Comprehensive understanding of vascular anatomy for endovascular treatment of intractable oronasal bleeding

  • Oronasal bleeding that continues despite oronasal packs or recurs after removal of the oronasal packs is referred to as intractable oronasal bleeding, which is refractory to conventional treatments. Severe craniofacial injury or tumor in the nasal or paranasal cavity may cause intractable oronasal bleeding. These intractable cases are subsequently treated with surgical ligation or endovascular embolization of the bleeding arteries. While endovascular embolization has several merits compared to surgical ligation, the procedure needs attention because severe complications such as visual disturbance or cerebral infarction can occur. Therefore, comprehensive understanding of the head and neck vascular anatomy is essential for a more effective and safer endovascular treatment of intractable oronasal bleeding.

  • Knowledge of variations of veins of head and neck in relation to internal jugular, external jugular, anterior jugular and facial veins is important to surgeons doing head and neck surgery as well as to radiologists doing catheterization and to clinicians in general. In the current case, congenital atresia of the left internal jugular vein is reported. During neck dissection, superficial veins of neck and left external jugular vein was engorged and the left internal jugular vein was look like a fibrotic band. The left common carotid artery and vagus nerve was normal.

Is Robot-Assisted Surgery Really Scarless Surgery? Immediate Reconstruction with a Jejunal Free Flap for Esophageal Rupture after Robot-Assisted Thyroidectomy

  • Esophageal perforation is a rare but potentially fatal complication of robot-assisted thyroidectomy (RAT). Herein, we report the long-term outcome of an esophageal reconstruction with a jejunal free flap for esophageal rupture after RAT. A 33-year-old woman developed subcutaneous emphysema and hoarseness on postoperative day1 following RAT. Esophageal rupture was diagnosed by computed tomography and endoscopy, and immediate surgical exploration confirmed esophageal rupture, as well as recurrent laryngeal nerve injury. We performed a jejunal free flap repair of the 8-cm defect in the esophagus. End-to-side microvascular anastomoses were created between the right external carotid artery and the jejunal branches of the superior mesenteric artery, and end-to-end anastomosis was performed between the external jugular vein and the jejunal vein. The right recurrent laryngeal nerve injury was repaired with a 4-cm nerve graft from the right ansa cervicalis. Esophagography at 1 year after surgery confirmed that there were no leaks or structures, endoscopy at 1 year confirmed the resolution of vocal cord paralysis, and there were no residual problems with swallowing or speech at a 5-year follow-up examination. RAT requires experienced surgeons with a thorough knowledge of anatomy, as well as adequate resources to quickly and competently address potentially severe complications such as esophageal rupture.

  • Objective : We have studied the part, definition, diagnostic generaliztion and pathological viewpoint of Qi mouth and Renying by refering literatures Methods : I refered to records from anciant to modern. The results are followings : 1. Qi mouth and Renying is ancient diagnostic method and the origin of diagnostic method through the pulsation of radial artery. And Qi mouth is recognized as the right and left Qi mouth and Renying is located on pulsation points of both common carotid arteries. 2. The yin of five viscera, nutritional blood and viscous liquid are diagnosed by Qi mouth which is inclued lung meridian. And the yang of six organs, guarding Qi and the rise and fall of stomach energy are diagnosed by Renying which is inclued stomach meridian. 3. It is possible that the position, process and transformational condition of disease are decided by comparing and examining Qi mouth and Renying.

A Study on the Effects of Needle Electrode Electrical Stimulation on the Number of c-Fos Response Cells and c-Fos Expression in the Global Ischemic Rats

  • c-Fos is known to related to synaptic plasticity and apoptosis in damage from ischemia or external injury. The purpose of this study was to investigate whether needle electrode electrical stimulation(NEES) is effective in increasing the number of c-Fos response cells and c-Fos expression in striatum after global ischemia in rats. There were no treatment and occlusion in the control group, global ischemia(GI) group were no treatment after carotid artery occlusion, and needle electrode electrical stimulation(NEES) group were treated with NEES after GI induced. The number of striatum c-Fos response cells and c-Fos protein expression significantly decreased in the NEES group compared to the GI group after 12, 24, 48 hours. The results of the present study suggest that NEES is ineffective in improving global ischemia in rats and may also be ineffective in the globally ischemic human brain.

  • This study was carried to identify the component of Large Intestine Meridian Muscle in human, dividing into outer, middle, and inner part. Brachium and antebrachium were opened widely to demonstrate muscles, nerve, blood vessels and the others, displaying the inner structure of Large Intestine Meridian Muscle. We obtained the results as follows; 1. Meridian Muscle is composed of the muscle, nerve and blood vessels. 2. In human anatomy, it is present the difference between a term of nerve or blood vessels which control the muscle of Meridian Muscle and those which pass near by Meridian Muscle. 3. Arterial Spin Labeling MRI for Quantitative Assessment of Cerebral Perfusion Before and After Cerebral Revascularization in Children with Moyamoya Disease

  • Objective: To determine the correlation between cerebral blood flow (CBF) on arterial spin labeling (ASL) MRI and the degree of postoperative revascularization assessed on digital subtraction angiography in children with moyamoya disease (MMD). Materials and Methods: Twenty-one children (9 boys and 12 girls; mean age, 8.4 ± 3.6 years; age range, 3-16 years) with MMD who underwent both pseudocontinuous ASL MRI at 1.5T and catheter angiography before and after superficial temporal artery encephaloduroarteriosynangiosis were included in this retrospective study. The degree of revascularization in the middle cerebral artery (MCA) territory was evaluated on external carotid angiography and was graded on a 3-point scale. On ASL CBF maps, regions of interest were manually drawn over the MCA territory of the operated side at the level of the centrum semi-ovale and over the cerebellum. The normalized CBF (nCBF) was calculated by dividing the CBF of the MCA territory by the CBF of the cerebellum. Changes in nCBFs were calculated by subtracting the preoperative nCBF values from the postoperative nCBF values. The correlation between nCBF changes measured with ASL and the revascularization grade from direct angiography was evaluated. Results: The nCBF value on the operated side increased after the operation (p = 0.001). The higher the degree of revascularization, the greater the nCBF change was: poor revascularization (grade 1), -0.043 ± 0.212; fair revascularization (grade 2), 0.345 ± 0.176; good revascularization (grade 3), 0.453 ± 0.182 (p = 0.005, Jockheere-Terpstra test). The interobserver agreement was excellent for the measured CBF values of the three readers (0.91-0.97). Conclusion: The nCBF values of the MCA territory obtained from ASL MRI increased after the revascularization procedure in children with MMD, and the degree of nCBF change showed a significant correlation with the degree of collateral formation evaluated via catheter angiography.

The Usefulness of Extradural Anterior Clinoidectomy for Low-Lying Posterior Communicating Artery Aneurysms : A Cadaveric Study

  • Objective : To confirm the usefulness of the extradural anterior clinoidectomy during the clipping of a low riding posterior communicating artery (PCoA) aneurysm through cadaver dissection. Methods : Anatomic measurements of 12 adult cadaveric heads (24 sides total) were performed to compare the microsurgical exposure of the PCoA and internal carotid artery (ICA) before and after clinoidectomy. A standard pterional craniotomy and transsylvian approach were performed in all cadavers. The distance from the ICA bifurcation to the origin of PCoA (D1), pre-anterior clinoidectomy distance from the ICA bifurcation to tentorium (D2), post-anterior clinoidectomy distance from the ICA bifurcation to tentorium (D3), pre-anterior clinoidectomy distance from the tentorium to the origin of PCoA (D4) and post-anterior clinoidectomy distance from the tentorium to the origin of PCoA (D5) and the distance of the ICA obtained after anterior clinoidectomy (D6) were measured. We measured the precise thickness of the blade for the Yasargil clip with a digital precision ruler to confirm the usefulness of the extradural anterior clinoidectomy. Results : Twenty-four sites were dissected from 12 cadavers. The age of the cadavers was 79.83±6.25 years. The number of males was the same as the females. The space from the proximal origin of the PCoA to the preclinoid-tentorium (D4) was 1.45±1.08 mm (max, 4.01; min, 0.56). After the clinoidectomy, the space from the proximal origin of the PCoA to the postclinoid-tentorium (D5) was 3.612±1.15 mm (max, 6.14; min, 1.83). The length (D6) of the exposed proximal ICA after the extradural clinoididectomy was 2.17±1.04 mm on the lateral side and 2.16±0.89 mm on the medial side. The thickness of the Yasargil clip blade used during the clipping surgery was 1.35 mm measured with a digital precision ruler. Conclusion : The proximal length obtained by performing an external anterior clinoidectomy is about 2 mm, sufficient for proximal control during PCoA aneurysm surgery, considering the thickness of the aneurysm clips. In a subarachnoid hemorrhage, performing an extradural anterior clinoidectomy could prevent a devastating situation during PCoA aneurysm clipping.

체위변화가 두부 및 하지의 분시혈류량에 미치는 영향 (Changes of Minute Blood Flow in the Large Vessels during Orthostasis and Antiorthostasis, before and after Atropine Administration)

  • 기립 및 도립의 체위면화가 두부 및 하지의 혈액순환계에 미치는 영향을 관찰하고, atropine의 정주에 의한 부교감신경의 차단이 체위변화에 대한 심맥관계의 내성을 증가시키는 지를 보기위하여 개를 경사대에 수평위로 고정하여 수동적으로 기립 및 도립위로 체위를 변화시키고, 각 체위에서 10분간 유지시켜 경동맥, 외경정맥, 고동맥 및 고정맥의 분시 혈류량, 분시 심박수 및 분시 호흡수, 그리고 혈액의 pH, $PCO_2$, $PO_2$ 및 Hct를 측정하였다. 다시 수평위에서 atropine 0.5mg을 1회 정주한 후 위의 실험과정을 반복하여 시행하고 atropine투여전과 비교 관찰하였던 바 다음과 같은 결과를 얻었다. 기립시 두부 및 하지 동정맥혈의 분시 혈류량은 감소하였고, 특히 두부로 가는 혈류량의 감소가 더 컸으나, atropine의 투여는 경동맥의 분시 혈류량의 감소를 억제하였다. 도립시 두부 및 하지의 분시 혈류량은 유의한 변화를 보이지 않았고, 개체에 따라 변화양상도 다르게 나타났다. atropine의 투여는 투여전에 비하여 유의한 차이를 보이지 않았다. 분시 심박수는 기립 및 도립시 모두 증가하였다. atropine의 투여는 수평위에서 분시 심박수는 투여전 보다 증가하나, 체위변화에 의한 분시 심박수의 증가를 경감시켰다. 분시 호흡수는 개체에 따라 변화양상에 차이가 있으나, 대체로 기립시는 감소하고 도립시는 증가하였다. atropine의 투여는 기립 및 도립시 다 같이 분시 호흡수의 변화를 억제하였다. 혈액소견은 기립시 정맥철의 pH 및 $PO_2$는 감소하였고, $PCO_2$는 증가하나 동맥혈의 $PCO_2$는 감소하였다. 도립시 동정맥혈의 소견은 수평위에 비해 별 변화가 없었으며 , atropine의 투여후도 기립 및 도립시 모두에서 투여전에 비하러 유의한 변하는 없었다. Hct는 기립 및 도립시 증가하였으며 atropine투여에 의한 변화는 관찰할 수 없었다. 이상에서와 같이 atropine의 투여는 기립시 두부로 가는 혈류량을 증가시키며, 기립 및 도립시 발생할 수 있는 분시 심박수의 과도한 증가를 억제함으로서 체위변화에 대한 심맥관계의 내성을 증가시키는데 어느정도 효과가 있다고 하겠다.

쥐에서 말초 자극에 따른 뇌피질 활성화의 자기공명 영상 (MR imaging of cortical activation by painful peripheral stimulation in rats)

  • As imaging technology develops, magnetic resonance imaging (MRI) techniques have contributed to the understanding of brain function by providing anatomical structure of the brain and functional imaging related to information processing. Manganese-enhanced MRI (MEMRI) techniques can provide useful information about functions of the nervous system. However, systematic studies regarding information processing of pain have not been conducted. The purpose of this study was to detect brain activation during painful electrical stimulation using MEMRI with high spatial resolution. Male Sprague-Dawley rats (250-300 g) were divided into 3 groups: normal control, sham stimulation, and electric stimulation. Rats were anesthetized with 2.5% isoflurane for surgery. Polyethylene catheter (PE-10) was placed in the external carotid artery to administrate mannitol and MnCl2. The blood brain barrier (BBB) was broken by 20% D-mannitol under anesthesia mixed with urethane and a-chloralose. The hind limb was electrically stimulated with a 2Hz (10V) frequency while MnCl2 was infused. Brain activation induced by electrical stimulation was detected using a 4.7 T MRI. Remarkable signal enhancement was observed in the primary sensory that corresponds to sensory tactile stimulation at the hind limb region. These results suggest that signal enhancement is related to functional activation following electrical stimulation of the peripheral receptive field.
