The Effects of Electrical Stimulation Therapy with Microchip for the Treatment of Bladder Irritability Symptoms in Stress Urinary Incontinent Women (방광 자극증상을 호소하는 복압성 요실금 환자에서 마이크로칩을 이용한 전기자극치료의 효과)

  • Purpose: This study was carried out to evaluate the efficacy of intravaginal pelvic floor electrical stimulation (FES) therapy on bladder irritability symptoms in stress urinary incontinent women. Materials and Methods: The evaluation before and after treatment included the Blaivas's female Bladder Questionnaire, the quality of life questionnaires and the overall satisfaction with present voiding state using visual analogue test (VAT). All patient were treated for 20 minutes, twice a week for 6 weeks, using a new intravaginal electrical stimulator with microchip (PIC16C74). Results: After the FES therapy, the overall success rate of the SUI was 50.4.% at 9 months. The bladder irritability symptoms such as frequency, nocturia, urgency, residual urine sensation and lower abdominal discomfort were improved. In particular, the symptoms of frequency, urge incontinence, dysuria were significantly improved after the FES therapy at 9 months. The VAT score of the overall satisfaction with the present voiding state was significantly lower after the FES therapy. 73.7% of patients were satisfied with the FES therapy and complications such as hemorrhage, vaginitis, urinary tract infection and pain were not encountered. Conclusion: These results suggest that FES therapy with microchip improves some bladder irritability in SUI women. Therefore, electrical stimulation therapy should be considered as a first line therapy in SUI women with bladder irritability symptoms.

  • Short-chain acyl-CoA dehydrogenase (SCAD) deficiency is a rare mitochondrial fatty-acid oxidation disorder that is inherited as an autosomal recessive pattern. SCAD deficiency is caused by mutations in the ACADS gene (Acyl-CoA Dehydrogenase, Short-chain, OMIM #606885), which encodes SCAD, the mitochondrial enzyme that catalyzes the first reaction in the beta-oxidation of fatty acids four to six carbons in length. Here, we describe one Korean pediatric case of SCAD deficiency, which was diagnosed during newborn screening through tandem mass spectrometry. An increased concentration of butyrylcarnitine was detected on the newborn screening test, and the urine organic acid analysis showed increased urinary excretion of ethylmalonic acid. The patient has been asymptomatic and has shown normal growth and development by 8 months of age without any intervention during follow-up period.

  • Amyloidosis comprises a diverse group of systemic and local diseases characterized by organ involvement by the extracellular deposition of fibrils composed of subunits of a variety of normal serum proteins. Secondary amyloidosis is caused by the deposition of amyloid A(AA) protein in chronic inflammatory disease. Juvenile rheumatoid arthritis(JRA) has been known to be the most common cause of secondary amyloidosis. We experienced one case of secondary renal amyloidosis in a 12-year-old girl who had suffered from JRA for several years who had visited our renal clinic to evaluate the proteinuria with microscopic hematuria which was detected by chance at school urine screening examination. Apple green birefringence was observed under polarized light with Congo red stain at)d characteristic electron microscopic findings was also noted in renal tissues which was obtained by percutaneous renal biopsy. In our knowledge, this is the first case report of secondary renal amyloidosis developed in pediatric age in Korea.

Effects of Astragali Radix (AR) on Chronic Renal Injury in Rats (황기가 흰쥐의 만성 신손상에 미치는 영향)

  • Objective : This study was designed to investigate the possibility of AR for chronic renal injury. Methods : The author first investigated the expression levels of DNA by inducing of chronic renal injury. Then, the author investigated the effects of AR on chronic renal injury induced by combination treatment with Adriamycin and cisplatin in terms of changes in body weights and renal tissues, urine volume, BUN and creatinine levels, creatinine clearance and histopathological changes in renal tissues. Total expression levels of 546 genes were elevated or lowered by induction of chronic renal injury. Genes of which whose expression levels were elevated by induction of chronic renal injury were related to the PPAR signaling pathway and fatty acid mechanism, etc. Genes of which expression levels were lowered by induction of chronic renal injury were related to the neuroactive ligand-receptor signaling pathway. Results : Oral administration of AR restored renal mass which was reduced by induction of chronic renal injury. AR also restored creatinine clearance and lowered serum BUN level. In histopathological observation, the AR group has a tendency to prevent tissue damages as shown in the chronic renal injury group. Conclusions : AR can be used to treat patients with chronic renal injury although further study will be needed to elucidate the exact mechanisms in the efficacy of AR on chronic renal injury.

Gel Precipitation Reaction in Rats experimentally infected with Clonorchis sinensis (간흡충감염백서에 있어서의 침강반응항체출현(沈降反應抗體出現)의 추이(推移))

  • In the diagnosis of parasitic helminthic diseases, the value of examining and identifying the parasitic eggs and/or adult parasites from patient's urine or stool are well appreciated. However, these methods have a limited value in the diagnosis of tissue or intracellular parasitism, and we have to rely on supplementary methods such as immune-serological test. The author tested the value of gel precipitation reactions as a diagnostic method of clonorchiasis by observing the appearance of bands in rats experimentally infected with Clonorchis sinensis, And the therapeutic effect of CIBA 35'058-Ba was evaluated by this serological method. The antigen was prepared from the adult worms infected in rabbits by Tsuji method. Rats infected with 40 metacercariae each were bled on 7,14,21,26,28,39,42(43),49(53) days after infection to observe the appearance of precipitin bands by both Oucterlony method and immunoelectrophoresis. Fifteen rats were separately infected and treated with CIBA 35'058-Ba in dose of 15mg/kg of body weight. The following results were obtained: 1. It was observed that there exist individual variations in the appearance of the first precipitin band with the range of 2-4 weeks after infection. 2. The number of precipitin bands was increased until 6-7 weeks after infection. In all cases, 3 precipitin bands were appeared by Oucterlony method and 6-7 bands were appeared by immunoelectrophoresis after 6-7 weeks of infecion. 3. It was hardly possible to notice any change in the number of bands after the administration of CIBA 35'058-Ba. This result suggested that the drug has no effect on clonorchiasis which was confirmed by the autopsy of the experimental rats.

Enhancing Mulberry Leaf Meal with Urea by Pelleting to Improve Rumen Fermentation in Cattle

  • Four, ruminally fistulated crossbred (Brahman${\times}$native) beef cattle with initial body weight of $420{\pm}15kg$ were randomly assigned according to a $4{\times}4$ Latin square design. The dietary treatments were mulberry leaf pellet (MUP) supplementation at 0, 200, 400 and 600 g/hd/d with rice straw fed to allow ad libitum intake. All steers were kept in individual pens and supplemented with concentrate at 5 g/kg of body weight daily. The experiment was 4 periods, and each lasted 21 d. During the first 14 d, all steers were fed their respective diets ad libitum and during the last 7 d, they were moved to metabolism crates for total urine and fecal collection. It was found that increasing MUP levels resulted in linearly increasing rice straw and total intakes (p<0.05). Ruminal temperature and pH were not significantly affected by MUP supplementation while $NH_3$-N concentration was increased (p<0.05) and maintained at a high level (18.5 mg/dl) with supplementation of MUP at 600 g/hd/d. Similarly, viable total bacteria in the rumen and cellulolytic bacteria were enriched by MUP supplementation at 600 g/hd/d. However, the rumen microbial diversity determined with a PCR-DGGE technique showed similar methanogenic diversity between treatments and sampling times and were similar at a 69% genetic relationship as determined by a UPGMA method. Based on this study, it could be concluded that supplementation of MUP at 600 g/hd/d improved DM intake, ruminal $NH_3$-N, and cellulolytic bacteria thus iimproving rumen ecology in beef cattle fed with rice straw.

DIGESTION OF ALKALI-TREATED ALFALFA SILAGE BY GOATS

  • First crop of alfalfa (Medicago sativa L.) was harvested, wilted and ensiled with or without NaOH or $NH_3$, and fed to three rumen fistulated goats in a $3{\times}3$ Latin-square design. Each alkali treatment (2.44% of alfalfa dry matter) was made by spraying its solution prior to ensiling. Silage pH, $NH_3-N$ and butyric acid concentration were increased with each alkali addition, and NaOH-treated silage showed the lowest chemical quality. Compared with untreated silage, digestibilities of organic matter, ADF and cellulose were depressed by both alkali treatments, and the reductions in NaOH-treated silage were significant. Crude protein digestibility was also significantly decreased in NaOH-treated silage, but the goats receiving the silage excreted less nitrogen in urine than those on the other two silages. Nitrogen retention of goats was not different among the treatments. Ruminal solubility and potential degradability of dry matter and nitrogen determined with the in situ bag technique were reduced, and rate of degradation of the two components were increased by the NaOH treatment. Addition of $NH_3$ provided ruminal soluble nitrogen to the silage, but the rate of degradation was similar to that of untreated silage. These results suggest that NaOH treatment would denature the protein and reduce the susceptibility to microbial degradation in the rumen, while no positive effect of alkali treatment on fiber digestion and nitrogen utilization was observed in this study.

Effect of Dietary Cellulose on Zinc Metabolism in Rats fed Different Level of Zinc. (식이내 섬유소와 Zn 첨가 수준이 흰쥐의 체내 Zn 대사에 미치는 영향)

  • This study was carried out to investigate the effect of addition of cellulose in the diet on the metabolism in rat fed high and low level of zinc. The experimental animals were consisted of 24 male weaning rats of Sprague-Dawley strain(mean weight 72.3g), and they were devided into 4 groups of 6 rats and fed experimental diets for four weeks. Dietary zinc levels used were 10 ppm, and 300ppm and cellulose levels were 2.5% and 10% of diet by weight. Throughout the experimental period, feed consumption and body weight gain were measured and feed efficiency ratio was calculated. The weight of live, kidney and spleen were measured, and the contents of zinc in feces, urine, liver, kidney, spleen and serum were determined. The results obtained are summarized as following ; 1. Body weight gain in high zinc-adequate cellulose group was significantly higher than the other groups. Feed consumptions were significantly higher in high zinc groups and no significant difference was found with dietary cellulose levels. 2. Fecal zinc excretions of four groups were not different at the first week, but at the end of fourth week, high zinc groups experince significantly more zinc excretion than low zinc groups, and also high cellulose groups had higher zinc contents in the feces than the adequate ones within the same zinc levels(p<0.05). There was no significant difference in the urinary zinc excretion. 3. The weights of liver, kidney and spleen were heavier in the high zinc groups than the lower ones, and higher in the high cellulose groups(p<0.05). The liver zinc contents were significantly lower in the low zinc and high cellulose groups. However zinc contents in the kidney and serum were not influenced by dietary zinc level but by cellulouse level. High cellulose diet lowered serum and kidney zinc concentrations(p<0.05).

Influence of Isoproterenol on Intra-renal Blood Flow in the Dog (개의 신내혈류(腎內血流)에 미치는 Isoproterenol의 영향(影響))

  • The direct effect of isoproterenol on renal function, when given intravenously, is usually obscured by its potent hypotensive action. To obviate the latter action, isoproterenol was infused directly into one renal artery of the dog, the other kidney serving as a control for the general action. And following results were obtained. In the first series of experiments, the directic action of isoproterenol was ascertained. $1.0\;{\mu}g/kg/min$. reduced on both kidneys the urine flow, clearances of PAH and creatinine, as well as the amount of sodium excreted, but the effect was weaker on the experimental side than on contralateral side. With $0.1\;{\mu}g/kg/min$., two cases among 6 experiments showed marked diuresis, two cases no apparent effect, and another two marked antidiuresis on the experimental kidney, whereas the contralateral kidney exhibited antidiuresis in all cases. Further reducing the dose unmasked the diuretic action on the ,experimental kidney. In another series, the effects of isoproterenol on the blood flow distribution within the kidney and on sodium concentration gradient within the kidney tissue were observed. $0.05\;{\mu}g/kg/min$ isoproterenol markedly increased the medullary plasma flow and slightly increased total renal plasma flow and glomerular filtration rate, along with concomitant increase in the amount of sodium excreted and osmolar clearance, and decrease in reabsorption of free water. Sodium concentration gradient markedly decreased in the experimental kidney, reaching 2/3 of the value observed in the contralateral kidney at the papilla. It is thus concluded that isoproterenol exerts a diuretic action, when infused directly into a renal artery, and the mechanism of the action rests on its hemodynamic action, substantiated as the increase in glomerular filtration and in the medullary blood flow, resulting in washout of hyperosmolality produced by the coutercurrent multiplier system.

A Case Report of Rhabdomyolysis Induced by a Combination of Exercise, Alcohol, and Medication (운동, 알코올, 약물 등 복합적 요인으로 유발된 횡문근융해증 환자 치험 1례)

  • Rhabdomyolysis is a syndrome that evokes either myopathy or cola-colored urine with elevated creatine kinase and myoglobin levels. The most common complication of rhabdomyolysis is acute renal failure (ARF). A large quantity of fluid must be injected promptly the first time symptoms appear, whereas sequela of symptoms has remained in the long run. In this report, a 58-year-woman was diagnosed with ARF due to rhabdomyolysis induced by exercise, alcohol, and medication. A treatment of acupuncture and herbal medicines was carried out for over 2 months. To evaluate the condition of the patient's lower limb, a short form of the McGill Pain Questionnaire, Visual Analogue Scale (VAS), European Quality of Life-5 Dimensions (EQ-5D), and Manual Muscle Test (MMT) points were used. As a result, clinical symptoms and quality of life improved noticeably after treatment with Korean medicine. This suggests that Korean medicine has an effect on rhabdomyolysis.
