Chondroid Syringoma on Face

  • Chondroid syringoma is a rare mixed tumor of the skin which is composed of both mesenchymal and epithelial cells. Its incidence at less than 0.1% and is frequently located on the head and neck. Chondroid syringoma is easily confused with epidermal cysts. Since malignant forms of chondroid syringoma have been reported, accurate and timely diagnosis is important for proper management. We report clinical and histological features of chondroid syringoma in 5 patients treated at our institution. In most of the cases, chondroid syringoma presented as a round, firm, nodular or cystic lesion that had well marginated heterogeneity in sonography. Clinically, all of the lesions were removed by simple excision. Microscopically, all five tumors were well circumscribed and consisted of epithelial, myoepithelial, and stromal components. The epithelial component formed tubules lined by one or more rows of eosinophilic epithelial cells. The outer layer of tubules appeared to be flattened myoepithelial cells. The stroma is myxoid and contained spindle shaped myoepithelial cells. We expect that the clinical, sonographic, and histological data from our report may help clinicians who are confronted with various kinds of analogous facial lesions to decide the most proper management for their patients.

Establishment of the Repair Cycle of the Components of the Apartment Housing

  • Building has been deteriorated over the year after construction. The deterioration has caused to hinder the living condition and to decrease the building value. Thus, it is important to prevent or delay the building deterioration as well as to proceed the maintenance. The long-term repair program for the apartment in Korea plays a key role to make the repair plan after construction and to assure the function decent. This is not flexible to the change of the material and component because the program is provided by the Korean-law. Many items are omitted in the long-term program so that the maintenance experts face many difficulties to make a repair plan. In this paper, it aimed at providing the repair cycle and repair ratio according to the repair scope, which are not provided in the current program. This study shows that the repair ratio is presented with quartile range in 25%, 50%, 75% and 100%. This has an advantage to take an overall look in repair items. Under this presentation of the repair ratio, each item has a unique repair value. Second, the repair scope is divided into partly repair, fully repair, partly alteration and fully alteration. If a fully repair has a repair ratio close to 100%, it would mean that a fully repair is a fully alteration. Third, the short repair cycle means that it requires to maintain the function or performance of the components in a short term.

Gabor 특징과 웨이브렛 영역의 BDIP와 BVLC 특징을 이용한 질감 특징 기반 언어 인식 (Texture Feature-Based Language Identification Using Gabor Feature and Wavelet-Domain BDIP and BVLC Features)

  • 본 논문에서는 Gabor 특징과 웨이브렛 영역의 BDIP와 BVLC 특징을 이용한 질감 특징 기반 언어 인식 방법을 제안한다. 제안된 방법에서는 먼저 시험 영상에 Gabor 변환과 웨이브렛 변환을 적용한다. 웨이브렛 영역의 상세 대역에는 Donoho의 연역치화를 적용하여 잡음을 제거한다. 이어서 Gabor 영상에는 크기 연산자를 적용하고 웨이브렛 부대역에는 BDIP와 BVLC 연산자를 적용한다. 그런 다음 Gabor 크기 영상과 BDIP, BVLC 부대역에 대하여 통계치를 계산하여 그 결과들을 벡터화하고 융합하여 특징 벡터로 사용한다. 분류 단계에서는 얼굴 인식에 주로 사용되는 WPCA를 분류기로 하여 시험 특징 벡터와 가장 유사한 학습 특징 벡터를 찾는다. 실험 결과 제안된 방법은 실험 문서 영상 DB에 대하여 비교적 낮은 특징 벡터 차원으로 매우 우수한 언어 인식 성능을 보여준다.

기후변화에 따른 도시 수종의 기후 적합성 평가모델 - 서울시를 대상으로 - (Modeling the Present Probability of Urban Woody Plants in the face of Climate Change)

  • The effect of climate change on urban woody plants remains difficult to predict in urban areas. Depending on its tolerances, a plant species may stay and survive or stay with slowly declining remnant populations under a changing climate. To predict those vulnerabilities on urban woody plants, this study suggests a basic bioclimatic envelop model of heat requirements, cold tolerance, chilling requirements and moisture requirements that are well documented as the 'climatic niche'. Each component of the 'climatic niche' is measured by the warmth index, the absolute minimum temperature, the number of chilling weeks and the water balance. Regarding the utility of the developed model, the selected urban plant's present probabilities are suggested in the future climate of Seoul. Both Korea and Japan's thermal thresholds are considered for a plant's optimal climatic niche. By considering the thermal thresholds of these two regions for the same species, the different responses observed will reflect the plant's 'hardening' process in a rising climate. The model illustrated that the subpolar plants Taxus cuspidata and Ulmus davidiana var. japonica are predicted to have low suitability in Seoul. The temperate plants Zelkova serrata and Pinus densiflora, which have a broad climatic niche, exhibited the highest present probability in the future. The subtropical plants Camellia japonica and Castanopsis cuspidata var. sieboldii may exhibit a modest growth pattern in the late 21C's future climatic period when an appropriate frost management scheme is offered. The model can be used to hypothesize how urban ecosystems could change over time. Moreover, the developed model can be used to establish selection guidelines for urban plants with high levels of climatic adaptability.

고령 양전동암각화의 암석학적 특징과 훼손양상 분석 (Petrological Characteristics and Deterioration Aspect of the Goryeong Yangjeondong Petroglyph)

  • 고령 양전동암각화는 녹색 사암으로 구성된 너비 6m 높이 3m의 암반의 절리면에 겹동그라미(3점)와 방패무늬(30점)의 그림들이 있다. 구성암석의 주 구성광물은 석영, 장석 및 녹니석이며, 교질물인 점토광물 중 녹니석이 대부분이나 일라이트와 소량의 방해석도 산출된다. 이들 점토광물들은 구성광물을 피복 및 성분을 교대하거나 공극을 충진하는 양상으로 산출된다. 수용성 염과 암석의 풍화현상과의 관계를 포함한 훼손양상과 암각화가 그려진 암반의 사면안정성을 조사하였다. 훼손양상은 주로 표면박리, 입자박락, 균열 및 파손 등이다. 그리고 절리와 균열이 발달한 암반은 낙석의 가능성이 있는 것으로 해석된다. 보존을 위해서는 균열 및 파손 부위에 대한 접착처리와 약해진 표면층에 강화처리가 필요하다.

A case of de novo duplication of 15q24-q26.3

  • Distal duplication, or trisomy 15q, is an extremely rare chromosomal disorder characterized by prenatal and postnatal overgrowth, mental retardation, and craniofacial malformations. Additional abnormalities typically include an unusually short neck, malformations of the fingers and toes, scoliosis and skeletal malformations, genital abnormalities, particularly in affected males, and, in some cases, cardiac defects. The range and severity of symptoms and physical findings may vary from case to case, depending upon the length and location of the duplicated portion of chromosome 15q. Most reported cases of duplication of the long arm of chromosome 15 frequently have more than one segmental imbalance resulting from unbalanced translocations involving chromosome 15 and deletions in another chromosome, as well as other structural chromosomal abnormalities. We report a female newborn with a de novo duplication, 15q24- q26.3, showing intrauterine overgrowth, a narrow asymmetric face with down-slanting palpebral fissures, a large, prominent nose, and micrognathia, arachnodactyly, camptodactyly, congenital heart disease, hydronephrosis, and hydroureter. Chromosomal analysis showed a 46,XX,inv(9)(p12q13),dup(15)(q24q26.3). Array comparative genomic hybridization analysis revealed a gain of 42 clones on 15q24-q26.3. This case represents the only reported patient with a de novo 15q24-q26.3 duplication that did not result from an unbalanced translocation and did not have a concomitant monosomic component in Korea.

The Chicken Thigh Adductor Profundus Free Muscle Flap: A Novel Validated Non-Living Microsurgery Simulation Training Model

  • Background Simulation training is becoming an increasingly important component of skills acquisition in surgical specialties, including Plastic Surgery. Non-living simulation models have an established place in Plastic Surgical microsurgery training, and support the principles of replacement, reduction and refinement of animal use. A more sophisticated version of the basic chicken thigh microsurgery model has been developed to include dissection of a type 1-muscle flap and is described and validated here. Methods A step-by-step dissection guide on how to perform the chicken thigh adductor profundus free muscle flap is demonstrated. Forty trainees performed the novel simulation muscle flap on the last day of a 5-day microsurgery course. Pre- and post-course microvascular anastomosis assessment, along with micro dissection and end product (anastomosis lapse index) assessment, demonstrated skills acquisition. Results The average time to dissect the flap by novice trainees was $82{\pm}24$ minutes, by core trainees $90{\pm}24$ minutes, and by higher trainees $64{\pm}21$ minutes (P=0.013). There was a statistically significant difference in the time to complete the anastomosis between the three levels of training (P=0.001) and there was a significant decrease in the time taken to perform the anastomosis following course completion (P<0.001). Anastomosis lapse index scores improved for all cohorts with post-test average anastomosis lapse index score of $3{\pm}1.4$ (P<0.001). Conclusions The novel chicken thigh adductor profundus free muscle flap model demonstrates face and construct validity for the introduction of the principles of free tissue transfer. The low cost, constant, and reproducible anatomy makes this simulation model a recommended addition to any microsurgical training curriculum.

  • Protein O-GlcNAcylation, dictated by cellular UDP-N-acetylglucosamine (UDP-GlcNAc) levels, plays a crucial role in posttranslational modifications. The enzyme GlcNAc kinase (NAGK, E.C. 2.7.1.59) catalyzes the formation of GlcNAc-6-phosphate, which is a major substrate for the biosynthesis of UDP-GlcNAc. Recent studies have revealed the expression of NAGK in different types of cells especially in neuronal dendrites. Here, by immunocytochemistry (ICC) and immunonucleochemistry (INC) of cultured rat hippocampal neurons, HEK293T and GT1-7 cells, we have showed that NAGK immuno-reactive punctae being present in the nucleoplasm colocalized with small nuclear ribonucleoprotein-associated protein N (snRNPN) and p54NRB, which are speckle and paraspeckle markers, respectively. Furthermore, NAGK IR cluster was also found to be colocalized with GTF2H5 (general transcription factor IIH, polypeptide 5) immuno reactive punctae. In addition, relative localization to the ring of nuclear lamin matrix and to GlcNAc, which is highly enriched in nuclear pore complexes, showed that NAGK surrounds the nucleus at the cytoplasmic face of the nuclear outer membrane. By in situ proximity ligation assay (PLA) we confirmed the colocalization of NAGK with snRNPN in the nucleus and in dendrites, while we also verified the interactions of NAGK with p54NRB, and with GTF2H5 in the nucleus. These associations between NAGK with speckle, paraspeckle and general transcription factor suggest its regulatory roles in gene expression.

고주파절제술을 통한 선택적 신경차단법을 이용한 미간주름의 개선 (Treatment of glabellar frown lines using selective nerve block with radiofrequency ablation)

  • Purpose: Corrugator supercilii muscle pulls eyebrow to inferomedial direction and produces the vertical component of the glabellar line formation. Current techniques for eliminating of glabellar frown include direct resection of corrugators and botulinum toxin injection. Muscle resection in endoscopic face lift procedure is relatively complex and has many disadvantages ranging from possible nerve injury, postoperative edema, pain and a long recovery period. The Botox treatment on the other hand is much more simple in technique but has a short duration of action. The authors have attempted new ways of finding improved treatment of the glabellar frown by selectively blocking of motor nerves innervating the corrugator supercili muscle by using radiofrequency ablation technique. Methods: A total of 80 patients were recruited in our study during the period between Feb. 2007 to June 2008. A probe was introduced from the supraorbital ridge and advanced to the corrugator supercilii muscle. Nerve stimulator was then used to locate the nerve innervating the corrugator and radiofrequency ablation of the nerve was done. Results: In all patients, there were marked improvement in glabellar frown after treatment. There were no reported cases of any relapses during the follow up period. No complication was noted such as facial nerve injury. No patient complained of any adverse symptoms other than slight discomfort due to swelling of the operation site. Conclusion: The treatment of glabellar frown lines using selective nerve block with radiofrequency ablation was not only less invasive but also excellent in surgical outcomes.

환승센터의 두 수단간 환승거리의 상대적 적정성 평가 (A model of a relative evaluation of the transfer distance between two modes)

  • 환승센터의 계획이나 개선시 제일 먼저 부딧히는 문제의 하나는 어느 수단을 얼마나 가깝게 위치시켜야 하는가 하는 것이다. 이때의 목표는 중심 수단을 이용하는 승객이 가능한 한 최단거리를 걸어서 다른 수단으로 연결될 수 있게 하는 것이다. 이 문제가 결정되고 난 후에 라야 연결 통로에 대한 서비스 수준을 향상시키는 문제를 제기할 수 있다. 물리적인 접근 거리가 너무 긴 경우 개별 통로의 서비스수준을 아무리 높여도 전체적인 환승만족도가 일정수준이상 향상되기 어렵기 때문이다. 본 연구에서는 한 수단의 도착지에서 다른 수단의 출발지까지의 전 과정 (환승패스)을 외부보도구간, 계단구간, 실내통로구간으로 나누어 이용자 설문조사를 통하여 상대적인 저항감을 구하여 합산된 환승저항을 계산하고, 이를 전반적인 환승만족도와 연결시켜 환승효용함수를 구하고 이를 토대로 환승패스의 개선 방향을 결정하는 방법을 논하였다.
