A Study on the Structural Analysis for Fatal Industrial Accidents using Multivariate Analysis Methods (다변량 분석기법을 활용한 중대재해 구조분석에 관한 연구)

  • The importance of the industrial safety has been growing gradually as well as the prevention activities on industrial accidents. Industrial accident rates have been decreasing by the prevention activities. However, the fatal industrial accidents such as the death tend to increase and risk per accident has increased. The previous studies on the industrial accidents focus on the entire accidents. However, these studies are lacking for the fatal industrial accidents such as the death. The purpose of this paper is to analyze the characteristics and trend of death which occurred by industrial accident, based on the real data of deaths collected last 5 years from 1999 to 2003 in korea. This paper suggests a analysis method using MDS(MultiDimensional Scaling) that considers accidents variables and properties simultaneously. We evaluate MDPREF (Multidimensional Analysis of Preference Data), one of the MDS analysis, to know the relations between the type of industry and region as well as the type of industry and occupation. This paper finds the type of industry which has high possibilities of death by regional groups. In addition, we find the type of occupation which has high possibilities of death by the type of industry. These findings indicate that industrial classification should be differently controled according to type of occupation and region.

Gas Chromatographic Analysis of Zipeprol in Blood (혈액중 지페프롤의 가스크로마토그래피에 의한 분석)

  • The abuse of zipeprol, an antitussive agent, is prevalent among young people. Ten fatal cases of zipeprol concentration in blood after its abuse had caused death are disscussed. GC equipped with TSD was used to quantify the drug in postmortem blood and GC/MS to identify the metabolites in urine after preliminary test. The calibration curve of zipeprol was linear (r = 0.998) and the recoveries of zipeprol 5, 10 and $20\;{\mu}g$ added to 1 ml postmortem blood were $97.2{\pm}3.9$, $92.5{\pm}3.9$ and $86.9{\pm}4.6%$ respectively. The blood zipeprol concentration varied 2.9 to $24.69\;{\mu}g/ml$ in 10 fatal cases. All the deaths were young adults with the age of 16-21. Three metabolites were identified in drug abuser's urine, 2-methoxy-2-phenyl ethylpiperazine, 2-hydroxy-3-methoxy-3-phenyl propylpiperazine and a demethylated zipeprol.

A fatal case of acute pulmonary embolism caused by right ventricular masses of acute lymphoblastic lymphoma-leukemia in a 13 year old girl

  • We report a case of a 13-year-old girl with acute lymphoblastic lymphoma- leukemia, who presented with a cardiac metastasis in the right ventricle, resulting in a pulmonary embolism. At the time of her leukemia diagnosis, a cardiac mass was incidentally found. The differential diagnosis for this unusual cardiac mass included cardiac tumor, metastasis, vegetation, and thrombus. Empirical treatment was initiated, including anticoagulation and antibiotics. She underwent plasmapheresis and was administered oral prednisolone for her leukemia. Five days later, she experienced sudden hemodynamic collapse and required extracorporeal membrane oxygenation insertion and emergency surgery. These interventions proved futile, and the patient died. Pathology revealed that the cardiac mass comprised an aggregation of small, round, necrotic cells consistent with leukemia. This is the first known case of acute lymphoblastic leukemia presenting as a right ventricular mass, with consequent fatal acute pulmonary embolism. A cardiac mass in a child with acute leukemia merits investigation to rule out every possible etiology, including vegetation, thrombus, and even a mass of leukemic cells, which could result in the fatal complication of pulmonary embolism.

  • Schwannomas of the jugular foramen, originating from the glossopharyngeal nerve, vagus and accessory nerve represent approximately 0.17-0.72% of all intracranial tumor, and consists of 1.4-2.9% of all intracranial schwannomas. The clinical presentation of these tumors varies significantly according to originated nerve and it's growth pattern. Magnetic resonance(MR) image and temporal bone computed tomography(CT) scan have a major role for diagnosis of such tumor. The treatment of choice is total resection whenever possible. Generally, suboccipital approach is sufficient for the removal of the tumor, but in case with large size, combination of resection of petrous part of temporal bone with or without transection of sigmoid sinus is may be necessory. We have recently experienced one case of giant jugular foramen schwannoma and postoperative fatal complication in a 34-year-old male who was treated with combined posterior petrous and suboccipital approach with transection of sigmoid sinus

EQUIVALENT DOSE, EFFECTIVE DOSE AND RISK ASSESSMENT FROM PANORAMIC RADIOGRAPHY TO THE CRITICAL ORGANS OF HEAD AND NECK REGION (파노라마 촬영시의 두경부 주요기관의 등가선량, 유효선량 및 위험도)

  • The purpose of this study was to evaluate the equivalent and effective dose, and estimate radiation risk to the critical organs of head and neck region from the use of adult and child mode in panoramic radiography. The results were as follows. 1. The salivary glands showed the highest equivalent and effective dose in adult and child mode. the equivalent and effective dose in adult mode were 837μSv and 20.93μSv, those in child mode were 462μSv and 11.54μSv, respectively. 2. Total effective doses to the critical head and neck organs were estimated 34.21μSv in adult mode, 20.14μSv in child mode. From these datas, the probabilities of stochastic effect from adult and child mode were 2.50x10/sup -6/ and 1.47x10/sup -6/. 3. The other remainder showed the greatest risk of fatal cancer. The risk estimate were 4.5 and 2.7 fatal malignancies in adult and child mode from million panoramic examinations. The bone marrow and thyroid gland showed about 0.1 fatal cancer in adult and child mode from these examinations.

  • The number of accidents is raising every year and severity is also increasing. A lot of studies are interested in the leading indicator such as safety culture or safety audit to reduce accidents. In particular, the studies on safety awareness are actively conducted. However, these studies showed that there was consciousness of scapegoatism which to pass responsibility of accident causes to others regardless of safety awareness. It means that there are probability about overestimating and making light of unsafe condition and act which have to manage and control. Eventually, the direct causes which lead to the fatal accident may be consistently arisen. Nevertheless, researches about consciousness of scapegoatism are shortage. Therefore, this study proposed the means to investigate the relationship between consciousness of scapegoatism and accidents through report from KOSHA and conducts the survey with suggested means. This paper will contribute to prevent accidents by identifying relationship between consciousness of scapegoatism and accidents.

  • As human factors are the most important cause of construction accidents, it is important to reduce human error in construction work to reduce accidents. However, the error forcing context in organizational situations acts as a factor behind human error. Therefore, fatal construction accidents were analyzed using the m-SHEL model, which can identify the factors behind human errors. Through such analysis, it was found that there are differences in the detailed factors behind human errors according to the type of fatal accidents in construction, This study is meaningful in that it confirmed through accident cases that it is important to understand and respond to organizational situations in order to reduce human error in construction work.

ESTABLISHMENT OF SAFETY GUIDELINES FOR ELDERLY CONSTRUCTION WORKERS

  • With the average life expectancy increasing thanks to better standards of living and medical technology, the number of elderly construction workers in construction sites rises every year to surpass the 450 thousand people in the construction industry (24.7% of all employees in the construction industry). Similarly, the percentage of fatal accidents involving elderly workers has stood at 41~46 percent for the past five years; note that this is significantly higher than the other age groups, making safety measures for dealing with this issue a matter of urgency. This study sought to propose appropriate safety guidelines for elderly construction workers aged 50 years and over by examining the changes in their physical and psychological functions and through the subsequent analysis of the current status and causes of fatal accidents involving them. The guidelines targeted ferroconcrete construction where accidents occur quite often; construction was classified into mold construction, reinforcing rod construction, and concrete construction. Mold construction was further classified into preparation, carry-in, processing, assembly, and disassembly, and reinforcing rod construction, into preparation/transport and processing/assembly. Safety guidelines for each process were presented by dividing them into three aspects considering the changes in the physical and psychological functions according to their ages and type of accident causes: work environment improvement, machinery and equipment improvement, and work method improvement.

Fatal pneumonia caused by extraintestinal pathogenic Escherichia coli in a young dog (강아지에서 장외 대장균 감염에 의한 치명적 폐렴 사례)

  • This paper describes a fatal case of pneumonia in a 14-day-old dog caused by extraintestinal pathogenic Escherichia coli (ExPEC). The necropsy showed that almost all of left lobes of the lungs had dark-red consolidation. A histopathology examination revealed moderate acute fibrino-hemorrhagic necrotizing pneumonia with intralesional bacterial colonies. Non-suppurative epicarditis, congestion in the liver, and necrosis in the white pulp of the spleen also were found. E. coli with cytotoxic necrotizing factor 1 and α-hemolysin was isolated from the lung. This case was confirmed to have fatal pneumonia caused by ExPEC that led to a systemic infection.

Effectiveness of Fatal Fall Accident Prevention through Design for Safety in Construction Industry (건설공사의 추락재해예방을 위한 설계안전기법의 효과성 분석)

  • Construction industry is considered as one of the most high-risk industries for work-related injuries and fatalities, accounting for more than half of fatalities in Korea. Advanced countries have recognized the critical role of designers in preventing construction accidents and have established regulations on design for safety. In line with this, the Korean government have also implemented regulations that require owners and designers to review the safety of design outcomes. However, it has been observed that designers face challenges in identifying hazards and integrating design solutions at the design stage mainly due to their shortage of required knowledge and skills. This study aimed to examine design solutions that can be applied to prevent fall accidents in the construction industry, and to establish a relationship between these solutions and fatal fall accidents occurred over the past three years in Korea. This study also analyzed the relationships of four variables (construction type, cost, work type, and fall location) with design solutions. The results indicated that all four variables have significant relationships with design solutions, with fall location showing the strongest relationship. The design solutions and their relationships with fatal fall accidents identified in this study can be utilized in identifying hazard and integrating design solutions to ensure design for safety.
