A Fetal ECG Signal Monitoring System Using Digital Signal Processor (디지털 신호처리기를 사용한 태아심전도 신호 추출 시스템)

  • This paper describes the implementation of a real time fetal ECG monitoring system in which an adaptive multi-channel noise canceller is realized using the Texas Instruments TMS32020 progrmmmable ditital signal processor. An ECG signal from the electrode placed on the mother's abdomen and three ECGs from those on the chest are applied as the desired signal and the referened inputs, respectively, of the multi-channel filter. The coefficients of the filter are updated using the LMS algorithm such that the output of the multi-channel filter copies the maternal ECG embedded in the abdominal ECG. The enhanced fetal ECG is obtained by subtracting the filter output from the abdominal ECG, and the difference signal is recorded. Both off-line and on-line experimental results are presented to verify the effectiveness of the parameters for the digital signal processing algorithms and the prototype system.

  • Fetal heart rate monitoring is important information to assess fetal well-being. Non-invasive fetal ECG (electrocardiography) can be derived from maternal abdominal signal. And various promising signal processing methods have been introduced to extract fetal ECG from mother's composite abdominal signal. However, non-invasive fetal ECG monitoring still has not been widely used in clinical practice due to insufficient reliable measurement and difficulty of signal processing. In application of signal processing method to extract fetal ECG, it might be lower signal to noise ratio due to time varying white Gaussian noise. In this paper, time varying Kalman smoother is proposed to remove white noise in fetal ECG and its feasibility is confirmed. Wiener process was set as Kalman system model and covariance matrix was modified according to white Gaussian noise level. Modified error covariance matrix changed Kalman gain and degree of smoothness. Optimal covariance matrix according to various amplitude in Gaussian white noise was extracted by 5 channel fetal ECG model, and feasibility of proposed method could be confirmed.

  • In this paper, we have proposed a new method to extract the fetal ECG from a pregnant woman's abdominal signal using least square acceleration (LSA) filter and adaptive impulse correlation (AIC) filter. To evaluate the performance, the proposed method and other fetal ECG extraction techniques were processed using the real ECG data and then the results were compared. According to comparative results, the proposed method is powerful and successful for extracting the fetal ECG. It was able to separate perfectly even though the fetal beats overlap with the QRS wave of the maternal beats and to extract fetal ECG using any single-channel abdominal signal measured from pregnant woman's abdominal surface. Also, it could be implemented easily by fast computation time and simple structure. It is sure that our method could be useful for portable fetal monitoring system.

  • Fetal monitoring is a routine procedure to obtain a record of physiologic functions during pregnancy and labor. It is required to determine fetal heart frequency accurately. There are various types of fetal heart rate(FHR) determination and the most frequently applied method is transabdominal Doppler ultrasound. However, in the case of weak or noise corrupted Doppler ultrasound signals, conventional peak detections and the autocorrelation function method have many difficulties to determine FHR precisely. Also the autocorrelation function is effected by threshold level and window size. To solve these problems, the high resolution pitch determination algorinthm is introduced to detect FHR from Doppler ultrasound signals. This scheme digitally processes Doppler ultrasound signal for digital rectification, envelope detection, decimation and correlation calculation of two interconnected segments and then FHR is determined by its maximal value. Even in the case of a greatly smeared noise signal, this algorithm is able to search FHR more accurately than autocorrelation function by means of compensating FHR with a constant correlation threshold. This algorithm is simulated by 386-MATLAB on PC 486/DX and verified that it is superior to the autocorrelation function method.

  • Digital FHR monitoring system based on the personal computer combined wi th the digital signal processing (DSP) board was implemented. The DSP board acquires and digitally processes ultrasound fetal Doppler signal for digital rectification, envelope detection, autocorrealtion function calculation and its peak position detection. The personal computer interfaced with the DSP board is in charge of graphic display, hardcopy, data taransmission and on-line analysis of fetal heart rate change including on-line warning system, base-line estmation, acceleration, deceleration and variability. The advantages of PC-DSP based system can be summarized as follows: 1) Minimum hardware realization. 2) Increased flexibility. 3) Total care system.

  • Sacrococcygeal teratoma (SCT) is an extragonadal germ cell tumor (GCT) that develops in the fetal and neonatal periods. SCT is a type I GCT in which only teratoma and yolk sac tumors arise from extragonadal sites. SCT is the most common type I GCT and is believed to originate through epigenetic reprogramming of early primordial germ cells migrating from the yolk sac to the gonadal ridges. Fetal SCT diagnosed in utero presents many obstetrical problems. For high-risk fetuses, fetal interventions (devascularization and debulking) are under development. Most patients with SCT are operated on after birth. Complete surgical resection is the key for tumor control, and the anatomical location of the tumor determines the surgical approaches. Incomplete resection and malignant histology are risk factors for recurrence. Approximately 10-15% of patients have a tumor recurrence, which is frequently of malignant histology. Long-term surveillance with monitoring of serum alpha fetoprotein and magnetic resonance imaging is required. Survivors of SCT may suffer anorectal, urological, and sexual sequelae later in their life, and comprehensive evaluation and care are required.

Precise segmentation of fetal head in ultrasound images using improved U-Net model

  • Monitoring fetal growth in utero is crucial to anomaly diagnosis. However, current computer-vision models struggle to accurately assess the key metrics (i.e., head circumference and occipitofrontal and biparietal diameters) from ultrasound images, largely owing to a lack of training data. Mitigation usually entails image augmentation (e.g., flipping, rotating, scaling, and translating). Nevertheless, the accuracy of our task remains insufficient. Hence, we offer a U-Net fetal head measurement tool that leverages a hybrid Dice and binary cross-entropy loss to compute the similarity between actual and predicted segmented regions. Ellipse-fitted two-dimensional ultrasound images acquired from the HC18 dataset are input, and their lower feature layers are reused for efficiency. During regression, a novel region of interest pooling layer extracts elliptical feature maps, and during segmentation, feature pyramids fuse field-layer data with a new scale attention method to reduce noise. Performance is measured by Dice similarity, mean pixel accuracy, and mean intersection-over-union, giving 97.90%, 99.18%, and 97.81% scores, respectively, which match or outperform the best U-Net models.

  • Despite the simplicity of processing, a conventional autocorrelation function (ACF) method for the precise determination of fetal heart rate (FHR) has many problems. In case of weak or noise corrupted Doppler ultrasound signal, the ACF method is very sensitive to the threshold level and data window length. It is very troublesome to extract FHR when there is a data loss. To overcome these problems, the high resolution pitch detection algorithm was adopted to estimate the FHR. This method is more accurate, robust and reliable than the ACF method. With a lot of calculation, however, it is impossible to process real time FHR estimation. This paper is presented a new FHR estimation algorithm for real time processing and studied the real time FHR monitoring system by high resolution pitch detection algorithm.

  • Continuous fetal heart beat monitoring has assisted clinicians in assuring fetal well-being during antepartum and intrapartum. Fetal heart rate (FHR) is an important parameter of fetal health during pregnancy. The Doppler ultrasound is one of very useful methods that can non-invasively measure FHR. Although it has been commonly used in clinic, inaccurate heart rate reading has not been completely resolved.. The objective of this study is to improve detection algorithm of FHR from Doppler ultrasound signal with simple method. We modified autocorrelation function to enhance signal periodicity and adopted adaptive window size and shifted for data segment to be analysed. The proposed method was applied to real measured data, and it was verified that beat-to-beat FHR estimation result was comparable with the reference fetal ECG data. This simple and effective method is expected to be implemented in the embedded system.

  • Oxygen saturation is an important parameter in clinical fields; fetal monitoring, apnea, emergency medicine etc. Because of monitoring patients continuously, pulse oximeter that measures oxigen saturation non-invasively is regarded attentively. But, though research about accuracy of signal extraction has been developed, it actually plays a supplementary part in hospital for not trusting the principle of measurement by clinicians. In this paper focusing on these things, first we suggested simple mathematical modelling on separating do components, ac components andnoise components in optical signal transmitted from fingertip or earlobe, and then we considered oxygen saturation computing algorithm using integral ratio of pulsating components. Last, we analyzed its effect by comparing received data.
