The Research Trends of Papers in The Korean Journal of Occupational Health Nursing (산업간호학회지 논문의 연구동향 분석)

  • This study examined all the papers published in the Journal of Occupational Health Nursing from 1991 to May 2000 based on an objective frame of evaluation. The purpose of study is to prospect the trend of occupational nursing research and to provide an optimal direction for future occupational nursing service. The results of total 94 studies were analyzed as follows. 1. The number of papers published in 1991-1992, 1993-1995, 1996-1997 was not significantly increased. However, the number of papers were obviously increased after 1998. 2. In the design of research, survey studies were definitely dominant. But, they are gradually decreasing and experimental studies are increasing. 3. In the fields of research, health problems, health risk factors and health awareness were higher than others. Occupational health nurse's task and job satisfaction were main areas of research until the middle of 1990's. However, after then, not only health problem, risk factor and awareness of health but also health promotion and preventive health behavior are increasing in the research areas. 4. The collaborative researches have gradually increased and the type of author has been varied. The number of researches completed by funds was undertaking only 3 among the total 94. 5. In the research analysis, except for case studies and literature studies, the research subjects have been shifted from the occupational health nurses to the industrial workers, and more diversified. The sample size has been enlarged and the major place of the study is industry. The questionnaire was used in many studies as instrument for data collection and measurement. But, complementary methods of diverse instruments have been increased. The method of data analysis has obviously showed some changes moving from descriptive statistics to inferential and advanced statistics. 6. The ways of nursing intervention have prominently diversified in experimental studies. The contents of case studies were mainly focused on the health management programs and activities in industry. The research concepts and the contents of literature study were also changed progressively in various way. Based on the above findings, conclusions are extended to the following discussion: Since the Korean Academic Society of Occupational Health Nursing was instituted in 1990, the researches of occupational health nursing have shown an acute methodological development in both quantity and quality. However, they were considered still in the lack of research verifying the effects of nursing intervention program. Also, the development of nursing intervention and nursing theory lacked. Therefore, more empirical researches are strongly needed to utilize the field of occupational health nursing in Korea Researchers are also encouraged to exert more efforts to get research funds.

Factors Influencing Suicidal ideation among Korean University Students

  • Korea has the highest suicide rate among the OECD countries, and the suicide rate is highest among young adults in their 20s, most of whom are university students. Therefore, suicide among Korean university students is a public health issue that is of interest to us. The purpose of this study is to investigate the factors affecting the suicidal ideation of university students in Korea, and to use them as a basis to establish effective intervention for university suicide prevention through it. This was a cross-sectional descriptive study using convenience sampling method. The participants were 344 university students at universities in S and G cities. Data were collected with a structured questionnaire and analyzed using descriptive analysis, t-test, ANOVA, Pearson correlation coefficients, and hierarchical regression analysis using with the SPSS/Win 23.0 program. There was a significant correlation between depression (r=.45, p<.001), drinking alcohol (r=.14, p=.008), social support (r=-.26, p<.001), quality of life (r=-.46, p<.001), and suicidal ideation. In the first step of hierarchical regression analysis, satisfaction of school life (${\beta}=.198$, p<.001) was the significant factor influencing the suicidal ideation. Explanatory power was 25.2%. In the second step of the hierarchical regression analysis, absence of parents (${\beta}=-.095$, p=.044), depression (${\beta}=.247$, p<.001), quality of life (${\beta}=-.280$, p<.001), and explanatory power were increased to 42.0%. The results of the study indicate the need to actively identify the group of university students in their 20s with high risk of suicide through continuous evaluation of depression, and to improve the quality of life as a method of preventing suicide. In addition, the effect of absence of parents on the suicidal ideation among Korean university students suggests that parental support may play an important role in suicide prevention.

Evaluating Chronic Care of Public Health Centers in a Metropolitan City (만성질환 관리 평가도구를 이용한 보건소 만성질환 관리수준 평가)

  • Background: To evaluate the quality of chronic care provided by public health centers located in a South Korean metropolitan city using a modified Assessment of Chronic Illness Care (ACIC). Methods: We conducted self-evaluation surveys and collected data using a modified ACIC from twenty five public health centers. Cultural validity of the original ACIC was examined by the public health and nursing science experts. Based on expert reviews, cognitive interviews, pre-test results, five items of the original ACIC that were not relevant were deleted. The response scale was changed from twelve-point Likert scale to Guttman scale but its scoring system was maintained. Results: Eighty eight percent of public health centers in this study reported that their overall quality of chronic care was at a limited or basic level. About 68% of the centers reported that the organization was as reasonably good or fully developed to provide chronic care. On the other hand, 96% of the public health centers reported that the clinical information system was at a very limited or basic support level. The decision support, the integration of Chronic Care Model components, the delivery system design, the community linkages, and the self-management support were evaluated as limited or basic level of support by more than half of the public health centers, respectively. Conclusion: In a metropolitan area of South Korea, quality of chronic care in public health centers was not found to reach acceptable levels of services. It is critical to enhance the quality of chronic care in public health centers.

How do Lung Cancer Patients Experience Stigma?: A Meta-synthesis of Qualitative Studies

  • Purpose: Lung cancer patients are often stigmatized since lung cancer is closely associated with smoking, which is a self-administered life style. The stigma of lung cancer has been examined in some qualitative studies; however, their findings were diverse and not yet synthesized. Therefore, this meta-synthesis study aimed to explore how lung cancer patients experience stigma. Methods: A meta-synthesis method, as suggested by Sandelowski and Barroso in 2007, was applied by aggregating the findings after an evaluation according to consolidated criteria for reporting qualitative research (COREQ). Results: By synthesizing the findings of the selected seven papers, a synthesized theme was emerged as "experiencing external and internal distances, which mandates authentic and consistent supports." The four sub-themes included 'experiencing some distance from the surrounded world,' 'experiencing self-made distance between the disease and oneself,' 'the disease experience causes social isolation and loneliness,' and 'there is lack of supportive care for myself.' Conclusion: Health care providers should be more attentive to supporting lung cancer patients by providing more effective advocacy programs that improve patients' quality of life.

The Effects of the Tai Chi Exercise on Metabolic Syndrome and Health-related Quality of Life in Middle-aged Women (타이치 운동이 중년여성의 대사증후군 위험인자 및 건강관련 삶의 질에 미치는 영향)

  • Purpose: The purpose of this study was to test the effects of the Tai Chi exercise on metabolic syndrome and health-related quality of life in middle-aged women. Methods: A quasi-experimental design was used. Subjects were sixty middle-aged women with metabolic syndrome. All of the subjects were met the criteria of the National Cholesterol Education Program-Adult Treatment Panel III (NCEP-ATP III). The subjects were divided into the experiment group (n=33) trained the Tai Chi for 12 weeks and the control group (n=27). Metabolic syndrome risk factors including blood pressure, waist circumference, glucose, triglyceride (TG), and high density lipoprotein cholesterol (HDL-C) were measured before and after the 12-week period. Euro Quality of Life Questionnaire 5-Dimensional Classification (EQ-5D) was used to evaluate the health-related quality of life. Results: The experiment group showed significant decreases in diastolic blood pressure, waist circumference, glucose, and TG; and increase in HDL-C compared to the control group. For the health-related quality of life evaluation, the experiment group showed significant improvement more than the control group. Conclusion: The Tai Chi exercise may be effective intervention in preventing cardiovascular disease caused by metabolic syndrome in middle-aged women.

  • The aim of the study is to analyze and evaluate the Child Health Management Service that was promoted as a part of Mother-Child Health Guidance Project by the Public Health Center of BukCheju-gun in 1999, to reflect the problems needed to be improved, and, thus, to present the integral program model for the Child Health Management Service that guides childhood health, regarded as the foundation of that of the adulthood period, to the optimum level. The common results of the evaluation of the Child Health Management Service of BukCheju-gun public health center are as below: 1. In the aspect of public information, the existing simple material and method of public information was not effective. 2. The opportunities for the necessary training and education to develop the persons concerned and their ability for the Child Health Management were not enough.3. The environment under which the service, aimed to promote Child's health, was carried out was not clearly divided from that of the services related with the prevention and treatment of disease. And the service environment for the child health was threatening to the clients.4. Still, the actual result was pursued more than the quality management, while carrying out the project. With the reflection of the above result, the research has presented the program model of Child Health Management Program for the next. The main concept of the model is, through the establishment of the network for information offerings and effective convergence of a variety of opinions for the community around the Public Health Center and for the correct evaluation of the project and the reflection of the result from it, to achieve the ultimate goal of the optimal growth and development of the childhood by expanding the support necessary for carrying out Child Health Management Program more effectively.

Evaluation of Physicians' Perception of Patient Safety Incidents Including Disclosure Utilizing Hypothetical Clinical Vignettes

  • Purpose:We investigated physicians' responses to a series of clinical vignettes consisting of patient safety incidents, with and without disclosure of patient safety incidents (DPSI). Methods: An anonymous survey was conducted to investigate physicians' responses to the DPSI via online communities of physicians, and additional participants were recruited using a snowballing sampling method. We evaluated physicians' responses to the DPSI using eight hypothetical scenarios (HS) from the following perspectives: thoughts regarding medical errors, revisiting the physician, recommendation, lawsuit, criminal prosecution, trust score, and compensation amounts. We used the chi-square test to evaluate the overall differences in response rates among the scenarios. Statistical analyses were performed using the Student's t-test to compare the trust scores and compensation amounts. Results: A total of 910 physicians participated in this survey. An overall comparison of trust scores among HS showed that HS 1 (unclear medical errors, minor harm, and DPSI) had the highest trust score. In contrast, in the opposite scenario, HS 8 (clear medical errors, major harm, and DPSI not conducted) received the lowest scores. Cases with minor harm to patients (HS 1, 2, 5, and 6) showed lower compensation amounts than the others (HS 3, 4, 7, and 8). Physicians were more likely to think of situations with DPSI as not having medical errors (53.1% vs. 55.2%). In addition, the scenarios with DPSI were evaluated favorably in terms of intention to revisit, recommend, suit, and engage in criminal proceedings. Physicians showed higher trust scores (6.2 vs 5.4) and gave lower compensation amounts ($27.7 million vs $28.1 million), although there was no significant difference in terms of compensation amounts to the physician conducting DPSI. Conclusion: Our study showed overall positive perceptions regarding DPSI among Korean physicians.

A Study on the Determinants Which Influence on Evaluating the Silver Welfare Facilities (노인복지시설평가에 영향을 미치는 요인에 관한 실증연구)

  • This study is to search the determinants which influence on evaluating the silver welfare facilities, of which data for this study were collected from the survey on the evaluation of social welfare facilities which were enforced by the Ministry of Health and Welfare in 2006. As the results, all the factors for the management of those facilities, such as facility and environment, management of organization and human resource, service quality, and community relation, influences significantly on silver welfare facility. Among the factors, the development of service quality should be taken care of, since service quality strongly explains the result of evaluation through the regression analysis. In regard of the fundamental factors, the juridical fund itself has no influence on the evaluation of the institutes, while the patronage fund for the facilities has significant influence on silver welfare facilities except elderly housing. However, the fundamental support from government has negative influence on the facilities for specialized nursing.

  • Platelet products are used to treat hemorrhagic or platelet dysfunction diseases. Plateletpheresis involves collecting the platelet components of blood using an apheresis blood-collection system. Various indicators are available for evaluating the qualities of the apheresis platelets. The productivity of platelet collection is evaluated through both the collection efficiency and collection rates. Platelet storage quality can be evaluated in vitro using several indicators, including visual appearance, metabolic activities, volume, platelet count, white blood cell count, microparticles, and various platelet activation markers. Platelet activation markers have been used as indicators of storage quality in various studies. Post-transfusion platelet quality can be evaluated based on the corrected count increment and the percentage of platelet recovery. Although various studies have investigated the aspects of plateletpheresis, no article has systemically presented assessments of the platelet products obtained from different plateletpheresis devices. The present study provides a review of plateletpheresis, including the specifics of the process, the types of devices employed, the platelet quality, the overall efficacy, and the evaluation indicator qualities. Furthermore, the differences in functionality among the different apheresis devices are discussed. Although adverse reactions to the citrate anti-coagulant have been reported, apheresis processing may provide a safer option for donors who are at a high risk for presyncopal or syncopal reactions related to whole blood collection.

Revised Clinical Ladder System Model Based on Nurses' Clinical Competence and Professional Activities for Nurses in General Hospitals (종합병원 간호사의 간호역량, 전문성 활동에 따른 임상경력관리체계 수정모형)

  • Purpose: The purpose of this study was to propose a revised Clinical Ladder System(CLS) Model for nurses based on the evaluation of clinical competence and professional activities of nurses working in general hospitals. Methods: Data were collected between September 10 and October 30, 2017. Participants were 50 head nurses from 10 general hospitals with over 400 beds located in Seoul City and Gyeonggi Province. Each head nurse evaluated clinical competence, qualifications, and professional activities of 5 staff nurses at each of the 5 levels of CLS in her unit. The total number of the nurses evaluated was 245. Data were analyzed with descriptive statistics and t-test, one-way ANOVA, and $Scheff{\acute{e}}$. Results: Over 80% of the nurses were university graduates. As the CLS levels increased, clinical competence, qualifications, and professional activities also increased significantly. Education material development and quality improvement activities were carried out by nurses from level 2, research and evidence based practice activities were carried out from level 3, and nurses at level 4 or 5 participated in most of the professional activities as leaders. Conclusion: In order to retain excellent nurses in general hospitals, recognizing and rewarding nurses according to the revised model of the CLS are recommended.
