Objective : To assess the utilization patternsand satisfaction factors of Traditional Korean Medicine(TKM) in Korea. Method : The study design of this paper is from a nationally representative random e-mails and interview surveys. In this survey, we asked 2000 Korean adults between the ages of 20 and 69, whether they have used TKM in the past three years and if so, what were their experiences and how satisfied they were with the care they have received. Results : Sixty percent of respondents(n=1239) answered that they had used TKM in the past three years. The odds of using TKM were higher in female(OR 1.48; 95% CI 1.24-1.78) and academic backgrounds of university graduate or beyond (OR 1.27; 95% CI 1.01-1.60), and lower in unmarried person (OR 0.68; 95% CI 0.55-0.84) in the multivariable analysis. The most frequent purpose for seeking TKM and receiving treatment was to 'treat illness'.In addition, the use herbal medicine among types of TKM is statistically significant in sexual groupings.748(60.4%) among TKM users responded that they had a satisfying experience during TKM treatment. In multivariable logistic model, the odds of satisfaction from using TKM were higher in employed groups(OR 1.34; 95% CI 1.02-1.77), the reason for using of personalized medicine into TKM(OR 1.50; 95% CI 1.00-2.26). Conclusion : Korean traditional medicine doctors need to develop and provide the personalized medicine for the group of high utilization rate and satisfaction, and make strategies to disseminate TKM utilization for the group of lower utilization.
Objectives: Bloodletting therapeutics is one of the most popular oriental medical treatments in Korea. In this study, we operate the Interview Survey for grasping clinical actual state bloodletting therapeutics in Korea. Methods : Survey questions were developed based on consensus of acupuncture professors. The list of the Korean medical doctors with experiences more than 10 years is provided by the Association of the Korean Oriental Medicine. The interviews were conducted to 39 members of the Korean medical doctors who answered to used bloodletting therapeutics over 30 percentage a day at previous telephone survey. Interview survey with them were conducted by the well-trained interviewers of College of Korean Medicine student from 29th May 2006 to 3rd June 2006. Results : Korean medical doctors prefer to use the bloodletting cupping treatment(89.5%) on the bloodletting therapeutics. Musculo-skeletal disorder was as frequent as 89.5% of treatment disease. The most common treatment area was back(57.9%), extremity(l5.8%), pain area(l0.5%). The most common instrument for treating was disposable lancet(57.9%), three-edged needle(26.3%). Most Korean medical doctors(60.5%) took up the position that bring symptom relief following good treatment area. 'Recover quickly from illness'(50%) was one of bloodletting good points but 'Sever pain'(34.2%) was a weak points. Conclusion : This survey provides unique insight into the perception of the Korean medical doctors at bloodletting therapeutics. Most doctors experienced symptom relief, received positive benefits from the treatment. Future research needs to provide more in-depth insight into doctor views of the experience.
One of the characteristics features of Chinese Medicine is recognized as the practical use of combinations of crude drugs in the forms of various prescriptions according to the symptoms of illness. Such crude drugs are mostly dried plant roots, rhizomes, leaves, fruits and seeds, sometimes dried animla organ preparations even fossils or minerals. In the oldest book of Chinese Medicine, Sang Han Ron, 113 prescriptions were described in which about 80 crude drugs were included, and in other old literature, Shin Nong Bon Tcho Keun, 363 crude drugs were recorded. Such drugs, in particular, those included in Sang Han Ron are generally low toxic, and the prescriptions are acting mild with less side effects, but obviously not so easy task to determine the effect of the indivisual drug from the total effects of various prescriptions which are generally consisted of many kinds of crude drugs. About 200 years ago, in the middle of Edo era, a Japanese medical doctor, Yoshimasu Todo wrote a series of books named "Yakucho", in which he described the essential various prescriptions in which the certain drugs plays the main role. This book is very suggestive to evaluate the effects of some Chinese drugs. The actual effects of Chinese medicine and drugs have widely been recognized by the East Asian peoples during their long age experiences, but still it has not been studied sufficiently by modern scientific methods to give generally acceptable evidences. During past some years, my collaborators and I have been engaged to study some well-known Chinese drugs chemically to find out the active principles in determining their chemical structures under the collaboration of pharmacologists, Prof. K. Takagi and his coworkers. In the present lecture, I would like to present two topics from our recent investigations: The studies on the principles of Paeony and Ginseng roots.

Purpose: The aim of this study is to investigate current status, indications, and complications of hyperbaric oxygen therapy. Methods: A retrospective investigation of patients who underwent hyperbaric oxygen therapy at a university medical center from September 2004 to August 2013 was conducted based on patients' medical records and results of an email survey for 99 emergency centers. Results: During the study period, a total of 233 patients underwent hyperbaric oxygen therapy. Indications for hyperbaric oxygen treatment of illness or injury were as follows: 1) 151 cases of acute carbon monoxide poisoning(65.4%), 2) flap wound management, including 42 cases(18.2%), 3) skin care transplanted, including 23 cases(10.4%), 4) Burger's disease, including 5 five cases(2.1%), respectively. Total application time$^*$ frequency was 1,088 and total time was 1,239 hours. Among 233 patients who underwent hyperbaric oxygen therapy, 32 patients(13.7%) had complications: 1) otalgia in 21 cases(9.0%), 2) mastoiditis?in six cases(2.6%), 3) hemotympanum in five cases(2.1%), respectively. There were only 8 emergency centers that currently had an operational hyperbaric oxygen chamber in 77 emergency centers(10.4%). Conclusion: Indications identified through this study showed difference from current indications worldwide. It seems necessary that physicians' perception regarding application of hyperbaric oxygen therapy for more indications be changed and improved. A hyperbaric chamber capable of providing respiratory assistance and intensive care is also needed. A good network for sharing treatment experiences and a specialized team for administration of hyperbaric oxygen therapy is also required.
Caring for patients with cancer is highly stimulating and rewarding, attracting health professionals to the field who enjoy the challenge of managing a complex illness. Health professionals often form close bonds with their patients as they confront ongoing disease or treatment impacts, which may be associated with multiple losses involving function and/or eventual loss of life. Ongoing exposure to patient loss, along with a challenging work setting, may pose significant stress and impact health professionals' well-being. The prevalence rates of burnout and compassion fatigue (CF) are significant, yet health professionals have little knowledge on these topics. A 6-week continuing education program consisting of weekly small-group video-conferencing sessions, case-based learning, and an online community of practice was delivered to health care providers providing oncology care. Program content included personal, organization and team-related risk and protective factors associated with CF, grief models, and strategies to mitigate against CF. Content analysis was completed as part of the program evaluation. In total, 189 participants (93% nurses) completed the program, which was associated with significant improvements in confidence and knowledge of CF and strategies to support self and team resilience. Qualitative themes and vignettes from experiences with the program are presented. Key themes included knowledge gaps, a lack of support related to CF and strategies to support resilience, organization-and team-based factors that can inhibit expression about the impacts of clinical work, the health professional as a "person" in caregiving, and the role of personal variables, self-skill practices, and recommendations for education and support for self and teams.

Objectives : This pilot study examined the physical and psychological sequela of Korean War victims. Methods : Of 255 elderly who completed the Mini International Neuropsychiatric Interview (MINI), we selected 16 subjects who suffered severe traumatic experiences during the Korean War and met more than 2 specified symptoms of PTSD in DSM-IV-TR and 16 controls. Demographic characteristics, medical history including hypertension, diabetes mellitus, hypercholesterolemia and brain infarction, Geriatric Depression Scale, State-Trait Anxiety Inventory, suicide scale in MINI, and a scale for the assessment of somatic symptoms were compared between subjects with trauma experience and controls. Results : Subjects with trauma experience were more likely to have brain infarction and diabetes mellitus compared to control group. Depressive symptoms and suicidal risk were significantly higher in subjects with trauma experience than controls. Subjects with trauma experience showed significantly higher scores in Trait Anxiety scale and more complained of pain than controls. Conclusion : This study suggests that traumatic experience probably induce physical and psychological problem even 60 years later. Further studies are needed to confirm the results of this pilot study.

Purpose : Recently, Korea has entered an aging society due to the steadily increasing proportion of the elderly population. Additionally, elderly suicidal rate in Korea is the highest among OECD member countries. Because old age is a time when all functions are weakened physically, mentally, and socially, negative experiences, including depression and poor quality of life in the elderly, may lead to suicidal intentions. This study attempts to understand the relationship between depression and quality of life experienced by the elderly and their suicidal intentions. Methods : The survey was conducted from January 2 to March 31, 2019 by administering a questionnaire targeting people aged ≥ 65 years, who were registered at public health centers located in U-city, Gyeonggi-do. In total, 1,015 copies of data were used for the final analysis. The collected data were analyzed using the SPSS/WIN 23.0 program. The χ2 test was performed to investigate the relationship between the demographic characteristics and the suicidal intentions of the elderly subjects. The t-test was performed to determine the relationship between suicidal intentions, depression, and quality of life in elderly subjects. Results : The study results revealed that 168 people (16.6 %) had thought of suicide and 3.2 % had actually tried to commit suicide. The reasons for suicidal thoughts were family problems in 60 (5.6 %), economic problems in 49 (4.6 %), and physical illness in 44 (4.1 %). Those with suicidal intentions had higher levels of depression (t=11.712, p<.001) and higher quality of life scores (t=-9.565, p<.001). Conclusion : Based on these results, it is proposed that a suicide prevention policy and program that can improve the elderly's depression and quality of life is necessary.

The occurrences of occupational illness and injury have been seriously underestimated in Korea. Surveillance systems for occupational diseases have recently emerged as important strategies for the control of occupational hazards and the implementation of intervention programs to protect workers. However, health service providers do not actively diagnose occupational diseases and are unwilling to report occupational diseases. With the rapid growth of Internet usage in Korea, the computer network has become the predominant means of communicating and sharing information. Therefore, we developed a web-based updated information and education network to assist the health services providers in reporting occupational diseases. Information systems for occupational disease surveillance were also designed to support occupational disease reporting. Commonly available database systems, such as web databases, are useful to manage occupational diseases data efficiently. Standardized case definitions and report guidelines were also established, which included cumulative trauma disorder, occupational asthma, occupational contact dermatitis, and occupational cancer. This system may provide the basis of an efficient and continuously updated source of educational information and provide specific information concerning the occurrence of occupational diseases in specific areas. Background information on occupational diseases obtained in this way will be invaluable for preventing hazards and enforcing occupational disease prevention programs. Moreover, our experiences in establishing these information systems will be of great use in other countries and settings.

Objectives : The objective of this report is to identify the utilization of hospital school service during hospitalization among patients in their childhood and adolescence with psychiatric disorders. Methods : We retrospectively reviewed the medical record of child and adolescent psychiatric who were hospitalized during March 2009 through October 2012. We compared the one-year successful schooling and outpatient follow up rate between users and nonusers of the inpatient hospital school service. The hospital schooling experiences of the users were investigated upon follow-up visits to the outpatient clinic. Results : Sixty-three students received hospital school service during hospitalization among total 122 child and adolescent inpatients. Hospital school participants showed a significantly higher school reentry rate (61.9%) than non-participants (40.7%). However, there was no difference on follow up rate between the two groups. More than 60% of the 22 interviewed participants expressed an above-average level of satisfaction about hospital school service. Conclusion : Many patients with mental illness experience difficulty in receiving school education during treatment. That induces deterioration in disease, academic failure, poor social skills, low self-esteem, economic difficulties, and future job opportunities. The results of this study emphasize the importance of hospital school service and offer useful guidance for hospital school operation.
Purpose: The aim of this study was to develop a substantive theory on self-management conducted by the adolescents with chronic kidney disease from their lived experience. Methods: Data was collected through in-depth interviews from May to December in 2015 with thirteen adolescents with chronic kidney disease. The data collected were analyzed on the basis of Strauss and Corbin's grounded theory. Results: The core of the category found in this study was "overcoming the unstable sense of self- control and integrating disease experience into their life". The causal conditions triggering the central phenomenon were "restriction in daily life" and "manifestation and aggravation of symptom". The central phenomenon in the experience of self-management within the adolescents with chronic kidney disease was "unstable sense of self control". The intervening condition for unstable self control were "micro system support" and "motivational resources". This study found that the adolescents with chronic kidney disease followed a series of strategies when they faced the central phenomenon, including; passive coping, reappraisal of illness, active coping, compliance with treatment, controlling physical activity, and adjusting school life. With these strategic approaches, the adolescents with chronic kidney disease could maintain their active lifestyles and achieve their health behaviors. The process of self-management by these adolescents passed through four phases; limited experience caused by diseases, effort for normalization, reorganizing their daily lives, and integration with daily lives and self-management. Conclusion: This Study explored the process and experience of self-management of adolescents with chronic kidney disease. These findings can be used for basis for developing substantive theory and nursing intervention strategy for adolescents with chronic kidney diseases.
