MYOEPITHELIOMA ON SOFT PALATE -A CASE REPORT- (연구개에 발생한 근상피종 1예 -증례보고-)

  • Myoepithelioma is composed exclusively of myoepithelial cells. Myoepithelial cells are ectodermally derived contractile cells that can be routinely identified in many normal tissues having secretory function such as major and minor salivary glands, lacrimal gland and sweat gland. Tumors composed exclusively of myoepithelial cells, so-called myoepitheliomas, are rare-less than 1% of all salivary gland tumors. Grossly, these tumors are well demarcated. The external surface is smooth and may be bosselated. The cut surface is white and homogenous. Microscopically, these tumors are surrounded by a thin fibrous capsule. They are composed of benign-appearing spindle- shaped and/or polygonal cells. Mitoses are rare. Frequently these tumors contain myxomatous stroma which is susceptible to alcial blue stain. Clinically, myoepitheliomas present as slow-glowing, painless masses and can not distinguished from pleomorphic adenomas. Treatment is the same as for pleomorphic adenoma, and the surgical excision should include a margin of normal tissue. Although the majority of myoepitheliomas have behaved in a beingn manner, pleomorphism and mitotic activity have been associated with local aggressiveness.

Characteristics of Spray Dried Polysaccharides for Microencapsulation (미세캡슐화를 위한 분무건조 다당류의 특성)

  • Characteristics of viscosity and spray dried particles for several polysaccharides were studied to investigate the possibilities as wall materials for microencapsulation. Viscosities of 10% maltodextrin, 10% gum arabic, 10% dextran, 1% gum locust bean, and 1% gum karaya were 2.2 mPa.s, 9.2 mPa.s, 13.0 mPa.s, 4660.0 mPa.s, and 77.0 mPa.s, respectively. In scanning electron micrographs for spray dried polysaccharides, gum arabic had spherical shapes at 20% and 30% emulsion concentration, while trailed shapes at 40%. Maltodextrin had uniform spherical shapes at 30%, while aggregated form with various kinds of capsule sizes at 40%. Dextran had spherical shapes at 20%, while trailed fibrous shapes at over 30%. Mixed polysaccharides with gum arabic:maltodextrin (1:3, w/w) had uniform spherical shapes at 20%, 30%, and 40% with increasing diameter with increasing concentration.

  • Fibroma of tendon sheath is an uncommon benign soft tissue tumor with a predilection for the hand or upper extremities with extremely rare intra-articular involvement. It is mostly comprised of slow-growing fibrous lobules made up of scattered fibroblasts in a dense stroma with slit-like vascular channels. An unique case in a 54-year-old male patient involving the glenohumeral joint arising from postersuperior joint capsule is reported. Magnetic Resonance Imaging (MRI) reveals a focal nodular mass with decreased signal intensity on all pulse sequences and pathology confirmation was made with arthroscopic excision. Intra-articular fibroma of tendon sheath should be considered in the differential diagnosis of intra-articular soft tissue masses during shoulder arthroscopy.

  • Bone grafting is widely used to bridge major bone defects or to promote bone union. In the evaluation of bone defect regeneration, 5 mm-diameter defects were created in rabbit calvaria. Concerning biocompatibility, fibrous capsule thickness of CBHA (hydroxyapatite from cuttlebone) was significantly thinner than that of CB (cuttlebone) and CHA (hydroxyapatite from coral) (p < 0.05) at 2 and 4 weeks after implantation. Concerning 12-week total changes of radiologic gray-level histogram, CBHA was significantly higher than CHA (p < 0.05). In the evaluation of bone defect regeneration, bone formation of CHA was significantly higher than that of CB and CBHA (p < 0.05). Based on the clinical and histological results, CBHA would be a safe material for use inside the body and has more effective osteoconduction than CB. It is suggested that CBHA is a valuable bone graft material.

Evaluation of Bioactivity of Titanium Implant Treated with H2O2/HCl Solution (H2O2/HCl 처리한 Ti 임플란트의 생체활성 평가)

  • Surface treatment play an important role in nucleating calcium phosphate deposition on surgical Ti implant. Therefore, the purpose of this study is to examine whether the precipitation of apatite on cp-Ti and Ti alloys are affected by surface modification in HCl and $H_2O_2$ solution. Specimens were then chemically treated with a solution containing 0.1 M HCl and 8.8M $H_2O_2$ at $80^{\circ}C$ for 30 mins, and subsequently heat-treated at $400^{\circ}C$ for 1 hour. All specimens were immersed in the HBSS with pH 7.4 at $36.5^{\circ}C$ for 15 days, and the surface was examined with XRD, SEM, EDX ana XPS. Also, pure Ti, Ti-6Al-4V and Ti-6Al-7Nb alloy specimens with and without surface treatment were implanted in the abdominal connective tissue of mice for 4 weeks. All specimens chemically treated with HCl and $H_2O_2$ solution have the ability to form a apatite layer in the HBSS which has inorganic ion composition similar to human blood plasma. The average thickness of the fibrous capsule surrounding the specimens implanted in the connective tissue was $38.57\;{\mu}m,\;62.27\;{\mu}m\;and\;45.64\;{\mu}m$ in the cp-Ti, Ti-6Al-4V ana Ti-6Al-7Nb alloy specimens with the chemical treatment respectively, and $52.20\;{\mu}m,\;75.62\;{\mu}m\;and\;66.56\;{\mu}m$ in the commercial specimens of cp-Ti, Ti-6Al-4V and Ti-6Al-7Nb without any treatment respectively. The results of this evaluation indicate that the chemically treated cp-Ti, Ti-6Al-4V ana Ti-6Al-7Nb alloys have better bioactivity and biocompatibility compared to the other metals tested.

AN EXPERIMENTAL STUDY OF THE EFFECTS ON THE BONE TISSUE BY IMPLANT OF ROOT CANAL FILLING MATERIALS (근관충전재(根管充塡材)의 매식(埋植)이 골조직(骨組織)에 미치는 영향(影響)에 관(關)한 실험적(實驗的) 연구(硏究))

  • The purpose of this study was the observe the toxic effects of root canal sealers in 108 white rats. Experimental animals were divided into control and experimental groups. Theree representative types of materials, such as AH26, Z.O.E. and F.R. were used in this study. Cavities were prepared on the left mandibular area of 108 white rats. Three different sealers were placed in as experiment and bone cavities were left without filling as control. The experimental animals were sacrificed by cervical dislocation at the intervals of 1, 3, 7, 14, 28 and 49 days after filling. Each specimen was fixed with 10% neutral formalin solution, decalcified with 5% nitric acid, embedded in paraffin and sectioned 5-7${\mu}$. in thickness. The paraffin sections stained with Hematoxylin - Eosin were observed through the ordinary light microscope. The results were as follows; 1. Slight toxic effect to surrounding tissue were found in every experimental specimen. 2. AH26 showed the highest inflammatory response, and F.R. showed the lowest inflammatory response which subsided and replaced by fibrosis at 4 weeks after filling. 3. The cavity filled materials, such as implanted root canal sealers, blood clots and necrotic tissue, showed a tendency to be absorbed gradually proportioned to the experimental periods. A small amount of cavity filled materials were observed in the bone cavities after 4 weeks. 4. Fibroblastic proliferation began to produce fibrous capsule around the bone cavity in 2 weeks after filling. Fibrosis was prominent at 4 weeks after filling. 5. Osteoblastic activity of surrounding bone was observed at first in 2 weeks after filling and prominent in 4 weeks after filling. Osteoblastic activity showed an increasing effect as the time prolonged. 6. Surrounding tissue of the bone cavities showed the features of tissue destruction and had very severe inflammatory response at an initial stage. Above-mentioned appeared to be recovered gradually proportioned to the experimental periods.

  • Extra-skeletal chondroma, characterized by its lack of connection with the adjacent bone, includes synovial chondromatosis, intra-articular and para-articular chondroma, and soft tissue chondroma. Among them, the last two lesions are extremely rare. This is the case about 20-year-old soldier, who had complained of tenderness of the knee, pain and fullness during knee flexion and limitation of motion after a hard military training. We found a mass in the x-ray and MRI and resect the mass surgically, which was a $5.5{\times}4{\times}3$ cm size hard solitary mass. On microscopic finding, it was consisted of lobulated hyaline cartilage and outer fibrous capsule, and we ascertained it as para-articular chondroma. We experienced a case of para-articular chondroma in the infrapatellar fat pad of the knee joint and present its clinical, radiologic and pathologic findings with literature review.

Effect of Inflammatory Responses to PLGA Films Incorporated Hesperidin: In vitro and In vivo Results (PLGA/헤스페리딘 함량별 필름에서 염증 완화 효과: In vitro, In vivo 결과)

  • Hesperidin (Hes) has known to having some functions like protection of blood circulatory system, anti-tumor effect, antioxidant effect and anti-inflammatory effect. The goal of this study is to demonstrate the relationship between Hes and inflammatory through in vitro and in vivo studies using poly(lactic-co-glycolic acid) (PLGA) film including Hes as a tissue engineered scaffold. To confirm the proliferation of cells on fabricated scaffold, cells (RAW 264.7 and NIH/3T3) were seeded on PLGA/Hes film then analyzed with MTT and SEM at 1 and 3 days after seeding. The results from ELISA, RT-PCR, and FACS for anti-oxident and anti-inflammatory effect showed that inflammatory response of PLGA/Hes film decreased more than that of PLGA film. Also, in vivo result confirmed that inflammatory response by implanted PLGA/Hes film decreased more comparing with PLGA film. This is because of anti-inflammatory effect of Hes reducing induced inflammatory cell and accumulation of fibrous capsule. The results showed that PLGA/Hes film's capacity on reducing inflammatory is better than PLGA film because of Hes.

Pathophysiology of Stiff Elbow (주관절 강직의 병태 생리)

  • Purpose: Stability of joints and maintenance of range of motion are needed for optimum function. The most common complaint about the elbow joint is joint stiffness. Recent articles have reported good outcomes in the treatment of stiff elbow joints. However, deciding which procedure to use is always difficult. Materials and Methods: Morrey et al. reported that the functional range of motion of the elbow joint is $30-130^{\circ}$ of flexion-extension and $50^{\circ}C$ of supination and pronation. About 90% of daily activities are done using this range of motion. Stiff elbow joints can be classified according to the traumatic events that caused the problem or the location of the main pathology. Intraarticular pathology includes severe articular mismatch, intraarticular adhesions, loss of articular cartilage, mechanical blockade by osteophytes, loose bodies, and hypertrophied synovium. Extraarticular pathology includes severe capsular adhesion due to the trauma or to dislocation, contracture of the collateral ligaments or muscles, bony bridge. Results and Conclusions: The main pathology underlying the loss of extension is the fibrous contracture of the anterior capsule. In this pathology, an anterior capsulectomy would be helpful. The main pathology underlying the loss of flexion is the contracture of the posterior band of medial collateral ligament.

The Effect of Microcurrent Stimulation Intensity on Osteoarthritis in Rat (미세전류자극 강도가 흰쥐의 골관절염 회복에 미치는 영향)

  • Osteoarthritis is a degenerative joint disease and is led to physical disability. Yet the development of effective disease-modifying treatments has lagged. In this study, I examined the effect of physical therapeutic intervention through microcurrent stimulation and attempt to find which degree of intensity, either 25 ${\mu}A$ or 500 ${\mu}A$ with a regular 5 pps pulse, is more effective in the osteoarthritis. Osteoarthritis was induced with a mixture of 2% carrageenan and 2% kaolin in 26 male Sprague-Dawley rats. The mixture (0.1 $m{\ell}$) was injected into the intra-articular capsule of knee joint once a week for three weeks. Five animals did not show degenerative changes by radiological findings and excluded in the following experiment. Osteoarthritic animals were randomly divided into 3 groups ($n_1$, $n_2$, $n_3$=7/each): untreated, treated with 25 ${\mu}A$, treated with 500 ${\mu}A$. All experimental groups received microcurrent stimulation for four weeks (15 min/day, 5 days/week). The ethological inspection of foot print analysis on the walking corridor was accomplished every week. Histological preparations and immunohistochemical staining with insulin-like growth factor-1 were also done in the articular cartilages. All of these parameters were compared with those of osteoarthritic control group (n=7). The ethological inspection of foot print analysis revealed that changes of walking track (paw width) and stride length was significantly increased in both experimental groups. The better results were observed in experimental group treated with 25 ${\mu}A$ intensity without significance than group treated with 500 ${\mu}A$. Histological preparations disclosed that routine hyaline cartilage of articular surface were altered to fibrous cartilage in untreated group and experimental group treated with 500 ${\mu}A$ intensity. But a little changes were seen in experimental group treated with 25 ${\mu}A$ intensity. Immunolocalization of insulin-like growth factor-1 was simultaneously decreased according to the duration of osteoarthritis, and did not show significant difference among the groups. In this study discovered that the microcurrent stimulation, especially 25 ${\mu}A$ intensity, had a positive effect by the ethological inspection, histological and immunohistochemical stainings. These results suggest that microcurrent stimulation with low-intensity might be effective in the promotion of healing process for the osteoarthritis.
