EVALUATION OF THE ACCURACY OF THE APEX FINDER A.F.A. (APEX FINDER A.F.A.의 정확성에 관한 연구)

  • Recently electronic apex locators have been used widely in root canal treatment, but, accuracy of electronic apex locators is controversial. The purpose of this study was to evaluate the accuracy of Apex Finder A.F.A(EIE Analytic Technology, U.S.A.) in vivo compared with Root-Zx and radiograph. The root canal lengths were determined with Root-Zx(32 tooth) in before pulp extirpation and after pulp extirpation. Then the radiographs were taken with a file in the canal. The root canal lengths were determined with Apex Finder A.F.A.(21 tooth) in before pulp extirpation and after pulp extirpation and under NaOCl. Then the radiographs were taken with a file in the canal. The results were as follows: 1. There was no significant statistical difference in Root-Zx between before pulp extirpation and after pulp extirpation(p > 0.05). 2. There was no significant statistical difference in Apex Finder A.F.A. between before pulp extirpation and after pulp extirpation(p > 0.05). But, there was significant statistical difference under NaOCl(p < 0.05). 3. There was no significant statistical difference in accuracy between Root-Zx and Apex Finder A.F.A.

Hemocyte Changes after the Extirpation of the Hemopoietic Organ-wing Disc Complexes in the Silkworm, Bombyx mori (Lepidoptera: Bombycidae)

  • We successfully extirpated all four hemopoietic organ-wing disc complexes of the fifth instar larvae of Bombyx mori, and found that most of the treated silkworms could still develop into the moths. We investigated the changes of the circulating hemocytes and evaluated the effects of extirpation on the hemopoiesis. The results showed that proliferation of circulating hemocytes was sufficient to allow development of the silkworms which complexes were totally extirpated. We also found that hemopoietic organ-wing disc complexes extirpation might cause a certain hemopoietic compensation of the remainder complexes during early spinning. Exogenous hormones such as $20-{\beta}-hydroxyecdysone$ and juvenile hormone analog had a positive effect on hemocytes proliferation.

Long-term outcomes after core extirpation of fibrous dysplasia of the zygomaticomaxillary region

  • Background: Fibrous dysplasia (FD) is a localized bone disorder in which fibro-osseous tissue replaces the normal bone structure. Patients with craniofacial FD often present with gradual swelling, deformity, and compromised vision or hearing. We previously introduced "the core extirpation method," a novel surgical technique that is minimally invasive like traditional bone shaving but has longer-lasting effects. This study presents the long-term outcomes of our core extirpation method. Methods: We conducted a retrospective analysis of patients who underwent core extirpation for FD of the zygomaticomaxillary region from 2012 through 2021. Computed tomography (CT) scans were performed 6 to 12 months before the operation, immediately before and after the operation, and during follow-up visits. We performed all operations using the upper gingivobuccal approach, and we extirpated the core of the lesion while preserving the cortical structures of the zygoma and the maxilla to maintain symmetrical facial contour. Results: In 12 patients with lesions in the growth phase, anteroposterior/mediolateral (AP/ML) length discrepancies and the volume increased between preoperative and immediate postoperative CT scans. All patients' immediate postoperative AP/ML discrepancies were stable up to 12-17 months postoperatively. Postoperative volume showed continuous lesion growth; the median volume growth rate was 0.61 cc per month. Conclusion: In this article, we present our experiences managing FD using the minimally invasive core extirpation technique, which entails small expected blood loss and can be performed as day surgery. It provides similar cosmetic outcomes as traditional bone shaving but with longer-lasting results. Although there are some limitations with the study's retrospective nature and small sample size, our 4-year follow-up results show promising results of the core extirpation method in well-indicated patients.

CHANGE OF VASOACTIVE INTESTINAL POLYPEPTIDE(VIP) IMMUNOREACTIVE CELLS FOLLOWING PULP EXTIRPATION IN RAT TRIGEMINAL GANGLION: A CONFOCAL LASER SCANNING MICROSCOPIC STUDY (치수제거 후 흰쥐 삼차신경절에서 VIP 면역반응세포의 변화: 공초점레이저주사현미경적 연구)

  • The purpose of this study was to investigate the distribution and fluorescence intensity of vasoactive intestinal polypeptide immunoreactive (VIP-IR) cells in rat trigeminal ganglion following pulp extirpation of rat mandibular molar. The animals were divided into control group(n=6) and experimental group(n=6). The experimental animals were sacrificed at 14 days after pulp extirpation. The trigeminal ganglion was removed and immersed in the 4% paraformaldehyde in 0.1M phosphate buffer. Serial frozen sections about $20{\mu}m$ in thickness were cut with a cryostat. The immunofluorescence staining was performed. The rabbit anti-VIP(1 : 8,000) was used as primary antibody and fluorescene isothiocynate(FITC) conjugated anti-rabbit IgG(1 : 80) as secondary antibody. The slides were observed under confocal laser scanning microscope (CLSM). Unprocessed optical sections were obtained and stored on a optical disk. Color pictures were printed by a video copy processor. The results were as follows; 1. The positive ratio of VIP-IR cells in mandibular part of trigeminal ganglion were 7.40% in control group and 28.42% in experimental group(14 days after pulp extirpation). 2. The relative fluorescence intensity of VIP-IR cells in mandibular part of trigeminal ganglion were 87.78 in control group and 138.65 in experimental group. The relative fluorescence intensity of experimental group was 58% higher than that of control group. 3. In optical serial section analysis of VIP-IR cells of experimental group, most of the 9 sections showed high fluorescence intensity. At high magnification, axons of the experimental group displayed greater VIP-IR than in the control group, and the positive cells were mainly of medium size. The result indicate that number and fluorescence intensity of VIP-IR cells were increased in the mandibular part of trigeminal ganglion following pulp extirpation of mandibular molar, and it suggests that VIP could play a role in processing of nociception.

Microsurgical Extirpation of Intracordal Cyst with a Microflap Technique (미세피판기법을 이용한 성대낭종제거술)

  • Background and Objectives : Microsurgical resection of intracordal cysts is technically difficult and challenging because the wall of cysts may be tightly attached to underlying vocal ligament and/or overlying epithelium, and therefore their thin wall will easily rupture during surgical dissection. We aimed to evaluate the voice outcomes of standard microflap subepithelial resection and the recurrence rate depending on the intraoperative rupturing of the cyst. Materials and Methods : Medical records of Samsung Medical Center, Seoul, Korea, were reviewed for sixty-four consecutive patients who received surgical resection of vocal cyst using microflap subepithelial dissection technique between the year 2004 and 2013. Meticulous dissection was performed to completely remove the cyst wall while preserving the mucosa and the lamina propria as much as possible. Voice outcomes and recurrence rates were compared according to the type, size and the intraoperative rupture of cyst. Results : Presence or absence of cyst rupture was clearly described in the operation records of 41 patients. Intraoperative rupture of the cyst occurred in 32 of 41 (78%) patients. The recurrence was detected in 5 of 64 (7.8%) total cases and 4 of 32 (12.5%) cases of ruptured cyst, but not in 9 cases of intact extirpation. Rupture was more common in case of mucous retention cyst compared with epidermoid cyst (p=0.036). Subjective and objective voice parameters were measured at before and 3 months after surgery, which improved regardless of the cyst rupture. Conclusion : Although complete microsurgical extirpation of intracordal cyst while keeping the cyst wall intact is technically difficult, meticulous dissection with maximal preservation of surrounding tissue may warrant the improvement of voice outcomes.

A study on recognition and practice of dental hygiene students for Infection control dental hygiene major courses (치위생과 학생의 감염관리에 대한 인지도와 실천도 조사)

  • The purpose of the study was to provide the basic for the prevention of infection control by analyzing the level of recognition and practice of dental hygiene students. It is important for dental clinic staffs to know and practice infection control measures in favor of effective infection control. Thus, this study conducted an one-month questionnaire survey (during October 2008) for dental hygiene students who ever experienced in 16-week clinical practice, and analyzed total 206 questionnaire forms as collected from them, so that it could determine potential associations between their recognition and practice of infection control and thereby suggest an efficient solution for infection control in dental hygiene. 1. It was found that our students' recognition about infection control averaged $4.49{\pm}.58$ points on the whole, and they were best aware of instrumental disinfection and sterilization among other measures, which was followed by hand washing, extirpation control, post-sterilization management, personal protective outfit and instrumental surface maintenance respectively. 2. It was found that our students' practice of infection control averaged $3.85{\pm}.57$ points on the whole, and they practiced hand washing most actively among other measures, which was followed by extirpation control, post-sterilization management, instrumental disinfection and sterilization, personal protective outfit and instrumental surface maintenance respectively. 3. It was found that our students scored higher mean points in recognition about every measure of infection control than those in practice (t=15.676, p=0.000). Particularly, it was notable that there were significant differences between their mean points in recognition and practice of some infection control measures, such as instrumental surface maintenance (t=15.361, p=0.000), personal protective outfit (t=15.245, p=0.000) and instrumental disinfection and sterilization (t=11.169, p=0.000). But there was least significant difference between mean points in recognition and practice of hand washing (t=5.460, p=0.000). 4. For potential associations between recognition and practice of infection control, it was found that higher recognition was in significantly positive associations with higher practice in every measure of infection control (r=.478, p=.000), such as extirpation control (r=.630, p=.000), instrumental disinfection and sterilization (r=.477, p=.000) and post-sterilization management (r=.433, p=.000). 5. It was found that there were differences in our students' recognition depending upon availability of infection control guideline (t=4.587, p=.011), and there were significant differences in practice depending on necessity of infection control education on a statistical basis (t=2.229, p=.027). Overall, it is found that our dental hygiene students are very likely to practice hand washing and extirpation control, because both of these measures are relatively easy to practice or are considered legally binding.

  • Thymic cyst is a relatively uncommon,usually asymptomatic benign lesion situated in the anterior mediastinum or in the neck,and sometimes in both regions. This is a report of two cases of congenital thymlc cyst in a 18 year old girl and a 43 tear old man. Those were treated with extirpation of the cystic lesion by thoracotomy successfully and had good postoperative results.

  • Cystic intrathoracic lesions of foregut origin are now well recognized and account for approximately 10% of lesions presenting as mediastinal tumors. The terminology used to describe mediastinal endodermal cysts has been confused and sometimes ambiguous. The embryological derivation of these lesions has been the cause of much speculation. It Is suggested that these lesions should be classified Into three main categories based on embryology bronchogenic cyst[resulting from a defect of lung budding], Intramural esophageal cyst[true duplication], and enteric cyst[resulting from the split notochord syndrome]. This communication describes a 26 year old man with intramural esophageal cyst who was diagnosed as posterior medlastlnai tumor preoperatively and cured with extirpation of the cyst.
