혈액투석 환자의 약물복용 이행도 및 이행 영향요인: 1차, 2차 의료기관 중심 (Factors Influencing Medication Adherence in Hemodialysis Patients: Focusing on Primary and Secondary Medical Care)

  • Purpose: The purpose of this study was to identify the factors influencing medication adherence in hemodialysis patients among primary medical care and secondary medical care. Methods: A cross-sectional survey design was utilized. Data were collected using questionnaires from 280 hemodialysis patients who had taken prescribed medication regularly as a result of chronic renal failure at primary and secondary medical care in Korea. Data were analyzed using t-test, ANOVA, Pearson correlation coefficients, and multiple regression. Results: There were statistically significant differences in medication adherence according to living area (p=.002), health condition (p<.001), amount of medication (p=.004), inconvenience for taking medication (p<.001), and depression level (p=.001). The mean of medication adherence was 3.72 points. Medication adherence was explained by perceived barrier related to medication taking (${\beta}=.338$), attitude (${\beta}=.250$), and depression (${\beta}=.132$). Conclusion: This study strongly recommended that nursing intervention program to improve medication adherence should be developed and a match control study in improvement of medication adherence for hemodialysis patients needs to be done.

수치해석에 있어 단단한 점성토 노반에 대한 회복탄성계수의 적용 (Implementation of the Resilient Modulus for the Stiff Cohesive Subgrade Soils on a Numerical Analysis)

  • 반복하중을 받는 도로 노반체의 경우 회복탄성계수를 이용한 해석 및 설계 방법이 이루어지고 있는데 유사한 형태의 하중조건인 철도 노반체의 경우에도 그 거동은 크게 다르지 않을 것으로 판단된다. 이러한 현상적인 특성을 감안하여 본 논문에서는 회복탄성계수를 기반으로 하는 구성방정식을 상용 유한요소 해석 프로그램에 적용하였다. 일반적으로 K-${\theta}$ 혹은 Uzan 모델을 기반으로 수치해석 프로그램과의 접목이 이루어져 왔다. 이러한 모델의 기본 가정은 반복하중으로 인하여 재료의 상태는 비선형탄성 상태까지 도달한 조건으로 보고 있으므로 추가적인 반복하중으로 인한 재료의 거동은 탄성구간 내에서만 발생하는 것으로 가정하고 있다. 그러나 점성토에 대한 회복탄성실험 결과를 보면 하중의 재하 횟수 증가에 따라 영구변형이 발생하는 상황이므로 기존의 모델을 사용한다면 발생하는 영구변형을 충분히 모델링 할 수 없게 된다. 따라서 본 논문에서는 실험결과로부터 도출된 소성특성과 간단한 파괴기준을 적용하여 영구변형이 발생하는 조건에서 구성모델을 개발하였으며 개발된 모델의 적용결과는 실험 결과값과 잘 일치하는 것으로 확인되었다.

서비스 품질 속성 IPA 분석을 활용한 테마 레스토랑 입지별 LSM 전략수립 (Development of LSM(Local Store Marketing) Strategy Using the IPA Analysis of Service Quality Attributes by the location in a Theme Restaurant)

  • The purposes of this study were to a) examine using status, b) measure the service quality attributes, c) analyze IPA and customer satisfaction, d) develop the LSM(Local Store Marketing) by location of K theme restaurant such as residential type(A), complex type(B) and downtown type(C). The questionnaires were handed out to 346 of 450(usage rate 76.9%). The statistical data analysis was completed using SPSS(ver 12.0) for descriptive analysis, $X^2$, t-test, ANOVA, reliability analysis, factor analysis, multiple linear regression. From an analysis on service quality attributes, most importance of service quality attributes was more highly perceived than performance of those, except for 'attractive exterior', 'variety of kid's menu', 'convenient facilities for kid' and perceived importance and performance were also significantly different by location of branch restaurant. The attributes to be improved were 'convenience of restaurant's parking lot', 'food tast', 'speedy service' and 'service competency' as branch A(residential type) restaurant, 'convenience of the restaurant location' as branch B(complex type) restaurant and 'convenience of the restaurant location' and 'speedy service' as branch C(downtown type) through IPA analysis. The Overall customer satisfaction was 3.74 and service quality factors had significant effect on overall customer satisfaction. Location of restaurant, which gives a great effect on its operation and marketing, has a important correlation with success or failure of restaurant business enterprises. Therefore, marketing strategy by restaurant location should be developed.

초기(初期) 갈색부후(褐色腐朽)에 따른 라디에타소나무의 역학적(力學的) 성질(性質) 감소(減少) (Reduction in Mechanical Properties of Radiata Pine Wood Associated with Incipient Brown-Rot Decay)

  • This study was performed to evaluate the reduction in bending properties of radiata pine sapwood associated with incipient brown-rot decay. Decayed bending specimens by Tyromyces palustris and Gloeophyllum trabeum for varoious periods were tested destructively. Brown-rot decay by T. palustris and G. trabeum caused serious reduction in bending properties at very early stages of decay, with about 30 percent decrease in bending strength observed for only 1~2 percent weight loss. In general, the reduction in bending properties caused by T. palustris was somewhat greater than that by G. trabeum. Work to maximum load was reduced most severely and rapidly from the onset of decay, while modulus of elasticity showed a much more moderate rate of reduction. Modulus of rupture was affected by decay to a greater extent than was modulus of elasticity. Since a relatively strong correlation between weight loss and bending strength was observed, the residual strength of decayed wood can be predicted by weight loss due to decay. The results of this study indicate that very early stages of brown-rot decay reduce the bending strength significantly. Thus, it is recommended that all load-bearing members in wooden structures, especially those that are periodically wetted, should be inspected regularly to prevent a sudden failure even though there are no definite signs of decay.

배양 심근세포에서 저농도 삼산화비소에 의한 산화적 스트레스 발생 (Oxidative Stress by Arsenic Trioxide in Cultured Rat Cardiomyocytes, $H_9C_2$ Cells)

  • Epidemiologic studies have showed a close correlation between arsenic exposure and heart disease such as, cardiovascular problem, ischemic heart disease, infarction, atherosclerosis and hypertension in human. It may increase the mortality of high risk group with heart disease. Regarding the mechanism studies of heart failure, blood vessel, vascular smooth muscle cells and endothelial cells have long been focused as the primary targets in arsenic exposure but there are only a few studies on the cardiomyocytes. In this study, the generation of oxidative stress by low dose of arsenic trioxide was investigated in rat cardiomyocytes. By direct measurement of reactive oxygen species and fluorescent microscopic observation using fluorescent dye 2',7'-dichlorofluorescin diacetate, reactive oxygen species were found to be generated without cell death, where cells are treated with 0.1 ppm arsenic for 24 hours. With the induction of reactive oxygen species, GSH level was decreased by the same treatment. However, DNA damage did not seem to be serious by DAPI staining, while high dose of arsenic (2 ppm for 24 hrs) caused fragmentation of DNA. To identify the molecular biomarkers of low-dose arsenic exposure, gene expression was also investigated with whole genome microarray. As results, 9,022 genes were up-regulated including heme oxygenase-l and glutathione S-transrerase, which are well-known biomarkers of oxidative stress. 9,404 genes were down-regulated including endothelial type gp 91-phox gene by the treatment of 0.1 ppm arsenic for 24 hours. This means that biological responses of cardiomyocytes may be altered by ROS induced by low level arsenic without cell death, and this alteration may be detected clearly by molecular biomarkers such as heme oxygenase-1.

  • 미국, 유럽 그리고 일본에서는 90년대 말에 큰 지진을 경험하면서부터 지진에 대한 성능 설계 개념이 설계 기준에 포함되기 시작하였다. 일반적으로 스트럿-타이 모델은 D-영역에서 비교적 강도를 잘 예측하지만 파괴 때 연성정보를 제공하진 않는다. 그러므로 RC 구조물의 강도와 연성을 예측할 수 있는 간단한 해석 도구가 필요하다. 이 연구에서 RC 구조물의 지진에 대한 거동을 해석할 수 있는 도구인 2차원 래티스 모델을 제안한다. 2차원 래티스 모델은 실험을 통한 상관관계 연구에서 RC 구조물의 강도뿐만 아니라 연성도 예측하는 것으로 나타났다.

Partial Confinement Utilization for Rectangular Concrete Columns Subjected to Biaxial Bending and Axial Compression

  • The prediction of the actual ultimate capacity of confined concrete columns requires partial confinement utilization under eccentric loading. This is attributed to the reduction in compression zone compared to columns under pure axial compression. Modern codes and standards are introducing the need to perform extreme event analysis under static loads. There has been a number of studies that focused on the analysis and testing of concentric columns. On the other hand, the augmentation of compressive strength due to partial confinement has not been treated before. The higher eccentricity causes smaller confined concrete region in compression yielding smaller increase in strength of concrete. Accordingly, the ultimate eccentric confined strength is gradually reduced from the fully confined value $f_{cc}$ (at zero eccentricity) to the unconfined value $f^{\prime}_c$ (at infinite eccentricity) as a function of the ratio of compression area to total area of each eccentricity. This approach is used to implement an adaptive Mander model for analyzing eccentrically loaded columns. Generalization of the 3D moment of area approach is implemented based on proportional loading, fiber model and the secant stiffness approach, in an incremental-iterative numerical procedure to achieve the equilibrium path of $P-{\varepsilon}$ and $M-{\varphi}$ response up to failure. This numerical analysis is adapted to assess the confining effect in rectangular columns confined with conventional lateral steel. This analysis is validated against experimental data found in the literature showing good correlation to the partial confinement model while rendering the full confinement treatment unsafe.

  • Purpose: The purpose of this study was to analyze research in the Korean Journal of Rehabilitation Nursing and to suggest further perspective for rehabilitation nursing research. Method: In this study we reviewed the contents of 134 articles published in this journal from its beginning year 1998 to 2004. Results: The majority of subjects were adult patients. The type of disease were Cerebrovascular Accident, Neuromuscular disease, Cancer patients, Urogenital disease such as Chronic Renal Failure, Spinal cord injury. Domain of research almost was client, type of research were survey(34.3%), quasi experimental study(20.9%), correlation study(19.4%). Nursing intervention of experimental study was exercise(39.0%), education(17.1%). Place of data collection was hospital (58.2%), position of researcher was professor(67.2%), method of data collection was questionnaire(50.7%). Conclusion: There has been a lack of written consent, child rehabilitation, measurement tool development, qualitative research. More studies about rehabilitation nursing are needed to extend the role and fields for professional nursing.

암반터널 예비설계를 위한 인공신경회로망 전문가 시스템의 개발 (Development of an Artificial Neural Network Expert System for Preliminary Design of Tunnel in Rock Masses)

  • 인공신경회로망을 이용하여 터널굴착설계를 위한 전문가 시스템 NESTED를 개발하였다. 이를 위하여 지하 암반의 안정성을 평가할 수 있는 신경회로망 모델과 대표적인 암반분류법인 RMR과 Q 시스템 사이의 상관관계를 결정할 수 있는 신경회로망 모델을 사용하였다. 또한 사용된 모델과 전산화된 암반분류법 프로그램이 동일한 사용자 환경을 통해 운용될 수 있도록 통합 시스템을 구성하였다. NESTED에 사용된 신경회로망의 구조는 역전파 학습 알고리즘을 채용한 다층 역전파 신경 회로망이다. 전문가 시스템에 필요한 지식기반을 구축하기 위해 이전의 현장 시공사례로 학습과정을 수행함으로써 불완전하거나 오류가 포함된 정보를 처리할 수 있는 공학 데이터베이스를 개발하였다. 일련의 실험을 통해 전문가 시스템을 현장사례에 적용해보고 여기서 출력된 결과를 문헌에 보고된 자료와 비교하였다. 이 결과 암반의 파괴거동을 추정하고 이에 따른 보강시기의 변화를 정확히 예측하는 신경회로망의 추론능력을 확인할 수 있었다. 이처럼 본 연구를 통해 개발된 신경회로망 전문가 시스템을 암반터널에 적용할 경우 부족한 지질자료에 대해 합리적인 기준을 제공하고 터널의 예비설계에 필요한 보강설계를 제시할 수 있었다.

강우로 인한 철도 연변 사면의 활동특성 : 현장사례 중심 (Characteristics of Rainfell Driven Landslides Near the Railway : Field Study)

  • 강우로 인한 철도사면의 활동은 사면의 안정성에 영향을 끼치며 결국은 철도의 운행에 큰 영향을 미친다. 비록 소량의 토체의 활동이 발생하여 선로상으로 이동되더라도 탈선을 일으킬 가능성이 있으므로 철도운영에 있어 사면의 안정은 중요한 분야중의 하나이다. 따라서, 본 연구에서는 철도연변 사면에서 관측된 강우로 인한 사면의 거동특성을 분석하였다. 전체 23개소에 대한 조사가 이루어 졌으며, 관측결과 강우로 인한 사면의 붕괴 패턴은 크게 4가지로 구분될 수 있었다. 이러한 분류는 붕괴된 사면의 형태를 기준으로 이루어 졌다. 본 연구결과에 따르면 사면의 제원(사면의 높이와 길이)은 관측된 활동 패턴과 직접적인 상관성을 보이지는 않았지만, 사면의 형태 및 지질학적 특성은 각기 다른 활동타입의 발생에 영향을 미치는 것으로 나타났다.
