The purpose of this study was to investigate if there were a significant difference between cephalometric measurements of mandibular position derived from a centric occlusion tracing compared to those of a converted centric relation tracing in the Class III malocclusion. The sample consisted of 25 Class III malocclusion and 25 normal occlusion persons who had no orthodontic treatment. The records included an lateral cephalometrics in centric occlusion, centric relation and centric occlusion bite registration and diagnostic casts mounted on the SAM II articulator in CR. The amount of CR-CO discrepancy of condyle was recorded using a MPI(Mandibular Position Indicator, MPI $200^{(R)}$, Great Lakes Orthodontics, USA). The conversion of the CO cephalogram to CR using the MPI readings was performed on the Conversion work sheet. Measures of mandibular position were chosen for the purpose of this study. The comparison of the difference between CO and CR cephalometric measurements in the normal occlusion and Class III malocclusion group were studied. The results were as follows: 1. In the features of CR-CO discrepancy of the condyle, the condyle was displaced posterior and inferior when the teeth were in centric occlusion. The horizontal component(${\Delta}X$) in Class HI malocclusion group was greater than the vertical component(${\Delta}Z$) and also greater than the horizontal component(${\Delta}X$) in normal occlusion group. There was no statistically significant correlation between MPI measurements and the groups of normal occlusion and Class III malocclusion group. 2. In the comparison of the cephalometric measurements in each group, Normal occlusion group showed significant difference in measurements such as ANB, Facial angle, Facial convexity and ODI. Class HI malocclusion group showed significant difference in measurements such as ANB, Facial angle, Facial convexity, ODI, SNB, APDI, L1-FP and it had more significance than the normal occlusion group. 3. The Value of cephalometric measurements was significantly different between CO and CR but there were no differences between the groups of normal occlusion and Class III malocclusion. The results of this study suggest that if the discrepancies are greater than the amount of normal displacement from clinically captured centric relation, centric relation should be considered as the starting point for proper diagnosis and treatment planning.
The purpose of this study to analyze characteristics of soft tissue profile in Korean young adults. The sample consisted of 50 young adults (25 males and B females) who had pleasing and normal occlusion. Soft tissue analysis (facial convexity angle, nasolabial angle, H-angle, Z-angle, E-line to upper lip, lower lip, Sn-pog' to upper lip, lower lip) was performed on lateral cephlograms. Mem and standard deviation was obtained. When compared by other studies, mean profile of this sample was relatively straighter and both the upper and lower lips was more protrusive.
The purpose of this study was to describe growth changes of soft tissue profile in koreans with normal occlusion and to get differences between male and female. The biennial serial cephalometric radiographs of 26 samples (15 males, 11 females) with normal occlusion from 8.5 years to 18.5 years of age were used in this study. The following results were obtained : 1. Both sexes had lower facial parts more protruded at 18.5 years of age than 8.5 years of age : in total facial convexity angle, male showed a decrease(P<0.01) and female showed no significant difference(P>0.05). 2. In the growth changes of soft tissue thickness, male outgrew female in the areas of nose and upper and lower lips, and female outgrew male in the pogonion area 3. In the growth changes of upper facial height and upper part of lower facial height, male's was higher than female's growth (P<0.05). 4. The ratio of the upper facial height over the lower facial height showed a decrease at 18.5 years of age than 8.5 years of age(P<0.01). The ratio of upper vs lower part of lower facial height and that of upper lip height vs lower lip height showed no significant difference(P>0.05). 5. Male showed a sustained change into older age compared with female in soft tissue profiles.

Purpose: Mid and lower facial convexity is more common in Oriental people than in Caucasian. Bimaxillary dentoalveolar protrusion is characterized by procumbent teeth, protruding lips, acute nasolabial angle, gummy smile, receding chin, facial convexity. Especially, pure maxillary dentoalveolar protrusion is less frequent than bimaxillary dentoalveolar protrusion. Therefore, it is important to make an accurate decision for the operation throughout the history taking, cephalogram, dental cast to arrive at accurate diagnosis and surgical plan. Methods: From December 2002 to June 2004, ten patients with maxillary dentoalveolar protrusion and microgenia were corrected by maxillary anterior segmental osteotomy and advancement genioplasty. 10 patients were analyzed by preoperative and postoperative clinical photography, posteroanterior and lateral cephalograms. Results: No major complications were occurred throughout the follow-up period except one of the over-recessed, otherwise most of the patients were satisfied with the result. Conclusion: We could correct the occulusal relationship with teeth and improve lower facial profile, asthetically and functionally, by maxillary anterior segmental osteotomy and advancement genioplasty.
This study was performed to compare the size of soft palate, tongue and airway according to the types of the malocclusion and evaluate the correlation between the size of soft palate, tongue, airway and dentofacial skeleton respectively. The sample of this study was 98 malocclusion female patients between the ages 12 and 17 years. The lateral cephalometric radiographs were taken and the distance, angle, ratio and area of the dentofacial skeleton, soft palate, tongue and airway were measured and evaluated statistically. The results obtained were as follows: 1. There was significant difference in SNB, ANB, facial angle, facial convexity, A-B plane angel, Y axis to FH, SN-MP, Wits appraisal, ODI and APDI according to the types of malocclusion. 2. The hyoid bone was more posteriorly positioned in Class II malocclusion group than other two groups and superio-inferior position of the hyoid bone was not different according to the malocclusion types. 3. The nasopharyngeal area of Class II and Class III malocclusion group was smaller than that of Class I malocclusion group, and the pharyngeal area of Class II malocclusion group was smaller than that of Class I and Class III maocclusion group. There was no difference of the area of the soft palate, tongue, oropharynx and hypopharynx according to malocclusion types. 4. The ramal height and mandibular body length(Go-Me) showed positive correlation with the area of tongue, nasopharynx, oropharynx, and pharynx. SNA did not correlated with the area of tongue and airway but SNB showed positive correlation with the area of hypopharynx and pharynx. The anterior, posterior facial height, upper and lower central incisor position to facial plane showed positive correlation with tongue area.
Objective: The aim of this study was to evaluate the volumes and areas of the upper airways in children with Class II malocclusion, using three dimensional cone-beam computed tomography (CBCT) and to compare the volumetric and cross-sectional measurements and cephalometric variables to investigate possible relationships between the upper airway and facial morphology. Methods: CBCT scans were obtained from 37 subjects (17 boys and 20 girls; average age, 11.02 years). The upper airway volumes and areas were measured, and compared with cephalometric variables. Results: The area of the PNS-posterior plane ($S_{PP}$) was significantly smaller in the Class II malocclusion group (p < 0.05). Also, the volumetric and cross-sectional measurements were lower in Class II than in Class I malocclusion groups, although the differences were not significant between the two groups (p > 0.05). The Class II malocclusion group showed significantly smaller values of PFH, mandibular body length, pog to N perp and showed larger values of FMA, ANB, and facial convexity than the Class I malocclusion group. The volume of the upper airway in front of PNS point (WN) showed negative correlation with ANB (p < 0.05). Conclusions: The Class II malocclusion group had a narrower upper airway associated with a decreased posterior facial height and a divergent growth pattern than the Class I malocclusion group.

This study was made on the facial profile of the normal Korean children using the roentgenographic cephalograms. The subjects consisted 51 males and 47 females children of primary dentition with the normal occlusion and acceptable profile. For this study 13 soft tissue profile landmarks were plotted and 14 liner length, 9 soft tissue thickness, 8 vertical height length, 12 angles of soft tissue profile, and 2 vertical proportion were measured. The mean and standard deviations in the subjects were calculated and compared between male and female. The following results were obtained : 1. By the significant test, total facial convexity angle and soft tissue thickness were no significant difference between both sexes. 2. Lower facial height was greater than upper facial height in both sexes. 3. The vertical length of the upper and lower lips were 21.95 mm, 40.74 mm in male and 21.62 mm, 39.63 mm in female. 4. In the relationship of the upper lip and lower lip to the Ricketts' esthetic line, the male was 1.3 mm, 1.18 mm and the female was 1.16 mm, 1.27 mm front of the esthetic line. 5. Compared with the angulation of flush terminal plane group and mesial step group, the mesial step group was greater than the flush terminal plane group except the chin angle.

Purpose: Rigid external distraction(RED) is a highly effective technique for correction of maxillary hypoplasia in patients with cleft or syndromic craniosynostosis. Despite many advantages of RED, it also has the problem of relapse as the conventional advancement surgery. Bicoronal approach, that is the common approach to gain access to the craniofacial skeleton, had some morbidity, such as hair loss, sensory loss, wide scar and temporal hollowing. We present our clinical experience of RED distraction with minimal invasive approach and early rigid fixation to overcome these disadvantages. Methods: A 27-year-old female patient with Crouzon's disease underwent Le Fort III osteotomy and RED device application through the minimal invasive direct skin incisions. After the latent period of 5 days, distraction was undertaken until proper convexity and advancement were obtained. During the rigid retention period, inflammation occurred on the right cheek, and proper conservative managements were done including continuous irrigation. To maintain the stability of distraction, early rigid fixation was undertaken on the osteotomy sites through another skin incisions. Preoperative and postoperative orthodontic treatments were performed. Serial photographs and cephalometric radiographs were obtained preoperatively, after distraction and 6 months after distraction. Results: The cephalometric analysis demonstrated postoperatively significant advancement of the maxilla and improvement of facial convexity. After 6-month follow-up period, the maxilla was stable in the sagittal plane and no relapse was found. Facial scars were not noticeable and other deformity and morbidity did not occur. Conclusion: This effective and stable technique will be a good alternative for the patients who need large amount of distraction and for adult patients with severe maxillary hypoplasia or syndromic craniosynostosis.
Class III malocclusion patients can be approached with many different types of treatment methods, and thus, each patient's problems must be accurately evaluated to allow selection of the best possible treatment method. Cephalometric analysis is an essential part of diagnosis and treatment planning of orthodontic patients, and it would certainly be helpful if reliable cephalometric guidelines could be set. The author divided 482 Class III malocclusion patients(253 males and 229 females) into fourgroups according to different types of treatment methods they have received to correct imbalance between upper and lower jaws: 1) orthopedic appliance (face mask & RPE), 2) camouflage treatment with fixed appliance, 3) surgical-orthodontic treatment, 4) cross-bite correction with removable plates/ functional appliance. Cephalometric values at the time of first clinical examination were compare among the four groups. Cephalometric analysis indicates the following results: 1)the amounts of antero-posterior and vertical skeletal discrepancies and dental compensation were greatest in surgery group 2) SNB, Wits, distance from Nasion Perpendicular Plane to point a facial angle, facial convexity, and APDI were greater in orthopedic appliance group than fixed appliance(camouflage) group, but there was no statistical difference 3) removable plates/ functional appliance group showed least amounts of skeletal discrepancies and dental compensation with statistical significance.
This study was performed to evaluate whether growth Prediction method can be used to diagnose and make treatment plan in skeletal Class III malocclusion patients or not. The sample was consisted of 25 patients(13 males, 12 females) who had been diagnosed with skeletal Class III malocclusion at first visit and after that had returned to take ortognathic surgery. Growth prediction performed with Ricketts' growth prediction method from first cephaogram. was compared with actual growth of the second cephalogram. The findings of this study were as follows ; 1. There was significant difference between actual growth and growth prediction in Porion Location, Ramus Position, Facial Depth, Facial Axis, Mandibular Plane angle, Maxillary Convexity. So, for these items Ricketts' growth prediction method is not proper to predict growth. 2. Although the growth amount of mandibular body was similar to normal growth amount, mandible was positioned anteriorly because of Porion Location and Ramus Position. 3. In skeletal Class III malocclusion patients, the tendency of mandibular prognathism might be aggreviated because of anterior placement of ramus and anterosuperior rotation of Pogonion.
