Two Cases of Plantar Fasciitis Treated with Magnetic Acupuncture

  • Objectives : The aim of the study was to ascertain the effectiveness of magnetic acupuncture in patients with plantar fasciitis. Methods : The patients were treated with magnetic acupuncture using a magnetic therapy stimulator (Whata 153), as well as with moxibustion therapy. Acupuncture treatment was conducted on the acupoints of Ashi-as well as on 太谿 (KI3), 崑崙 (BL60), and 委中 (BL40)-for 15 minutes. The intensity of pain was evaluated using the visual analogue scale (VAS), and the tenderness threshold (TT) was measured at each visit using an algometer. Results : After treatment, evaluation using the VAS had improved, as had the TT; furthermore, plantar fasciitis pain was reduced. Conclusion : Magnetic acupuncture relieved pain in plantar fasciitis. In further clinical study, more research on magnetic acupuncture is required.

두경부 영역에 발생한 괴사성 근막염: 증례보고 (NECROTIZING FASCIITIS OF HEAD AND NECK AREA : 4 CASES REPORTS)

  • Necrotizing fasciitis has been recognized as a potentially lethal and rapidly progressing infection. Necrotizing fasciitis of head and neck area is rare but fatal disease that should be prompt diagnosis and recognition. If not promptly recognized and treated, infection can spread into the deep spaces of the neck and compromise the airway. It may also spread into the mediastimum producing life threatening sepsis. In this report, we describe the treatment of 4 cases of necrotizing fasciitis of head and neck area and discuss diagnosis, treatment, complication and consideration with review of literatures.

경부 괴사성 근막염의 임상적 고찰 (Clinical Analysis of Cervical Necrotizing Fasciitis)

  • Background and Objectives Cervical necrotizing fasciitis is a fulminant disease associated with necrosis of connective tissue, spread along the fascial plane, and high mortality. We analyzed the clinical characteristics and treatment outcome of this rare fatal disease. Materials and Methods We retrospectively reviewed the medical records of 19 patients treated for cervical necrotizing fasciitis from January 1999 to January 2009. Mean age was 53.7 years. Results The most common predisposing illness was tonsillitis (36.8%), followed by odontogenic infection (15.7%). Diabetes mellitus was most common underlying disease. Liver cirrhosis and chronic renal failure were found in 2 patients each. All patients were treated with combination of parenteral antibiotics and wide surgical debridement by transcervical and/or thoracotomy approach. Multiple surgical debridements were performed in 7 patients. Tracheotomy was performed in most of the patients (88.8%). Period of total hospitalization and Intensive care unit was 23 days and 10.1 days. Two patients died of disease and overall survival rate was 89.4%. Conclusion Early surgical management and care in intensive care unit are essential for cervical necrotizing fasciitis. Possible complications such as respiratory failure, mediastinitis or sepsis should be carefully evaluated.

Nodular Fasciitis of the Periorbital Area

  • Nodular fasciitis is a reactive, non-neoplastic lesion that is most commonly found in the subcutaneous or superficial fascia of the extremities and trunk. Head and neck lesions are relatively uncommon and reports vary from 7% to 15% depending on the authors. Nodular fasciitis grows quickly, and shows a pleomorphic spindle cell pattern with increased mitotic activity. Such factors lead to cases where the lesion is mistaken for a malignancy such as fibrosarcoma and the case may end up with unnecessarily aggressive treatments. The intent of this paper is to report a relatively rare case of nodular fasciitis occurring in the periorbital area and also to highlight the importance of accurate diagnosis and non-aggressive management of this benign lesion.

  • Objectives : Plantar fasciitis is most common cause of heel pain which starts from anterior tubercle of calcaneus. It is chronic inflamation of plantar fascia, reduces collagen and water content of heel pad which incur the degenerative changes with elastic fiber weakness. We have evaluated the effect of myofacial releasing therapy and acupuncture therapy by experimenting two patients suffering from plantar fasciitis. Methods : Two patients were diagnosed as plantar fasciitis through their symptoms. We used myofacial releasing therapy and acupuncture therapy to the patients and measured VAS(Visual analogue scale). Results and Conclusions : After treating myofacial releasing therapy and acupuncture therapy in two cases, We figured out that the patients were on the mend. These results suggest that myofacial releasing therapy and acupuncture therapy were effective to heel pain.

턱벼각 절골술 후 발생한 괴사근막염의 치험례 (A Case of Necrotizing Fasciitis of Lower Leg Following Mandible Angle Ostectomy)

  • Mandible angle ostectomy is the most widely-operated facial contour surgery. We experienced a rare case of necrotizing fasciitis of the lower leg following mandible angle ostectomy. A 20 years old female visited our department 5 days after mandible angle ostectomy. At the time, she was diagnosed as sepsis and on her 2nd admission day, she was diagnosed as necrotizing fasciitis on her left leg. She was treated by fasciotomy and necrotized tissue removal. Necrotizing fasciitis is a rare complication of any kinds of operation but once it develops, it causes fatal results, especially to cosmetic surgeries. Early diagnosis and aggressive operation is known as only treatment for the disease.

족저근막염의 초음파유도하 봉약침 치험1례 (A Case Report of Ultrasound-guided Bee Venom Pharmacopuncture on Plantar Fasciitis)

  • Objectives: This study is to report the usefulness of ultrasound in diagnosing plantar fasciitis and the effectiveness of ultrasound-guided bee venom pharmacopuncture as a treatment of it. Methods: A 61-year-old woman suffered from plantar fasciitis on her right foot for 1 month. The sonographic findings were fascial thickening, blurring of perifascial border and perifascial effusion. The ultrasound-guided bee venom pharmacopuncture for 6 times and conventional Korean medicine therapies like acupuncture and moxibustion for 12 times in 5 weeks. Numeric rating scale (NRS) and foot function index(FFI) was used to evaluate the pain and its progress. and thickness of plantar fascia was measured by sonography every week. Results: After treatments, NRS and FFI were reduced from 10 to 1 and from 190 to 72. The thickness of fascia was reduced from 0.43cm to 0.40. Blurring and effusion of perifascial border were also improved. Conclusion: This report suggests that the Ultrasound-guided Bee Venom Pharmacopuncture is effective for Plantar Fasciitis. Ultrasonography could be one of the most valuable items in the clinical practice of Korean medicine doctors who seek minimally invasive treatment.

Pseudoaneurysm Leading to Necrotizing Fasciitis at the Femoral Arterial Puncture Site

  • In this article, we present two cases of femoral pseudoaneurysm (PA) at the femoral arterial puncture site followed by necrotizing fasciitis, which is rare but can be fatal when not managed appropriately. PA was revealed by lower-extremity angiography and color-flow Doppler ultrasonography. Hematoma removal, thrombolysis, and bleeder ligation with Gelfoam were repeatedly performed by a vascular surgeon. When necrotizing fasciitis developed, aggressive surgical drainage and creation of a viable wound bed for reconstruction were mandatory. We adopted a vacuum-assisted closure device (Kinetics Concepts International) as the standard treatment for complicated, serious, infected PA of the puncture site. Excellent clinical outcomes were obtained.

  • 소아에서 매우 드문 질환으로 알려진 호산구성 근막염은 근막의 염증과 말초혈액에서 호산구 증가가 특징인 질환이며 면역학적 이상이 그 원인으로 지목되고 있다. EBV는 숙주의 면역기전에 변화를 일으켜 여러 가지 질환을 유발하는 것으로 알려져 있어 호산구성 근막염을 일으킬 수 있는 선행인자가 될 수 있음을 추정할 수 있다. 이에 저자들은 22개월 남아에서 EBV 감염과 연관하여 발생한 호산구성 근막염 1례를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

비부에 발생한 결절성 근막염 치험 1례 (Nodular Fasciitis of the Nose: A Case Report)

  • Nodular fasciitis is an unusual benign tumor which is composed of myofibroblast. Typical histologic findings include haphazardly arranged pleomorphic arranged spindle cells in a myxoid stroma. These lesions may easily be misinterpreted as malignancy clinically and histologically because it presents as a rapidly growing mass from subcutaneous or deep fascia. These lesions are usually located over upper extremity and rarely in the head and neck region. It's important to emphasize the need of clinical suspicion and accurate histopathology of this rare benign lesion usually misdiagnosed as a malignacy to avoid unnecessary and inappropriate aggressive wide resection, when encountering subcutaneous nodules of the face and neck region. In this article, we report a rare case of Nodular fasciitis on the Nose and some reviews of the literature.
