  • Patient care services are provided to individual patients in response to their health needs produced by illnesses or injuries. The services are often addressed to very serious conditions, and also they constitute the most expensive component of health care services. Therefore, the importance of quality is emphasized, but there are many indications that patient care quality is far from a satisfactory state in most of the countries. Based upon this observation, it is attempted to examine obstacles and approaches to quality improvement in patient care services. In doing so, following Taguchi's(1986) definition of product quality, quality of patient care services is conceived of as better when the less is the sociental loss attributalbe to variability of intended function and harmful side effects they emhibit after being delivered. Some distinguishing features of medical care sector pose difficulties in implementing effective quality improvement programs in patient care services. Nevertheless, newly proposed method of quality management, based on industrial quality management approach, seems to have a great deal of potential to effectively cope with such difficulties. This method, unlike the traditional approach to quality assurance, focuses on total organisational processes, not individuals, as the obproach to quality assurance, focuses on total organizational processes, not individuals, as the objects of quality improvement; variation, not comparison with standards, in quality measurement; and continuous improvement, not removing only bad quality care, as an ideal. Prerequisite to a successful use of any quality mangement method is motivating providers to improve quality. Conceivable approaches for such motivation are self-regulation, external controls and promotion of competition. Since these approaches are not mutually exclusive, they may be employed in an appropriate combination. In Korea, medical care providers are now functioning under the circumstances where they have little reason for making efforts to improve quality of their services. Once these circumstantial conditions are changed to exert pressures on providers to improve quality, the use of adequate quality management method becomes an issue. In this connection, much attention shoould be directed to the newly proposed method described above. In all these efforts for improving quality of patient care services, health insurance would be able to play a pivotal role. Poviders of medical care, buth indiciduals and organizations, are usually very responsive to the measures that affect their financing, and thus health insurance can be a strong instrument for motivationg providers to improve quality. Also, the insurance continuously acquires data on patient care, which could be processed to produce information required to effective quality control.

  • This study discusses the institutional change of Chinese early childhood education quality assurance institution (ECEQAI) from the three dimensions of the perspective of historical institutionalism of that refer to the external environment, dynamic mechanism containing with nation-market-society, and the invisible ideology of the institutional change. Chinese ECEQAI was divided into five historical periods, which were 17 years after the founding of China (1949-1966), the Great Cultural Revolution (GCR) period (1966-1976), the early stage of Reform and Opening-up (RO) (1976-1995), the social change period (1995-2010) and the new period (2010-now). This study also concludes five separate change models for Chinese ECEQAI that correspond to the five periods. The first was a compulsory and gradual institutional change model influenced by the Soviet model and dominated by national power. The second was compulsory and radical change model controlled by the counter-revolutionary group. The third was a compulsory and gradual institutional change model dominated by national power. The fourth was induced and gradual institutional change model driven by market power and forced by society power. The fifth was a compulsory and gradual institutional change model led by national power and forced by society power. Finally, the future change direction of China's ECEQAI was further pointed out, that was the government-leading and tend to balanced three-pole power model of "Nation-Market-Society", as well as the "three-pole power relationship" of indepth communication and positive interaction.

  • The evaluation of Varian enhanced dynamic wedges (EDW) were performed in terms of quality assurance in external radiotherapy. The seven (10, 15, 20, 25, 30, 45, 60 deg.) EDW angles were evaluated for 6 and 15 MV x-rays in Varian Linac. The STT (segmented treatment table) for a field were calculated and compared with actual movement of the jaw using Dynalog files in order to evaluate mechanical operation. Two dimensional array detector and an ionization chamber were used to measure dose distributions in phantom from Linac. The mechanical movement of jaw was agreed with its expectation and two dimensional dose distributions including beam profiles were in agreement with RTP data approximately. In comparison with RTP calculations the percentage difference of output dose values for 100 MU irradiation was less than 2.9% and measured wedge factor was less than 2.6%. These results are shown that there is no problem in clinical applications of EDW equipped on this linac.

  • Background: The aim of this study was to evaluate the quality of the Second Korean Working Conditions Survey (KWCS), focusing on its validity and reliability. Methods: The external validity was evaluated by the assessment of sampling procedures and the response rate, in order to investigate the representativeness of the sample. The content validity was evaluated by the assessment of the development of the questionnaire, and the consistency of questions for the selected construct. The test-retest method was used to evaluate the reliability by means of a phone call survey of 30% of the respondents, who were randomly selected. The respondents' satisfaction regarding the survey procedures and interview time were analyzed to evaluate the quality of survey data. Results: The external validity was assured by an acceptable sampling procedure, rigid multi-stage stratified cluster random sampling. The content validity was also guaranteed by a reasonable procedure for the development of the questionnaire with a pretest. The internal consistency of the questions for work autonomy was maintained, with 0.738 of Cronbach's alpha. The response rate of 36% was lower than that of the European Working Conditions Survey (EWCS), with a contact rate of 66%, compared to 76% for the EWCS. The matching rates of the five retested questions were more than 98% reliable. Conclusion: The quality of the second KWCS was assured by the high external and content validity and reliability. The rigid sampling procedure and development of the questionnaire contributed to quality assurance. The high level of reliability may be guaranteed by the sophisticated field survey procedures and the development of a technical manual for interviewers. The technical strategies for a high response rate should be developed for future surveys.

  • Purpose Radioimmunoassay implement quality control by systematizing the internal quality control system for quality assurance of test results. This study aims to contribute to the quality assurance of radioimmunoassay results and to implement systematic quality control by measuring the average CV of internal quality control and external quality control by plenty of institutions for reference when setting the laboratory's own acceptable range. Materials and Methods We measured the average CV of internal quality control and the bounce rate of more than 10.0% for a total of 42 items from October 2020 to December 2021. According to the CV result, we classified and compared the upper group (5.0% or less), the middle group (5.0~10.0%) and the lower group (10.0% or more). The bounce rate of 10.0% or more was compared by classifying the item of five or more institutions into tumor markers, thyroid hormones and other hormones. The average CV was measured by the overall average and standard deviation of the external quality control results for 28 items from the first quarter to the fourth quarter of 2021. In addition, the average CV was measured by the overall average and standard deviation of the proficiency results between institutions for 13 items in the first half and the second half of 2021. The average CV of internal quality control and external quality control was compared by item so we compared and analyzed the items that implement well to quality control and the items that require attention to quality control. Results As a result of measuring the precision average of internal quality control for 42 items of six institutions, the top group (5.0% or less) are Ferritin, HGH, SHBG, and 25-OH-VitD, while the bottom group (≤10.0%) are cortisol, ATA, AMA, renin, and estradiol. When comparing more than 10.0% bounce rate of CV for tumor markers, CA-125 (6.7%), CA-19-9 (9.8%) implemented well, while SCC-Ag (24.3%), CA-15-3 (26.7%) were among the items that require attention to control. As a result of comparing the bounce rate of more than 10.0% of CV for thyroid hormones examination, free T4 (2.1%), T3 (9.3%) showed excellent performance and AMA (39.6%), ATA (51.6%) required attention to control. When comparing the bounce rate of 10.0% or more of CV for other hormones, IGF-1 (8.8%), FSH (9.1%), prolactin (9.2%) showed excellent performance, however estradiol (37.3%), testosterone (37.7%), cortisol (44.4%) required attention to control. As a result of measuring the average CV of the whole institutions participating at external quality control for 28 items, HGH and SCC-Ag were included in the top group (≤10.0%), however ATA, estradiol, TSI, and thyroglobulin included in bottom group (≥30.0%). Conclusion As a result of evaluating 42 items of six institutions, the average CV was 3.7~12.2% showing a 3.3 times difference between the upper group and the lower group. Cortisol, ATA, AMA, Renin and estradiol tests with high CV will require continuous improvement activities to improve precision. In addition, we measured and compared the overall average CV of the internal quality control, the external quality control and the proficiency between institutions participating of six institutions for 41 items excluding HBs-Ab. As a result, ATA, AMA, Renin and estradiol belong to the same subgroup so we require attention to control and consider setting a higher acceptable range. It is recommended to set and control the acceptable range standard of internal quality control CV in consideration of many things in the laboratory due to the different reagents and instruments, and the results vary depending on the test's proficiency and quality control materials. It is thought that the accuracy and reliability of radioimmunoassay results can be improved if systematic quality control is implemented based on the set acceptable range.

  • Visual inspection is regarded as one of the critical activities for quality control in a manufacturing company. it is thus important to improve the performance of detecting a defective part or product. There are three probable working modes for visual inspection: fully automatic (by automatic machines), fully manual (by human operators), and semi-automatic (by collaboration between human operators and automatic machines). Most of the current studies on visual inspection have been focused on the improvement of automatic detection performance by developing a better automatic machine using computer vision technologies. However, there are still a range of situations where human operators should conduct visual inspection with/without automatic machines. In this situation, human operators'performance of visual inspection is significant to the successful quality control. However, visual inspection of components assembled into a mobile camera module belongs to those situations. This study aims to investigate human performance issues in visual inspection of the components, paying more attention to human errors. For this, Abstraction Hierarchy-based work domain modeling method was applied to examine a range of direct or indirect factors related to human errors and their relationships in the visual inspection of the components. Although this study was conducted in the context of manufacturing mobile camera modules, the proposed method would be easily generalized into other industries.
