The Development of the Evaluation Tool of Group Occupational Health Program for Workers of Small and Medium Sized Industries (중소기업 근로자 보건관리대행기관 QA 평가지표의 개발과정)

  • Background: Group occupational health program by non-for-profit agency, started five years ago, for management of health problems in small and medium sized industries, is one of the measures to cope with limited human and financial resources in occupational health. The program has been rapidly expanding to include 54 participating institutions, private as well as public, all over the country. In spite of its potential impact on health of employees and practice of occupational health in small and medium sized industries, comprehensive evaluation in terms of quality has not been tried. Objectives : The aim of this study are to develop and apply criteria to assess quality of newly developed Korean group occupational health program. Methods : By defining occupational health services, in particular for small and medium sized industries, as one of the primary health care, we included followings as core elements of sound occupational health program; accessibility, continuity, intersectoral collaboration, comprehensiveness, community participation, technical quality, adequacy, focus on preventive services, acceptability, and workers' satisfaction. Again we divided each elements into five major components of national health system infrastructures developed by World Health Organization; development of health resources organized arrangement or resources, delivery of health care, economic support, and management. In turn, we categorized each component into input, process and outcome aspects. After discussions in expert panel, several criteria were selected for evaluation of program. The criteria were modified according to each group of interviewees. Results: We developed five sets of questionnaire that evaluate the quality of 'Group occupational health program'. Conclusion : The refining of the measurement tool and the continuing evaluation process for the 'Group occupational health institute' should be done further.

Effects of Qi Gong Exercise on the Immune Response, Pulse Wave Parameter and Heart Rate Variability(HRV) for Post Mastectomy Women (기공(氣功) 운동이 유방암 절제술 여성의 면역, 맥상파 및 심박변이에 미치는 영향)

  • Objectives The purposes of this study was to develope a Qi gong exercise that suits characteristics of post mastectomy women, and to evaluate the effect of Qi gong exercise on immune response, blood circulation index, pulse wave parameter and heart rate variability. Methods This study was applied to total 35 post mastectomy women, including 17 for experiment group and 18 for control group. The Qi gong exercise was composed of total 24 times of 90 minutes per each time, twice a week, and 12 weeks and it was conducted by the oriental medicine professor who was an expert of Qi gong exercise. Results 1. Two group comparison revealed that the experimental group had significantly improved immune response(p<.021), HR(beats/min)(p<.001), ESV(ml/beat)(p=.038), ESI($ml/beat/m^2$)(p=.040), ECO (L/min)(p=.019), ECI($L/min/m^2$)(p=.023), ECRI($dyne^*sec/cm$)(p=.015), Left Kwan($div^3$)(p=.021), Right Kwan($div^3$)(p=.038), Mean HRV(cycle/min)(p<.001), SDNN(ms)(p=.043), RMSSD(ms)(p=.040), and TP(log $ms^2$)(p=.039). 2. Two group comparison revealed that the experimental group had significantly decreased ECR ($dyne^*sec^*cm^{-5}$) (p=.034), Left RAI(p=.044), Right RAI(p=.042), and pNN50(%)(p=.038). Conclusions These results from Qi gong exercise program can be used as basic data for development of health promotion program for Post Mastectomy Women.

Comparison of Standardized Peristomal Skin Care and Crusting Technique in Prevention of Peristomal Skin Problems in Ostomy Patients

  • Purpose: This study was performed to compare the effects of standardized peristomal skin care (SPSC) and crusting technique (CT) on the peristomal skin of ostomates. SPSC was developed by a consensus among the expert group based on a comprehensive review of the relevant literature and hospital protocols. Methods: A randomized controlled pilot trial with 2 parallel arms was used. A total of 81 ostomates, who were recruited from a tertiary hospital, completed the baseline, 1-month, 2-month, and 3-month follow-up (SPSC group, n=45; CT group, n=36). SPSC consisted of water cleansing and direct application of ostomy appliances. CT involved crusting hydrocolloid powder and patting with water sponge or protective barrier liquid film. The outcomes of the study were assessed by skin problems, such as discoloration, erosion and tissue overgrowth; the domains of the evaluation tool used in examining the peristomal skin. A generalized estimating equation model was used to examine the effects according to time and group. Results: In both SPSC and CT groups, the likelihood of occurrence of discoloration (OR, 1.99; 95% CI, 1.61-2.46), erosion (OR, 1.87; 95% CI, 1.55-2.25) and tissue enlargement (OR, 1.94; 95% CI, 1.36-2.77) increased with time. There was no significant difference in discoloration between the groups, whereas the probability of erosion (OR, 0.38; 95% CI, 0.16-0.89) and tissue overgrowth (OR, 0.09; 95% CI, 0.02-0.55) was lower in the SPSC group than in CT group. Conclusion: SPSC was sufficient in preventing peristomal skin problems of ostomates compared to the CT.

Biomedical Ethics Education for Nursing Students: The Effect on Awareness and Application of Nursing Code of Ethics, Consciousness of Biomedical Ethics and Moral Sensitivity (생명의료윤리 교육 프로그램이 간호 대학생의 간호사 윤리강령인식, 간호사 윤리강령 활용 정도, 생명의료 윤리 의식 및 도덕적 민감성에 미치는 효과)

  • Purpose: This study examines the effectiveness of an educational program of nursing biomedical ethics for nursing students to increase awareness of the Code of Ethics for Nurses, moral sensitivity, and application of the Code of Ethics for Nurses. Methods: A quasi-experimental research design and t-test were used. The experimental group (n=31) and control group (n=32) were second-year university nursing students in two major cities. The experimental group received 15 hours of nursing ethics education during the 15-week semester. The training was not provided to the control group. Results: The application of the Code of Ethics for Nurses increased significantly in the experimental group (t=-1.06, p=.017), nurses and patient (t=-2.23, p=.029), obligation of nurse as an expert (t=-2.08, p=.042), nurse and cooperator (t=-2.54, p=.014). The consciousness of biomedical ethics increased significantly in the experimental group (t=4.28, p=.021), newborn's right to live (t=-2.61, p=.011), euthanasia (t=-2.36, p=.021). Conclusion: The results of the study show that providing a nursing biomedical ethics program to nursing students is an effective method to enhance the application of the Code of Ethics for Nurses and Consciousness of Biomedical Ethics. Implementing an intervention program of the Code of Ethics for Nurses in the regular nursing curriculum may reduce conflicts involving ethical decision making by nurses.

A Study on the health care with eating food and exercise (음식섭취와 운동를 통한 건강관리 유형 연구(전문가 집단을 중심으로))

  • This article a little bit lack of elements for the basis of study of health care in the respect of taking a certain expert group as the sample. However researcher chose people who is in same situation with himself and tried to make clear how to do for their behavior of health care to know that how the experts groups are taking care of their health. For the study, the main aim of this article is studying the health care situation of experts group which is taking crucial role in the economy, society and cultural field. When it comes to the experts group's health care by eating habits and exercise related program is, first, it indicates that most people are eating for their health, lives and work through the analysis of responds; they eat food for their health and their health, lives or task.(Surely they family-centered structure. Third, 78% of 40s group responded that they enjoy finding good restaurants while 20s to 30s are enjoying fastfood and instant food instead of Korean traditional foods. fourth, health care by the exercise is done in one or half one hour and the dependant exercises like jogging or swimming which can be done whenever they have time are perferred to group exercise. It's desirable that one has to take care of himself with systematic and effective way. But it's very difficult and care for most people to take care of themselves, so they have to make the program and practice it systematically. The result from this study shows that practicing the health related program rationally, at the same time increasing leisure time and doing dependant or group exercise, taking a walk, and various kind of exercise considered spending money are more common than eating foods for health care. These are regarded as a desirable phenomenon in terms of the health care.

  • This study intended to analyze the effectiveness to obtain by developing the critical pathway presented as the method to improve the quality-betterment and cost effectiveness the through the continuous and consistent patient management for the hysterectomy patient and applying it to the real practice. This study was designed to develop and effect the critical pathway for hysterectomy patients in the way to be possible the intergrated in patient management. It was adopted the process of seven phase to develop a critical pathway. To analyze the application effect of the developed critical pathway, this author offered health care service applying the critical pathway to the hysterectomy patient from July. 20 to Oct. 19. 1999. The study method had been done by investigating the experimental group and control group through the questionnaire on 40 patients who had been inpatient hysterectomy. Dependent variables were measured by modified from satisfaction, and cost and length of hospital stay. The data anlyzed by frequency, x2-test, t-test. The results of this study was as follows; 1. It was decided that the vertical line of the critical pathway was made up of eight items such as monitoring/assessment, treatment, activity, medication, consult. Lab test, diet, patient teaching and horizontal line was 7days from admission to discharge. 2. After the verifying the validity of the expert group about the critical pathway, the horizontal line was amended from hospitalization to five postoperative days and taking their consensus, some contents of the horizontal line was amended and deleted. 3. There was no significant differences in the experimental group and control group in the satisfaction, and significant differences in the cost, the length of hospital stay.

Clustering Analysis of Films on Box Office Performance : Based on Web Crawling (영화 흥행과 관련된 영화별 특성에 대한 군집분석 : 웹 크롤링 활용)

  • Forecasting of box office performance after a film release is very important, from the viewpoint of increase profitability by reducing the production cost and the marketing cost. Analysis of psychological factors such as word-of-mouth and expert assessment is essential, but hard to perform due to the difficulties of data collection. Information technology such as web crawling and text mining can help to overcome this situation. For effective text mining, categorization of objects is required. In this perspective, the objective of this study is to provide a framework for classifying films according to their characteristics. Data including psychological factors are collected from Web sites using the web crawling. A clustering analysis is conducted to classify films and a series of one-way ANOVA analysis are conducted to statistically verify the differences of characteristics among groups. The result of the cluster analysis based on the review and revenues shows that the films can be categorized into four distinct groups and the differences of characteristics are statistically significant. The first group is high sales of the box office and the number of clicks on reviews is higher than other groups. The characteristic of the second group is similar with the 1st group, while the length of review is longer and the box office sales are not good. The third group's audiences prefer to documentaries and animations and the number of comments and interests are significantly lower than other groups. The last group prefer to criminal, thriller and suspense genre. Correspondence analysis is also conducted to match the groups and intrinsic characteristics of films such as genre, movie rating and nation.

A Survey on Usages of Decision Support Functionalities in Korean Medicine Electronic Charts (한의전자차트에서 진단 지원 기능의 활용도에 대한 설문조사)

  • Objectives : A Survey was conducted to find out usages of Korean medicine electronic charts and requirements of clinical decision support functionalities in the charts. Methods : An e-mail was sent to about 12,000 Korean medicine doctors that was affiliated to the Association of Korea Oriental Medicine. 250 doctors answered the questionnaires during one week. Results : Most doctors of 83% answered in use the electronic charts and use it mainly to insurance claims. 46% of them felt that diagnosis functions need to be improved first in the electronic charts. Moreover, 66% of them answered that expert systems to support diagnosis is required if provided. Conclusions : The clinical decision support systems help doctors diagnosis patients in a desirable manner. Many researches have been proposed about them in modern medical science, while a few studies suggested in Korean medicine. In the future, more researches in the field of diagnosis of electronic charts should be proceeded.

  • Generally, the method of the policy analysis has been used for making a policy decision of the development project of the road through the expert interview based on AHP. According to introducing PI, the study of the method of the integrating process for various opinion of the related groups is required. On this study I evaluate what is a difference between the results of integrated process and the AHP analysis on what is major considering factor in decision-making process. From the analysis, each evaluate factor is in level of significance by ANOVA. First, there is a significance level difference between Group A, C and Group B Second, there is also a significance level difference on the result of AHP analysis for each group in order to evaluate by integrated method. Finally, the method of AHP analysis which considers priority factor is expected as to be useful tool on decision-making of various group.

Effects of an Integrated Self-Management Program on Self-Management, Glycemic Control, and Maternal Identity in Women with Gestational Diabetes Mellitus (통합 자가관리 프로그램이 임신성 당뇨병 임부의 자가관리, 혈당 및 모성 정체성에 미치는 효과)

  • Purpose: The purpose of the study was to investigate the effects of an integrated self-management program on self-management, glycemic control, and maternal identity in women with gestational diabetes mellitus (GDM). Methods: A non-equivalent control group non-synchronized quasi-experimental design was used. A total of 55 women with GDM were recruited from Cheil General Hospital, Seoul, Korea and were assigned to an experimental (n=28) or control group (n=27). The participants were 24-30 weeks pregnant women who had been diagnosed with GDM as of July 30, 2010. The program was conducted as a 1 hour small group meeting 3 out of 5 times and by telephone-counseling 2 out of 5 times. The integrated self-management program was verified by an expert panel. Results: Although there was no significant reduction in HbA1c (U= -1.17, p=.238), there were statistically significant increases in self-management (U= -3.80, p<.001) and maternal identity (U= -4.48, p<.001), and decreased 2-h postprandial glucose levels (U= -2.43, p<.015) in the experimental group compared to the control group. Conclusion: These findings suggest that an integrated self-management program for women with GDM improves self-management, maternal identity, and glycemic control. Further studies are needed to identify the effects of an integrated self-management program on pregnancy and neonatal outcomes.
