전침 및 봉약침치료를 시술한 양성전립선비대증 환자의 임상증상개선에 대한 후향적 단면연구 (A Retrospective and Cross-sectional Study on Symptom Improvements in Benign Prostatic Hyperplasia Patients Treated with Electroacupuncture and Bee Venom Acupuncture)

  • Obejectives : To investigate the clinical effect of electroacupuncture and bee venom acupuncture for benign prostatic hyperplasia(BPH) symptoms. Methods : Among 39 outpatients who were treated at a BPH clinic, 8 were finally selected for analysis by charts-review, retrospectively. They were treated by electroacupuncture and bee venom acupuncture more than 3 times. We estimated the effectiveness of the treatments by using the International Prostate Symptom Score (IPSS) questionnaire and checking their nocturia frequency. On the basis of these variables, we analyzed the statistical significations of symptom improvements. Results : Among 8 patients, the IPSS and nocturia frequency showed significant improvement after the final treatment compared with baseline (P = .0078, P = .0078). The IPSS significantly decreased even after the 1st treatment and also after the 2nd treatment compared with the baseline (P = .0078, P = .0078). The IPSS at post final-treatment, post 1st- and post 2nd treatment also showed significant improvement compared with IPSS at baseline in 6 severe symptomatic patients(P = .031, P = .031, P = .031). Nocturia frequency decreased after the 1st treatment compared with baseline and showed significant improvement between baseline and post 2nd treatment (P = .0078). Conclusions : In a retrospective and cross-sectional study investigating the clinical effects of electroacupuncture and bee venom acupuncture on BPH, these treatments proved to have significant therapeutic effects, particularly for various symptoms involving frequent nocturia.

Effects of Supplemental Mannanoligosaccharides on Growth Performance, Faecal Characteristics and Health in Dairy Calves

  • Twenty Holstein calves were used to investigate the effects of mannanoligosaccharides (MOS) supplementation in the whole milk on growth performance, faecal score, faecal pH, selected faecal bacterial populations and health during the preweaning period. Healthy calves selected by clinical examination were allocated to one of the two groups (control [CG] and experimental [EG]) at 5 days old. Each group consisted of 5 male and 5 female calves. Each calf in EG was supplemented with 7 g/d of a MOS product (Celmanax) from 5 days to 56 days of age. MOS supplement was mixed with the whole milk once in the morning and administered to the calves in EG via nipple bottle, whereas the calves in CG were fed the whole milk without MOS. Calves were weaned at 56 days of age. The final body weight, average daily weight gain (ADG) and average daily feed intake (ADFI) were statistically similar (p>0.05) but were higher by 3.70%, 6.66%, and 10.97%, respectively, in MOS than in control calves. Feed efficiency (ADG/ADFI) was also similar in two calves group. While faecal scores did not differ on day 5, 7, 14, 21, 28, 42, 49, and 56 between groups, EG had a higher faecal score (p = 0.05) than CG on day 35. Faecal concentration of Lactobacillus was lower (p<0.05) in EG compared with CG. No differences (p>0.05) in faecal concentrations of Bifidobacterium, Clostridium perfringens, and Escherichia coli were found between groups. Although there were no significant differences (p>0.05) in the incidence of diarrhoea, treatment days for diarrhoea and the costs associated with diarrhoea treatments between groups, collectively, the observed reductions in treatment days and the cost of diarrhoea treatments accompanying increases in final body weight, ADG and ADFI for EG may indicate potential benefit of MOS in treatment of diarrhoea.

무지외반증 치료에서 근위 중족골 절골술과 원위 연부조직 교정술 후 종자골의 교정정도 (Correction of Sesamoid after Proximal Metatarsal Osteotomy and Distal Soft Tissue Procedure in Hallux Valgus)

  • Purpose: A retrospective review of the radiographs of the proximal metatarsal osteotomy and distal soft tissue procedure for hallux valgus, evaluating the correction of the tibial sesamoid, was undertaken. We evaluated the correlation between the reduction of the tibial sesamoid and the clinical outcomes. Materials and Methods: 17 patients (23 cases) with moderate to severe hallux valgus deformity underwent the proximal metatarsal osteotomy and distal soft tissue procedure. The preoperative and last follow-up radiographs were reviewed according to the tibial sesamoid grade classification recommended by the Research Committee of the American Orthopedic Foot and Ankle Society (AOFAS). We divided them into two groups according to the reduction of the tibial sesamoid. We anaylyzed the clinical outcomes in each group according to Mayo Clinic Forefoot Scoring System (FFSS). Results: In all of the patients, the preoperative tibial sesamoid position were grade 2 or greater. At the last follow-up, 52% (n=12) were grade 1 or less (Group I) and 48% (n=11) were grade 2 or greater (Group II). In group I, the forefoot score was improved from preoperative mean value of 32.0 points to final follow-up value of 66.3 points. In group II, the forefoot score was improved from preoperative mean value of 31.7 points to final follow-up value of 65.9 points. There was no statistical significance between postoperative, average scores in group I and II (p>0.05). Conclusion: The position of the tibial sesamoid was corrected insufficiently in almost half of all cases. In view of clinical outcomes, there was no significant difference between the corrected group and the other group.

Expression Pattern and Prognostic Significance of Claudin 1, 4 and 7 in Pancreatic Cancer

  • Background: Tight junctions (TJs) organise paracellular permeability and they have an important role in epithelial and endothelial cell polarity and permanence of barrier function. It has been demonstrated that the Claudin family constitutes an important component of them. In this study, we assessed expression patterns of of Claudin1, 4 and 7 and whether they have any relation with prognosis in patients with pancreatic cancer. Materials and Methods: Expression patterns of Claudin 1,4 and 7 were examined by immunohistochemistry in 25 patients with a histopathological diagnosis of pancreatic cancer using a semiquantitative scoring of the extent and intensity of staining. After grouping the staining scores as low (final score 0-2) and high (final score 3-9) the relation between expression of Claudin 1,4 and 7 and survival was evaluated. Results: There was no significant relation between expression of Claudin 1,4 and 7 and gender and stage. No statistically significant relation was found between Claudin 1 and 4 expression and survival whereas a statistically significant relation was found between decrease in Claudin 7 expression and decrease in survival. Conclusions: Claudins have important functions other than their popular function known as adhesion. Supporting this hypothesis, we found a statistically significant relationship between increased Claudin 7 expression and increased survival time, and this suggests that Claudin 7 may exert different tumorigenic effects in pancreatic cancer other than its well-known adhesion effect.

견관절 만성 충돌 증후군의 관절경적 견봉하 감압술 (Arthroscopic Subacromial Decompression for Chronic Impingement)

  • The purpose of this study was to assess the results of arthroscopic subacromial decompression in patients with chronic impingement and to evaluate the results according to the rotator cuff pathology. We evaluated the clinical results of treatment for chronic impingement syndrome in 28 patients from Feb 1996 to Feb 1997. There were twenty men and eight women in age from 24 to 72 years (mean age 51) with dominant arm involvement in sixteen patients. Follow up evaluations averaged 15(range 12-24)months. The average duration of symptoms were 15(range 6­60)months. The final diagnoses which were based on the physical examination, plain radiographs and arthroscopic findings, were stage II impingement in 16 patients and stage ill impingement in 12 patients. We excluded the patients with acromioclavicular arthritis or glenohumeral instability in this study. All patients were managed non-operatively a minimum of six months. During the operation we performed contouring and smoothing the acromial undersurface and only resecting of the anterolateral band of the coracoacromial ligament. The clinical results were quantitated using UCLA shoulder rating score. Satisfactory results were obtained in 23(80%) patients. Unsatisfactory results were obtained in 5(18%) patients with posterior cuff tear. The average UCLA pain score showed significant improvement from 2.8(constant pain) to 7.2(present during heavy activities) at final follow up. The function and active forward flexion scores also increased from their preoperative value. There was no significant differences according to the surface and severity of tear and NeeI' stage (P>0.05). These results compared favorably with those reported following open acromioplasty. While arthroscopic subacromial decompression is a demanding technique with a learning curve, it is a reliable treatment for chronic impingement syndrome. A less aggressive approach to subacromial decompression and preserving the posteromedial band of the coracoacromialligament does not appear to compromise results.

급성 족근 관절 원위 경비인대결합 손상에서 TightRopeTM를 이용한 수술의 단기 치료 결과 (Short-Term Results of Surgical Treatment Using TightRopeTM for Acute Ankle Syndesmosis Injury)

  • Purpose: The purpose of this study was to evaluate the clinical and radiologic outcome of syndesmosis fixation using TightRope$^{TM}$ (Arthrex, Naples, FL, USA) in acute syndesmosis injuries. Materials and Methods: Twenty-five consecutive patients with acute syndesmosis injuries, treated using TightRope$^{TM}$, were reviewed. Patients were evaluated preoperatively and at the last follow-up (at least 12 months postoperatively). Clinical outcomes were assessed using American Orthopaedics Foot and Ankle Society (AOFAS) ankle-hindfoot score and self-subjective satisfaction survey. Three radiologic parameters were evaluated two times at the preoperative and final follow up from the nonweightbearing ankle anteroposterior radiographs. Results: The mean AOFAS ankle-hindfoot score was 95.5 at the final follow-up. According to the satisfaction survey, 21 patients chose excellent, and four patients chose good. All radiologic parameters, including the mean tibiofibular clear space, mean tibiofibular overlap, and mean medial clear space on nonweightbearing ankle anteroposterior view, significantly improved after surgery. Complications occurred in only one patient who experienced knot irritation with infection. Conclusion: The short-term surgical results of syndesmosis fixation using TightRope$^{TM}$ were good to excellent, both clinically and radiographically. These results suggest that the fixation using TightRope$^{TM}$ is a valid option for acute syndesmosis injury.

얼굴아토피 피부염환자 45명의 한의학적 치료효과에 대한 후향적 연구 (Retrospective Study about the Effectiveness of a Korean Medicine Treatment on 45 Facial Atopic Dermatitis Patients)

  • Objective : The purpose of this study is to report the effectiveness of a Korean Medicine Treatment on 45 facial atopic dermatitis patients. Methods : Total 45 facial atopic dermatitis patients, who has visited korean medical clinic in year 2011 were analyzed by Objective SCORAD Index(OSI) and Investigator's Global Assessment(IGA). Subanalysis of OSI and IGA were done according to topical ointment user/non-user, treatment period and change in IGA distribution. Results : 1. Male and female percent was 20%:80%. 17.8% were teens, 55.6% were twenties, 13.3% were thirties, 13.3% were above age forty. 64.4% were on topical ointment treatment of corticosteroid or calcineurin inhibitor, and 35.6% were not at the initial visit. Patients with family history were 44.4% and 62.2% had atopic dermatitis past history. 2. OSI and IGA were significantly lower after 1~3, 3~6, 6~9 month of treatment. Average post-treatment score was lower in longer-treated group. 3. IGA distribution has changed from average 3.42 at the first visit to 1.76 at final visit. 91.1% of total patients reported decrease in IGA at the final visit. 4. OSI and IGA improvement rate were significantly higher in non-topical ointment user than the user. Age, treatment period, initial OSI and IGA score difference were not significantly different. Conclusion : A significant percent of 45 facial atopic dermatitis patients who were treated with Korean Medicine Treatment reported decrease in OSI and IGA. The difference increased with the treatment period. Non-topical ointment users' improvement was significantly higher than topical ointment user.

Comminuted Radial Head Fracture in All-arthroscopic Repair of Elbow Fracture-dislocation: Is Partial Excision of the Radial Head an Acceptable Treatment Option?

  • Background: In elbow fracture-dislocation, partial excision of the comminuted radial head fracture that is not amenable to fixation remains controversial considering the accompanying symptoms. This study was undertaken to evaluate the results of radial head partial excision when the comminuted radial head fracture involved <50% of the articular surface in all-arthroscopic repair of elbow fracture-dislocation. Methods: Patients were divided into two groups based on the condition of the radial head fracture. In Group A, the patients had a radial head comminuted fracture involving <50% of the articular surface, and underwent arthroscopic partial excision. Group B was the non-excision group comprising patients with stable and non-displacement fractures. Follow-up consultations were conducted at 6 weeks and at 3, 6, 12, and 24 months after surgery. Results: In all, 19 patients (Group A: 11; Group B: 8) met the inclusion criteria and were enrolled in the study. At the final follow-up, all 19 patients showed complete resolution of elbow instability. No significant differences were observed in the range of motion, visual analogue scale score, and Mayo elbow performance score between groups. Radiological findings did not show any complications of the radiocapitellar joint. However, nonunion of the coracoid fracture was observed in 3 patients (Group A: 1; Group B: 2), without any accompanying instability and clinical symptoms. Conclusions: Considering that the final outcome is coronoid fracture fixation and lateral collateral ligament complex repair for restoring elbow stability, arthroscopic partial excision for radial head comminuted fractures involving <50% of articular surface is an effective and acceptable treatment for elbow fracture-dislocation.

Effects of feeding ethanol on growth performances, carcass characteristics, and lipid metabolism of finishing Korean cattle (Hanwoo) steers

  • Objective: The purpose of this study was to determine the effects of feeding ethanol on growth performances, carcass characteristics, and lipid metabolism of finishing Korean cattle (Hanwoo) steers. Methods: Thirty (30) Hanwoo steers (average 25.1 months of age, body weight 660.1 kg) were assigned to three treatments: control (0% ethanol), E-3 (1.44% ethanol for 3 months), or E-5 (0.72% ethanol for 2 months followed by 1.44% ethanol for 3 months). The animals were allotted by treatment group into six pens and fed concentrate and perennial ryegrass. Ethanol (30%, v/v) was supplemented into drinking water twice a day to meet final concentrations based on average water consumption of finishing Hanwoo steers. Results: There were no statistical differences among the groups in final body weight, average daily gain, or carcass yield grade indices such as cold carcass weight, fat thickness, and loin area. The marbling score tended (p = 0.228) to increase with the highest score (6.7) in the E-5 group followed by 6.3 and 6.0 in E-3 and control groups, respectively. The appearance frequencies of quality grades of $1^{{+}{+}}$ (the best), $1^{+}$, 1, and 2, were; 30%, 50%, 0%, and 20% for control, 10%, 80%, 10%, and 0% for E-3, and 10%, 80%, 0%, and 10% for E-5 groups, respectively, indicating improvements of quality grades by feeding ethanol. Concentrations of serum glucose tended to decrease whereas those of insulin and non-esterified fatty acid to increase by feeding ethanol (E-3 and E-5; p>0.05). Conclusion: Feeding ethanol directly into drinking water of finishing Hanwoo steers stimulated lipogenesis in intramuscular adipose tissue (marbling) and thereby improved carcass quality grade. The serum metabolites results supported the hypothesis of lipolysis of existing adipose tissue, such as abdominal fats, and lipogenesis in intramuscular adipocytes.

정보화사업 성과 영향 요인 분석: 중앙행정기관 정보화사업 평가결과를 중심으로 (A Study on the Factors Affecting IT Project Performance: Focusing on the Results of K-PART of Central Government)

  • 1997년 정보화평가 시작 이후 평가 주관부처 및 평가방식 등의 많은 변화를 겪으며 현재 중앙행정기관 정보화사업에 대한 성과평가는 기획재정부의 재정사업자율평가가 시행되고 있다. 최근 정보화사업은 디지털뉴딜을 거쳐 디지털플랫폼정부 구현을 위해 예산이 큰 폭으로 증가하고 있으며 이에 따라 성과에 대한 관심도 높아지고 있다. 본 연구는 선행연구를 통해 성과 관련 특성을 선정하여 이에 따라 실제로 정보화사업 성과 차이가 있는지 실증 분석하였다. 분석 결과 신규사업, 처나 청에서 수행하는 사업은 최종평가점수에 통계적으로 유의미한 양의 영향을 미쳤다. 반면, 정보화지원사업과 기금사업은 최종평가점수에 통계적으로 유의미한 음의 영향을 미쳤다. 본 연구는 향후 새 정부의 디지털정부플랫폼 정부 구현, 공공데이터 개방 사업 등 정보화사업을 추진하고 이에 대한 성과를 평가할 때 사업의 고유한 특성에 따라 성과 차이가 발생할 수 있다는 점을 시사한다.
