Changes in Corneal and Internal Astigmatism with Age (연령에 따른 각막난시와 내부난시의 변화)

  • The purpose of present study was to determine the frequency of RA with age and to investigate the age-related trends and changing-factors in RA, CA and IAs. The refractive power of the eye and the power of corneal anterior surface were measured with auto-refractor among 1,017 inhabitants aged 5 to 59 years in Cheongju in July 2010. The overall frequency of RA was 22.7%, and frequency of RA increased from 8.9% in 5~9 years age group to 36.8% in 20~29 years age group. It then dipped to 19.2% in 40~49 years age group but increased again 28.6% in 50~59 years age group. $J_{45}$ components for RA, CA, and IAs were fairly stable in different age groups, the changes in $J_0$ components for both RA and CA appeared to be decreased after age of 30 years. In addition, the refractive power on the vertical direction was changed slightly with age, but the refractive power on the horizontal direction was changed significantly with age. It was expected that the change in the frequency of astigmatism with age was due to the change in the refractive power of horizontal meridian.

A Study on the Criteria for Collision Avoidance of Naval Ships for Obstacles in Constant Bearing, Decreasing Range (CBDR) (방위끌림이 없는 장애물에 대한 함정의 충돌회피 기준에 관한 연구)

  • Naval ships that are navigating always have the possibility of colliding, but there is no clear maneuvering procedure for collision avoidance, and there is a tendency to depend entirely on the intuitive judgment of the Officer Of Watch (OOW). In this study, we conducted a questionnaire survey when and how to avoid collision for the OOW in a Constant Bearing, Decreasing Range (CBDR) situation wherein the naval ships encountered obstacles. Using the results of the questionnaire survey, we analyzed the CBDR situation of encountering obstacles, and how to avoid collision in day/night. The most difficult to maneuver areas were Pyeongtaek, Mokpo, and occurred mainly in narrow channels. The frequency appeared on average about once every four hours, and there were more of a large number of ships encountering situations than the 1:1 situation. The method of check of collision course confirmation was more reliable with the eye confirmation results, and priority was given to distance at closest point of approach (DCPA) and time at closest point of approach (TCPA). There was not a difference in DCPA between the give-way ship and stand-on ship, but a difference between day and night. Also, most navigators prefer to use maneuvering & shifting when avoiding collisions, and steering is 10-15°, shifting ±5knots, and the drift course was direction added stern of the obstacles to the direction of it. These results will facilitate in providing officers with standards for collision avoidance, and also apply to the development of AI and big data based unmanned ship collision avoidance algorithms.

A Study on 3D Model Building of Drones-Based Urban Digital Twin (드론기반 도심지 디지털트윈 3차원 모형 구축에 관한 연구)

  • In this study, to build a spatial information infrastructure, which is a component of a smart city, a 3D digital twin model in the downtown area was built based on the latest spatial information acquisition technology, the drone. Several analysis models were implemented by utilizing. While the data processing time and quality of the three types of drone photogrammetry software are different, the accuracy of the construction model is ± 0.04 in the N direction and ± 0.03m in the E direction. In the m and Z directions, ± 0.02m was found to be less than 0.1m, which is defined as the allowable range of surveying performance and inspection performance for the boundary point in the area where the registration of the boundary point registration is executed. 1: 500 to 1 of the aerial survey work regulation: The standard deviation, which is the error limit of the photographic reference point of the 600 scale, appeared within 0.14 cm, and it was found that the error limit of the large scale specified in the cadastral and aerial survey was satisfied. In addition, in order to increase the usability of smart city realization using a drone-based 3D urban digital twin model, the model built in this study was used to implement Prospect right analysis, landscape analysis, Right of light analysis, patrol route analysis, and fire suppression simulation training. Compared to the existing aerial photographic survey method, it was judged that the accuracy of the naked eye reading point is more accurate (about 10cm) than the existing aerial photographic survey, and it is possible to reduce the construction cost compared to the existing aerial photographic survey at a construction area of about 30㎢ or less.

The Comparison of Lens Movement by the Fitting States of Soft Contact Lenses in Normal and Dry Eyes (정상안과 건성안에서 피팅상태에 따른 각막에서의 소프트렌즈 움직임 비교)

  • Purpose: The study was performed to compare the differences in lens rotation, lens movement by blinking and lens centration with alignment or steep fitting of soft contact lens in normal and dry eyes. Methods: Total 40 eyes (aged 20~30 years) were classified into the normal (n=20) or dry eye group (n=20) by the diagnosis methods for dry eyes and worn soft contact lens (polymacon material) with alignment fitting or steep fitting. Lens rotation, lens movement by blinking and lens centration were separately measured immediately after lens wearing and after stabilization of tear film and compared by fitting states of soft contact lenses. Results: With steep fitting of soft contact lens in dry eyes, averaged lens rotation immediately after lens wearing was not significantly different from that of the normal eye group with alignment fitting however, lens rotation after stabilization in dry eyes was significantly larger than that in normal eyes. Any significant difference in lens movement by blinking was not shown in normal eyes. However, lens movement by blinking in dry eyes was increased with steep fitting. The range of lens centration on cornea in normal eyes with alignment fitting was more vertically distributed. On the other hand, the range of lens centration on cornea in dry eyes with alignment fitting was more horizontally distributed. Lens centration was shown to be changed by stabilization of tear film. That is, lens centrations were somewhat vertically widespread immediately after lens wearing and restrictively distributed in horizontal direction, respectively, with steep fitting in dry eyes. Conclusions: These results suggested that lens movements and centration in dry eyes were different from those of normal eyes. Especially, those differences between normal and dry eyes were much bigger with steep fitting of soft contact lenses. Thus, those differences should be considered for the comfortable and safe fitting of soft contact lens in dry eyes.

Study of Masking Effect of Soft Contact Lenses on Cornea after Refractive Surgery (각막굴절교정 수술 안에 대한 소프트콘택트렌즈 착용 시 Masking 효과)

  • Purpose: This study investigated the masking effect of the hydrogel lens and silicone hydrogel lens on the cornea with refractive surgery and without surgery. Methods: 24 university students (means age: $23.48{\pm}2.89years$) without refractive surgery (12, control group) and with refractive surgery (LASIK: 8, LASEK: 4, experimental group) participated in the study. Mean refractive errors of right eyes were -2.73 D for control group and -0.24 D for experimental group. The differences in the refractive power and corneal topography map between pre- and post-wearing the -3.00 D lenses were compared, and 2 kinds of hydrogel contact lenses (0.89 Mpa, 0.49 Mpa) and 2 kinds of silicone hydrogel lenses (1.5 Mpa, 0.8 Mpa) were used for -3.00 D lenses. NVision-K5001 (Shin nippon, Japan) was used to measure the refractive power and Keratograph 5M (Oculus, Germany) to measure the corneal topography map change. Results: Variations in the refractive power increased to the plus direction in the experimental group after wearing soft contact lenses. The corneal topography map showed significant changes on the both groups after wearing soft contact lenses (p<0.05). However there were no significant differences in the refractive power and corneal topography map variations by lens materials. Conclusions: Wearing soft contact lenses showed corneal topography map changes. Especially wearing soft contact lenses on the flat cornea after corneal refractive surgery showed greater corneal power changes. Therefore, it should pay attention to refractive change in case of prescribing soft contact lenses to patients with corneal refractive surgery.

Development of Neuropsychological Model for Spatial Ability and Application to Light & Shadow Problem Solving Process (공간능력에 대한 신경과학적 모델 개발 및 빛과 그림자 문제 해결 과정에의 적용)

  • The purpose of this study is to develop a neuropsychological model for the spatial ability factor and to divide the brain active area involved in the light & shadow problem solving process into the domain-general ability and the domain-specific ability based on the neuropsychological model. Twenty-four male college students participated in the study to measure the synchronized eye movement and electroencephalograms (EEG) while they performed the spatial ability test and the light & shadow tasks. Neuropsychological model for the spatial ability factor and light & shadow problem solving process was developed by integrating the measurements of the participants' eye movements, brain activity areas, and the interview findings regarding their thoughts and strategies. The results of this study are as follows; first, the spatial visualization and mental rotation factors mainly required activation of the parietal lobe, and the spatial orientation factor required activation of the frontal lobe. Second, in the light & shadow problem solving process, participants use both their spatial ability as a domain-general thought, and the application of scientific principles as a domain-specific thought. The brain activity patterns resulting from a participants' inferring the shadow by parallel light source and inferring the shadow when the direction of the light changed were similar to the neuropsychological model for the spatial visualization factor. The brain activity pattern from inferring an object from its shadow by light from multiple directions was similar to the neuropsychological model for the spatial orientation factor. The brain activity pattern from inferring a shadow with a point source of light was similar to the neuropsychological model for the spatial visualization factor. In addition, when solving the light & shadow tasks, the brain's middle temporal gyrus, precentral gyrus, inferior frontal gyrus, middle frontal gyrus were additionally activated, which are responsible for deductive reasoning, working memory, and planning for action.

Usefulness of Non-coplanar Helical Tomotherapy Using Variable Axis Baseplate (Variable Axis Baseplate를 이용한 Non-coplanar 토모테라피의 유용성)

  • Purpose: Helical Tomotherapy allows only coplanar beam delivery because it does not allow couch rotation. We investigated a method to introduce non-coplanar beam by tilting a patient's head for Tomotherapy. The aim of this study was to compare intrafractional movement during Tomotherapy between coplanar and non-coplanar patient's setup. Materials and Methods: Helical Tomotherapy was used for treating eight patients with intracranial tumor. The subjects were divided into three groups: one group (coplanar) of 2 patients who lay on S-plate with supine position and wore thermoplastic mask for immobilizing the head, second group (non-coplanar) of 3 patients who lay on S-plate with supine position and whose head was tilted with Variable Axis Baseplate and wore thermoplastic mask, and third group (non-coplanar plus mouthpiece) of 3 patients whose head was tilted and wore a mouthpiece immobilization device and thermoplastic mask. The patients were treated with Tomotherapy after treatment planning with Tomotherapy Planning System. Megavoltage computed tomography (MVCT) was performed before and after treatment, and the intrafractional error was measured with lateral(X), longitudinal(Y), vertical(Z) direction movements and vector ($\sqrt{x^2+y^2+z^2}$) value for assessing overall movement. Results: Intrafractional error was compared among three groups by taking the error of MVCT taken after the treatment. As the correction values (X, Y, Z) between MVCT image taken after treatment and CT-simulation image are close to zero, the patient movement is small. When the mean values of movement of each direction for non-coplanar setup were compared with coplanar setup group, X-axis movement was decreased by 13%, but Y-axis and Z-axis movement were increased by 109% and 88%, respectively. Movements of Y-axis and Z-axis with non-coplanar setup were relatively greater than that of X-axis since a tilted head tended to slip down. The mean of X-axis movement of the group who used a mouthpiece was greater by 9.4% than the group who did not use, but the mean of Y-axis movement was lower by at least 64%, and the mean of Z-axis was lower by at least 67%, and the mean of Z-axis was lower by at least 67%, and the vector was lower by at least 59% with the use of a mouthpiece. Among these 8 patients, one patient whose tumor was located on left frontal lobe and left basal ganglia received reduced radiation dose of 38% in right eye, 23% in left eye, 30% in optic chiasm, 27% in brain stem, and 8% in normal brain with non-coplanar method. Conclusion: Tomotherapy only allows coplanar delivery of IMRT treatment. To complement this shortcoming, Tomotherapy can be used with non-coplanar method by artificially tilting the patient's head and using an oral immobilization instrument to minimize the movement of patient, when intracranial tumor locates near critical organs or has to be treated with high dose radiation.

The Comparative Study on Travel Behavior and Traffic Accident Characteristics on a Community Road - With Focus on Seoul Metropolitan City (생활도로에서의 교통행태와 교통사고특성에 관한 연구 - 서울특별시를 중심으로)

  • In Korea, the number of crash accident victims per 100,000 population is three times higher than the average of OECD. In particular, 60% of it occurs on the community road. Thus, this study intends to analyze the causes of such accidents through a pedestrian and vehicle traffic survey. The purpose is to establish practical safety enhancement measures for community roads. In recent years, lots of changes have occurred in the pedestrian environment. A traffic survey shows that 65% of pedestrians walk on the right and 17% of people use smart-phones while walking. An eye camera experiment shows that the operation load of drivers on the community roads is more than 4 times higher than those in urban roads. According to a speed survey, 62% of vehicles drive at 30km/h or above. The characteristics of accidents on community roads are as follows. First, the ratio of accidents on the edge of the road is 2.3 times as high as those on other roads. Second, when people walk on the right, the ratio of accidents is 2.5 times as high as that of walking on the left. Third, it becomes more dangerous when people cross the road from the right to the left. The majority of accidents is caused by unsafe driving (84.4%). When a vehicle makes a left turn, the likelihood of accidents is 2.3 times as high as those caused by a right turn. The ratio of accidents caused by vehicles going backwards is 14% among all accidents. In community roads, the focus of drivers should be at least 4 times higher than those on urban roads. Thus, walking in the opposite direction of vehicles and careless behaviors are highly likely lead to accidents.

A Study on Survey of Non Face to Face Realtime Education Focused on Firefighter in COVID-19 (코로나19 상황에서 소방공무원의 비대면 실시간 교육에 관한 의식조사연구)

  • Purpose: Due to the coronavirus infection-19 (COVID) pendemics, all educational institutions were required to provide full non-face-to-face real-time education, and fire officials were required to provide fire-fighting education by applying non-face-to-face education. In this difficult situation, the National Fire Service Academy tries to find the direction of the non-face-to-face real-time education and suggest ways to improve it through a survey of the status of non-face-to-face real-time education conducted by the NFSA to fire officials. Method: A survey was conducted on fire officials under the theme of "Consciousness Survey for Improving the Quality and Specialization of Non-face-to-face Real-Time Remote Education" and an in-depth analysis was conducted based on the results. Result & Conclusion: First, professors or educational operators shall actively utilize remote education programs suitable for educational characteristics by utilizing various programs. Second, a dedicated notebook for non-face-to-face training should be provided to provide an educational environment where all learners can participate in the training without difficulty. Third, in the case of education and training that requires the use of equipment due to the nature of fire officials' education and training, it is necessary to consider it as a non-face-to-face training place by arranging educational equipment at each fire station. Fourth, it is hard to expect a satisfactory educational effect to cope with practical education with theoretical education. Therefore, facilities and programs that enable non-face-to-face real-time hands-on training should be developed. It is worth considering the proper combination of face-to-face education while maintaining the social distance as much as possible until such non-face-to-face training is possible. Fifth, non-face-to-face education is considered to have high eye fatigue due to the light and electromagnetic waves of the computer screen, and as time goes by, the concentration level decreases. Therefore, it is necessary to form an education time to reduce the eye fatigue of learners and increase concentration through proper class and rest time. Finally, professors should operate a learner participation-oriented education that allows professors and learners to interact rather than one-sided knowledge transfer education. In addition, technical problems of non-face-to-face remote education should be thoroughly prepared through preliminary system checks to ensure that education is not disrupted.

Survey for the Understanding of Elderly Disease and Work Environment for Physical Therapists in Elderly Care Facilities (노인요양시설물리치료사들의 노인복지정책과 노인질환에 대한 이해도 및 직무 환경에 대한 조사)

  • The results of researching on duties of physical therapist, understanding of elderly diseases and policies related with elderly welfare came out from the survey before beginning of education to physical therapists who attended National Elderly Welfare Physical Therapist Workshop during Sep. 11th to 14th, 2007. They have already understood about long-term elderly care insurance from the policy, and responded that those policies will bring more advantage to elders. On contrary, negative response had reason of low service quality. About elderly care system, there were positive answer of appropriate alternatives to support them, but also other critics had opinion of inevitable choice for living conservation. About understanding of elderly diseases, there were more comprehension of diseases as age gets higher, but most did not understand dementia and paralysis. Past experience and process of medical treatment for their disease were such arthritis, paralysis, broken bone, and dislocation of joint. Prognosis of technical training was mostly answered positively, but linking with other related disease programs were not welcomed. Most respondents thought that professional training were needed. The results of understanding diseases during senescent had high percentage of understanding as age and education was high, and it had eye upon statistics of the survey (p < 0.05) Most of those questioned responded that it is appropriate to have six working days per week and eight hours of daily work, however, present wage is not reasonable. Most of those questioned had negative response of work independence, and had an-swered that there should be certificates needed in related fields. For difficulties during work period, they had hardship instituting direction of treatment by lack of program, level of wage, and physical difficulties. Physical therapists averagely wished to have fewer than thirty patients per therapist. After the research, physical therapists who work in those facilities of elderly care systems agreed on policies, though it should not lower service quality. In addition, they were pros to have elderly care system, yet they are still inevitable option to maintain living. For work environment, the period of work was appropriate, but had different opinions for work independence and regular number of patients. Therefore, there should be more organized treatment program that also should be linked to other practical programs at this beginning point of the policy enforcement and the time that many therapists hope to work in elderly care system.
