according of the Sasang constitution that is used to treat and manage the diabetics. We collected 98 patients with diabetes who were diagnosed Sasang constitution at department of Sasang constitutional Medicine in Dongeui Oriental Hospital during 1 year from August. 2000 to July. 2002. and then we investigated and analyzed the clinical symptoms of the diabetics according of the Sasang constitution, and 98 patient was examinated. We investigated the clinical symptoms of the diabetes according of the Sasang constitution to find relationship between the symptoms and the Sasang constitutions. The result of it, we found that it is different between the symptoms according of the Sasang constitution in first symptoms of the diabetes mellitus, weight, appetite, urine and drinking water. Therefore we have found that it is possibility to classify the clinical symptoms of the diabetes mellitus patients according of the Sasang constitution.

Prevalence and antimicrobial resistance of Klebsiella species isolated from clinically ill companion animals

  • Background: Klebsiella spp. is an important conditional pathogen in humans and animals. However, due to the indiscriminate use of antibiotics, the incidence of antimicrobial resistance has increased. Objectives: The purpose of this study was to investigate antimicrobial resistance in strains of Klebsiella strains and the phylogenetic relatedness of extended-spectrum cephalosporin (ESC)-resistance among Klebsiella strains isolated from clinically ill companion animals. Methods: A total of 336 clinical specimens were collected from animal hospitals. Identification of Klebsiella species, determination of minimum inhibitory concentrations, detection of ESC resistance genes, polymerase chain reaction-based replicon typing of plasmids by conjugation, and multilocus sequence typing were performed. Results: Forty-three Klebsiella strains were isolated and, subsequently, 28 were identified as K. pneumoniae, 11 as K. oxytoca, and 4 as K. aerogenes. Eleven strains were isolated from feces, followed by 10 from ear, 7 from the nasal cavity, 6 from urine, 5 from genitals, and 4 from skin. Klebsiella isolates showed more than 40% resistance to penicillin, cephalosporin, fluoroquinolone, and aminoglycoside. ESCresistance genes, CTX-M groups (CTX-M-3, CTX-M-15, and CTX-M-65), and AmpC (CMY-2 and DHA-1) were most common in the K. pneumoniae strains. Some K. pneumoniae carrying CTX-M or AmpC were transferred via IncFII plasmids. Two sequence types, ST709 and ST307, from K. pneumoniae were most common. Conclusions: In conclusion, this is the first report on the prevalence, ESCresistance genotypes, and sequence types of Klebsiella strains isolated from clinically ill companion animals. The combination of infectious diseases and antimicrobial resistance by Klebsiella in companion animals suggest that, in clinical veterinary, antibiotic selection should be made carefully and in conjunction with the disease diagnosis.

Evaluation of antiproteinuric and hepato-renal protective activities of propolis in paracetamol toxicity in rats

  • BACKGROUND/OBJECTIVES: Propolis has a rich source of bioactive compounds and has renal and hepatic protective properties. The purpose of this study was to investigate the beneficial effect of hydro-ethanolic extract of propolis against paracetamol-induced liver damage and impairment of kidney function, as well as hematological changes in rats. MATERIALS/METHODS: Six groups of rats were used; the first group was served as a control; the second and third groups were treated by propolis extract at a dose of 50 and 100 mg/kg.B.WT. respectively; the fourth group was treated by paracetamol (200 mg/kg.B.WT.); the fifth group was treated by propolis (50 mg/kg.B.WT.) for eight days and then received similar dose of propolis for following seven days with paracetamol at a dose of 200 mg/kg.B.WT. daily for the seven days; and the sixth group was treated with propolis (100 mg/kg.B.WT.) for eight days and then received similar dose of propolis for following seven days with paracetamol at a dose of 200 mg/kg.B.WT. daily for the seven days. All the animals were treated for a period of 15 days. At the end of the experimental period, blood samples were collected for measurement of the liver enzymes, serum albumin, protein and creatinine, blood urea nitrogen, hematological parameters, and urine volume, protein and albumin. RESULTS: Paracetamol over dose significantly lowered hemoglobin, serum total protein, albumin, and uric acid, while it significantly increased blood creatinine, blood urea nitrogen, alanine aminotransferase, aspartate aminotransferase and lactate dehydrogenase activities, white blood cells, and platelet count as compared to the control. However, these alterations were significantly attenuated by the use of propolis extract and the effect was dose dependent. Interestingly, propolis prevented paracetamol induced proteinuria, low hemoglobin and body weight loss. CONCLUSIONS: Propolis significantly prevented paracetamol induced renal, hepatic and hematological toxicity and might be useful in the management of liver and renal diseases particularly proteinuria.
