안면마비 환자에 대한 Sunnybrook Facial Grading System의 적용 실태 분석 : 후향적 관찰연구 (The Application State of the Sunnybrook Facial Grading System for Facial Palsy Patients : A retrospective study)

  • Objectives : Among the assessment tools for evaluating facial function, the House-Brackmann scale is used as a standard tool, but it has some shortcomings. The Sunnybrook Facial Grading System can assess the after effects of facial palsy and facial movement by each part of the face. By understanding the application state of this Sunnybrook Facial Grading System, we intend to analyze the relationship between House-Brackmann scale score and Sunnybrook Facial Grading System score so that we can examine the advantages of the Sunnybrook Facial Grading System as a more accurate tool. Methods : We screened both inpatients and outpatients who visited the Facial Palsy Center at Kyung Hee University Hospital for Korean medical treatment and were evaluated with the Sunnybrook Facial Grading System from December 2015 to October 2016. A total of 159 out of 166 patients were studied, including basic characteristics and missing data. We used descriptive statistics for general features of patients and SPSS Ver.18 for statistical analysis. Results : House-Brackmann scale and Sunnybrook Facial Grading System have high negative correlation through Pearson Correlation Coefficient with a score of -0.884. Analyzing outlier data resulting from relation analysis between the House-Brackmann scale and the Sunnybrook Facial Grading System showed many outliers when the damaged state of each part of the face is different. Conclusion : Sunnybrook Facial Grading System can make up for faults of the House-Brackmann scale, which is inferior in accuracy when each damage status of each part of the face is different. Sunnybrook Facial Grading System performs a detailed assessment of facial function and sequelae of facial palsy easier than the House-Brackmann scale.

부분 광배근 피판을 이용한 안면마비의 단단계 재건 (One Stage Reconstruction of Facial Palsy Using Segmental Latissimus Dorsi Muscle Free Flap)

  • The goal in facial paralysis treatment is to achieve the normal appearance of the face as well as to reconstruct the natural symmetrical smile. In cases of facial paralysis, a widely accepted procedure is the two stage method, which combines neurovascular free muscle transfer with cross face nerve grafting. Although the results are promising, the two operations of this method, which are about 1 year apart, impose an economic burden on the patients and require a lengthy period before results are obtained. In order to overcome these drawbacks, one stage method, using latissimus dorsi neurovascuular free muscle flap was introduced. Between January 2000 and January 2004, fifteen patients with long standing facial paralysis were treated in the Korea University Anam Hospital. The segmental latissimus dorsi with long nerve and pedicle was transferred to the paralyzed side of the face. The first postoperative movement of the transferred muscle was reported at 8.9 months, faster than that of the two stage method. During the next 24 months, a constant increase in the power of muscle contraction was observed. The fifteen cases were evaluated within an average of 31.7 months following the surgery and satisfactory results including muscle contraction were obtained in eleven of the cases but muscle contraction was not found in three cases.

  • Objectives : The purpose of this study was to report a case of a patient efficacy of electromagnetic acupuncture using Whata 153 in facial nerve palsy in Herpes zoster oticus. Methods : We treated the patient with magnetic acupuncture combined with electro-acupuncture. Acupuncture points were at the face (BL2, GB14, TE23, ST4, ST6, ST3). All the acupuncture points were stimulated with magnetic field and 4 of them were combined with electro-acupuncture. Results & Conclusions : The improvement of facial movement and symptom was evaluated by Yanagihara grading system(Y-system), House-Brackmann scale(HB scale) and Sunnybrook facial grading system(SFGS). After treatment, all of the scales(Y-system, HB scale and SFGS) and symptom of the patient were improved. From the above results, we suggest that magnetic acupuncture and electro-acupuncture might be effective on facial nerve palsy in Herpes zoster oticus.

해면상 혈관종을 동반한 안면신경마비 환자의 한방치험 1례 (A Case Report of a Facial Palsy Patient with a Cavernous Malformation)

  • Objective: To report the effect of Korean medicine treatment on a facial palsy patient with a cavernous malformation. Methods: The patient was treated with herbal medicine (Jodeung-san, 釣藤散) and acupuncture. The degree of treatment was measured by the Yanagihara-scale and by mobility of the face. Results: During treatment, the patient’s symptoms were relieved and facial movement was improved. Conclusion: This clinical case study showed the effect of herbal medicine and acupuncture on symptoms of facial palsy.

근재교육훈련이 급성안면신경마비 환자의 안면마비 회복정도와 기능에 미치는 효과 (The Effect of Muscle Reeducation Training on Recovery and Function in People with Acute Facial Nerve Paralysis)

  • Purpose: The purpose of study was to determine the effects of muscle reeducation training in patients with acute facial nerve paralysis. Methods: Thirty patients were randomly assigned to either the experimental group (n=15) or the control group (n=15). The experimental group received muscle reeducation training for 20 minutes after electrotherapy and the control group received conventional electrotherapy for only 40 minutes. Therapeutic intervention for each group was performed four times per week for four weeks. The patients were measured for recovery of paralysis using the House-Brackmann Grading System (H-B grade), the Movement Distance of Mouth, Nasolabial Angle (NA), and Facial Disability Index (FDI). Results: In within group comparison, the experimental group showed significant improvements for all variables (p<0.01). In comparison between two groups, the experimental group showed relatively greater significant improvements for all variables (p<0.01). Conclusion: These findings suggest that muscle reeducation training is more effective than conventional therapy in improving the condition of patients with facial nerve paralysis. In particular, the results of this study indicate that muscle reeducation training can be recommended by clinicians since it provides more benefits.

  • Objectives : The purpose of this study is to investigate the mechanism of Cosmetic Acupuncture through reinterpretation of anatomy and physiology. Methods : The causes of wrinkle increases and rapid aging of facial skin were studied and the theoretical system of Cosmetic Acupuncture treatment was analyzed through anatomy and physiology reinterpretation. Results and Conclusions : An increase in wrinkles and rapid aging of facial skin is caused by xerosis. Skin condition represents the condition of subcutaneous muscle. The reason why skin becomes easily dry is the heat produced by craniofacial part. Craniofacial part always generates lot of physiological fever because of the muscles. This physiological fever is produced from the muscles that are responsible for maintaining skull suture, controlling the movement of temporomandibular joint, maintaining head and neck posture. Controlling this fever is the crux of Cosmetic Acupuncture mechanism. These muscles correspond to Foot Taeyang meridian-muscle, Foot Soyang meridian-muscle and Foot Yangmyung meridian-muscle. Cosmetic Acupuncture is effective for preventing facial skin from aging and wrinkle increase by mechanical stimulus on facial muscles, and for controlling craniofacial part meridian-muscle system producing the heat.

  • Facial nerve schwannomas (FNSs) are usually painless, slow-growing, and without specific symptoms, so that early diagnosis may be difficult. They are particularly liable to being misdiagnosed as parotid gland origin benign tumor before surgery, which can lead to unnecessary parotidectomy or unexpected facial nerve injury. To prevent these complications, it is important that the correct diagnosis is performed at least in intraoperative time. When an adhesion between the mass and the facial nerve is exist or when electrical stimulation of the mass triggers facial movement, FNS is highly suggested diagnosis. In such cases, frozen section analysis should always be performed. In this case, the pre-operative diagnosis from clinical examination and MRI was pleomorphic adenoma. However, intraoperative features led us to suspect that the mass originated from facial nerves, and intraoperative frozen section analysis yielded results consistent with a schwannoma. Based on this intraoperative diagnosis, we carried out a successful conservative treatment with preservation of facial nerve.

  • 하드웨어의 급속한 기술발전은 3차원 영상정보를 포함한 다양한 디지털 영상정보의 급속한 발전과 확대를 가져왔다. 애니메이션, 가상현실, 영화, 광고, 게임 분야에서도 3차원 디지털 영상처리는 매우 다양하고 광범위하게 사용되고 있다. 이러한 3차원 디지털 영상 속에 등장하는 3D캐릭터는 의인화된 동작이나 음성, 표정등을 통해 관객에게 감동과 정보를 전달하는 중요한 역할을 담당한다. 3D캐릭터의 활용도와 사용범위가 확대되어감에 따라 효과적인 3D캐릭터의 동작연구나 표정연출에 대한 관심도 또한 높아지고 있다. 본 연구에서는 내적감정상태의 효과적 수단으로 사용되는 표정에 대해 연구해본다. 해부학적 구조에 기반한 얼굴근육의 운동방향과 변화에 따른 3D 캐릭터의 표정구현에 대해 살펴보고 효과적인 표현방법을 모색해보고자 한다. 또한 선행연구로 진행된 2D캐릭터 표정연구와의 비교를 통하여 그 차이점과 특징에 대해 살펴보고자 한다.

Orthodromic Transfer of the Temporalis Muscle in Incomplete Facial Nerve Palsy

  • Background Temporalis muscle transfer produces prompt surgical results with a one-stage operation in facial palsy patients. The orthodromic method is surgically simple, and the vector of muscle action is similar to the temporalis muscle action direction. This article describes transferring temporalis muscle insertion to reconstruct incomplete facial nerve palsy patients. Methods Between August 2009 and November 2011, 6 unilateral incomplete facial nerve palsy patients underwent surgery for orthodromic temporalis muscle transfer. A preauricular incision was performed to expose the mandibular coronoid process. Using a saw, the coronoid process was transected. Three strips of the fascia lata were anchored to the muscle of the nasolabial fold through subcutaneous tunneling. The tension of the strips was adjusted by observing the shape of the nasolabial fold. When optimal tension was achieved, the temporalis muscle was sutured to the strips. The surgical results were assessed by comparing pre- and postoperative photographs. Three independent observers evaluated the photographs. Results The symmetry of the mouth corner was improved in the resting state, and movement of the oral commissure was enhanced in facial animation after surgery. Conclusions The orthodromic transfer of temporalis muscle technique can produce prompt results by applying the natural temporalis muscle vector. This technique preserves residual facial nerve function in incomplete facial nerve palsy patients and produces satisfying cosmetic outcomes without malar muscle bulging, which often occurs in the turn-over technique.

감정확률을 이용한 동적 얼굴표정의 퍼지 모델링 (Dynamic Facial Expression of Fuzzy Modeling Using Probability of Emotion)

  • 본 논문은 거울 투영을 이용하여 2D의 감정인식 데이터베이스를 3D에 적용 가능하다는 것을 증명한다. 또한, 감정 확률을 이용하여 퍼지 모델링 기반의 얼굴표정을 생성하고, 표정을 움직이는 3가지 기본 움직임에 대한 퍼지이론을 적용하여 얼굴표현함수를 제안한다. 제안된 방법은 거울 투영을 통한 다중 이미지를 이용하여 2D에서 사용되는 감정인식에 대한 특징벡터를 3D에 적용한다. 이로 인해, 2D의 모델링 대상이 되는 실제 모델의 기본감정에 대한 비선형적인 얼굴표정을 퍼지를 기반으로 모델링한다. 그리고 얼굴표정을 표현하는데 기본 감정 5가지인 행복, 슬픔, 혐오, 화남, 놀람, 무서움으로 표현되며 기본 감정의 확률에 대해서 각 감정의 평균값을 사용하고 6가지 감정 확률을 이용하여 동적 얼굴표정을 생성한다. 제안된 방법을 3D 인간형 아바타에 적용하여 실제 모델의 표정 벡터와 비교 분석한다.
