노인의 낙상과 균형 (Fall-Related Injury and Balance of the Elderly)

  • 낙상은 노인에서 중요한 건강문제와 직결된다. 노인의 활동력 저하와 노화로 인한 생리학적 변화는 균형과 기능적 가동성 감소를 초래하고 낙상의 주요 요인이 된다. 노년기의 신체활동은 근력유지뿐만 아니라 균형감각 같은 신경계의 기능유지를 위해서도 필요하다. 노화 되어감에 따라 활동이 줄어들고 대근의 활동보다는 소근의 활동을 중심으로 소일하는 경우가 대부분이다. 따라서 신체의 운동범위가 좁아짐에 따라 활동할 때 신체균형감각이 둔화되어 낙상을 일으키게 되고 심각한 사회적 문제를 초래하게 된다. 그러므로 노인의 낙상방지와 생활의 질적 향상을 위해 적당한 강도의 운동과 균형수행력을 향상시키는 훈련이 필요하다.

사지에서 전이성 골종양의 수술적 치료 (Surgical Treatment of Metastatic Bone Tumor in Extremity)

  • The incidence of metastatic bony lesion has been increased recently, with the advanced therapeutic modalities of malignant tumors. The purpose of this study was to evaluate the effectiveness of surgical treatment in impending or established pathologic fractures due to metastasis in extremity. From 1981 to 1992, thirty two patients who had established or impending pathologic fractures in extremity had been treated surgically. The locations of metastasis, were lower extremity in 24 cases(femsral head and neck;5, intertrochanteric;7, subtrochanteric;8, femoral shaft;4) and upper extremity in 8 cases(proximal humerus;2, humoral shaft;6). The method of surgery was excision of tumor mass and rigid internal fixation to allow immediate mobilization, except 2 cases of amputation. The results were analysed by two criteria. The first was the grade of performance status that was made by modification of Functional Classification of New York Heart Association. The second was the degree of pain relief. The performance status was improved at least one grade in all patients and the mean improvement was 1.7 grades. The satisfactory pain relief was achieved in 81% of patients. Postoperatively, most of the patients had only slight to moderate limitation of physical activities and they became comfortable at rest and ordinary activities. The solitary metastatic lesions showed better pain relief(92%) than multiple metastasis(75%). The curative surgery was done in 3 cases, and they showed more than 24 months of disease-free survival.

Bone Mineral Density and Osteoporotic Vertebral Fractures in Traditional, Unassisted, Free-Diving Women (Haenyeos)

  • Background: Water pressure and muscle contraction may influence bone mineral density (BMD) in a positive way. However, divers experience weightlessness, which has a negative effect on BMD. The present study investigated BMD difference in normal controls and woman free-divers with vertebral fracture and with no fracture. Methods: Between January 2010 and December 2014, traditional woman divers (known as Haenyeo in Korean), and non-diving women were investigated. The study population was divided into osteoporotic vertebral fracture and non-fracture groups. The BMD of the lumbar spine and femoral neck was measured. The radiological parameters for global spinal sagittal balance were measured. Results: Thirty free-diving women and thirty-three non-diving women were enrolled in this study. The mean age of the divers was $72.1{\pm}4.7$ years and that of the controls was $72.7{\pm}4.0$ years (P = 0.61). There was no statistical difference in BMD between the divers and controls. In divers, cervical lordosis and pelvic tilt were significantly increased in the fracture subgroup compared to the non-fracture subgroup (P = 0.028 and P = 0.008, respectively). Sagittal vertical axis was statistically significantly correlated with cervical lordosis (Spearman's rho R = 0.41, P = 0.03), and pelvic tilt (Spearman's rho R = 0.46, P = 0.01) in divers. Conclusion: BMD did not differ significantly between divers and controls during their postmenopausal period. When osteoporotic spinal fractures develop, compensation mechanisms, such as increased cervical lordosis and pelvic tilt, was more evident in traditional woman divers. This may be due to the superior back muscle strength and spinal mobility of this group of women.

Bisphosphonates for Osteoporosis in Nonmetastatic Prostate Cancer Patients Receiving Androgen-deprivation Therapy: A Systematic Review and Meta-analysis

  • This systematic review was conducted to assess the efficacy and safety of bisphosphonates for prevention and treatment of osteopenia or osteoporosis in men with non-metastatic prostate cancer receiving androgendeprivation therapy. We searched for randomised controlled trials (RCTs) of bisphosphonates compared with placebo from Pubmed, Embase, the Cochrane Library, and ISI - Science Citation Index. Meta-analyses of prespecified outcomes (bone mineral density, fractures, and adverse events) were performed using Review Manager. Ten RCTs with a total patient population of 1,017 were identified. There was generally more improvement in bone mineral density of the lumbar spine for patients who received bisphosphonate treatment than placebo or other medical treatment at 12 months (WMD 6.02,95%CI 5.39 to 6.65). Similar effects were also observed for total hip, trochanter or femoral neck bone mineral density. However, there was no significant reduction in fractures. Fever and gastrointestinal symptoms were the most common adverse events (10.4% vs. 1.2%; 0.10% vs. 0.03%). Currently, our meta-analysis suggested that oral and intravenous bisphosphonates caused a rapid increase in spine and hip or femoral BMD in non-metastatic prostate cancer patients receiving androgen-deprivation therapy. Fever and gastrointestinal symptoms were common with the use of bisphosphonates. These short-term trials (maximum of 12 months) did not show fracture reduction. In future, more efficient performance of higher quality, more rigorous, large sample, long-term randomised controlled trials (>12 months) are needed where outcomes are detailed.

The Effects of COVID-19 Pandemic on the Recovery of Hip Fracture Patients

  • Purpose: To figure out how complete control of family visits to prevent infection of coronavirus disease 2019 (COVID-19) affected the activity recovery of hip fracture patients admitted to nursing hospitals. Materials and Methods: Eighty-one patients with hip surgery in the two years prior to COVID-19 pandemic were classified as Group A, and 103 patients in the next two years were designated as Group B. The subjects' walking ability was evaluated by using the modified Koval index (MKI). In order to analyze the impact of the family visit control to the subjects, each group was classified into two different groups: (1) inpatients group who admitted to nursing hospitals and (2) home-treated patients. Additionally, statistical elements were processed in consideration of other factors that may affect the results of the experiment. Results: The MKI evaluated at 6 months postoperative was 3.31±1.79 in Group A and 2.77±1.91 in Group B, and it was meaningfully low after the pandemic (P=0.04). There was significantly low among both of Group A 2.74±1.76 and Group B 1.93±1.81 after the pandemic (P=0.03) among those treated at the nursing hospital. The rate of deterioration of the MKI was 35 (43.2%) in Group A and 57 (55.3%) in Group B, which increased by 12.1% after the pandemic. Conclusion: The pandemic had a negative effect on the recovery of postoperative activities of elderly hip fracture patients who admitted to nursing hospitals when family access was completely restricted to prevent infection.

대퇴경부 골절 환자의 입원 생활 (The Hospital Life of the Patient with Femoral Neck Fracture)

  • Nowerdays, the increase of traffic accidents and old age population make the Femoral Neck Fracture(FNF) patients increase. By the improvement of education and standard of living the patients demand better medical service than before. This study is designed to give practical help for the FNF patients by observing their hospital life and establish practical nursing strategies for the FNF patients. For these purposes the Ethnographic Participant Observation was adopted. By this study is focused on the hospital life patient's view. For this end, the field study adopted orthopedic ward in the C University Hospital with 400 beds in Seoul. The object patients of the study were twelve patients. The patients experienced five stages : Embarrassment, Conflict, Stability, Independent, and Extension Stage. The findings and prepared nursing strategies are stated as follows. First, in the Embarrassment Stage they suffered embarrassment, anxiety, pain, they could not do ordinary things. The patients who accidental fractures had anxiety from unfamiliar tests and from hospitalization itself. They lamented that they could not ordinary things, and do nothing but obeying the hospital, and endure the pain. They recognized the changed environment and resigned themselves to life in the ward. In this stage, full openness by the nurses is needed. Second, the attribute of the Conflict Stage were conflict, fear, curiosity, belief, reflection. When they sign the consentment form, they experience conflicts about the possibility of complication, fear of recovery from anesthesia, curiosity about the operation procedure, post - operation state, reflection on their past life, and promise to care for their family members after discharge and keep their religious life faithfully. And they accepted the operation depending on God, believing in modern medicine, and the surgeon. Asking for their changed informations, they expected positive results from the operation. In this stage, an empathic attitude by the nurses is needed. Third, the attribute of the Stability Stage were relief, gratitude, difficulty with excretion, and pain. When they awoke from anesthesia, they felt relief because of a the end of the operation, but they experienced extreme pain, difficulty of excretion in bed. They accepted the changed environment and expected recovery. In this stage, support by the nurses is needed. Fourth, the attributes of the Independence Stage were freedom, exercise, nurturing, anxiety, and discomfort. When they ambulated and exercised, they experienced freedom. They showed exhibited weakness of the digestive organs and discomfort hospital's space, structure, and facilities, the delay of medical certificate issue the lack of prompt response by the medical agents. They ate nurturious food and felt anxiety on the end of hospital life and returning to their ordinary life. They showed the independence of overcoming their environment by increasing exercise and expected their discharges. In this stage, respect by the nurses is needed for the patients to, overcome their environment and prepare for their independence. Fifth, the attributes of the Extension Stage were pessimism, isolation, dissatisfaction, and pain. Accompanied injury and old age made their ward life extend to over seven weeks. They exhibited weariness, melancholy, skeptisis, general pessimistic feeling, and desperation caused by their isolated life. They experienced the digestive discomfort caused by the prolonged medication and psycological pain caused by long-time hospitalization. As a, result, their dissatisfaction on the human, physical, and systematic environments had been increased. They acquired critical power and sought for something to do spending their time. They expected vaguely about the returning of their ordinary life. In this stage, counseling is needed by the nurse to overcome positively their psychological, social, and physical problems. The process of the FNF patient's ward life starts from the dependent state, when they are hospitalized, and gradually progresses to self-fulfillment in order to keep independent life. As a result, the FNF patients showed "Response in Challenge" or "Adaptation in Conflict" through their experiences of social, physical, and psychological difficulties.

고관절 주위 골절환자에서 술 전 색 도플러 초음파를 이용하여 진단한 심부 정맥 혈전의 빈도 (Incidence of Deep Vein Thrombosis before Hip Fracture Surgery Diagnosed by Color Doppler Sonography Surveillance)

  • 목적: 고관절 주위 골절 환자에서 술 전 색 도플러 초음파 검사를 이용하여 심부 정맥 혈전증을 진단하고 그 발생 빈도를 알아 보고자 하였다. 대상 및 방법: 2013년 6월부터 2014년 5월까지 본원에 내원한 고관절 주위 골절 환자 중 이전에 심부 정맥 혈전의 과거력이 없으며 술 전 색 도플러 초음파 검사에 동의한 환자 27예를 대상으로 하였으며 남자가 8예, 여자가 19예이었다. 평균 연령은 74.3세(41-87)였다. 고관절 주위 골절의 진단명은 대퇴 경부 골절 15예, 대퇴 전자간 골절 11예, 비구 골절이 1예였다. 모든 수술은 본원 내원 후 48시간 이내에 시행하는 것을 원칙으로 하였으며 내원 24시간 이내에 색 도플러 초음파 검사를 시행하였다. 결과: 심부 정맥 혈전은 총 6예(22.2%)에서 발견되었다. 이중 2예(7.4%)는 근위 심부 정맥 혈전이었고 4예(14.8%)는 원위 심부 정맥 혈전이었다. 심부 정맥 혈전 발생 군이 평균 79세(75-87), 발생하지 않은 환자 군이 평균 72세(65-86)로 유의하게 많았다(p=0.038). 결론: 고관절 주위 골절 환자에게 수술 이전 실시한 색 도플러 초음파 검사상 22.2%로 비교적 높은 빈도로 심부 정맥 혈전이 발견되었다. 고관절 주위 골절 환자에서 술 전 심부 정맥 혈전증에 대한 보다 적극적인 검사가 필요할 것으로 사료된다.

멀티 모달리티 데이터 활용을 통한 골다공증 단계 다중 분류 시스템 개발: 합성곱 신경망 기반의 딥러닝 적용 (Multi-classification of Osteoporosis Grading Stages Using Abdominal Computed Tomography with Clinical Variables : Application of Deep Learning with a Convolutional Neural Network)

  • 골다공증은 전 세계적으로 주요한 건강 문제임에도 불구하고, 골절 발생 전까지 쉽게 발견되지 않는 단점을 가지고 있습니다. 본 연구에서는 골다공증 조기 발견 능력 향상을 위해, 복부 컴퓨터 단층 촬영(Computed Tomography, CT) 영상을 활용하여 정상-골감소증-골다공증으로 구분되는 골다공증 단계를 체계적으로 분류할 수 있는 딥러닝(Deep learning, DL) 시스템을 개발하였습니다. 총 3,012개의 조영제 향상 복부 CT 영상과 개별 환자의 이중 에너지 X선 흡수 계측법(Dual-Energy X-ray Absorptiometry, DXA)으로 얻은 T-점수를 활용하여 딥러닝 모델 개발을 수행하였습니다. 모든 딥러닝 모델은 비정형 이미지 데이터, 정형 인구 통계 정보 및 비정형 영상 데이터와 정형 데이터를 동시에 활용하는 다중 모달 방법에 각각 모델 구현을 실현하였으며, 모든 환자들은 T-점수를 통해 정상, 골감소증 및 골다공증 그룹으로 분류되었습니다. 가장 높은 정확도를 갖는 모델 우수성은 비정형-정형 결합 데이터 모델이 가장 우수하였으며, 수신자 조작 특성 곡선 아래 면적이 0.94와 정확도가 0.80를 제시하였습니다. 구현된 딥러닝 모델은 그라디언트 가중치 클래스 활성화 매핑(Gradient-weighted Class Activation Mapping, Grad-CAM)을 통해 해석되어 이미지 내에서 임상적으로 관련된 특징을 강조했고, 대퇴 경부가 골다공증을 통해 골절 발생이 높은 위험 부위임을 밝혔습니다. 이 연구는 DL이 임상 데이터에서 골다공증 단계를 정확하게 식별할 수 있음을 보여주며, 조기에 골다공증을 탐지하고 적절한 치료로 골절 위험을 줄일 수 있는 복부 컴퓨터 단층 촬영 영상의 잠재력을 제시할 수 있습니다.
