A Study On Related to Test and Evaluation of Nursing Information System (간호정보시스템의 시험평가에 관한 연구)

  • Currently, the domestic nursing information system software products are being developed competitively with the participation of many companies, generating quantitative development with high value added. However, it is true that the study that concurrently adds qualitative enhancements to such quantitative development has been inadequate. In order to secure international competitiveness in the field of nursing information system software, it is necessary to establish quality requirements on this system, and along with it, establish an evaluation system on the nursing information system software products. The purpose of this study is to analyze the accompaniment and technology of the nursing information system, and by identifying the requirements, establish test and evaluation system of the said system.

Development of Evaluation Indicators for Integrated Home Care (통합가정방문급여 평가지표 개발)

  • Purpose: The purpose of this study was to develop integrated evaluation indicators of home care services in the hope that the increasing group of long-term home care patients could receive quality care services. Methods: The development involves a methodological study on a development phase and a verification phase. The main survey at a verification stage was conducted by the staffs at 146 institutions who agreed to participate on this study. Results: The evaluation index for the integrated home visit care consisted of five categories and 57 indicators including Managing Institution (12), Environment and Safety (3), Right and Responsibility (7), Process of Care (31) and Results of Care (4). The criterion-related validity was verified in regard to the participation in the 2010 evaluation of long-term home-care institutions by the National Health Insurance Corporation. Conclusion: The evaluation index of the integrated home visiting care developed in this study is considered suitable to utilize as evaluating indicators in managing and evaluating the way of how institutions integrate and provide home visit care services as well as home nursing care services.

Validity and Reliability of the Korean Version of the Observable Indicators of Nursing Home Care Quality Evaluation Instrument (한국형 노인요양시설 질 관찰평가도구의 타당도와 신뢰도 분석)

  • Purpose: The purposes of this study were; 1) to test the validity and reliability of the Korean version of Rantz's Observable Indicators of Nursing Home Care Quality Instrument (ONHQ) and 2) to evaluate the quality of Korean nursing homes. Methods: The study employed a three-phase methodological research design. 1) The original instrument of Rantz's. ONHQ was translated into Korean and modified by Korean nursing home experts. 2) A pilot study using the modified instrument was done in 20 nursing homes to examine inter-rater reliability. 3) The validity and reliability were tested.in 98 nursing homes. Results: Seven factors were extracted through factor analysis: 'communication', 'care delivery', 'grooming', 'odor, 'environment-basics', 'environment-access', and 'environment-homelike'. These factors explained 86.07% of the total variance. Cronbach's alpha coefficient of the 30 items was .97 indicating a high internal consistency of the instrument. Inter-rater reliability according to Kappa was .82. The average score of nursing home quality was 112.07 indicating an average range of quality level. Conclusion: The Korean version of the ONHQ was identified as a tool with a high degree of validity and reliability. This tool can be effectively used to assess the quality of nursing homes by professions as well as family members.

An Integrative Review of the 'QUality Of care Through patient's Eyes' (환자의 눈을 통한 간호의 질(QUOTE) 평가의 통합적 고찰)

  • Purpose: The patient's perspective in the assessment of quality of care has become increasingly important. The purpose of this paper was to introduce the concept of 'QUality Of care Through patient's Eyes (QUOTE)', to analyze the papers using QUOTE through an integrative review method, and to present the attributes of the QUOTE evaluation. Methods: Data were collected from electronic databases. Inclusion criteria were publication in English from 1997 to 2013, as a peer-reviewed research article, with an empirical study focused on QUOTE. Thirty two papers met the criteria and were analyzed. Results: QUOTE had been applied to various groups of patients such as those with various diseases since 1997 in the Netherlands. Four themes were retrieved from synthetic analysis of the thirty-two papers using QUOTE; 1) acceptance of individuation of the patient and family, 2) evaluation of diverse nurses' competencies, 3) evaluation of quality of nursing environments, and 4) participation of patient in the whole process of evaluation. Conclusion: The strengths of QUOTE are that it attempts to overcome the conceptual and methodological problems associated with evaluation of quality of care. Our results imply that QUOTE evaluation could be an effective strategy to improve care of patients in clinical setting.

Development of Detailed Clinical Models of Nursing Information for Initial Assessment (초기사정을 위한 간호정보조사지의 임상내용 모델 개발)

  • Purpose: The purpose of this study is to develop a detailed clinical model for recording initial nursing assessment items, and to test the applicability of the model to facilitate semantic interoperability for sharing and exchanging nursing information. Methods: First, the researchers extracted items by analyzing initial nursing assessment records. Second, defining characteristics were identified by analyzing nursing records and reviewing the literature. Third, value sets for defining characteristics were identified and types and cardinalities of defining characteristics were defined based on the value sets. Finally, the detailed clinical model was tested through evaluation by experts and comparison with the initial nursing assessment in a clinical setting. Results: Sixty-one detailed clinical models were developed with 178 defining characteristics and value sets. The experts evaluation and comparison with the initial nursing assessment in a clinical setting showed that the detailed clinical model developed in this study was valid. Conclusion: Use of this detailed clinical model can ensure that the Electronic Health Record contains meaningful and valid information and supports semantic interoperability of nursing information. This use will promote quality in the nursing records and eventually quality of nursing care.

A study of the Nursing Service Quality and Satisfaction that Admitted Patients Perceived - being used SERVQUAL - (입원환자들이 지각하는 간호서비스 질과 만족에 관한 연구 - SERVQUAL을 중심으로 -)

  • This study was performed to measure the nursing service quality being used SERVQUAL model and satisfaction that the admitted patients perceived. Method: The questionnaire founded on the SERVQUAL was developed and distributed to 300 patients at the three general hospitals in three provincial city, Korea. For data analysis, Cronbach's α, frequencies, percentages, paired t-test, Pearson Correlation Coefficient were used. Result: In expectation, patients most highly perceived the assurance factor that was one among the 5 factors being constituted nursing service. In performance, patients most highly perceived the responsiveness factor. The performance degrees of the 5 factors and 20 attributes being constituted nursing service did not exceed the expectation degree of those. So the calculated figures for nursing service quality of the three subjected hospitals were all minus. In relation of the patients' demographics and nursing service characteristics to their general satisfaction, patients' sex, age, income and the all factors and attributes of nursing service had relation to their general satisfaction. Conclusion: It could be concluded that the nursing service quality of the three subjected hospitals was poor and the patients' demographic and nursing service characteristics had relation to their general satisfaction.

Influence of Oral Health Knowledge and Awareness of Caregivers in Charge on Oral Health-Related Quality of Life of the Elderly in Nursing Homes (요양보호사의 구강보건 지식, 구강건강 인식이 요양시설 노인의 구강건강 관련 삶의 질에 미치는 영향)

  • The purpose of this study was to identify the influence of oral health knowledge and awareness of caregivers in charge on the oral health-related quality of life of the elderly in nursing homes. Data were collected from 115 elderly without dementia and their 115 caregivers in nursing facilities in S and C cities. The data were analyzed using SPSS/WIN 22.0 program. The average score for oral health knowledge and awareness of the caregivers were 11.62, 39.22 points each and the oral health-related quality of life of the elderly was 40.62 points. Oral health knowledge, awareness of caregivers and oral health-related quality of life of the elderly showed a difference according to oral health education experience of the caregivers (𝜌<.001), the nursing facility evaluation grade (𝜌=.016), and the oral health education experience (𝜌=.008), working hours of 40 hours or less per week of caregivers (𝜌=.008) each in order. The influencing factors on the oral health-related quality of life of the elderly were the oral health education experience, the working hours per week of the caregivers and the facility evaluation grade. This findings imply that developing customized program and the work environment improvement for caregivers should be considered to improve the oral health-related quality of life of the elderly in nursing homes.

A study for quality assurance of visiting nurses service of a public health center (보건소 방문 간호 사업의 질보장을 위한 연구)

  • The purpose of the study is to evaluate the visiting nurses service of a public health center. Data were collectd from the 36 clients who received services from a public health center. In terms of the process evaluation, the tool is composed 4 parts, 27 items such as assessment planning, implementation, and evaluation. It was measured through the health records by 2 peer review. In terms of the outcome evaluation, the level of client satisfaction was measured by self report or interview by 2 supervisor. The result were as follows: 1. 30% of 36 health records showed narsing process was not and out of them, nursing care plann including spectific activities were rarely established or unclear. 2. The lack of systematic data collection' showed and nursing diagnosis was not adressed in health records review. 3. Client satisfaction score was 32, 97, out of maximum score 36. 4. The lack of sufficent objective data, care plan, record of client's health status change, and evaluation was founded therefore quality assurance for visiting nurses service and in-service education are required and the development of standardized record system need.

Development of the Quality Indicators in Long Term Care Service (장기요양서비스의 질 평가 지표 개발)

  • Purpose: This study was designed to develop quality outcome indicators for nursing homes and community-based home care that would contribute to an appropriate evaluation and improvement of quality of long term care in Korea. Methods: The preliminary quality indicators of long term care were developed from a literature review and clinical expert panel. A content validity testing was done using a panel of experts who were selected from academic and clinical field of long-term care. The final quality indicators were confirmed after application in four nursing homes and four home care agencies to test clinical validity. Results: The preliminary quality indicators consisted of 3 domains and 19 indicators. The final quality indicators were composed of 4 domains and 17 indicators. Conclusion: This study demonstrated the feasibility of outcome quality indicators in long term care. These quality indicators can be effectively used to evaluate the quality of nursing home and home care and to improve the quality of care in the Korean long-term care system.

A Study on the Differences of Perception between Consumers and Providers about Nursing Service Quality, and the Usefulness of Tools Measuring Nursing Service Quality (간호서비스 질에 대한 지각차이 및 측정도구에 관한 연구)

  • Service quality is, unlike goods quality, an abstract and elusive term. However, quality is very important to marketers and consumers in that it has many strategic benefits in contributing to profitability of marketing activities and consumers' problem-solving activities. The main purpose of this study is 1) to explore the differences of perception between consumers and providers about nursing service quality. 2) to identify the useful tool between two tools measuring nursing service quality. To achieve these purposes of the study, the questionnaire was developed and distributed to 210 nurses who worked at seven subjected hospitals in Seoul. Also, 280 people who had a direct experience with the subjected hospital nursing services at the time of screening were involved. They were randomly selected at the seven subjected hospitals during August to September of 1998. Among them, 165 responses from nurses and 229 responses from others resulted in worthy finds. The measurement instrument for a nursing services quality evaluation was modified from the SERVQUAL model originated from Parasuraman, Zeithaml, and Berry (1988). The reliability coefficient of the scale was calculated and showed high degree of internal consistency (Cronbachs' Alpha = .9353). For data analysis, SPSS/PC was used for descriptive statistics, t-test, ANOVA and regression analysis. The results were as follows : 1) In the perception about nursing services quality, there were gap between consumers and providers. Especially the critical attributes in point of perception nursing services quality, naming satisfy, hygiene and performance factors, are very different contents. 2) In the comparison analysis of the usefulness tools it was turned out that SEVPERF model is more appropriate than SERVQUAL model. And in the analysis to identify the construct validity of the tool, it is turned out that the relationship between the constructing factors of the tool and the general satisfaction is significant.
