Optimum Screening Procedures Using Prior Information

  • Optimum screening procedures using prior information are presented. An optimal cutoff value on the screening variable X minimizing the expected total cost is obtained for the normal model; it is assumed that a continuous screening variable X given a dichotomous performance variable T is normally distributed and that costs are incurred by screening inspection and misclassification errors. Methods for finding optimal cutoff values based on the prior distributions for unknown parameters are presented.

  • This paper presents a linear programming formulation of the aggregate production planning problem for an optical instrument manufacturing company. Taking into account of the various requirements of the company, an L.P model is developed whose objective is to minimize the total cost of production during the planning horizon. The application results and its implications are well understood by the management and expected to be used as a guide line for future production planning.

  • We consider the problem of determining the spare inventory level for a multiechelon repairable-item inventory system. Our model extends the previous results to the system which has an inventory at the central depot as well as bases. We develop an optimal algorithm to find spare inventory leveis, which minimize the total expected cost and simultaneously satisfy a specified minimum service rate. The algorithm is tested using problems of various sizes to verify the efficiency and accuracy.

  • This paper is concerned with preventive maintenance model for the items whose failures are dependent on their wear level. The previous maintenance models have used time as their decision variable, but it is not appropriate for the case which have wear dependent processes for their failures. In this paper, we consider an operating item which is under periodic review and which is subject to degradation. The scheduled maintenance (overhaul ) is based on the level of item degradation rather time. A functional equation for the total expected cost over an infinite horizon period is formulated and solved.

  • Optimal numbers of repeat inspections are obtained for a single inspector who has a fixed probability of detecting a nonconforming item on each inspection and will continue to inspect until further inspection is not warranted when comparing the expected increase of total gain with the inspection cost. It is assumed that the detection probability decreases as the number of repeat inspections increases, and that the lot to be inspected contains an unknown but Poisson distributed number of nonconforming items.

  • The object of this study is to suggest the direction of building the defence logistics system of Korea Armed Forces in the future. For the purpose of this, we surveyed the best practices among the enterprises and U.S. Defence which reformed successfully the physical distribution and logistic in recent year. Also, we analyzed our current logistics system and suggested the vision, operational concept, and the way of building this system. This system is expected to reduce the total logistics cost, speed up the logistics velocity and increase the efficiency of the combat support.

  • The probabilistic order level inventory model is developed when a supplier allows some credit period T for settling the accounts for purchase quantity. The credit period T is known constant. Mathematical models are derived for both the cases i) T'.leq. T and ii) T'>T. Expressions are derived for average expected total cost of the system, the optimum cycle time and for obtaining optimum order level S = S$_{0}$ in each case. The exmaples are given to illustrate the model.

Cost Effectiveness Analysis of Different Management Strategies between Best Supportive Care and Second-line Chemotherapy for Platinum-resistant or Refractory Ovarian Cancer

  • Background: There is no standard treatment for patients with platinum-resistant or refractory epithelial ovarian cancer. Single agent chemotherapies have evidence of more efficacy and less toxicity than combination therapy. Most are very expensive, with appreciable toxicity and minimal survival. Since it is difficult to make comparison between outcomes, economic analysis of single-agent chemotherapy regimens and best supportive care may help to make decisions about an appropriate management for the affected patients. Objective: To evaluate the cost effectiveness of second-line chemotherapy compared with best supportive care for patients with platinum-resistant or refractory epithelial ovarian cancer. Materials and Methods: A Markov model was used to estimate the effectiveness and total costs associated with treatments. The hypothetical patient population comprised women aged 55 with platinum-resistant or refractory epithelial ovarian cancer. Four types of alternative treatment options were evaluated: 1) gemcitabine followed by BSC; 2) pegylated liposomal doxorubicin (PLD) followed by BSC; 3) gemcitabine followed by topotecan; and 4) PLD followed by topotecan. Baseline comparator of alternative treatments was BSC. Time horizon of the analysis was 2 years. Health care provider perspective and 3% discount rate were used to determine the costs of medical treatment in this study. Quality-adjusted life-years (QALY) were used to measure the treatment effectiveness. Treatment effectiveness data were derived from the literature. Costs were calculated from unit cost treatment of epithelial ovarian cancer patients at various stages of disease in King Chulalongkorn Memorial Hospital (KCMH) in the year 2011. Parameter uncertainty was tested in probabilistic sensitivity analysis by using Monte Carlo simulation. One-way sensitivity analysis was used to explore each variable's impact on the uncertainty of the results. Results: Approximated life expectancy of best supportive care was 0.182 years and its total cost was 26,862 Baht. All four alternative treatments increased life expectancy. Life expectancy of gemcitabine followed by BSC, PLD followed by BSC, gemcitabine followed by topotecan and PLD followed by topotecan was 0.510, 0.513, 0.566, and 0.570 years, respectively. The total cost of gemcitabine followed by BSC, PLD followed by BSC, gemcitabine followed by topotecan and PLD followed by topotecan was 113,000, 124,302, 139,788 and 151,135 Baht, respectively. PLD followed by topotecan had the highest expected quality-adjusted life-years but was the most expensive of all the above strategies. The incremental cost-effectiveness ratios (ICER) of gemcitabine followed by BSC, PLD followed by BSC, gemcitabine followed by topotecan and PLD followed by topotecan was 344,643, 385,322, 385,856, and 420,299 Baht, respectively. Conclusions: All of the second-line chemotherapy strategies showed certain benefits due to an increased life-year gained compared with best supportive care. Moreover, gemcitabine as second-line chemotherapy followed by best supportive care in progressive disease case was likely to be more effective strategy with less cost from health care provider perspective. Gemcitabine was the most cost-effective treatment among all four alternative treatments. ICER is only an economic factor. Treatment decisions should be based on the patient benefit.

Economic Efficiency of the Korean Container Terminals: A Stochastic Cost Frontier Approach

  • Purpose - Recent issues such as vessel enlargement, strengthening of environmental regulations, and port smartization are expected to increase costs and intensify competition in the port industry. In the new normal era, when external growth has reached its limit, the efficient operation of ports is becoming indispensable for achieving sustainable growth. This study aims to identify the determinants of inefficiency by examining the cost structure and efficiency of container terminals in Korea and furthermore propose the political implications to derive the maximization of efficiency. Design/methodology - This study estimates the cost function of container terminal operators and identifies the efficiency of container terminals using stochastic cost frontier (SCF) in the first stage. In the second step, the SCF results are compared with the data envelopment analysis (DEA). Last, this paper proposes efficiency determinants on container terminal operation to establish appropriate strategies. Out of the 29 container terminal operators in South Korea, 13 operators participated in the survey. The translog cost function was estimated utilizing a total of 116 observations collected over the 2007-2017 period. Findings - Empirical analysis shows that economies of scale exist in Korea's container ports, which provides a rationale for the government's policy to establish the global terminal operator by integrating small terminal operators to enhance competitiveness. In addition, as a result of the determinants analysis, container throughput, weight of direct employment costs, and labour cost share have positive effects on improving cost efficiency, while inefficiency increases as the length of quay increases. More specifically, cost efficiency improves as the proportion of direct employment costs to outsourcing service costs increases. Originality/value - This study contributes to analyzing the inefficiency factors of container terminals through efficiency analysis with respect to a cost function. In addition, this study proposes the practical and political implications, such as establishing a long-term manpower pool, the application of the hybrid liner terminal system, and the construction of a statistical data system, to improve the cost inefficiency of terminal operators.

A Study on Operation-Effectiveness Analysis of Clinical Nurse Specialists : Focused on Patient Satisfaction and Expected Profit (임상 전문간호사 운영 효과 분석)

  • Purpose: The aim of this study was to evaluate patient satisfaction and indirect marketing effects (recommendation, hospital re-visit intention) and additionally to calculate the expected incomes by clinical nurse specialists (CNS). Methods: Sixty-six patients who had lower varicose vein removal surgery performed at two general hospitals from August 1 to November 30, 2008, were recruited. The effects were evaluated by degree of patient satisfaction, the number of personal recommendations given, rate of hospital re-visit intention, and indirect marketing effects. Indirect marketing effects were calculated as the total cost of surgery multiplied by the score of other variables. Results: The patient satisfaction score was 0.82 for CNS and 0.43 for general nurses (GN). The number of personal recommendations given was 2.5 for CNS and 1.1 for GN. The rate of hospital re-visit intention was 0.31 for CNS and 0.21 fot GN. The ratio of expected incomes between CNS and GN is 2.07 to 1. Conclusion: We found that when a CNS serves a patient, satisfaction, number of personal recommendations, hospital re-visit intention, and indirect marketing effects were higher than those of GN. These results show that CNS have dominant roles and functions and that the effects of CNS employment can be measured using economics parameters.
