Comparative Analysis between Spinning and Other Causes in Exercise-Induced Rhabdomyolysis

  • Purpose: Spinning-induced rhabdomyolysis (SIR) has been increasing in recent years and accounts for a large proportion of exercise-induced rhabdomyolysis (EIR). The purpose of this study was to compare the clinical features between SIR and non-spinning exercise-induced rhabdomyolysis (NSIR), and to analyze each of these clinical features. Methods: A retrospective chart review was conducted on patients treated due to EIR from January 2006 to March 2018. Patients were divided into the SIR and NSIR groups, and their clinical factors, outcome, and blood chemistries were compared and analyzed. Results: Sixty-two patients were enrolled in this study, with 23 (37.1%) and 39 (62.9%) patients categorized in the SIR and NSIR groups, respectively. The SIR group were mostly women (78.3% vs. 38.5%, p=0.002), more frequent EIR occurrence in the first exercise class (60.9% vs. 15.4%, p=0.001), and had most complaints of thigh pain (91.3% vs. 43.6%, p=0.001). The SIR group had a higher incidence rate despite its shorter exercise duration (90.5% vs. 62.9%, p=0.024), longer hospital stay (6.0 [4.5-7.0] vs. 5.0 [3.5-6.0] days, p=0.080), and higher rate of peak CPK (15,000 U/L or higher) (91.3% vs. 74.4%, p=0.182) compared to the NSIR group. Conclusions: SIR occurs at a higher rate during the first exercise class in women compared to NSIR, and the incidence rate is higher in SIR than in NSIR despite its shorter exercise duration (less than 60 minutes). It is necessary to recognize these risks during spinning exercises and to perform these exercises sequentially and systematically.

Effects of a 12-week Combined Exercise Program on Gait Parameters in Elderly Women with Osteoarthritis

  • Objective: The purpose of this study was to investigate the effects of a 12-week combined exercise program on gait parameters in elderly women with osteoarthritis. Method: The subjects of this study were 11 elderly women (age: $67.09{\pm}2.47$, height: $157.35{\pm}4.30cm$, weight: $62.49{\pm}6.36kg$) with knee osteoarthritis. The combined exercise program of this study was divided into aerobic exercise and lower extremity strengthening exercises. The exercises were performed for 60 minutes per session, three times a week, for a total of 12 weeks. The maximum joint moments of the hip, knee, and ankle joints with walking were obtained with 8-3D cameras (MX-T20, Vicon, USA) and 2-force plate (AMTI OR6-7-400, AMTI, USA). SPSS Windows version 23.0 was used for statistical analysis. A paired t-test was used for pre-post comparison. An alpha level of .05 was utilized in all tests. Results: The maximum extension moment was significantly higher in the hip joint after P1 on the X axis. The maximum joint moment of P3 in extension was statistically significantly lower after intervention. On the Z axis, the maximum joint moment was significantly lower after the exercise intervention at P3. There was a statistically significant increase in the extension moment of the left and right knee joints in the X axis after exercise intervention. In the right ankle joint, P1 (plantar flexion moment) showed a statistically significant high moment after exercise intervention. Conclusion: These results suggest that combined exercise, including lower limb and aerobic exercise, may have a positive effect on mobility and walking moments in patients with osteoarthritis of the knee.

Effect of Modified Clamshell Exercise on Gluteus Medius, Quadratus Lumborum and Anterior Hip Flexor in Participants with Gluteus Medius Weakness

  • PURPOSE: This study compared the effects of three different clamshell exercises (CLAM) on the gluteus medius (GMED), quadratus lumborum (QL), anterior hip flexor (AHF), gluteus medius/quadratus lumborum ratio, and gluteus medius/anterior hip flexor ratio by studying the activities of participants with GMED weakness. METHODS: Eleven subjects with weak GMED participated in this study. Subjects performed CLAM under three different conditions (standard, and modified 1 and 2). Surface electromyography was then used to measure the muscle activity and one-way repeated-measures analysis of variance was used to assess the statistical significance of the measured variables. RESULTS: GMED and the QL muscle activities did not differ significantly between the standard CLAM and the modified CLAM with the 2 different foot positions (F=4.74, P=.02; F=4.57, P=.02, respectively). AHF activity was significantly different in the two different foot positions when compared to the standard CLAM (F=11.17, P=.00). However, there was no significant difference between the AHF activities for the two different foot positions (P=.09). Finally, GMED/QL and GMED/AHF ratios were not significantly different between the three different CLAM exercises (F=.63, P=.55; F=.82, P=.45, respectively). CONCLUSION: Modified CLAM can be recommended as a good method to minimize AHF activity while maintaining GMED activity in subjects with weak GMED.

Effects of Kinesio Taping and Upper Coordination Exercises on Pain and Function of Shoulder Myofascial Pain Syndrome (키네시오 테이핑 요법과 상지 협응 운동이 견관절 근막동통증후군의 통증과 기능에 미치는 효과)

  • Purpose: The purpose of this study was conducted to investigate the effects of kinesio taping and coordination exercise on the myofascial pain syndrome and shoulder function. Methods: The patients with myofascial pain syndrome were participated in this study and divided randomly 2 groups. Control group (n=22) was taken only physical therapy program. Experimental group (n=22) was taken physical therapy with Kinesio taping and coordination exercise. The Kinesio taping in experimental group applied on levator scapulae and supraspinatus. The coordination exercise performed in supine position and sitting position on 15 times during 10 seconds each positions. We measured the pain degree using visual analog scale (VAS), pain rating score (PRS), pressure pain threshold (PPT), myofascial pain subjects symptoms index and shoulder motor function using constant shoulder assessment scale; CSA before and after experiment. Results: The significant test of CSA, myofascial pain subjects symptoms index, VAS, PRS according to applying the Kinesio taping and coordination exercise between groups used ANCOVA. In the result following analysis, there was significance on VAS (F=13.071, p=.031), PRS (F=12.130, p=.014), PPT (F=7.378, p=.016), CSA (F=5.302, p=.026) between control group and experimental group. Conclusion: Then, Kinesio taping and coordination exercise has benefit on the VAS, PRS, PPT, CSA in patients with myofascial pain syndrome. So, it may suggest that Kinesio taping combined with coordination exercise will be helpful of the pain and shoulder function improvement the patients with myofascial pain syndrome.

Effect of Chin Tuck Exercises on Various Postures and Muscle Activity of the Neck and Shoulder (다양한 자세에 따른 턱 당기기 운동이 목과 어깨의 근활성도에 미치는 영향)

  • Purpose: This study examined how performing chin tuck exercises in supine, sitting, and standing positions affects changes in the muscle activity of the neck and shoulder. Methods: Seventeen men and three women with forward head posture participated in the study. The subjects performed the chin tuck exercise using a pressure biofeedback unit. After determining the ideal order for performing the chin tuck exercise in supine, sitting, and standing positions through randomized controlled trials, muscle activity was measured in the dominant-side sternocleidomastoid, scalenus anterior, upper trapezius, and levator scapulae. Muscle activity was measured three times in each muscle for each position, and the average of the three measurements was used for analysis. Results: In this study, the chin tuck exercise performed in a supine position reduced muscle activity of the sternocleidomastoid, scalenus anterior, upper trapezius, and levator scapulae significantly more than performing the exercise in either sitting or standing positions (p < 0.05). No significant difference in muscle activity was observed when the exercise was performed in a sitting position versus a standing position (p < 0.05). Conclusion: Performing a chin tuck exercise in a supine position is more effective for stabilizing the neck and shoulder than performing it in a sitting or standing position, as it reduces excessive tension and fatigue in the neck and shoulder.

Comparison of Abdominal Muscle Activation During Lifting with Stabilization Method (물건 들기 시 복부 안정화 방법에 따른 몸통 근육 활성도 비교)

  • PURPOSE: This study examined the muscle activity of the abdominal muscle when lifting with abdominal hollowing with visual feedback and lifting with a pelvic compression belt. This study suggests how to lift an object safely in the workplace for people who bend their backs repeatedly. METHODS: The study was conducted on healthy men in their 20s and 30s. When lifting a 7kg object, lifting with abdominal hollowing with visual feedback, and lifting an object with a pelvic compression belt were performed three times in random order. The muscle activities were measured rectus abdominis (RA), external oblique (EO), internal oblique/transverse abdominis (IO/TrA) muscles, and abdominal hollowing exercises, and box lifting exercises were carried out in advance before the experiment. One-way ANOVA was used to compare muscle activities, and a Tukey HSD was used for post-analysis. The level of significance was set to .05. RESULTS: According to the study, there was no significant difference in muscle activity of the RA muscle depending on the lifting method (p > .05). There were significant differences between the EO and IO/TrA muscle (p < .05). The IO/TrA muscle activity showed the largest increase in lifting an abdominal hollowing with visual feedback (p < .05). The EO muscle activity increased in pelvic compression belt lifting (p < .05). The muscle activity was increased in RA, but there was no significant difference (p < .05). CONCLUSION: Abdominal hollowing lifting with visual feedback increases the muscle activity of the IO/TrA muscle, which is higher than normal, and affects the core stability of the body.

Analysis of the Effects on the Level of Pain and Functional Improvement After Integrated Korean Medicine in Patients with Shoulder Impingement Syndrome: A Retrospective Chart Review

  • Background: This study investigated the clinical effectiveness of Korean medicine (KM) treatment for shoulder impingement syndrome (SIS). Methods: There were 61 patients who were diagnosed with SIS in the Jaseng hospital network of KM (7 hospitals located in Korea: Gangnam, Daejeon, Bucheon, Haeundae, Bundang, Ulsan, and Gwangju) between January 1st, 2015 and December 31st, 2020 who were retrospectively reviewed. The patients were grouped according to complications, intake of analgesics, duration of illness preadmission, and treatment. Treatments consisted of herbal medicine, acupuncture, cupping, Chuna, pharmacopuncture, bee venom pharmacopuncture, medicinal steaming, Daoyin exercises, and physical therapy. By comparing the Numeric Rating Scale (NRS), Shoulder Pain and Disability Index, and European Quality of Life 5-Dimensions questionnaire scores, the effectiveness of integrated KM treatment was evaluated. Results: There were 14 males and 47 females. For inpatients diagnosed with SIS, the mean NRS score decreased from 5.78 ± 1.33 to 3.40 ± 1.43 (p < 0.001). The mean Shoulder Pain and Disability Index score decreased from 53.87 ± 14.76 to 38.56 ± 18.87 (p < 0.001), and the mean European Quality of Life 5-Dimensions questionnaire increased from 0.67 ± 0.13 to 0.76 ± 0.09 (p < 0.001) after KM treatment. Medicinal steaming (0.398; p < 0.001), acupuncture (0.290), cupping (0.288), bee venom pharmacopuncture (0.282), and Daoyin exercises (0.262; p < 0.05) had a positive correlation with improved changes in the NRS score. Conclusion: Conclusion: Treatment with integrated KM treatment improved the pain, range of motion, shoulder function, and quality of life of patients with SIS.

Effects of Posterior Oblique Sling Activation on Gluteus Maximus Muscle Activity during Prone Hip Extension Exercises in Healthy Male Individuals

  • Purpose: The purpose of this study was to investigate the effects of posterior oblique sling activation on the muscle activities of the gluteus maximus (GM), multifidus (MF), and biceps femoris (BF) during three different prone hip extension exercises in healthy male individuals. Methods: Twenty healthy subjects participated in this study. An electromyography device was used to measure the muscle activities of the GM, MF, and BF. Each subject was asked to perform three different prone hip extensions as follows: [1) Prone hip extension with knee flexion + hip abduction 30°; PHE1, 2) Prone hip extension with knee flexion + hip abduction 30° and shoulder abduction 125°; PHE2, 3) Prone hip extension with knee flexion + hip abduction 30° and shoulder abduction 125° with 1kg loading; PHE3, in random order. A one-way repeated measures analysis of the variance and a Bonferroni post hoc test were used to analyze the results. The statistical significance was set at α=0.01. Results: The muscle activity of the GM was significantly different between the three positions (Padj<0.01). The muscle activity of the GM was significantly greater during PHE3 compared with PHE1 and PHE2 (Padj<0.01). The BF muscle activity was significantly lower during PHE3 compared with PHE1 and PHE2 (Padj< 0.01). There was no significant difference in the muscle activity of the MF (Padj<0.01). The ratio of the muscle activity (ratio=GM/BF) during PHE3 was significantly greater compared to PHE1 and PHE2 (Padj< 0.01). Conclusion: The GM activity and GM/BF ratio during the PHE3 exercise were significantly greater compared to that during PHE1 and PHE2. Therefore, the PHE3 exercise could be recommended as a selectively effective GM activation exercise while decreasing the muscle activity of the BF.

Effects of Core Exercise on Body Composition, Abdominal Muscle Thickness, Trunk Muscular Endurance, and Psychological Factors in Normal-weight Obese Women (코어운동이 마른 비만 여성의 신체조성과 복부 근육두께, 몸통 근지구력, 심리적 요인에 미치는 영향)

  • Background: Normal-weight obese women have a normal weight with relatively low muscle mass resulting from high body fat. We aimed to investigate the effects of core exercise on body composition, abdominal muscle thickness, trunk muscular endurance, and psychological factors in normal-weight obese women. Design: One group pre-, mid-, and post-test design. Methods: A total of 20 normal-weight obese women were recruited voluntarily. Core exercises were performed for 30 minutes, three times a week for 4 weeks. InBody measurements, ultrasound imaging, and clinical tests were used to assess body composition, abdominal muscle thickness, trunk and core muscular endurance, and psychological condition(satisfaction with appearance and self-esteem). The dependent variables were measured three times: before the intervention (pre-test), 2 weeks after the intervention (mid-test), and 4 weeks after intervention (post-test). One-way repeated measures ANOVA and Friedman test were used for statistical analysis. Results: Although there was no change in the body composition (p>0.05), thickness of the transversus abdominis and internal and external oblique muscles, endurance of the trunk flexors and extensors and core muscles, and psychological condition showed significant differences after the intervention (p<0.05). Conclusion: Our findings will help develop exercise programs for normal-weight obese individuals that utilize the beneficial effect of core exercises.

Validity Analysis of Python Automatic Scoring Exercise-Problems using Machine Learning Models (머신러닝 모델을 이용한 파이썬 자동채점 연습문제의 타당성 분석)

  • This paper analyzed the validity of exercise problems for each unit in Python programming education. Practice questions presented for each unit are presented through an online learning system, and each student uploads an answer code and is automatically graded. Data such as students' mid-term exam scores, final exam scores, and practice questions scores for each unit are collected through Python lecture that lasts for one semester. Through the collected data, it is possible to improve the exercise problems for each unit by analyzing the validity of the automatic scoring exercise problems. In this paper, Orange machine learning tool was used to analyze the validity of automatic scoring exercises. The data collected in the Python subject are analyzed and compared comprehensively by total, top, and bottom groups. From the prediction accuracy of the machine learning model that predicts the student's final grade from the Python unit-by-unit practice problem scores, the validity of the automatic scoring exercises for each unit was analyzed.
