CLINICAL STUDY ON THE ETIOLOGY, DIFFERENTIAL DIAGNOSIS AND TREATMENT OF TRISMUS (개구장애 환자의 병인, 감별진단 및 치료방식에 대한 임상연구)

  • Trismus is a common problem to most people experiencing at once in his or her life and to most dental practitioners experiencing frequently. It has a number of potential causes which are single factor or complex factors. Its treatment will depend on the cause. The purpose of this study was to discuss the causes of trismus condition and the various treatments available. This study was made by reviewing of collected data from 86 patients complained of trismus among patients who were diagnosed by TMD, tumor, infection including tetanus, soft tissue anomalies, bony fracture and ankylosis from Jan 2002 to Dec 2004 on department of oral and maxillofacial surgery at Pusan National University Hospital, South Korea. The clinical reviews regarding chief complaints, clinical characteristics, diagnostic examination, treatments and the results on the patients were given as follows. 1. The etiology of trismus commonly were derived from temporomandibular joint(TMJ) disorder, TMJ ankylosis, TMJ tumor, odontogenic maxillofacial infection, mandibular condylar fracture, tetanus. 2. The chief complaints of trismus patients were progressive mouth opening limitation, TMJ pain, malocclusion, facial asymmetry, retrognathic state. 3. Especially, for the differential diagnosis between the fibrous ankylosis and true bony ankylosis, computed tomogram (CT) was useful. Surgical gap arthroplasty on bony ankylosis patients was applied and the gain of mouth opening after operation was average 35.8 mm during 19 months. 4. The tetanus, rarely, also induced the trismus with the range of mouth opening less than 10 mm. The average serum level of tetanus anti-toxin was 0.02-0.04 IU/mL. The limitation of mouth opening was improved into average 38 mm on 4 weeks after injection of 10,000 units of tetanus immune globulin. 5. In the treatment of osteochondroma, TMD, odontogenic infection and fracture, and the others inducing trismus, to obtian the maximum result and decreased inadequate time and effort, it is important to finding the causes from the exact clinical examination and diagnosis.

The way of characters creation in this research suggest as follows. First, animation character action and a way of thinking, which makes regulation factor is a global outlook. It expresses a outer factors include appearance, style, action, facial expressions, tone of voice, hobbies, occupation, status and internal factors shows nature, conception, culture. Second, the given condition of general tendency of animation character, Cultural background, and Key characteristics and experiences is built up by global outlook. Third, the attractive of animation character show up the familiarity, originality, fun, various senses, and flexibility. Fourth, it is harmony of new global outlook and character. The establishment of animation character is that the familiarity which is easy approach to audience with special global outlook, the originality which is different from other works, fun with laugh, at different times in different ways to interact with more flexibility, various sense to be delivered to audience for easy setting is the most desirable.

A CEPHALOMETRIC STUDY ON CORRELATION BETWEEN MANDIBULAR SYMPHYSIS AND CRANIOFACIAL SKELETON (하악이부와 두개안면골격의 상관성에 관한 측모두부방사선 계측학적 연구)

  • The configuration of mandibular symphysis is likely to be dependent upon the genetic predeterminants and/or compensatory adjustments onto functional demands. The interrelation of morphological distinctives of symphysis in conjunction to the craniofacial skeleton had been scarcely anecdotal, therefore, the objective was to determine the correlation of morphological modifications between the mandibular symphysis and craniofacial complex. Lateral cephalometric headfilms of 212 subjects were employed for the conventional measurements. The proportion of chin height against chin depth length was referred as chin ratio, then, Low Symphysis (IS) and High Symphysis (HS) groups were turned out by means of the chin ratio. These samples yielded 35 in LS and 35 in HS groups. The data per capita were statistically analyzed and the following results were drawn ; 1. Overall characteristics of the craniofacial skeleton in HS group manifested hyperdivergence and LS group showed hypodivergence. 2. Gonial angle increased as chin ratio increased and was highly correlated to the chin ratio. 3. The chin ratio presented high correlation to the vertical face height, especially in terms of the chin height to anterior face height and the chin depth to posterior face height. 4. The morphological configuration of chin was hardly correlated with hyoid bone position.

Phenotypic and Cytogenetic Delineation of Six Korean Children with Kabuki Syndrome (한국인 Kabuki 증후군 환아들의 임상적 표현형 및 세포유전학적 양상)

  • Purpose : Kabuki syndrome is a multiple congenital malformation syndrome with mental retardation. It was named after its characteristic appearance, a face resembling that of an actor in a Kabuki play. To date, six Korean cases of Kabuki syndrome have ever been reported. Here, we present the phenotypic and genetic characteristics of six patients with Kabuki syndrome. Materials and Methods : Between 2003 and 2009, six Korean girls have been diagnosed and followed up as Kabuki syndrome at Center for Genetic Diseases of Ajou University Hospital. Their clinical and laboratory data were collected and analyzed by the retrospective review of medical records. Results : All six patients showed the characteristic facial dysmorphism and developmental delay. Persistent fingertip pads were also found in all patients. Most patients showed postnatal growth retardation (83.3%) and hypotonia (83.3%). Opthalmologic problems were common, particularly for strabismus (83.3%). Congenital heart defects were present in three patients (50%). Skeletal abnormalities including 5th finger shortening (83.3%), clinodactyly (50%), joint hypermobility (50%) and hip dislocation (16.7%) were also observed. There was no patient who had positive family history for Kabuki syndrome. Cytogenetic and molecular cytogenetic analyses including karyotyping and array CGH could not reveal any underlying genetic cause of Kabuki syndrome. Conclusion : Korean patients with Kabuki syndrome showed a broad spectrum of clinical features affecting multiple organ systems. Although clinical manifestations of Kabuki syndrome have been well established, our results failed to detect recurrent chromosome aberrations which could cause Kabuki syndrome. Its natural history and genetic background remains to be further studied for providing appropriate management and genetic counseling.

A Method for Determining Face Recognition Suitability of Face Image (얼굴영상의 얼굴인식 적합성 판정 방법)

  • Face recognition (FR) has been widely used in various applications, such as smart surveillance systems, immigration control in airports, user authentication in smart devices, and so on. FR in well-controlled conditions has been extensively studied and is relatively mature. However, in unconstrained conditions, FR performance could degrade due to undesired characteristics of the input face image (such as irregular facial pose variations). To overcome this problem, this paper proposes a new method for determining if an input image is suitable for FR. In the proposed method, for an input face image, reconstruction error is computed by using a predefined set of reference face images. Then, suitability can be determined by comparing the reconstruction error with a threshold value. In order to reduce the effect of illumination changes on the determination of suitability, a preprocessing algorithm is applied to the input and reference face images before the reconstruction. Experimental results show that the proposed method is able to accurately discriminate non-frontal and/or incorrectly aligned face images from correctly aligned frontal face images. In addition, only 3 ms is required to process a face image of $64{\times}64$ pixels, which further demonstrates the efficiency of the proposed method.

Development of Virtual Makeup Tool based on Mobile Augmented Reality

  • In this study, an augmented reality-based make-up tool was built to analyze the user's face shape based on face-type reference model data and to provide virtual makeup by providing face-type makeup. To analyze the face shape, first recognize the face from the image captured by the camera, then extract the features of the face contour area and use them as analysis properties. Next, the feature points of the extracted face contour area are normalized to compare with the contour area characteristics of each face reference model data. Face shape is predicted and analyzed using the distance difference between the feature points of the normalized contour area and the feature points of the each face-type reference model data. In augmented reality-based virtual makeup, in the image input from the camera, the face is recognized in real time to extract the features of each area of the face. Through the face-type analysis process, you can check the results of virtual makeup by providing makeup that matches the analyzed face shape. Through the proposed system, We expect cosmetics consumers to check the makeup design that suits them and have a convenient and impact on their decision to purchase cosmetics. It will also help you create an attractive self-image by applying facial makeup to your virtual self.

Optimization of In Vivo Stickiness Evaluation for Cosmetic Creams Using Texture Analyzer (Texture Analyzer (TA)를 이용한 화장품 크림의 In Vivo 끈적임 평가법의 최적화)

  • There have been continuous attempts to quantify sensory attributes of cosmetic products by measuring relevant physical properties. The most representative method to evaluate stickiness is to measure axial force using texture analyzer. Stickiness is known to correlate with AUC which abbreviates area under curve in the obtained axial force curve as a function of time. Recently, Normandie University research group developed in vivo stickiness evaluation method considering the characteristics of skin along with established evaluation method[8]. Based on the study, we tried to optimize in vivo stickiness evaluation method especially for cosmetic creams. The experiment was carried out on 5 different facial creams products by changing the amount and the times of rolling of creams, and the shape and material of probes. Based on the results of the sensory evaluation, the most consistent conditions were established as the optimal evaluation method. As a result, applying 70 μL of cream and rubbing 10 times for 7 s inside the 3.4 cm circle were judged to be suitable. As for the probes, spherical metallic probe was more proper due to its reproducibility. We conducted the settled method on 10 subjects to check its validity. Although the absolute values of AUC differed depending on the individuals, the AUC values were all ranked the same. Finally, for the standardization of stickiness of AUC, polyvinylpyrrolidone (PVP) was set as a reference material and we measured AUC of its aqueous solution by changing concentration. Then, the degree of stickiness recognition for 5 different creams was surveyed to check the correlation between AUC and stickiness.

Clinical Assessment and Cephalometric Characteristics of Patients with Condylar Resorption in Teenagers (10대 하악과두흡수 환자의 임상적 평가 및 악안면 골격형태에 대한 연구)

  • The aim of this study was to investigate clinical assessment and cephalometric characteristics in 10s patients with condylar resorption, who visited in the Department of Oral Medicine Kyungpook National University Hospital at 2006, by use of panorama, transcranial view and lateral cephalometric radiograph. The results were as follows; 1. Clinical assessment 1) Total number of patients were 59. Female was 47 and male was 12, Females were predominant and patient's age ranged from 12 to 19. 2) Most of the patients had Grade II condyle resorption. 3) The number of who had Class I occlusion was 27, which was the largest group. The number of patients with openbite was 20. Average overjet was 3.58mm and average overbite was 0.97mm. 4) Most of the patients had parafunctional habit. 5) The patients of showing the pain in condylar resortion was 41 and the case of not showing the pain was 18. 6) Treatment duration of 23 patients were less than 1 month, 28 patients were treated with supported therapy. 2. Cephalometric Characteristics 1) A 16-year-old female patients showed smaller SNA, SNB and larger articular angle significantly as compared with those of normal group. 2) A 17-year-old female patients showed smaller SN, SAr, TPFH, ramus height and larger SN-GoMe, FMA, articular angle significantly as compared with those of normal group. 3) Over 18 years old female patients showed smaller SN, SNB, TPFH, ramus height and larger ANB, FMA, SN-GoMe, LAFH, articular angle, gonial angle significantly as compared with those of normal group. 4) Over 18 years old male patients showed smaller SN, TPFH, ramus height and larger FMA, SN-GoMe, LAFH, articular angle, gonial angle significantly as compared with those of normal group. 5) There was no significant difference between 10s and normal group in mandibular body length.

CHARACTERISTICS OF CRANIOFACIAL STRUCTURES IN SEVERE ADULT CLASS III MALOCCLUSION (심한 골격성 III급 부정교합을 가진 성인 두개안면골격의 특성)

  • The purpose of this study is to define the characteristics of the skeleton and soft tissues of severe adult class III malocclusion. The materials selected for this study were lateral cephalograms of 112 adult class III malocclusion patients with ANB difference below -2 degrees. and the mean age was 22.9 years old. The normal control sampler consisted of lateral cephalograms of 50 adults in normal occlusion and the mean age was 22.1 years old. The Horizontal reference line was FH line and the vertical reference line was nasion perpendicular to FH line. The skeletal and soft tissue characteristics of Class III malocclusion are as follows : 1. In the skeletal profile evaluated by vertical reference line (Nasion perpendicular to FH), the forehead and maxilla was similar to normal, but the mandible was protruded significantly. 2. The soft tissue profile is concave. The thickness of soft tissue covering forehead area and nose is within normal range. but the upper lip is thicker and the nasolabial angle is smaller than normal. The lower lip and inferior labial sulcus is thinner than normal. The degree of eversion of lower lip is lesser than normal. 3. The cranial base of class III malocclusion is shorter and saddle angle is smaller than normal. 4. The location of midface evaluated in relations to cranial base is within normal range but, the length of midface is shorter than normal when compared from the deep portion of the facial skeleton. 5. The location of maxilla in reference to cranial base is within normal range but the length of maxilla was shorter in class III malocclusion. 6. The mandible was protruded, ramus height and body length, gonial angle were greater than normal, and the chin angle was smaller. 7. Upper incisor was proclined, lower incisor was retroclined.

Clinical Study about Warning Signs of Patients with Acute Stroke (급성기(急性期) 중풍환자(中風患者)에 대한 중풍전조증(中風前兆症) 조묘(調杳) 연구(硏究))

  • Objectives : This study was aimed to investigated the warning signs and its relationship with the other characteristics in acute stroke patients. Methods : 225 acute stroke patients were recruited at the Department of Cardiovascular and Neurologic Diseases (Stroke Center) of Kyung Hee University Oriental Hospital from October 2005 to September 2006. We evaluated their stroke type with brain MRI, their warning signs, and general characteristics such as age, sex, past history, risk factors, etc. Results : 225 subjects were included in the final analysis. In the subjects' general characteristics, the most common etiology of stroke was small vessel occlusion. In the assessment of the subjects' warning signs, the frequency of tension felt at the cervical area was highest followed by blepharospasm, sensory dysfunction (one side of numbness, tingling sensation, dead sensation), one side paralysis or weakness, etc. After analyzing etiology, cerebral hemorrhage had more facial spasm sign and hypertension than cerebral infarction. On the other hand, cerebral infarction had more diabetes and sensory dysfunction (one side of numbness, tingling sensation, dead sensation) than cerebral hemorrhage. Among stroke locations, subjects with their brain lesion in the cortex had more warning sings of motor dysfunction such as one side paralysis, or weakness. Multiple lesions showed a close relationship with smoking habit and were more common in males than in females. The under 65 years old group were more commonly associated with alcohol consumption, accidental mental stress and blepharospam than the over 65 years old group. In the group of under 65 years old, males more commonly had lesions in occipital lobe, alcohol consumption and smoking habit than females. Otherwise, females more commonly had vision dysfunction and blepharospasm than males. In the group of over 65 years old, males more commonly had cortex lesion than females. On the other hand, Females more commonly had accidental mental stress than males. Conclusions : We observed various warning signs and their distribution in acute stroke patients. The subjects' brainlesions and their etiology seemed to affect the features of the warning signs. Hypertension, diabetes and hyperlipidemia were also related to etiology of stroke and some habitual problems such as smoking and drinking seemed to reduce the age of stroke ictus. Although a concrete conclusion can hardly be drawn from this study, it reminds physicians of the importance of warning signs which appear among their patients.
