양혈사물탕가미방(凉血四物湯加味方)이 Allergy성(性) 접촉(接觸) 피부염(皮膚炎)에 미치는 영향(影響) (The Effects of Yanghyulsamultanggamibang to Allergic Contact Dermatitis)

  • Experimental drug was applied to a white male mouse taken ill with allergic contact dermatitis to investigate the effects of Yanghyulsamultanggamibang. 1. In case of skin moisture, when it was checked 72 hours later, there showed up meaningful differences among the normal, control, and sample groups. Especially, the moisture increased more noticeably in the sample group than in the control group. 2. With regards to RBC count in blood, it increased more in the control and sample groups than in the normal group when it was checked both in 24 hours and 72 hours. When it was checked in 48 hours, the number of RBC in the sample group increased the most, and the count in the control group increased more than in the normal group. 3. About the neutrophil rate in WBC count, it increased meaningfully more both on the control and sample groups than in the normal group in 24 hours. When the rate was checked in 48 hours, it increased the most in the sample group, and it increased more in the control group than in the normal group. When in checked72 hours later, it increased substantially more in the control group than both the sample and normal group. 4. As of the lymphocyte rate in WBC count, when measured in 24 hours, it increased in the normal group than in the control and sample groups. In 48 hours, it increased the most in the normal group, and it increased meaningfully more in the control group than in the sample group. 5. Regarding the total IgE, when measured 24 hours later, it increased noticeably more in the normal group than in the control and sample groups. In 48 hours, there showed up substantial differences among the normal, control and sample groups. In particular, it increased meaningfully more in the control group than in the sample group. 6. When the sample of skin tissue was examined, the corneal layer restoration increased more in the sample group than in the normal group. The above-mentioned results prove that Yanghyulsamultanggamibang can be taken when allergic contact dermatitis develops on the skin. Long-term observation afterward is also advised.

한국 소아알레르기 질환의 유병률 (The prevalences of asthma and allergic diseases in Korean children)

  • Asthma and allergic diseases are one of the most common disorders in children. Due to its increased prevalence, as well as the increased morbidity and mortality from these diseases, asthma and allergic diseases have come to be recognized as a major worldwide public health issue. In addition, socioeconomic burden of asthma and allergic diseases has increased in Korea also. The International Study of Asthma and Allergies in Childhood (ISAAC) investigated the worldwide prevalence of asthma and other allergic diseases using simple standardized methods because of the comparison of asthma and allergic diseases between the countries. In Korea, several epidemiologic studies have been conducted to determine the prevalence of asthma in children. Although these studies showed increased prevalence of asthma among Korean children (from 3.4% in 1964 to 10.1% in 1989), these findings were based on data from small numbers of subjects. The first Nationwide Study of Asthma and Allergies in Korean Children, which utilized the Korean version of ISAAC written and video questionnaire, was conducted in 1995 and the second Nationwide Study of Asthma and Allergies in Korean Children was conducted in 2000, directed by the Korean Academy of Pediatric Allergy and Respiratory Diseases. We report here the prevalence of asthma and other allergic diseases in Korean children and adolescents, and show the changes that occurred over this 5 year period. We also describe the risk factors for development of these diseases in Korean children. We developed Korean versions of the ISAAC written (WQ) and video (AVQ) questionnaires for allergic diseases. In 1995, the enrolled population consisted of 25,117 children selected from 34 elementary school and 14,946 children selected from 34 middle school across the nation, the response rate was 94.8%. In 2000, 27,831 children selected from 34 elementary school and 15,214 children selected from 34 middle school, and the response rate was 96.4%. From these studies, we can confirm that increase of the prevalences of asthma, allergic rhinitis, and atopic dermatitis in Korea, except the prevalence of food allergy. Especially, the video questionnaire showed increases in the lifetime and 12 month prevalence rates of wheeze at rest, exercise-induced wheeze, nocturnal wheeze, nocturnal cough, and severe wheeze over this period of time in middle school children. In addition, the increase of prevalences of symptoms, diagnosis and treatment of atopic dermatitis was noted significantly. Risk factor analysis showed that body mass index (BMI), passive smoking and living with a dog or cat were associated with higher risk of wheeze. Also the occurrence of fever during infancy and the frequent use of antibiotics were associated with the risk of wheeze. In conclusion, during the 5 year period from 1995 to 2000, the prevalences of asthma, allergic rhinitis, and atopic dermatitis has increased in Korean children. BMI, passive smoking, living with a dog or cat, the fever episodes in infancy, and the frequent use of antibiotics in infancy are important risk factors to development of asthma and atopic dermatitis. In the near future, the birth cohort study will be needed to investigate the causes of this increase and the natural course of allergic diseases, then we develop the methods to control asthma and allergic diseases.

Influence of age at complementary food introduction on the development of asthma and atopic dermatitis in Korean children aged 1-3 years

  • Background: Complementary food in infancy is necessary for human growth, neurodevelopment, and health. However, the role of allergen consumption in early infancy and its effects on the development of food allergy or tolerance remain unclear. Purpose: To investigate the influence of age at the time of complementary food introduction on the development of asthma and atopic dermatitis in Korean children aged 1-3 years. Methods: We combined data from the Korea National Health and Nutrition Examination Survey collected from 2010 to 2014 and analyzed 1619 children aged 1-3 years who were included in the survey. Multivariate regression analysis was used to identify associations among type of feeding, age at the time of complementary food introduction, and doctor-diagnosed atopic dermatitis and asthma. Results: Age at the time of complementary food introduction was not significantly associated with doctor-diagnosed atopic dermatitis and asthma in children aged 1-3 years. In the univariate analysis, children with asthma showed higher water and sodium intake levels than nonasthmatic children. However, this relationship was not significant in the multivariate regression analysis. Conclusion: The present study revealed no statistically significant relationship between age at the time of complementary food introduction and the risk of atopic dermatitis and asthma in young Korean children. A national prospective study is needed to clarify the influence of age at the time of complementary food introduction on the development of allergic diseases.

Intermittent Fasting Modulates Immune Response by Generating Tregs via TGF-β Dependent Mechanisms in Obese Mice with Allergic Contact Dermatitis

  • People with obesity maintain low levels of inflammation; therefore, their exposure to foreign antigens can trigger an excessive immune response. In people with obesity or allergic contact dermatitis (ACD), symptoms are exacerbated by a reduction in the number of regulatory T cells (Tregs) and IL-10/TGF-β-modified macrophages (M2 macrophages) at the inflammatory site. Benefits of intermittent fasting (IF) have been demonstrated for many diseases; however, the immune responses regulated by macrophages and CD4+T cells in obese ACD animal models are poorly understood. Therefore, we investigated whether IF suppresses inflammatory responses and upregulates the generation of Tregs and M2 macrophages in experimental ACD animal models of obese mice. The IF regimen relieved various ACD symptoms in inflamed and adipose tissues. We showed that the IF regimen upregulates Treg generation in a TGF-β-dependent manner and induces CD4+T cell hypo-responsiveness. IF-M2 macrophages, which strongly express TGF-β and inhibit CD4+T cell proliferation, directly regulated Treg differentiation from CD4+T cells. These results indicate that the IF regimen enhances the TGF-β-producing ability of M2 macrophages and that the development of Tregs keeps mice healthy against ACD exacerbated by obesity. Therefore, the IF regimen may ameliorate inflammatory immune disorders caused by obesity.

호도약침이 알레르기에 미치는 영향 (Effect of Juglandis Semen Aqua-acupuncture and Acupuncture on the Allergic Response)

  • Experimental studies were done to research the dinical effects of Juglandis Semen aqua-acupuncture and acupuncture(BL 13) on the anti-allergic response. anaphylaxis provoked by the compound 48/80, delayed type hypersensitivity response to picryl chloride and SASC and inflammation response to egg albumin. The following results have been obtained: Juglandis Semen aqua-acupuncture and acupuncture(BL13) group were increased the survival rate in compound48/80 induced systemic anaphylactic reaction. Picryl chloride induced contact dermatitis and delayed type hypersensitivity in SRBC challenged mouse were significantly decreased in Juglandis Semen aqua-acupuncture and acupuncture (BL13) group. Inflammation response - WBC, CRP and Nitric Oxide in egg albumin induced allergic rat were significantly decreased in Juglandis Semen aqua-acupuncture and acupuncture(BL13) group. According to the above results, Semen aqua-acupuncture and acupuncture(BL13) both depress the allergy reaction.

Prevalence and comorbidity of allergic diseases in preschool children

  • Purpose: Allergic disease and its comorbidities significantly influence the quality of life. Although the comorbidities of allergic diseases are well described in adult populations, little is known about them in preschool children. In the present study, we aimed to assess the prevalence and comorbidity of allergic diseases in Korean preschool children. Methods: We conducted a cross-sectional study comprising 615 Korean children (age, 3 to 6 years). Symptoms of allergic diseases were assessed using the Korean version of the International Study of Asthma and Allergies in Childhood (ISAAC) questionnaire that was modified for preschool children. Comorbidities of allergic diseases were assessed by 'In the last 12 months, has your child had symptoms?'. Results: The prevalence of symptoms of asthma, allergic rhinitis, and atopic dermatitis as recorded using the ISAAC questionnaire, within the last 12 months was 13.8%, 40.7%, and 20.8%, respectively. The symptom rates of allergic conjunctivitis, food allergy, and drug allergy were 14.8%, 10.4%, and 0.8%, respectively. The prevalence of allergic rhinitis in children with asthma was 64.3% and that of asthma in children with allergic rhinitis was 21.6%. The prevalence of rhinitis in children with conjunctivitis was 64.8% and that of conjunctivitis in children with rhinitis was 23.6%. Conclusion: The prevalence of current rhinitis in our preschool children is shown to be higher than that previously reported. Allergic conjunctivitis is closely associated with asthma and allergic rhinitis. However, further studies are warranted to determine the prevalence and effects of these comorbidities on health outcomes in preschool children.

알레르기 질환에서 장내미생물 조절을 통한 한약의 효과 연구동향 (Current Trends of Traditional Herbal Medicine Research on Allergic Disease with Dysbiosis)

  • Objectives : The purpose of this study is to analyze the current trends of various herbal medicine research on allergic disease with dysbiosis. Methods : Electronic searches were performed using Pubmed, Research Information Sharing Service(RISS), Korean studies Information Service System(KISS), Oriental medicine Advanced Searching Integrated System(OASIS). Results : We analyzed ten studies on the effect of herbal medicine on allergic disease with dysbiosis. Eight studies were animal experimental studies, and two were randomized clinical trial(RCT) study and one-group pretest-posttest research, respectively. Among the studies, three studies were on atopic dermatitis, two on allergic rhinitis, and five on asthma. All different herbal medicines were used in the studies. Changes in gut microbiota composition were observed in nine studies except for 1 RCT study. In eight animal experimental studies, there was significant reduction in allergy-related inflammatory markers. Six studies evaluated the change of metabolites related to gut microbiota and three of them showed significant increase in short-chain fatty acids(SCFA). Conclusion : This study provides current trends of studies on herbal medicine research on allergic disease with dysbiosis. Most research is conducted using animal experiments, and this is a relatively recent trend. These studies offer basic knowledge on the correlation between herbal medicine, gut microbiota, and anti-inflammatory effects in allergic disease.

  • 알러지성 접촉피부염은 T세포와 대식세포가 관여하는 세포매개성 면역반응으로 항원에 노출된 뒤 수 일 후에 증상이 발현되는 지연형 반응으로 특이 T세포의 활성을 통해 다양한 cytokine이 분비되어 항원 유입부위로 염증세포의 유주로 염증이 발생한다. 본 연구에서는 DNCB로 유발한 알러지성 접촉피부염에 대한 아로마 오일 합제의 항염효과를 평가하였다. 아로마 오일 합제는 8일간 적용하였고 실험기간 동안 2일 간격으로 피부 생물리학적 지표를 측정한 후 피부생검을 통해 피부조직을 평가하였다. DNCB 적용 후 피부 생물리학적 지표인 표피경유수분손실, 피부 수화도, 피부 두께 그리고 홍반지수가 증가하였다 (p < 0.05). 피부조직 평가에서 염증세포 침윤과 부종성 변화에 의한 상피두께 증가 및 진피 결합조직의 감소가 특징적으로 나타났다. 또한 진피내 pro-inflammatory cytokine인 TNF-${\alpha}$와 IFN-${\gamma}$ 면역반응세포수 및 표피내 apoptotic change의 지표인 caspase-3와 PARP 면역반응세포의 수가 유의적으로 증가 하였다 (p<0.01). 그러나 아로마 오일 합제 적용후에는 피부 생물리학적 지표 (p<0.05) 및 피부조직의 병리적 변화는 (p<0.01) 기저상태로 회복되었다. 따라서 본 연구를 통해 개에서 DNCB에 의한 알레르기성 접촉피부염 유발 및 아로마 오일 합제의 우수한 항염증 효과를 확인하였고 아로마 오일 합제는 향후 피부염에 대한 치료제로 사용할 수 있을 것으로 기대된다.

The effect of Korean red ginseng on allergic inflammation in a murine model of allergic rhinitis

  • Korean red ginseng (KRG) is reported to have anti-allergic properties, including beneficial effects on asthma and atopic dermatitis. However, its effect on allergic rhinitis has not been studied extensively. This study examined how KRG affected allergic inflammation of the nasal cavity in an allergic mouse model. A total of 40 Balb/c female mice were divided into four experimental groups according to treatment and allergic state: group 1 (G1), saline only; group 2 (G2), ovalbumin (OVA); group 3 (G3), OVA+KRG; and group 4 (G4), OVA+dexamethasone. Serum IgE levels were significantly lower in the KRG treatment group (G3) than in the allergic group (G2). However, serum IgG1 levels did not differ between G2 and G3. In the nasal lavage fluid, IL-4 and IL-5 levels were significantly lower in G3 than in G2 (p<0.05). H&E and Luna staining revealed that the eosinophil count was lower in G3 and G4 than in G2 (p<0.05). Immunohistochemical staining revealed that there were fewer IL-4-, IL-5-, and MUC5AC-positive cells in G3 and G4 than in G2 (p<0.05). These results indicate that KRG reduces the nasal allergic inflammatory reaction in an allergic murine model by reducing Th2 cytokines.

Clinical efficacy and mechanism of probiotics in allergic diseases

  • A complex interplay between genetic and environmental factors partially contributes to the development of allergic diseases by affecting development during prenatal and early life. To explain the dramatic increase in the prevalence of allergic diseases, the hygiene hypothesis proposed that early exposure to infection prevented allergic diseases. The hygiene hypothesis has changed to the microbial hypothesis, in which exposure to microbes is closely linked to the development of the early immune system and allergic diseases. The intestinal flora may contribute to allergic disease through its substantial effect on mucosal immunity. Based on findings that exposure to microbial flora early in life can change the Th1/Th2 balance, thus favoring a Th1 cell response, probiotics may be beneficial in preventing allergic diseases. However, evidence from clinical and basic research to prove the efficacy of probiotics in preventing allergy is lacking. To date, studies have yielded inconsistent findings on the usefulness of probiotics in allergic diseases. It is difficult to demonstrate an exact effect of probiotics on asthma, allergic rhinitis, and food allergy because of study limitations, such as different first supplementation period, duration, different strains, short follow-up period, and host factors. However, many studies have demonstrated a significant clinical improvement in atopic dermatitis with the use of probiotics. An accurate understanding of the development of human immunity, intestinal barrier function, intestinal microbiota, and systemic immunity is required to comprehend the effects of probiotics on allergic diseases.
