Workers at construction sites are easily exposed to many dangers and accidents involving falls, tripping, and missteps on stairs. However, researches on construction site monitoring system to prevent work-related injuries are still insufficient. The purpose of this study was to develop a wearable textile pressure insole sensor and examine its effectiveness in managing the real-time safety of construction workers. The sensor was designed based on the principles of parallel capacitance measurement using conductive textile and the monitoring system was developed by C# language. Three separate experiments were carried out for performance evaluation of the proposed sensor: (1) varying the distance between two capacitance plates to examine changes in capacitance charges, (2) repeatedly applying 1 N of pressure for 5,000 times to evaluate consistency, and (3) gradually increasing force by 1 N (from 1 N to 46 N) to test the linearity of the sensor value. Five subjects participated in our pilot test, which examined whether ascending and descending the stairs can be distinguished by our sensor and by weka assessment tool using k-NN algorithm. The 10-fold cross-validation method was used for analysis and the results of accuracy in identifying stair ascending and descending were 87.2% and 90.9%, respectively. By applying our sensor, the type of activity, weight-shifting patterns for balance control, and plantar pressure distribution for postural changes of the construction workers can be detected. The results of this study can be the basis for future sensor-based monitoring device development studies and fall prediction researches for construction workers.
Walking is not only an essential component of the human mobility, but also is a good exercise. Inability to walk freely can reduce an individual's quality of life and independence substantially. Being a relatively low impact activity, walking is particularly good for the elderly and research has shown that regular walking in the elderly reduces the chance of fall-related injuries and mental diseases as well. In spite of the documented benefits of regular walking, it is still difficult to walk without the aid of assistive devices for the frail elderly who have lower extremity problems. Assistive walking devices(AWD), such as crutches, canes, hiking-poles, T-Poles and walkers, are often prescribed to the elderly to make their walking be safe and efficient. Many researchers have demonstrated the effects of AWDs such as reducing lower extremity loading, improved dynamic/gait stability, yet, no study has been done for gait pattern when the elderly gait with AWDs. Therefore, the purpose of this study was to examine whether T-Poles, one of the AWDs, change the elderly gait pattern. Eight community-dwelling female elderly participated in this study. Laboratory kinematics during walking with T-Poles(PW) and with out T-Poles(NPW) was assessed. PW showed significant increase in step width, stride length, gait velocity and decrease in swing time. No significances were found in lower body joint angles but meaningful trend and pattern were found. Maybe the reason was due to the participants. Our participants were healthy enough so that the effect of T-Poles was minimum. PW also showed typical gait phases which are no single support phase during a gait cycle. It indicates that walking with T-Poles may guarantee safe and confident walking to the frail elderly.
We reviewed the charts and photos taken during arthroscpy of 218 knees of 214 patients(240 menisci) retrospectively. The male was 156 cases(73%) and the female 58 cases(27%). The mean age of the patients was 35 years(range, 7-68). The patients who had definite trauma history were classified as trauma group(Group 1), and the patients who had no or could not recall trauma history were as atrauma group(Group 2). The trauma group was subclassified into the the patients with sports injury, traffic accident, fall down, slip down, direct injury, and miscellaneous according to the causes of the trauma. The patterns of meniscal tear were classified into longitudinal, bucket-handle, horizontal, transverse, flap, complex, and degenerative tear on the basis of O'Connor's classification. The aim of this study was to compare the meniscal tear patterns between trauma group(Group 1) and atrauma group(Group 2) and between the patients before and after the age of 40. The results were as follows ; 1) The difference in the incidence of tear between medial and lateral meniscus was not significant statistically. 2) In Group 1, 60% of the cases showed the longitudinal and bucket-handle tear and 52% of the cases of Group 2 were horizontal tear. 3) In the patients before the age of 40, the longitudinal and bucket-handle tear were 52% of the cases and in the patients over 40, tear patterns which were thought to be related to degenerative change, horizontal and degenerative tear were more than half of the cases (51%).
본 연구의 목적은 낙상 예방을 위한 12주간의 수중 운동 수행 후 장애물 보행의 특성을 운동학 및 운동역학적으로 분석하는 것이다. 여성 노인 8명이 참여하였으며, 대상자들은 수중 운동 전 후에 네 높이의 장애물(0, 2.5, 5.1, & 15.2cm)을 자기선호 속도로 넘었다. 수중 운동 수행 후 고관절의 최대각, 최소각, ROM(Range Of Motion)이 유의하게 증가하였으며, Swing 과 Stance 국면에서 소요시간은 줄어들었다. 수중 운동 후 모든 높이에서 보폭은 유의하게 증가하였고, 보간은 줄어들었다. 수중 운동 후 장애물을 넘는 순간 장애물과 오른발 사이의 수직 최단거리는 증가하였고(15.2cm 장애물 제외), 장애물을 넘는 속도는 증가하였다. 수중 운동 수행 후 제동력, 추진력, 제동 운동량, 추진 운동량은 통계적으로 유의하게 변화하였다. 12주간의 수중 운동은 여성 노인의 근력과 평형성을 향상시켰으며 이는 낙상과 관련된 장애물 보행의 운동학 및 운동역학적 변인의 변화를 가져와 여성 노인들이 장애물을 안전하고 신속하게 넘을 수 있었다. 따라서 노인에게 보행 능력 향상과 낙상 예방 운동으로 수중 운동이 추천된다.
배경: 흉부 둔상은 전체 흉부 손상의 90%정도를 차지하며 외상과 관련된 사망률의 20%를 유발시킨다. 흉분 둔상에 의한 손상 후 이환율과 사망률의 주요 원인은 발견되지 못한 손상이 남아있기 때문이다. 그리하여 흉부전산화단층 촬영은 외상환자의 진단적 검사에서 매우 자주 사용되어져 왔다. 그러나 흉부 컴퓨터 촬영은 가격이 비싸고, 방사선 노출을 증가시킬 수 있다. 이번 연구를 통하여 흉부둔상 환자에서 흉부단순촬영과 비교하여 흉부전산화단층촬영을 통하여 얼마나 더 많은 정보를 얻을 수 있는지 조사해 보고 그리고 흉부 컴퓨터 촬영의 진단과 치료에 있어서의 역할을 알아보고자 한다. 대상 및 방법: 2006년 11월부터 2007년 7월까지 응급실로 내원한 환자 100명을 대상으로 하였다. 이 중 74명의 자동차 관련사고 환자와 26명의 낙상 사고 환자가 있었으며 흉부엑스선과 흉부전산화단층 촬영을 동시에 시행한 환자를 전체 응급실 환자 중에 선택하였다. 자료는 차트를 통하여 혈역학적 소견, 중재적 치료 여부, 손상의 중증도(RTS)와 종류를 조사하였으며, 초기 응급실 내원환자 중 흉부단순촬영과 흉부전산화단층촬영을 시행한 환자를 대상으로 발견되지 못한 병적 소견을 각각 질환별로 분석하였다. 결과: 100예의 환자 중 흉부엑스선 검사상 하나 이상의 병적소견을 보인 환자가 79예였으며 21예의 환자에서는 흉부엑스선 검사상 정상 소견을 보였으며, 이 21예의 환자 중 17예에서 흉부전산화단층촬영상 이상소견이 발견되었다. 흉부엑스선 검사상 발견하지 못한 소견으로는 기흉, 혈흉, 폐좌상, 흉골 골절 등이 있었으며 이러한 병적 소견의 진단은 흉부전산화단층촬영이 흉부엑스선 검사보다 통계적으로 유의하게 우수한 것으로 나타났다. 하지만 치료에 있어서는 흉부전산화촬영 시행 후 발견된 병변으로 흉관삽입술등의 치료를 시행한 환자는 31명에 불과했고 흉관삽입술, 개흉술 등의 흉부외과적인 치료 없이 집중관찰을 위하여 입원한 환자가 42명이었으며, 흉부엑스선촬영과 환자의 이학적 소견으로 진단되어 치료를 시행한 환자가 27명이었다. 결론: 흉부전산화단층촬영은 진단에 있어서 흉부엑스선 촬영보다 통계적으로 의미 있게 진단에 도움이 되는 것을 알 수 있었으나 발견되지 못한 병변이 증가할지라도 치료의 변화와 방법의 변화가 있는 경우는 매우 소수에 불과했다. 그리하여 응급실에서의 흉부전산화단층촬영의 오남용을 막기 위하여 선별적인 흉부전산화단층 촬영을 고려해야 할 것이다.
