The quantitative sensory testing is an efficient objective method for assessment of nerve injury

  • Background: This study evaluated Somatosensory evoked potentials (SEP), Quantitative sensory testing (QST), and thermography as diagnostic methods for nerve injury. Methods: From 2006 through 2011, 17 patients (mean age: 50.1 years) from ${\bigcirc}{\bigcirc}{\bigcirc}{\bigcirc}$ Hospital who sought care for altered sensation after dental implant treatment were identified. The mean time of objective assessment was 15.2 months after onset. Results: SEP of Inferior alveolar nerve(IAN) was $15.87{\pm}0.87ms$ on the normal side and $16.18{\pm}0.73ms$ on the abnormal side. There was delayed N20 latency on the abnormal side, but the difference was not statistically significant. In QST, the abnormal side showed significantly higher scores of the current perception threshold at 2 KHz, 250 Hz, and 5 Hz. The absolute temperature difference was $0.55^{\circ}C$ without statistically significance. Conclusion: These results indicate that QST is valuable as an objective method for assessment of nerve injury.

  • The purpose of intra-operative neurological monitoring (INM) is to minimize surgically induced nerve damage, sensory nerves and motor neurons without affecting the operations to proceed during surgery such as evoked potentials (EP), electromyography (EMG), electroencephalography (EEG), transcranial doppler (TCD), etc. During the course of checking a patient's condition, surveillance of ambulatory patients is a very different thing to check if the test is done under general anesthesia. INM can be possible or impossible depending on the type of drugs used and their concentrations because the monitoring is performed under anesthesia. Therefore, it is emphasized on the necessity of reviewing anesthesia which influences on INM.

천막아래 뇌경색 환자에서 전정유발근육전위 (Vestibular Evoked Myogenic Potentials in Infratentorial Infarction Patients)

  • Vestibular evoked myogenic potentials (VEMP) have been known to useful in documenting abnormality in patients with various vestibular disorders but the studies of VEMP in stroke patients are rare. We recorded VEMP in 17 consecutive patients with acute ischemic stroke in the brainstem lesions. All patients underwent magnetic resonance imaging and we compare VEMP results with the lesion documented by brain imaging. VEMP were defined to be abnormal when they were very asymmetrical (one is 2 times of more as large as the other), or absent in one side. VEMP abnormalities were found in 71%(12/17) of acute infarction patients with brainstem lesions. Most abnormalities found in the ipsilateral side of the lesion(9/12) but abnormalities in contralateral side of lesion were found in 25%(3/12) of patients.VEMP would be considered a useful complementary neurophysiological tool for the evaluation of brainstem dysfunction in acute stroke patients.

  • Evoked Potentials signals occur as a result of neuroelectric responses of the brain to sensory stimulation. In this paper, to analysis such signals we utilize a time-frequency analysis technique called wavelet transform. The wavelet analysis is performed based on a single prototype function,which can be thought of as a bandpass filter. Because the wavelet transform in a fine temporal analysis decomposes time-varying signals in EP into a dilated lowpass and a contracted highpass components, EP signal fetures can be obtained and analysed quantitatively at the levels of resolution. In the results, we analyze the VEP signal with the wavelet transform.

  • 부중심소엽 주변의 경막하 전극들에서 기록한 PTSEP는 전극의 위치에 따라 다양한 파형을 나타내며, 파형의 최대치는 주로 부중심소엽의 후 상부에 위치한다. 특히 최대값이 주변보다 확연히 차이 나며 10uV이상의 파형으로 기록될 경우는 40 msec에서 양극성을 나타낸다. 또한 주변의 전극과 상역전을 형성할 수 있는데 최대 전극의 뒤에서 형성될 경우는 더욱 분명한 상역전을 관찰할 수 있다. 따라서 경막하전극을 통한 PTSEP는 내측 반구의 부중심소엽 주변의 위치를 파악하는데 도움을 줄 수 있다고 생각된다.

Intraoperative Monitoring of Motor-Evoked Potentials for Supratentorial Tumor Surgery

  • Objective : The purpose of this study was to assess the feasibility and clinical efficacy of motor evoked potential (MEP) monitoring for supratentorial tumor surgery. Methods : Between 2010 and 2012, to prevent postoperative motor deterioration, MEP recording after transcranial stimulation was performed in 84 patients with supratentorial brain tumors (45 males, 39 females; age range, 24-80 years; median age, 58 years). MEP monitoring results were correlated with postoperative motor outcome compared to preoperative motor status. Results : MEP recordings were stable in amplitude (<50% reduction in amplitude) during surgery in 77 patients (91.7%). No postoperative motor deficit was found in 66 out of 77 patients with stable MEP amplitudes. However, postoperative paresis developed in 11 patients. False negative findings were associated with edema in peri-resectional regions and postoperative bleeding in the tumor bed. MEP decrease in amplitude (>50%) occurred in seven patients (8.3%). However, no deficit occurred postoperatively in four patients following preventive management during the operation. Three patients had permanent paresis, which could have been associated with vascular injury during tumor resection. Conclusions : MEP monitoring during supratentorial tumor surgery is feasible and safe. However, false negative MEP results associated with postoperative events may occur in some patients. To achieve successful monitoring, collaboration between surgeon, anesthesiologist and an experienced technician is mandatory.

Near-Infrared Laser Stimulation of the Auditory Nerve in Guinea Pigs

  • This study has investigated the feasibility of 980-nm low-energy pulsed near-infrared laser stimulation to evoke auditory responses, as well as the effects of radiant exposure and pulse duration on auditory responses. In the experiments, a hole was drilled in the basal turn of the cochlea in guinea pigs. An optical fiber with a 980-nm pulsed infrared laser was inserted into the hole, orientating the spiral ganglion cells in the cochlea. To model deafness, the tympanic membrane was mechanically damaged. Acoustically evoked compound action potentials (ACAPs) were recorded before and after deafness, and optically evoked compound action potentials (OCAPs) were recorded after deafness. Similar spatial selectivity between optical and acoustical stimulation was found. In addition, OCAP amplitudes increased with radiant exposure, indicating a photothermal mechanism induced by optical stimulation. Furthermore, at a fixed radiant exposure, OCAP amplitudes decreased as pulse duration increased, suggesting that optical stimulation might be governed by the time duration over which the energy is delivered. Thus, the current experiments have demonstrated that a 980-nm pulsed near-infrared laser with low energy can evoke auditory neural responses similar to those evoked by acoustical stimulation. This approach could be used to develop optical cochlear implants.

이상운동 질환에서의 신경생리적 검사법의 유용성 (Usefulness of Electrophysiological Tests in Movement Disorders)

  • In clinical neurology various different electrophysiological tests are widely used to demonstrate the unsuspected malfunctioning in the nervous system and to monitor over time the clinical status of patients. In addition clinical neurologists and neurosurgeons take advantage of the intraoperative monitorings to increase the quality of neurosurgical operations in the posterior fossa, in the spinal cord, or in visual pathways. In the field of movement disorders, elecrophysiolgical tests provide neurologists with making accurate differential diagnoses with useful therapeutic stratergies as well as with investigating the pathophysiological machanisms. By using the electromyographic tests it could be possible for us to evaluate the types of blephalospasm, the extent of hemifacial spasm, the level of myoclonus, and the prime muscles of torticollis etc. Sometimes the myographic guidance may be critical for choosing the exact injecting site of botulinum toxin. These several decades various electroencephalographic and evoked potential tests has been utilized in the electrophysiological laboratories to understand the basic pathophysiology of myoclonus, spasticity and other central motor dysfunctions. It could be one of the breakthroughs in the area of behavorial neurology that the brain function can be mapped by the spontaneous or evoked electrical activities of nervous system since the movement related potentials (MRPs) had been studies for several decades. Various reflex tests such as masseter reflex, blink reflex, click evoked vestibulocollic reflex, facial reflex, stretch reflex, flexor reflex, H-reflex, H-reflex recovery curve, vestibular inhibition of H-reflex, reciprocal inhibition, recurrent or Renshaw reflex, Ib inhibition, cutaneous reflex have been also used to understand normal or abnormal physiology in movement disorders. Polysomnography, posturography and gait studies are also applied in clinical neurology in association with with movement disorders which are useful in deciding the treatment regimen.

근위축성 측삭 경화증의 하 운동 신경원 과흥분성: 운동유발전위를 이용한 분석 (Lower Motor Neuron Hyperexcitability in Amyotrophic Lateral sclerosis: Analysis Using Motor Evoked Potentials)

  • Background & Objectives: Hyperexcitablity of motor system is a well-established characteristic pathophysiologic finding of amyotrophic lateral sclerosis (ALS). Whereas little is known about the source of excitability according to the progression of the disease. We evaluated the excitability and its source in advanced ALS patients using transcranial magnetic stimulation (TMS). Meterial & Methods: Motor evoked potentials (MEP) by TMS were recorded for abductor pollicis brevis muscles in 20 patients, 11 men and 9 women, with ALS. Mean age was $54.2{\pm}12.1years$, and mean disease duration was $13.9{\pm}13.4years$. Serial magnetic stimulations were applied to get the parameters; excitability threshold (ET), amplitude and latency of MEP. We also had a facilitated MEP (fMEP). Results: The parameters were analyzed according to the clinical settings. ET was higher in ALS(mean $63.5{\pm}18.1$) than normal control (mean $46.0{\pm}8.4$, p<0.01). Amplitudes of MEP were reduced in ALS ($2.6{\pm}3.6mV$; control $6.5{\pm}3.1mV$, p<0.01). Duration of the disease and ET showed significant inverse correlation (Spearson correlation coefficient = -0.57, p<0.01). Duration of the disease and fMEP/MEP ratio showed less but also significant inverse correlation (Spearson correlation coefficient, r = -0.52, p < 0.05). Conclusions: Lower ET in advanced ALS patients, in spite of decreased fMEP/MEP ratio, may indicate the hyperexcitability of lower motor neurons in these patients. This study suggests that lower motor neurons is hyperexcitable due to upper motor neuron dysfunction at advanced stage.

  • Motor evoked potential of spinal surgery is known to cause damage due to the movement path of the continuous scan operation and surgery can be performed with minimized disability after surgery. However, if it is not at all formed at the wave motion evoked potential can occur during surgery and, in some cases the size of the waveform to be measured is very small and intermittent. In this case, the surgery cannot provide information about whether there is neurological damage. Increased intensity of the wave-induced motion of the dislocation does not occur if it appears in a very small amplitude stimulus, but changing the inspection area that electrical stimulation of the waveform changes could not be found. However, stimulation of a wide area in the cerebral cortex was found to occur with a waveform in the patients who underwent examination. Through this study, we propose a useful motor evoked potential test. From November to December 2015 three spine surgery patients visited Samsung Medical Center as neurosurgery patients with omission discomfort, gait disturbance, and no symptom of strength before surgery. In spine surgery patients with motor grade weakness, when motor evoked potential waveform has not been measured, in examination of the site of electrical stimulation of the cerebral cortex from entering the C3+C5/C4+C6 or C3+C1/C4+C2 if by the activity of more motor neuron unit, it was found that the waveform is better formed.
