The Effect of Social Support and Resilience on Sick Role Behavior of Hemodialysis Patients (혈액투석환자의 사회적지지, 회복탄력성이 환자역할행위 이행에 미치는 영향)

  • This was a descriptive study to investigate the effect of social support and resilience on sick role behaviors of hemodialysis patients, and aims to provide base data on nursing intervention programs for improving the sick role behaviors of hemodialysis patients. The study subjects was patients with chronic kidney disease undergoing hemodialysis treatment at a general hospital in D city. The final analysis has been made on 131 questionnaires. The average of social support score was $3.93{\pm}0.84$, and resilience score was $2.67{\pm}0.80$. At last sick role behavior score was $3.99{\pm}0.80$. According to the analysis on the correlations among the study subjects' social support, resilience and sick-role behaviors, the social support had statistically and significantly positive correlation. In regression analysis, frequency of dialysis, social support and resilience have appeared to be the variables influencing sick role behavior, and the explanatory power of this model is 44.0%.

Feasibility of a Clinical-Radiomics Model to Predict the Outcomes of Acute Ischemic Stroke

  • Objective: To develop a model incorporating radiomic features and clinical factors to accurately predict acute ischemic stroke (AIS) outcomes. Materials and Methods: Data from 522 AIS patients (382 male [73.2%]; mean age ± standard deviation, 58.9 ± 11.5 years) were randomly divided into the training (n = 311) and validation cohorts (n = 211). According to the modified Rankin Scale (mRS) at 6 months after hospital discharge, prognosis was dichotomized into good (mRS ≤ 2) and poor (mRS > 2); 1310 radiomics features were extracted from diffusion-weighted imaging and apparent diffusion coefficient maps. The minimum redundancy maximum relevance algorithm and the least absolute shrinkage and selection operator logistic regression method were implemented to select the features and establish a radiomics model. Univariable and multivariable logistic regression analyses were performed to identify the clinical factors and construct a clinical model. Ultimately, a multivariable logistic regression analysis incorporating independent clinical factors and radiomics score was implemented to establish the final combined prediction model using a backward step-down selection procedure, and a clinical-radiomics nomogram was developed. The models were evaluated using calibration, receiver operating characteristic (ROC), and decision curve analyses. Results: Age, sex, stroke history, diabetes, baseline mRS, baseline National Institutes of Health Stroke Scale score, and radiomics score were independent predictors of AIS outcomes. The area under the ROC curve of the clinical-radiomics model was 0.868 (95% confidence interval, 0.825-0.910) in the training cohort and 0.890 (0.844-0.936) in the validation cohort, which was significantly larger than that of the clinical or radiomics models. The clinical radiomics nomogram was well calibrated (p > 0.05). The decision curve analysis indicated its clinical usefulness. Conclusion: The clinical-radiomics model outperformed individual clinical or radiomics models and achieved satisfactory performance in predicting AIS outcomes.

Quality Characteristics of White Bread with Arrowroot Powder (칡 분말을 첨가한 식빵의 품질 특성)

  • The objective of this study was to investigate the effects and availability of arrowroot powder in making white bread. The characteristics of pasting, farinogram and alveogram of the dough containing arrowroot powder were analyzed. Further, the physicochemical properties of white bread were analyzed by different mixing ratios (0, 3, 5 and 7%) of arrowroot powder during storage periods. When 7% of arrowroot powder was added, the initial pasting temperature of the dough by using a RVA (rapid visco analyzer) was significantly increased. Peak viscosity, holding strength, break down, final viscosity and set back were decreased by increasing added arrowroot powder. With the increasing amounts of arrowroot powder, the values of farinogram and alveogram parameter for the dough showed a tendency to decrement. The texture profile analysis of the white bread revealed that hardness, springiness, cohesiveness and gumminess were increased by adding arrowroot powder and further, the storage time was longer. By increasing the amount of added arrowroot powder, the L value of the white bread was decreased, whereas the a, b value were increased. In the sensory evaluation, the white bread of the control and that of the added 3% arrowroot powder showed the highest preference in total score.

Prioritization of Anti-Icing Spray System for Active Snow-Removal Works (능동적 제설작업을 위한 염수분사장치 설치 우선순위 선정)

  • PURPOSES: This study aims to establish the priority of introducing anti-icing spray system for regions of the National Highways in South Korea. Using this study, a logical plan for instituting such an anti-icing spray system can be established for the National Highways. METHODS : The Analytical Hierarchy Process (AHP) was employed to prioritize the implementation of an anti-icing spray system on Korean highways. For this purpose, an existing scoring table developed by the Ministry of Land, Infrastructure Transport Affair was slightly modified in order to reflect recent trends in winter maintenance. A survey was conducted to gather the preferences regarding the developed hierarchy of road experts and agencies. Finally, the final score was produced by integrating the scoring results with estimated weights for each evaluation criterion. RESULTS: In general, Honam and the metropolitan areas have relatively high priority while other areas such as Chungcheong, Young Nam, and Gang Won appear to be uniform in importance in terms of establishing an anti-icing spray system. This result may indicate that historical weather data and traffic volumes are significant factors in deciding in winter maintenance polices CONCLUSIONS : In this study, useful insights are suggested regarding winter maintenance by simultaneously performing rapid snow removal and proactive treatment. Issues of resource allocation may be potential research items in the field transportation engineering.

A SPEECH-PHONETIC STUDY ON THE PRONUNCIATION OF THE OPENBITE PATIENTS (개교환자의 발성에 관한 언어 음성학적 연구)

  • This study aimed at examining speech defects of openbite patients, which were analized in terms of formant frequency for vowels and word pronunciation length for consonants. In addition, the upper and lower lip (perioral m.) activity was tested by the EMG. The tongue force was measured by the strain gauge, and the speech discrimination test was carried out. One experimental group and one control group were used for this study and they were respectively composed of six female openbite patients and six normal-occlusion females. Eight monophthongs, two fricatives and two affricatives were chosen for speech analysis. Speeches of the above-mentioned groups were recorded and then analized by the ILS/PC-1 software. Four hundred most frequently used monosyllables were also chosen for discrimination score. Openbite patients showed the following characteristics: 1. Abnormality in case of /a/, $/\varepsilon/$, /e/, /i/ $F_2$ and /e/, /a/ $F_1$. 2. Significantly elongated length in their pronunciation of /h/ and $/C^h/$ and somewhat elongated length also in their pronunciation of /s/ and /c/. 3. Significant upper lip activity according to the EMG test during pronunciation of the bilabial consonants. 4. Relatively weak tongue force according to the strain gauge measurement. 5. According to the speech discrimination test, high rate of misarticulation in case of (a) initial /p/ /s'/ and /ts'/, (b) /a/,$/\varepsilon/$,/e/,/je/,/o/, $/\phi/$,/jo/,/u/,/we/, and /i/ (c) final (equation omitted).

  • Purpose: The purposed of this study was to develop and evaluate a Web-based for medical terminology educational program. Method: For the development of this program, the NBISD model was applied as a basic model and WBI ID Process Action model was applied. It was executed for 15 weeks along with off-line lectures. After the operation of this program to 210 nursing students, learners' responses were analyzed twice(at 5th week and 15th week). Result: The satisfaction with this program was slightly above the center point. But the structure of system and composition of design were high score. The results of the final evaluation were more positive than those of the interim one. This program have the advantage of easy accessibility and easy understandability with picture and testing. Conclusions: This study proved the importance of the suitability of learning contents and learning form in developing WBI programs and showed possibility for applying WBI to medical terminology. In order to enhance the effects of Web-based medical terminology educational program for, it is necessary to perform analytic comparison with the effect of WBI based on self-directed learning.

Development of An Inventory to Classify Task Commitment Type in Science Learning and Its Application to Classify Students' Types

  • The purpose of this study is to develop an inventory to classify task commitment types of science learning and to classify highschool students' task commitment types. Firstly, inventory questions were designed following the literature analysis on the task commitment components which involve self confidence, high goal setting, and focused attention. Prototype inventory underwent the content validity test, pilot test, and reliability test. Through these steps, final inventory was input to 462 high school students and underwent the factor analysis and cluster analysis. Factor analysis confirmed three components of task commitment as the three factors of inventory questions. In order to find how many clusters exist, factors of developed inventory became new variables. Each factor's factor mean was calculated and served as the new variable of the cluster analysis. Cluster analysis extracted five clusters as task commitment types. The 5 clusters were suggested by the agglomarative schedule and dendrogram gained from a hierarchical cluster analysis with the setting of the Ward algorithm and Squared Euclidean distance. Based on the factor mean score, traits of each cluster could be drawn out. Inventory developed by this study is expected to be used to identify student commitment types and assess the effectiveness of task commitment enhancement programs.

Risk Factors and Methods in Balance Assessment Associated with Fall in Older Adults (노인의 낙상과 연관된 위험요소와 균형 측정 방법)

  • Purpose : The purpose of this study was to determine risk factors and methods in balance assessment associated with fall in older adults. Methods : This article describes many of the tools that can be used to evaluate the physical parameters associated with fall risk in older adults. Results : Composite ratings of performance(Tinetti balance assessment, Guralnik test battery, Berg balance scale, modified-physical performance test) measures the score compounding the balance measure to determine fall risk. Static balance instruments are composed of FICSIT-4 that measures the ability of maintaining foot positions and CTSIB that measures postural stability. Dynamic balance instrument is composed of functional reach test. To measure walking velocity and mobility, 8-foot up-and-go test and walking around two cones are used. We can use 1-RM and to measure muscular strength, isokinetic dynamometery, and 30-second chair stand to measure lower extremity muscle strength. Conclusion : The described instruments are easy to use and widespread. To select and use these tool kits carefully is considered to be helpful in identifying those who are most likely to fall. The final part of the article includes a brief discussion of the potential role of exercise training interventions to improve these physical parameters and prevent falls.

High Spinal Block for Chronic Intractable Pain -A case report- (만성 난치성 통증의 치료를 위한 상위척수차단 -증례보고-)

  • Total spinal block is used as final choice for chronic intractable pain which doesn't respond to other treatments. A 35 years old male patient was admitted to pain clinic due to severe cramping and throbbing pain of whole body, especially left lateral side since 1980. The result of cervical and lumbar epidural block was not good enough, so we decided to try total spinal block. At first, C7-T1 interspace was punctured and 2% mepivacaine 20 ml was injected, but the result was not satisfied. Next day, L3-4 interspace was punctured and 1.5% mepivacaine 40 ml was injected. The sensory block level was C6 and pain score on Visual Analog Scale, was changed from 9 to 4. In both trials, actually even though they were not complete total spinal block, the effect was good enough. If complete total spinal block had been accomplished, better result might be obtained in this chronic intractable pain.

  • We propose a novel monitoring system for diagnosing crack faults of the wind turbine using image information. The proposed method classifies a normal state and a abnormal state for the blade parts of the wind turbine. Specifically, the images are input to the proposed system in various states of wind turbine rotation. according to the blade condition. Then, the video of rotating blades on the wind turbine is divided into several image frames. Motion vectors are estimated using the previous and current images using the motion estimation, and the change of the motion vectors is analyzed according to the blade state. Finally, we determine the final blade state using the Support Vector Machine (SVM) classifier. In SVM, features are constructed using the area information of the blades and the motion vector values. The experimental results showed that the proposed method had high classification performance and its $F_1$ score was 0.9790.
