환자 맞춤형 전자기장 요실금 치료 시스템 개발 (Development of Individually Adapted Electromagnetic Therapy System in Incontinence)

  • 요실금은 자신의 의지와는 무관하게 소변이 나오는 배뇨이상으로 삶의 질을 떨어뜨리는 대표적인 여성 질환이다. 치료방법 중 자기장을 이용한 치료는 자발적인 운동을 요구하지 않고, 수치심을 유발하지 않으며, 감염의 위험이 없기 때문에 가장 높은 발전 가능성을 가지고 있다. 하지만 획일화된 치료 프로토콜, 환자 의존적 치료방법, 환자 상태 평가 부재 등의 개선점도 갖고 있다. 이에 본 연구에서는 자기장을 이용하여 골반저근을 자극하고, 치료 중 환자의 상태 변화를 실시간 모니터링하는 시스템을 제안하였다. 또한, 환자의 요실금 정도에 따른 환자 맞춤형 자기장 치료 시스템을 제안하였다. 제안된 시스템은 자기장 발생장치, 냉각장치, 치료의자, 맥박 모니터링을 이용한 환자 감시 장치, 제어 소프트웨어로 구성되었으며, 시뮬레이션을 통한 고전력 시스템 설계 과정과 개발 후 성능 평가를 수행하였다. 제어 소프트웨어 개발을 통하여 사용의 편의성과 관리의 편의성을 확보하였으며, 환자의 증상별 치료 프로토콜을 적용하였다. 본 연구를 통하여 개발된 환자 모니터링을 포함한 환자 맞춤형 자기장 치료 시스템은 자기장 치료에서 발생할 수 있는 위험성을 줄이고, 환자의 증상에 맞게 패턴화된 치료법을 제시함으로써 요실금 자기장 치료의 효율성을 높일 수 있고, 전/후 치료를 통하여 환자에게 보다 접근된 치료법이라고 판단되었다. 이러한 결과는 요실금 환자들에게 보다 안전하고 체계적인 치료법을 제공하며, 요실금 자기장 치료 방법의 다양한 연구를 유도할 수 있을 것으로 사료되었다.

림프부종 환자의 자가관리에 관한 실태 조사 (A Study on Self-care among the Lymphedema Patients)

  • Purpose: The purpose of this descriptive study was to gain basic data to develop a self-care protocol for the lymphedema patients. Method: The subjects of this study consisted of 115 patients with lymphedema from 8 hospitals and two community health and welfare centers in Busan and Seoul. The data was collected with questionnaire by self reporting of patients between March 2001 and December 2001. Data was analyzed by mean and percentage. Result: For self-care activities in daily life, compliance of 'use skin care preparations', 'use heat and cold', 'protect from local compression on affected limbs', 'protect from insect biting', 'use aids to protect affected limbs', 'take diuretics and take protein diet' did not reach to 50%. For self-care activities related to complex physical therapy, 28.7% of subjects complied with compression garment, 14.8% with manual lymph drainage, and 13.0% with exercise. 20.0% of subjects tried to treat with acupuncture and 13.9% with heat therapy. Conclusion: From this study, it is suggested that patients need to get a self-care education with correct information about self care activities and health care professionals need to develop more convenience self-care techniques of massage and exercise.

국내 비특이적 만성 요통 환자에게 적용한 비약물적 중재의 효과: 체계적 문헌고찰 및 메타분석 (The Effectiveness of Non-pharmacologic Interventions for Chronic Nonspecific Low Back Pain: A Systematic Review and Meta-analysis)

  • Purpose: The purpose of this study was to evaluate the effectiveness of non-pharmacologic interventions for chronic nonspecific low back pain (CLBP) in adults aged 18-64 years. Methods: We searched for potentially relevant randomized controlled trials and non-randomized controlled trials through five Korean electronic databases (i.e., Korean Studies Information Service System, Research Information Sharing Service, Korean Medical Database, KoreaMed, and National Assembly Library) published from January 2010 to May 2019. Two investigators independently selected the studies based on the criteria and assessed risk of bias in the included studies. We estimated the effect size of interventions using Comprehensive Meta Analysis 3.3. Results: Of 10,151 studies, 26 studies met the inclusion criteria and 15 studies were included in the meta-analysis. Exercise reduced low back pain (Hedges's g=-1.53, 95% CI: -2.22 to -0.85) and pain-related disabilities (Hedges's g=-0.92, 95% CI: -1.40 to -0.45). We found that taping was effective in decreasing low back pain (Hedges's g=-1.12, 95% CI: -1.51 to -0.73) and pain-related disabilities (Hedges's g=-0.50, 95% CI: -0.93 to -0.07). Manual therapy yielded a marginally significant reduction in low back pain (Hedges's g=-2.32, 95% CI: -4.64 to 0.00), the therapy was not effective in decreasing pain-related disabilities. Conclusion: Although there was little evidence for the effectiveness of manual therapy in adults with CLBP, exercise and taping were effective to relieve pain and pain-related disabilities. Based on these findings, we suggest the development of non-pharmacologic interventions or a nursing intervention protocol for the CLBP management. Also, nurses should consider implementation of effective non-pharmacologic interventions for CLBP.

직물형 피트니스 밴드 디자인 및 개발 (Design and development of fabric-type fitness band)

  • This study aims to contribute to the development of sports wearables. It was conducted by a convergence team of professionals in the fashion industry, kinesiology and sports studies, and computer science and engineering. The purpose of the current study was to design and develop a fabric-type fitness band for a sensor to measure acceleration during jump rope exercises. Computer science and engineering professionals developed the Arduino board and sensor, kinesiology and sports studies provided the necessary exercise protocol, and the fashion industry professionals developed the band. First, a fitness band preference survey was completed by men and women between the ages of 20 and 50. Typical uses of the band included tracking exercise amount as measured by the number of steps taken and calories burned. Strap watch closure, a single color and achromatic color, and soft and smooth touch materials were preferred as band design. Second, two fabric-type fitness bands were designed and developed. Design 1 had a 3-dimensional pocket for the sensor, bright blue color, and stretch binding around the edges and for a loop. Design 2 had a flat pocket for the sensor, achromatic color, mesh binding around the edges and two metal loops. Both designs had Velcro as a closure. Third, wear testing of both bands with the sensor were conducted of 15 women in their 20s. They wore the bands during jump rope exercises. Both bands generally satisfied the participants. The Design 2 band was slightly more satisfying than the Design 1 band.

Reference Equations for the Six-Minute Walk Distance in Healthy Korean Adults, Aged 22-59 Years

  • Background: The six-minute walk test has been widely used in people with chronic cardiopulmonary disorders as an outcome assessment with regards to therapeutic or prognostic determinants. This study was undertaken to determine the six-minute walk distance (6MWD) in a sample of healthy Koreans and to create a reference equation. We also compared the 6MWD of our cohort with previously published equations. Methods: Two hundred fifty-nine healthy subjects (95 males) aged 22-59 years performed two walking tests using a standardized protocol. 6MWD was defined as the greatest distance achieved from the two tests. The effect of anthropometrics on the 6MWD was also investigated. Results: The average 6MWD was $598.5{\pm}57.92m$, with significantly longer distances by males ($628.9{\pm}59.51m$) than females ($580.9{\pm}47.80m$) (p<0.001). Age, height, weight, and body mass index were significantly correlated with 6MWD in univariate analysis. Stepwise multiple regression showed height to be single independent predictor of 6MWD ($r^2$=0.205, p<0.001). The reference equations derived in Caucasian and North African populations tend to overestimate the distance walked by Korean subjects, while Asian equations underestimate it. Conclusion: The average 6MWD in these Korean populations was 600 m. The regression equation revealed that individual's height was the most significant predictor of distance, explaining 20.5% of the distance variance.

여름철 작업자들의 고체온증 예방을 위한 액체냉각복 개발 및 효과적인 냉각온도와 인체 냉각부위 탐색 (Developing Liquid Cooling Garments to Alleviate Heat Strain of Workers in Summer and Exploring Effective Cooling Temperature and Body Regions)

  • The purpose of the present study was to explore the most effective body region and cooling temperature to alleviate heat strain of workers in hot environments. We developed liquid cooling hood, vest, sleeves and socks and applied the water temperatures of 10, 15, 20, and 25℃ through the liquid cooling garments in a hot and humid environment (33℃ air temperature and 70% RH air humidity). A healthy young male participated in a total of 16 experimental trials (four cooling garments × four cooling temperatures) with the following protocol: 10-min rest, 40-min exercise on a treadmill and 10-min recovery. The results showed that rectal temperature, mean skin temperature, and ratings of perceived exertion during exercise; heart rate and diastolic blood pressure during recovery; and total sweat rate were lower for the vest condition than other garment conditions(p < .05). However, there was no differences in mean skin temperature among the four cooling garments when we compared the values converted by covering area(%BSA). When we classified the results by cooling temperature, there were no consistent differences in thermoregulatory and cardiovascular responses among the four temperatures, but 25℃ water temperature was evaluated as being the most ineffective cooling temperature in terms of subjective responses. In conclusion, the results indicated that wearing cooling vest with < 20℃ cooling temperature can alleviate heat strain of workers in hot and humid environments. If the peripheral body regions are cooled with liquid cooling garments, larger cooling area with lower cooling temperature than 10℃ would be effective to reduce heat strain of workers. Further studies with a vaild number of subjects are required.

일주일간의 간헐적 좌식차단의 혈관기능 개선 효과 (Seven Days Breaking Up Prolonged Sitting Improves Systemic Endothelial Function in Sedentary Men)

  • PURPOSE: To examine the cumulative (7 days) effect of breaking up prolonged sitting on systemic endothelial function in sedentary men. METHODS: Thirty sedentary men ($33.93{\pm}5.72years$) participated in two randomized 7 days sitting trial (Sit group (control) vs. Breaks group). The protocol of Breaks group is as follows: 4-minute of moderate-intensity marching in place (walking) every 1 hour during business hour (total: 8 breaks/day). Assessment of brachial artery endothelial function using flow-mediated dilation (FMD) and arterial stiffness indices (augmentation index, arterial pressure and pulse wave velocity) were measured before and after 7 days treatment. RESULTS: Brachial artery FMD significantly increased after 7 days breaking up prolonged sitting treatment (Breaks groups, $9.65{\pm}2.61$ to $9.62{\pm}2.6%$) compared with 7 days prolonged sitting (Sit group, $8.37{\pm}3.41$ to $10.11{\pm}3.75%$) (interaction effect, p=.004). Arterial pressure (AP) significantly increased after treatment (Breaks group, $2.75{\pm}2.19$ to $2.38{\pm}1.63mmHg$, p=.002) in Sit group but there was no change (Sit group, $1.00{\pm}3.18$ to $2.50{\pm}9.23mmHg$) in Breaks groups (interaction effect, p=.008). CONCLUSIONS: These finding show that 7 days regular breaking up prolonged sitting improve in FMD, compared with prolonged sitting. Therefore, regular breaking up prolonged sitting may improve systemic endothelial function in sedentary men.

선박기인 온실가스 배출에 대한 IMO의 규제와 이행방향 (A Study on the IMO Regulations regarding GHG Emission from Ships and its Implementation)

  • 교토의정서에서 국제항해 선박의 온실가스 배출규제문제를 IMO에 위임하여 현재 IMO MEPC에서 논의중에 있다. 유엔해양법협약에 따라 모든 국가들은 해양환경보호를 위한 동등한 의무와 책임을 부담하여야 하며, 항만국은 입항하는 외국적 선박을 비차별적 원칙에 따라 통제하도록 하고 있다. 그러나 교토의정서의 기본협약인 기후변화협약은 온실가스 배출의 역사적 책임을 일부 선진국에 부과하는 차별적 공동책임을 기본원칙으로 내세우고 있기 때문에 IMO가 마련하고 있는 선박기인 온실가스 배출규제의 동등한 의무와 책임 원칙과 상반되는 양상을 보이고 있다. 따라서 이 논문에서는 기후변화협약의 발전과정과 기본원칙을 살펴보고 IMO의 선박기인 온실가스 배출규제 최근동향을 통하여 유엔해양법협약과 IMO의 규제에 있어 국제법상 제기될 수 있는 문제점을 검토하여 선박기인 온실가스 규제를 유엔해양법협약과 IMO 협약의 원칙에 따라 현실적으로 구현할 수 있는 이론적 기초와 함께 이행수단을 제공하고자 한다.

Variability in physical therapy protocols following total shoulder arthroplasty

  • Background: Physical therapy (PT) plays an important role in the recovery of function following anatomic total shoulder arthroplasty (aTSA). While several PT protocols have been published for these patients, there is no standardized protocol for aTSA rehabilitation. This lack of standardization may lead to confusion between patients and physicians, possibly resulting in suboptimal outcomes. This study examines how PT protocols provided by academic orthopedic surgery programs vary regarding therapeutic goals and activities following aTSA. Methods: PT protocols for aTSA available online from the Accreditation Council for Graduate Medical Education accredited orthopedic surgery programs were included for review. Each protocol was analyzed to evaluate it for differences in recommendation of length of immobilization, range of motion (ROM) goals, start time for and progression of therapeutic exercises, and timing for return to functional activity. Results: Of 175 accredited programs, 25 (14.2%) had protocols publicly available, programs (92%) recommended sling immobilization outside of therapy for an average of 4.4±2.0 weeks. Most protocols gave recommendations on starting active forward flexion (24 protocols, range 1-7 weeks), external rotation (22 protocols, range 1-7 weeks), and internal rotation (18 protocols, range 4-7 weeks). Full passive ROM was recommended at 10.8±5.7 weeks, and active ROM was 13.3±3.9 weeks, on average. ROM goals were inconsistent among protocols, with significant variations in recommended ROM and resistance exercise start times. Only 13 protocols (52%) gave recommendations on resuming recreational activities (mean, 17.4±4.4 weeks). Conclusions: Publicly available PT protocols for aTSA rehabilitation are highly variable. 한의약소재 스포츠음료수 섭취가 운동-유발성 피로의 단시간 회복에 미치는 영향 (Effect of Oral Sport Beverages with Medicinal Herbs Added on Short-term Recovery from Exercise-induced Fatigue)

  • Objectives : Ginseng Research Group in Korea Food Research Institute developed Saeng Mac San (KFRI-2)and Je Ho Tang (KFRI-3) with their sensory factors more acceptable. And we examined their effects on the short-term recovery capacity for cycling exercise (EX) maintained to all-out. Methods : Seven healthy young subjects (aged $24.0{\pm}2.1yr$) were volunteered at this double blind test. Each of KFRI-2, 3, a commercial sport beverage and control (CON) was offered randomly on a series of EX protocol including 65% VO2max-90min EX (D-ride). 1h-recovery and 85% VO2max EX to all-out (P-ride) under the control of their heart rate (HR) and rating perception of exertion (RPE). Blood samples were collected before D-ride, 30, 60 and 90min in D-ride, 30 and 60min in the recovery period and each 10min in P-ride. Plasma analysis items were glucose, insulin, cortisol (CORT), testosterone (TEST), free fatty acid (FFA), $Na^+$, Cl-and $K^+$. The collected data (Means${\pm}$SE) were analysed by two-way ANOVA and statistically significant differences between treatments (p<0.05) by LSD.; the significant level in FFA, $Na^+$, Cl-and $Na^+$ was p<0.01 Results : At 30min during recovery. plasma glucose level in KFRI-3 was significantly higher than CON, and also insulin in KFRI-3 was than CON and KFRI-2. FFA in KFRI-3 was significantly lower than CON during recovery. $Na^+$ in KFRI-3 significantly higher than CON at 90min in D-ride, and also KFRI-2 was at 60min during recovery. However CORT, TEST, Cl-and $Na^+$ in treated beverages were not significant. KFRI-2, 3 elevated the time for P-ride more than CON did. Conclusions : KFRI-2, 3 elevated the time for P-ride about 12% more than CON did. It is based on rapid recovery of plasma glucose level and inhibition of lipolysis during recovery.
