Survey on Personal Medicines in Cheju Island (제주도 민간요법에 관한 조사연구)

  • The purpose is to inspect the personal medicines in Cheju island that are used traditionally and to get their characteristics. The subjectives are 39 Cheju people, men 10, women 29, who were horned, grown up in Cheju and agreed with this study. The ranges of age are 10 persons(m : 1, w: 9) over 40 less 50 years old, 14(m : 7, w : 7) over 50 less 60, 5(m : 1, w : 4) over 60 less 70, 6(m : 0, w : 6) over 70 less 80, and 4(m : 1, w : 3) over 80. The average age is 64.5 years old. The method to collect the data is 20 structured opening questionaires that are based on references. The duration to collect datas is 11days from 4th, Aprill 1997 to 14th, Aprill 1997. The workers who are trainned the interview methods went their villages and got answers after explaining the purpose and contents to them. Somtimes they used to record the answers. The analysis was identified the subjectives to four regions of Cheju, arranged answers with items, rearranged the same answers, counted with number and percentage. And classified the materials and characteristics. The results of this study are followed: The things that is used as personal medicines are the effects through experiences ans misteries that have hand down by word of mouth, even though they are not scientific. People used the materials arround their circumference. It is an accumulation of experiences. The ways used in eating, doing acupuncture or sting, exposing to smoke, wheedling, fixing after pounding. Almost materials are plants. Mugwort is effective in fever, gastritis, hemorrhoid, diarrhea, edematous hands or feet and dermatitis. Citron and Arrowroot in fever, gastritis. Seeds of Pumpkin in indigestive, hemorrhoid, edematous hands or feet. Gallic in fever, diarrhea, frostbite, dermatitis, and toothache. Motherwort in diarrhea, gastritis. Radish juice in indigestive, jaundice. Bean paste in burn, wound. Acupuncture in fever, gastritis, indigestive, back pain, edematous hands or feet. Sting as similar with it in fever, indigestive, edematous hands or feet. Cigarrette in hemorrhoid, wound, toothache. Cowstools in edematous hands or feet, wound. Sault is usded a lot in fever, gastritis, indigestive, hemorrhoid, uneffective voiding, edematous hands or feet, dermatitis, having a boil around the mouth, toothache, and eye disease. Japanese Parsley in fever, gastritis. Egg Apple in diarrhea, edematous hands or feet, frostbite. And Wild Chrysanthemum is effective in jaundice. In the conclusion, people used the things arround. A things is effective in several symptoms. If these are not effective, they would used the magic as god's anger. As locally, they used the grasses and fruits in the middle of Mt. Halla and seafood on the sea village.

A Study of Hospital Infection in the Postoperative Patients (수술후 병원 감염 발생과 입원일수 및 수술 소요 시간과의 관계)

  • This study was aimed to observe the incidences and types of hospital infections and to compare the length of hospitalization and of the operation between the infected Patients and those of non-infected after the operations. The subject of study were 465 patients who had been operated surgically in a University Hospital from March 1 to April 30, 1985. The data were collected by reviewing medical charts of subjects. The criteria to diagnose hospital infection for this study had been revised the one utilized at University of Virginia Hospital in the U.S. Summary of the results were as follows: 1. The incidence rate of hospital infection was 6 %. The type of infection with higher incidence rate in order were wound infection(28.5%)urinary infection(28.5%), fever of unknown origin (25%) and septicemia (18%). 2. There was a statistically significant difference in the duration of hospitalization between the patient group without it (t=265.2, p<0.005). 3. There was a statistically significant difference in the duration of operation between the patient group with it (t=75.0, p<0.005).

Clinical Manifestations of PFAPA (Periodic Fever, Aphthous Stomatitis, Pharyngitis, and Adenitis) Syndrome from a Single Center (단일기관에서 진단한 PFAPA (Periodic Fever, Aphthous Stomatitis, Pharyngitis, and Adenitis) 증후군의 임상양상)

  • Purpose: Periodic fever, aphthous stomatitis, pharyngitis, and adenitis (PFAPA) syndrome is a leading cause of periodic fever in children. This study describes the clinical characteristics of PFAPA syndrome in patients from a single center. Methods: Thirteen children diagnosed with PFAPA syndrome at Seoul National University Children's Hospital were included in this study. Retrospective medical chart reviews were performed. Results: Among the 13 patients, 8 (61.5%) were male. The median follow-up duration was 3.3 years (range, 10 months-8.3 years). The median age of periodic fever onset was 3 years (range, 1-6 years). All patients had at least 5 episodes of periodic fever and pharyngitis, managed with oral antibiotics, before diagnosis. The median occurrence of fever was every 3.9 weeks and lasted for 4.2 days. All patients had pharyngitis and 12 (92.3%) had cervical lymphadenitis. Blood tests were performed for 12 patients, and no patients had neutropenia. Both the C-reactive protein and erythrocyte sedimentation rate were elevated at medians of 4.5 mg/dL (range, 0.4-13.2 mg/dL) and 29 mm/hr (range, 16-49 mm/hr), respectively. Throat swab cultures and rapid streptococcal antigen tests were negative. Nine (69.2%) patients received oral prednisolone at a median dose of 0.8 mg/kg, and in 6 (66.7%) patients, fever resolved within a few hours. Three (23.1%) patients received tonsillectomy and adenoidectomy. Conclusions: PFAPA syndrome should be considered when a child presents with periodic fever along with aphthous stomatitis, pharyngitis, or cervical lymphadenitis. Glucocorticoid administration is effective for fever resolution and can reduce unnecessary use of antibiotics.

Effect of Chest Physical Therapy in Pediatric Patients (소아 폐염 환자에서 시행한 흉부 물리치료의 효과)

  • Object: We evaluated the efficacy of chest physical therapy in pediatric patients with pneumonia. Method: Retrospective study was performed in 89 pneumonia patients admitted to pediatric unit. The subjects were divided into two groups: control group; treatment group. Control group included 42 patients who didn't receive chest physical therapy. Treatment group included 47 patients who received chest physical therapy. The chest physcial therapy employed were postural drainage, chest percussion, deep breathing training and enhancement of coughings. The efficacy was evaluated by x-ray outcome before discharge. Results: There were no significant difference in age, sex, type of pneumonia, and symptom duration between two groups. However the duration of fever after admission, duration of antibiotic use and hospital stay were longer in treatment group. In treatment group, longer the day to initiate chest physical therapy, longer the hospital stay. The final outcome was not different between groups. Conclusion: The results suggest that the referred patients for chest physical therapy tends to be of severe cases. Nevertheless, the result that the outcome was not different in two groups means that the chest physical therapy could be used as a effective treatment method in pediatric patient with pneumonia.

Clinical features according to chest radiologic patterns of Mycoplasma pneumonia in children

  • Background: Clinical differences in Mycoplasma pneumonia (MP) in children and adolescent patients according to abnormal infiltrate patterns on the chest X-ray were compared. Methods: From 2012 to 2015, patients (n=336) diagnosed with MP at Yeungnam University Medical Center have been classified as eiher lobar pneumonia or bronchopneumonia based on the infilterate patterns observed on chest X-ray. Cases were analyzed retrospectively for gender, age, seasonal incidence rate, main symptoms (fever duration, extrapulmonary symptoms), and laboratory results, including white blood cell count, hemoglobin, platelets, C-reactive protein (CRP), and erythrocyte sedimentation rate (ESR), as well as concurrent respiratory virus infection. Results: The following results were observed. First, lobar pneumonia affected 22.0% of all MP patients and was the most common in preschool children, with a high incidence rate in November and December. Second, lobar pneumonia had a longer fever duration than bronchopneumonia (p<0.001), and also showed significantly higher platelets (336.8 vs. $299.1k/{\mu}L$, p=0.026), ESR (46.3 vs. 26.0 mm/hr, p<0.001) and CRP (4.86 vs. 2.18 mg/dL, p=0.001). Third, viral co-infection was more common in bronchopneumonia (p=0.017), affecting 66.7% of infants and toddlers (p=0.034). Finaly, lobar consolidation was most common in both lower lobes. Conclusion: MP in children has increased in younger age groups, and the rate of lobar pneumonia with severe clinical symptoms is higher in older children.

Are the Clinical outcomes of Neonates and Infants Under 2 Months Old with Urinary Tract Infections Similar to those in Infants 2 to 12 Months Old?

  • Purpose: Although the American Academy of Pediatrics provides clinical guidelines for urinary tract infection (UTI) infants, guidelines are not appropriate for neonates and infants less than 2 months of age due to insufficient data. The aim of this study was to evaluate the characteristics of neonates and young infants less than 2 months old (group 1) with UTI compared to older infants from 2 to 12 months old (group 2). Methods: We reviewed UTI patients aged 0 to 12 months admitted to the pediatric department in the last 5 years. Clinical characteristics such as age, sex, fever duration, recurrence, progression to acute pyelonephritis (APN), malformations like hydronephrosis and vesicoureteral reflux (VUR), and laboratory results were compared between group 1 and group 2. Results: 615 patients were included in this study. Group 1 had 94 cases and group 2 had 521 cases. Escherichia coli was the most commonly isolated pathogen in urine cultures. Fever duration was shorter in group 1 (vs.) 2 ($1.91{\pm}1.43$ days vs. $3.42{\pm}2.40$ days, P<0.05). As compared to group 2, group 1 had a higher proportion of patients with antenatal hydronephrosis and hydronephrosis found after admission (10.6% vs. 3.6% and 75.5% vs. 55.9%, P<0.05). There were differences between two groups in white blood cell (WBC) count (Group 1: $13,694{\pm}5,315/{\mu}L$, Group 2: $15,271{\pm}6,130/{\mu}L$, P<0.05) and C-reactive protein (Group 1: $32.02{\pm}35.17mg/L$, Group 2: $46.51{\pm}46.63mg/L$, P<0.05). Conclusion: Compared to older infants, UTI in neonates and young infants shows milder clinical manifestations except higher rates of hydronephrosis but outcome is alike.

Clinical features of Epstein-Barr virus-associated infectious mononucleosis in hospitalized Korean children

  • Purpose: Few studies have been conducted on the recent status of infectious mononucleosis (IM) in Korean children. The aim of this study was to evaluate the recent trend in the clinical manifestations of Epstein-Barr virus (EBV)-associated IM as well as the clinical differences according to age. Methods: A retrospective study was performed on 81 children hospitalized with EBV-associated IM who fulfilled the serological criteria for the diagnosis of EBV infection (viral capsid antigen immunoglobulin M positive). The patients were divided into 3 age groups: <5 years, 5 to 9 years, and ${\geq}10$ years. We evaluated the recent trend in clinical manifestations and the differences in clinical and laboratory findings among the 3 age groups. Results: Thirty (37%) children were under 5 years of age, 38 (46.9%) were 5 to 9 years of age, and 13 (16%) were 10 years of age or older. The differences in the symptoms and signs among the 3 age groups were not statistically significant, except for headache. The mean duration of fever was 7.7 days (range, 0 to 18 days). A comparison of liver enzyme elevation among the age groups showed an association with advancing age (26.6%, 63.1%, and 76.9%, respectively, P=0.04) Conclusion: This study showed that EBV-associated IM in Korean children continues to occur mostly in children under 10 years of age. In children with EBV-associated IM, the incidence of headache and liver enzyme elevation, the duration of fever, and the proportion of females to males were all positively associated with advancing age.

Plasma Real Time-Quantitative Polymerase Chain Reaction of Epstein-Barr Virus in Immunocompetent Patients with Hepatitis

  • Purpose: Epstein-Barr virus (EBV) hepatitis is a usually asymptomatic and self-limiting disease in immunocompetent patients. However, the range of severity is wide, and the serological diagnosis is typically difficult until the convalescent phase. Thus, we examined the value of plasma EBV DNA real-time quantitative polymerase chain reaction (RT-qPCR) in EBV hepatitis for the timely diagnosis and the relationship between EBV viral load and clinical severity. Methods: Sixty samples were confirmed as having EBV infection by RT-qPCR with the EBV BALF5 gene sequence. We examined the clinical characteristics of EBV hepatitis by reviewing medical records. Results: The median total duration of fever was 8 days (range: 0-13 days). The mean peak value of aspartate aminotransferase (AST) was $241{\pm}214$ U/L, and the mean peak value of alanine aminotransferase (ALT) was $298{\pm}312$ U/L. There was no correlation between the serum levels of liver enzyme and plasma EBV DNA titer ($p$=0.1) or between median total duration of fever and EBV DNA titer ($p$=0.056). The median age of the EBV VCA IgM-negative group was lower compared with the EBV VCA IgM-positive group in EBV hepatitis (2 years vs. 6 years, $p$=0.0009). Conclusion: The severity of EBV hepatitis does not correlate with circulating EBV DNA load according to our data. Furthermore, we suggest that plasma EBV PCR may be valuable in young infants in whom the results of serology test for EBV infection commonly are negative.

An Epidemiological Study for on Outbreak of Typhoid Fever in Busan Area (부산 일부지역에 유행한 장티푸스 환자에 대한 역학적인 조사)

  • An epidemiological study was conducted, from April 28 to June 3, 1975, to investigate source and course of infection of typhoid fever occurred in Munhyeon Dong, Nam Gu, Busan. The obtained results were as follows: 1. The entity of unknown febrile disease was identified as Salmonella typhi, D group and it's symptoms were abdominal pain, diarrhea, hepatomegaly, nausea and vomiting, aplenomegaly, rose spots, tenderness of ileocecal region etc., in that order. 2. The average duration from onset to recovery was 25.2 days and incidence rate was 39.5 per 1,000 population. 3. The source of infection was presumed a charity patient. 4. By the duration of report on the cases from onset, 10-19 days group was the highest. 5. The positive rate of initial stool culture was 38.9% and secondary was 5.6%. 6. Distribution of the cases by age and sex showed that 5-9 years old group was the highest as 30.5%, and male was higher than female. 7. Distribution of the cases by education level, under primary school as 66.6% was the highest. By the living standard, the highest was low income earner group as 77.8%. 8. Utilization rate of medical facilites was drug store (41.7%), herbal drug (8.3%), clinic (5.6%) in that order.

Treatment of Peritonsillar Abscess: Needle Aspiration versus Incision and Drainage

  • Background and Objectives: The Objective of this study was to compare the effectiveness of the needle aspiration method and the I&D method in the treatment of peritonsillar abscess. Materials and Methods: A prospective clinical study was performed on 83 patients. All the patients were hospitalized after random treatment with either I&D or needle aspiration alone, received the same intravenous antibiotic therapy. Among the 83 patients, 73 patients who could be observed for longer than 6 months and had not undergone a tonsillectomy during the follow-up period, were analyzed for treatment outcomes. Results: There was no statistically significant difference in the initial failure rate(p=0.572), the hospitalized days(p=0.956), the recurrence rate(p=0.531) for the needle aspiration(35 patients) and I&D groups(38 patients). But, The mean duration of fever were statistically different in the needle aspiration(1.51 hours) and I&D groups(3.05 hours) (p=0.031). Conclusion: Two methods are thought to be similar in effectiveness, except that duration of fever was longer in the I&D group than in the needle aspiration group. However, taking advantages of the needle aspiration method into consideration, the needle aspiration of peritonsillar abscess may be more appropriate than I&D as an initial method for peritonsillar abscess
