Saddle Embolism Caused by Left Atrial Myxoma -A Case Report- (안장 색전증을 유발한 좌심방 점액종 -수술 치험 1례-)

  • Systemic emboli occur in approximately one-third of patients with cardiac myxoma. Embolization is common because of the friability of the tumor and intracardiac location. Embolic episodes in young patients with normal sinus rhythm should arouse suspicions of cardiac myxoma in the absence of active endocarditis. We present one case of 17 years old girl having saddle embolism combined with left atrial myxoma. We planned staged operation. First, the emergency thromboembolectomy of aortic bifurcation was performed through bilateral transfemoral approach with use of Forgaty catheter. One week later, the extirpation of myxoma was successfully done with ECC.

  • The management of teeth in the line of a mandibular fracture is controversial despite the general agreement that most of these teeth can be preserved. Teeth should be retained if bony attachments are adequate for survival, the tooth is sound and important in maintaining fixation of the fractured segment of bone. Teeth should be removed if they are loose and interfere with the reduction of fragments, are devitalized and potentially a source of wound infection, are damaged beyond their usefulness or may become devital and interfere with healing by becoming infected. However, tooth removal will increase the level of trauma, extend the severity of the wound and require expensive prosthetic treatment. Therefore, it is very important to conserve infected teeth in the line of a mandibular fracture through early primary endodontic treatment (pulp extirpation, canal enlargement and canal opening drainage) and splinting. The basic principles underlying the treatment of pulpless teeth are those underlying general surgery. Therefore, debridement of the infected wound (pulp extirpation and canal enlargement), drainage (canal opening) and gentle treatment of the tissues (occlusal reduction and teeth splinting) are the principles of surgery. This is a representative case report of conservative care by the early endodontic drainage of infected teeth in the line of a mandibular fracture.

A CASE REPORT ON INTENTIONAL REPLANTATION OF MAXILLARY FIRST BICUSPID (Intentional Replantation에 의한 상악 제 1 소구치 치료의 증례보고)

  • Intentional replantation of the posterior teeth has been performed as a last resort except extraction when it is impossible to perform the conventional endodontic or surgical endodontic treatment. Many studies have done with the prognosis, and root resorption, of which the responsibility might have been injury of periodontal ligament and remaining periodontal lesion, proved to be major cause of failure. Intentional replantation, however, can exclude anatomic difficulty, risk of nerve injury, and maxillary sinus trauma that surgical access can raise, especially in posterior teeth, and it is reported that success rate is not less than surgical endodontics, independent of replantation of traumatically avulsed teeth. The success of intentional replantation can be evaluated with, clinically, reduction of pocket depth, pain and mobility(physical mobility), and, radiographically, hard tissue formation, reduction of periapical radiolucency. In this case report, for the maxillary left first bicuspid with dens evaginatus that had periapical lesion, painfulness, mobility and showed no reduction of mobility after even extirpation and a number of irrigation, extraction followed by extraoral root filling with calcium hydroxide and replantation was performed. Based on many studies, it is concluded that hard tissue formation capacity of calcium hydroxide, preceded extirpation before extraction, and careful extraction without injuring periodontal ligament affected successful results of this case.

  • The prominent malar region has been recognized a sign of youth and beauty in caucasian who generally have a dolichocephalic and long face. But in the orients, especially Koreans who generally have a mesocephalic or brachycephalic face, it is considered an agressive, unesthetic facial appearance. So many patients require the shaving of prominent malar eminence and arch, and many methods of its reduction have been devised. For the exposure of malar complex, infraorbital skin incision, intraoral approach, preauricular approach, supraauricular scalp incision, and coronal approach have been used. And for the reduction of bony structure, direct shaving, contouring and repositioning of the malar complex after extirpation, and medially fracture of zygomatic arch have been used with its own merits. We performed the reduction malarplasty through intraoral approach. After two parallel oeteotomy at medial part of the zygomatic bone, the midsegment is removed. The posterior arch of zygoma was bended or green stick fractured. When more correction was required, the posterior arch was fractured medially through the step incision at skin. This method has a some advantages. Compared with the method for extirpation of malar complex, the infection rate is diminished, the resorption is small because of no free bone graft. And cheek drooping is prevented. Compared with the method of coronal approach, the surgical trauma is minimal. Now we report some cases of reduction malarplasty performed through intraoral approach and disscus the surgical technique and results.

Schwannoma of the Hand: Importance of Differential Diagnosis & Microsurgical Dissection (수부의 신경초종: 감별진단과 미세수술의 중요성)

  • Purpose: The schwannoma is a benign peripheral nerve tumor arising from the Schwann cell of the nerve sheath. Only 2-8% of schwannomas arise in the hand and wrist. Misdiagnosis is frequent such as ganglion and neurofibroma. This article documents and clarifies the clinical features of schwannomas arising in the hand and wrist, and emphasizes importance of differential diagnosis and meticulous surgical extirpation under magnification. Methods: The author reviewed clinical features of 15 patients with pathologic final diagnosis of schwannoma developed in hand and wrist during the last 12 years from 1998 through 2009. The review included the sex, age of onset, duration, preoperative diagnosis, location, involved nerve, preoperative symptoms and. Postoperative sequelae after surgical extirpation of the lesion with magnification, or without magnification of the surgical fields. Results: The chief complaints were slow growing firm mass in all patients, and followed by pain in 40%, and paresthesia in 40% respectively. The lesions were developed solitarily in 14 patients (93%). The postoperative pathologic diagnosis and preoperative diagnosis were coincided with only in 6 patients (40%). Other preoperative diagnosis were soft tissue tumor in 4 patient (26.6%), and ganglion in 3 patients (20%), and neurofibroma in 2 patients (13%). In all patients who were undergone surgical excision under the fields of magnification, all symptoms were subsided without any sequelae. Meanwhile muscle weakness, paresthesia, hypoesthesia and /or accidental nerve resection developed after surgical excision with naked eye. Conclusion: Schwannoma in hand most commonly appears as a slow growing solitary mass with pain or paresthesia. The chance of preoperative misdiagnosis was 60% in this series. To provide good prognosis and less sequelae, careful and elaborate diagnostic efforts and meticulous surgical excision under the magnification are necessary in management of schwannoma.

The effects of gingival blood flow on pulpal blood flow detection using ultrasound Doppler flowmetry: animal study

  • Objectives: This study evaluated the effect of adjacent gingival blood flow on detection of pulpal blood flow (PBF) using ultrasound Doppler flowmetry (UDF) through animal study. Materials and Methods: The study included 36 right and left maxillary the third incisors and canines in 9 experimental dogs. The study included 2 main steps: In the first step, the pulse sound level (PSL) was recorded on the cervical part of each tooth without flap elevation (Group 1), with flap elevation (Group 2), and after it was repositioned in place (Group 3). In the second step, the PSL was recorded on the cervical part of each tooth (Group 4), after pulpotomy (Group 5), after partial pulp extirpation (Group 6), after complete extirpation (Group 7), and after canal filling (Group 8). In Groups 5-8, the study was performed with and without flap elevation in the left and right teeth, respectively. The PSL was graded as follows: 0, inaudible; 1, heard faintly; and 2, heard well. The difference between each group was analyzed using Friedman's test with Wilcoxon signed-rank tests (α = 0.05). Results: In step 1, the PSL results were Group 1 > 2 and 3. In step 2, there was no significant difference between the groups when the flap was not elevated, while PSL results were Group 4 > 5 ≥ 6 and 7 ≥ 8 when the flap was elevated. Conclusions: PBF is affected by gingival blood flow when measured with UDF. UDF measurements require isolation of gingiva from the tooth.

  • 타석증을 동반한 만성 악하선 타액선염의 경우 악하선 적출은 일반적인 치료 방법이다. 일반적으로 악하선 적출술은 수술의 용이성, 수술시야의 확보 등의 장점으로 구강외접근에 의해서 시행되어져 왔다. 그러나 시대적으로 심미적인 요구가 증대되고 있는 요즈음 구외 접근으로 인한 술후 반흔은 구외접근법의 가장 결정적인 단점이라 할 수 있다. 또한 설하선과 함께 적출하여야 할 경우 구내접근과 구외접근을 동시에 실시할 경우 술후 치유과정상의 후유증을 유발할 수도 있다. 반면에 구강내 접근에 의한 악하선의 적출은 수술상의 고도의 난이도로 수술자체의 어려움은 있으나 구강의 반흔을 남기지 않아 심미적으로 만족할 만한 결과를 얻을 수 있다. 이에 본 저자 등은 타석증을 동반한 악하선 타액선염의 환자에 있어 구내 접근으로 악하선 및 설하선 적출술을 시행하여 좋은 결과를 얻었기에 문헌고찰과 함께 보고하는 바이다.

The Study on the States of Treatment in Pedodontic Field (소아치과 치료양상에 관한 고찰)

  • 72559 children, who visited the Dept. of Pedodontics, SNU Hospital from 1982 to 1985 were surveyed on the yearly tendency of dental treatment. The data were compared with those of previous reports, and the results were as follows, 1. The number out-patients were decreasing year by year. 2. Compared with those of 1982, in 1985, total number of visiting patient decreased to 14%, amalgam filling, 73.8% vital pulpotomy ; 69.55%, gold inlays ; 42.57%, Space maintainer ; 69.55%, tooth extraction ; 63.99% on the other hand, some items increased such as pulp extirpation ; 192%, canal filling ; 290%, 3. There revealed overall increasing tendency in almost all treatment items except Gold Inlay7s in from 1982 to 1985, compared with those from 1976 to 1979.

  • Since the upper lip does not receive direct actinic radiation, only 5% of lip tumors develop in the upper lip, while the lower lip is the site of the remainder. Among the lower lip cancer, squamous cell carcinoma is the most common tumor, especially the vermillion border of lower lip is the most common site. The aims of reconstruction of the lip are both aesthetic effect and functional restoration and the ideal procedure must produce a aesthetically normal, not-tao-tight lip and a good sensation and muscle tone of the lip. We have a satisfactory reconstruction of a subtotal loss of lower lip after squamous cell carcinoma extirpation using Gillies fan flap and the case is presented with reviewing a many published reports.

Intralesional OK-432 Sclerotherapy for Plunging Ranula (OK-432 경화요법으로 치유된 Plunging Ranula)

  • The incidence of plunging ranula has been reported as about 10% of all ranula cases. Treatment consists of wide surgical excision or marsupialization. However, it seldom provides a satisfactory result and various modalities of treatment have been proposed in addition to surgical extirpation. We reported our experience using intralesional OK-432 sclerotherapy for a case of plunging ranula in 20 year-old male patient. The OK-432 solution was prepared by dissolving 0.1 KE in 2ml of physiologic saline and was instilled in the same amount of the aspirated cystic fluid. Following two instillations, the lesion completely disappeared. The side effects were mild fever for 3 days and a mild local induration for several days. At present, 9 months after instillation, no recurrence was observed. OK-432 sclerotherapy appears to be simple, safe and effective method for the treatment of plunging ranula.
