동의보감(東醫寶鑑) 중 치자(梔子)가 배오(配伍)된 방제(方劑)의 활용(活用)에 대한 고찰(考察) (A Study on the prescriptions including Gardenia fructus in Dongeuibogam)

  • Objectives : This report describes 241 prescriptions related to the use of Gardenia Fructus blended prescriptions from Dongeuibogam. Methods : The following conclusions were reached through investigations on the prescriptions that use Gardeniae Fructus. Results : The Gardeniae Fructus is used in 49 fields of treatment such as urinary diseases, jaundice and furuncle, etc. Prescriptions that utilize Gardeniae Fructus are used in the treatment of fever, eye disease, hemorrhage, skin disease, ear disease, internal disease and so on. In the view of the causative agent of a disease, the prescriptions which are compounded with Gardeniae Fructus are related to endogenous agents such as jaundice, seven emotion, alcohol, exogenous agents such as cold, wind, heat and non-endo-exopathogenic factors like disease due to external factor, injury. The dosage of Gardeniae Fructus is 1pun(about 0.375) to 5don 5pun(about 21g), however 5pun(about 1.87g)~1don(about 3.75g) has been taken the most for clinical application. Conclusions : Hwangryunhaedok-tang is the most base prescriptions which use the Gardenia Fructus. Gardeniae Fructus is mainly used as a raw material.

안와파열골절 환자에서 중증 외상성 안구내 손상 발생과 연관인자 분석 (Severe Traumatic Intraocular Injuries Related to Blowout Fractures)

  • Purpose: Blunt trauma can cause a wide range of ocular injuries. This study was performed to describe the prevalence of severe intraocular injuries (SIOI) and their correlation with the severity of blunt orbital trauma. Methods: We retrospectively analyzed 117 eyes of 107 patients with orbital wall fractures who visited the emergency room at Konyang University Hospital from July 2006 to June 2008. Clinical features such as age, sex, causes of injury, revised trauma score (RTS), type of orbital wall fractures were recorded. The patients were divided into two groups: blowout fracture with severe intraocular injuries (SIOI) and blowout fracture without SIOI. We compared the clinical and the injury-related characteristics between two groups and analyzed the SIOS-related factors. Results: Among the 107 patients (117 eyes) with blowout fractures, 29 (27.1%) patients with 32 eyes (25.6%) had complicated severe intraocular injuries. Retrobulbar hemorrhage (14.5%), hyphema (13.7%), traumatic optic nerve injury (4.3%), and sustained loss of visual acuity (4.3%) were the most common SIOI disorders. A logistic regression analysis revealed that loss of visual acuity (odds ratio = 4.75) and eyeball motility disorder (odds ratio=7.61) were significantly associated with SIOS. Conclusion: We suggest that blowout fracture patients with loss of visual acuity or eyeball motility disorder are mostly likely to have severe intraocular injuries, so they need an ophthalmologic evaluation immediately.

Sarcocystosis among Wild Captive and Zoo Animals in Malaysia

  • Sarcocystis sp. infection was investigated in 20 necropsied captive wild mammals and 20 birds in 2 petting zoos in Malaysia. The gross post-mortem lesions in mammals showed marbling of the liver with uniform congestion of the intestine, and for birds, there was atrophy of the sternal muscles with hemorrhage and edema of the lungs in 2 birds. Naked eye examination was used for detection of macroscopic sarcocysts, and muscle squash for microscopic type. Only microscopically visible cysts were detected in 8 animals and species identification was not possible. Histological examination of the sections of infected skeletal muscles showed more than 5 sarcocysts in each specimen. No leukocytic infiltration was seen in affected organs. The shape of the cysts was elongated or Circular, and the mean size reached $254{\times}24.5{\mu}m$ and the thickness of the wall up to $2.5{\mu}m$. Two stages were recognized in the cysts, the peripheral metrocytes and large numbers of crescent shaped merozoites. Out of 40 animals examined, 3 mammals and 5 birds were positive (20%). The infection rate was 15% and 25% in mammals and birds, respectively. Regarding the organs, the infection rate was 50% in the skeletal muscles followed by tongue and heart (37.5%), diaphragm (25%), and esophagus (12.5%). Further ultrastructural studies are required to identify the species of Sarcocystis that infect captive wild animals and their possible role in zoonosis.

Hypertensive Retinopathy and the Risk of Hemorrhagic Stroke

  • Objective : Hemorrhagic stroke (HS) and hypertensive retinopathy are known end organ damage of the brain and eye respectively, with HS having deleterious consequence to the patients. This study is to correlate between hypertensive retinopathy and HS in hypertensive disease. Methods : A control group of hypertensive patients only, and an investigated group of hypertensive HS patients. Fundoscopic examination to determine the grade of retinopathy was performed and then divided into low or high severity hypertensive retinopathy. Clinical and radiological parameter included are demography, vital signs, Glasgow coma scale (GCS) on admission, clot volume, site of clot, Intracerebral hemorrhage (ICH) score and Glasgow outcome scale (GOS). Data were correlated with the severity of hypertensive retinopathy. Results : Fifty patient in the control group and 51 patients in the investigated group were recruited. In the hypertensive HS group, 21 had low severity retinopathy (no or mild retinopathy) accounting for 41.2% and 30 patients had high severity (moderate or severe retinopathy). In the hypertensive patients 49 had low severity and one had high severity (p-value of 0.001). In HS group low severity showed better GCS score of 9-15 on admission (p-value of 0.003), clot volume less than 30 mL (p-value 0.001), and also a better 30 days mortality rate by using the ICH score (p-value 0.006), GOS score of 4 and 5 the low severity retinopathy fair better than the high severity retinopathy (p-value of 0.001), and the relative risk to develop HS in low severity and high severity retinopathy was 0.42 and 29.4, respectively. Conclusion : Hypertensive retinopathy screening could be used as an indicator in hypertensive patient, to evaluate the risk of developing hypertensive HS in the future.

청열탕(淸熱湯)의 생물전환을 통한 항아토피피부염 효능(效能) 연구(硏究) (Effect of Cheongyeoltang (CYT) using Bioconversion on Atopic Dermatitis)

  • In order to improve efficacy of oriental medicines and to study the application of fermented oriental medicine in clinicals, the efficacy of CYT and CYTBH on atopic dermatitis were evaluated. The results and conclusions are as follows. CYT and CYTBH significantly improved the atopic dermatitis symptoms in NC/Nga mice by naked eye evaluation and significantly decreased clinical index in both groups. CYT and CYTBH both decreased the cell numbers of CD3+, CD11b+Gr-1+ cells in dorsal skin. Of the cells, CYT significantly decreased CD11b+Gr-1+ cells whereas CYTBH significantly decreased all immune cells. CYT and CYTBH both decreased the production rate of IL-4 and IFN-${\gamma}$ activated by CD3/CD28. In the case of CYTBH, significant decrease in all cases was observed. CYT and CYTBH decreased the production rate of IL-5, IL-13 and IL-17 in serum. Significant decrease of IL-5 in the case of CYT and IL-5 and IL-13 in the case of CYTBH were observed. CYT and CYTBH significantly decreased transcription of IL-5 mRNA and IL-13 mRNA in skin. Significant decrease in IgG1 and IgE immunoglobulins in serum were oberved in both groups. Significant decrease was only observed in the case of CYTBH. Both CYT and CYTBH significantly decreased the secretion of histamine. Both CYT and CYTBH suppressed erythema, hemorrhage, edema, excoriation, erosion of skin tissues of NC/Nga mice resulting in the decrease of thickness of epidermis. Significant decrease of infiltration of obese cells was also observed. The results above indicated that both CYT and CYTBH had significant efficacy in the treatment of atopic dermatitis through immune modulation. Animal studies showed that CYTBH had superior activity than that of CYT suggesting further and continuous studies on the changes in ingredients or absorption improvement by fermentation should follow.

동의보감(東醫寶鑑)에 수록(收錄)된 인삼(人蔘)이 주약(主藥)으로 배오(配伍)된 방제(方劑)의 활용(活用)범위, 병증, 주치(主治), 병리(病理) 및 구성내용(構成內容) 조사(調査) (Studies on Therapeutic range, Symptom, Pathology, and composition of Ginseng Radix -main blended Prescriptions from Donguibogam)

  • In the Encyclopedia Medica Koreana(Dongeuibogam), I have researched 245 prescriptions in which Panax Ginseng plays an important role. And I have got the following results. The healing scope and frequency of ginseng-mainly-included prescriptions are Child Part 29(11.83%), Violent Cough Part 23(9.38%), Sick-by-Cold Part 21(8.57%), Oncosis Part 16(6.53%), Overwork Part 14(5.71%), Gynecologic Part 14(5.71%), Internal Part 13(5.3%), Apoplexy Part 11(4.48%), Mind Part 10(4.08%) and Fecal Part 10(4.08%) prescriptions. And also each of Nausea Part, Anger Part, and Spirit parts has the same 5 (2.04%) prescriptions. And each of Qi Part, Diabetes Meatus Part, Malaria Part, and Humoral Part has 4(1.63%) prescriptions. And each of Foot Part, Choleraic Part, Genital Part, Blood Part, and Voice Part has 3 (1.2%). All of these prescriptions cover 88.88%. And besides listed parts above, Panax Ginseng is all used in 48 Parts: Body-Mind Part. Mouth-Tongue Part, Breast Part, Muscle Part, Swelling Part, Urine Part, Epidermis Part, Heat Part, Anus Part, Stomach Part, Eye Part, Laryngopharynx Part. Uterus Part" Heavy Stomach Part, Head Part, Pulse Part, Hair Part, Navel Part, Emetic Part, Costal Part, Edema Part, Vomiting Part, Superstitious Part, and Cardiac Part, etc. Of the prescriptions in which Panax Ginseng plays an important role, the most representative diseases, which more than 86.8% prescriptions cure, are shock, numbness from cold, Taeeum disease, oncosis, overwork, sick from eating, numbness of extremities, diarrhea, tachycardia, forgetfulness, nausea, heat from kidney, nocturnal emission, short breath, diabetes meatus, malaria, sweating, sweating overnight, beriberi, cholera, insomnia from enervation, sialitis, navel pain, hemorrhage, and loss of voice. The pathology of the prescriptions in which Panax Ginseng plays an important role is divided into the organ problems, six natural factors, seven extreme feelings, unbalanced humoral status, overwork, and, unbalance of qi and blood. Spleen, heart, and uterus is the main cause of organ problems; wind and cold are the main cause of six natural factors; heavy humors are the main cause of unbalanced humoral status; the stasis of seven feelings are the main cause of seven extreme feelings; the lack of stamina and overwork are the main cause of the overwork; the lack of qi, the lack of blood, and, the lack of qi and blood are the main cause of the unbalance of qi and blood. After I have researched the contents of the prescriptions in which Panax Ginseng plays an important role, I could understand the addition of the different prescriptions, combination of medicines, and the role of medicine groups associated with Panax Ginseng. So from now on, the results I have got could be used as the data which show the theoretical basis on the prescriptions.

Classification of Diabetic Retinopathy using Mask R-CNN and Random Forest Method

  • 본 논문에서는 딥러닝 기법의 하나인 Mask R-CNN과 랜덤포레스트 분류기를 이용해 당뇨병성 망막병증의 병리학적인 특징을 검출하고 분석하여 자동 진단하는 시스템을 연구하였다. 당뇨병성 망막병증은 특수장비로 촬영한 안저영상을 통해 진단할 수 있는데 밝기, 색조 및 명암은 장치에 따라 다를 수 있으며 안과 전문의의 의료적 판단을 도울 인공지능을 이용한 자동진단 시스템 연구와 개발이 가능하다. 이 시스템은 미세혈관류와 망막출혈을 Mask R-CNN 기법으로 검출하고, 후처리 과정을 거쳐 랜덤포레스트 분류기를 이용하여 안구의 정상과 비정상 상태를 진단한다. Mask R-CNN 알고리즘의 검출 성능 향상을 위해 이미지 증강 작업을 실시하여 학습을 진행하였으며 검출 정확도 측정을 위한 평가지표로는 다이스 유사계수와 Mean Accuracy를 사용하였다. 비교군으로는 Faster R-CNN 기법을 사용하였고 본 연구를 통한 검출 성능은 평균 90%의 다이스 계수를 통한 정확도를 나타내었으며 Mean Accuracy의 경우 91% 정확도의 검출 성능을 보였다. 검출된 병리증상을 토대로 랜덤포레스트 분류기를 학습하여 당뇨병성 망막 병증을 진단한 경우 99%의 정확도를 보였다.

Behçet 증후군에서 폐침범의 임상양상에 관한 고찰 (Clinical Manifestations of the Lung Involvement in Behçet's Syndrome)

  • 연구배경: Behçet 증후군은 전신적인 장기에 혈관염에 의한 다양한 병변을 일으키는 원인미상의 질환이다. Behçet 증후군에서 폐침범은 약 5%에서 발생하고, 주로 폐동맥염에 의한 폐동맥류, 혈전증, 폐출혈 등으로 나타나며, 이는 예후에 영향을 미치는 중요한 인자가 된다. 이에 저자 등은 Behçet 증후군 환자에서 폐침범의 유병율, 임상적 소견, 방사선 소견, 치료경과 및 예후 등에 관한 전반적인 고찰을 시행해 보고자 한다. 방법: 1986년부터 1995년까지 본원에 입원한 Behçet 증후군 환자 총 487예중 폐침범의 소견을 보인 10예를 대상으로 하였으며, 폐침범의 진단은 중상 및 이학적 소견을 토대로 하여 단순 흉부방사선, 흉부 전산화단층촬영, 폐환기-관류주사 및 폐동맥조영술 등에 의거하였다. 결과: 1) 전체 Behçet 증후군 환자에서 폐침범의 유병률은 2%(10/487) 였다. 평균연령은 34세, 남녀비는 8 : 2 였다. 객혈은 5 예에서 나타났고, 이중 대량객혈은 2예에서 관찰되었다. 다른 호흡기 증상으로는 기침이 6예, 호홉곤란이 4예, 흉통이 2예에서 나타났다. 구강궤양은 전예, 외음부궤양은 9예, 피부병변은 7예, 안병변은 6예에서 나타났다. 2) 검사실 소견상 특이한 것은 없었고, 단순 흉부방사선 소견상 다발성 경화가 6예, 원형 결절 또는 종괴양 음영이 4예에서 발견되었고 2 예는 정상소견이었다. 흉부 전산화단층촬영은 8예에서 시행하였는데 이중 6예에서 다발성 경화의 소견을 보였고, 4예에서 폐동맥류를 관찰할 수 있었다. 폐관류 또는 환기-관류주사는 8예에서 시행하여 7예에서 다발성 결손 및 불일치를 보였고, 특히 정상 흉부 단순방사선소견을 보인 2 예에서 다발성 환기-관류 불일치의 소견을 보였다. 폐동맥조영술은 3예에서 시행하여, 2예에서 폐동맥류를 관찰할 수 있었다. 3) 치료는 colchicine, 스테로이드, cyclophosphamide, 항응고제 등을 사용하였고, 1예에서 폐동맥류의 절제술을 시행하였다. 4) 10예 중 9예를 추적관찰한 결과, 3예가 호전되어 외래 경과관찰 중이며, 2예는 악화되어 가료중이고, 4예가 대량객혈, 대량 폐색 전증 및 패혈증 등으로 사망하였다. 결론: Behçet 증후군의 폐침범은 주로 혈관염에 의한 출혈, 동맥류의 형성, 및 혈전증 등으로 나타나며, 그 발병율은 낮지만 대량객혈 및 호흡부전증 등을 초래하여, 질환의 경과를 악화시키고 사망을 초래할 수 있는 주원인이 되므로, 이의 조기진단이 필요하며 적극적인 치료로서 예후의 향상을 기대할 수 있을 것이다.

안구 내 종양의 진단에 있어서 F-18-FDG PET의 유용성과 검사 방법의 영향 (The Usefulness of F-18-FDG PET and The Effect of Scan Protocol in Diagnosis of Intraocular Tumors)

  • 목적: 안구 내 종양의 치료계획에 있어서 악성여부의 결정은 매우 중요하다. 하지만 안구 내 종양의 생검은 시력의 손상을 가져오므로 임상적으로 권장할 수 없는 방법이다. 저자들은 안구 네 종양의 악성 여부를 진단하는 데 있어서 F-18-FDG PET의 유용성과 검사 방법이 결과에 미치는 영향에 대해 연구하였다. 대상 및 방법: 임상적으로 악성 안구 내 종양이 의심되는 13명의 환자(15병소)를 대상으로 F-18-FDG PET 스캔을 시행하였다. 세 명은 양성의 병소를 가지고 있었고(망막 박리, 맥락막 삼출, 맥락막출혈) 열 명을 열두 개의 악성 병소를 가지고 있었다(흑색종 3, 망막아종 7, 전이암 2). 양성 병소들과 악성 병소들의 크기는 각각 $164{\pm}85mm^3$$598{\pm}718mm^3$이었다. 영상은 GE Advance PET 스캐너(전체반값두께 4.6 mm)를 이용하여 얻었다. 각각의 영상에 대해 매트릭스 크기를 바꾸어 가면서($256{\times}256$$128{\times}128$) 감쇠보정을 하거나 하지 않는 두 가지방법으로 재구성하여 재구성 방법에 의한 영향을 비교하였다. 여덟 명의 환자(양성 3, 악성 병소 6)에서 추가로 전신스캔을 시행하였다. 육안 판독 후 반정량적으로 분석하여 표준섭취계수를 구하여 분석하였다. 결과: 치료하지 않은 악성 병소는 모두 진단할 수 있었고 양성 병소는 육안적으로 관찰되지 않았다. 10개의 악성 병소들의 픽셀당 최대 표준섭취 계수는 $2.64{\pm}0.57g/ml$로 한 예를 제외하고는 모두 2.0 g/ml보다 높았다. 종양의 크기와 표준섭취계수는 통계적인 상관관계가 없었다. 전신스캔에서는 두개의 $1000 mm^3$ 보다 큰 악성 병소는 높은 섭취를 보였으나 중간 크기의 병소들은 희미하게 관찰되었고 $100 mm^3$ 이하의 병소 두 개는 관찰되지 않았다. 결론: F-18-FDG PET는 안구 내 악성 종양의 진단에 있어서 높은 예민도를 보여 주었다. 악성 종양은 모두 육안적으로 진단할 수 있었다. 작은 크기의 안구 내 종양을 진단하기 위해서는 전신스캔은 예민도가 낮아 적당하지 않고 스캔시간을 충분히 길게 하여 안구 국소촬영을 할 필요가 있다. 영상은 $128{\times}128$ 매트릭스보다 $256{\times}256$ 매트릭스로 재구성하는 것이 좋다.
