Effect of the Physical Activity Promotion Program on Physical Function and Quality of Life in Elderly (신체활동증진 프로그램이 노인의 신체 기능 및 삶의 질에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effect of the physical activity promotion program on physical function and quality of life in the elderly. Method: The research was designed with one single pre-post group. The study group of 295 elderlies were selected through the convenience sampling form in a senior citizen's center in Ulsan city. The subjects received the physical activity promotion program for 60 min three times a week for 3 months. To evaluation the effects of the physical activity promotion program, body mass index (BMI), physiological index, physical function (muscle endurance, agility, balance) and quality of life were measured at before and after the program. The data was analyzed through SPSS 10.0 program for descriptive statistics and paired t-test. Result: After performing the physical action promotion program for 3 months, the results were follows. 1) There were significantly increased in BMI (t=4.36, p=.000). 2) There were significantly decreased in diastolic pressure (t=.68, p=.009). 3) There were significantly improved in muscle endurance (t=6.50, p=.000), agility (t=4.74, p=.000) and Rt. leg balance (t=4.15, p=.000). 4) There were significantly improved in quality of life (t=7.28, p=.000). Conclusion: These findings showed that the physical activity promotion program was effective in improving physical function and quality of life in the elderly. Therefore, the physical activity promotion program could be applied as an effective nursing intervention program for the elderly to promote their health.

Variations in Nurse Staffing in Adult and Neonatal Intensive Care Units (의료기관 및 중환자실 특성에 따른 간호사 배치수준)

  • Purpose: This study was done to analyze variations in unit staffing and recommend policies to improve nursing staffing levels in intensive care units (ICUS). Method: A cross-sectional study design was used, employing survey data from the Health Insurance Review Agency conducted from June-July, 2003. Unitstaffing was measured using two indicators; bed-to-nurse (B/N) ratio (number of beds per nurse), and patient-to-nurse (P/N) ratio (number of average daily patients per nurse). Staffing levels were compared according to hospital and ICU characteristics. Result: A total of 414 institutions were operating 569 adult and 86 neonatal ICUs. Tertiary hospitals (n=42) had the lowest mean B/N (0.82) and P/N (0.76) ratios in adult ICUs, followed by general hospitals (B/N: 1.34, P/N: 0.97). Those ratios indicated that a nurse took care of 3 to 5 patients per shift. Neonatal ICUs had worse staffing and had greater variations in stafnng ratios than adult ICUs. About 17% of adult and 26% of neonatal ICUs were staffed only by adjunct nurses who had responsibility for a general ward as well as the ICU Conclusion: Stratification of nurse staffing levels and differentiation of ICU utilization fees based on staffing grades are recommended as a policy tool to improve nurse staffing in ICUs.

Analysis of Published Papers by Keywords and Research Methods in the Korean Journal of Women Health Nursing (2007~2009) (여성건강간호학회지의 최근 3년간의 논문분석(2007~2009년))

  • Purpose: This study s aim was to analyze literature using the identified key words of published literature within the Korean Journal of Womens Health Nursing from 2007~2009. The study explored the major subjects and research methodology used and evaluated the quality of this Journal articles. Methods: Survey design using a structured analysis format was used for consistent evaluation of research subjects, type of research, research methodology, data analysis, along with comparing intervention research, theoretical framework, ethical consideration, and the implications. The frequency of the use of each keyword was classified by 8 domains. Results: Ninety eight papers were analyzed altogether. Most of the subjects in these papers were women with 86.7% being quantitative research. In these studies, 76.5% used convenience sampling; 70.6% used a survey design whereas 23.5% used experimental design. Health related concepts (n=159) was mentioned the most among the 8 domains. The key words frequently used were women, depression, knowledge, quality of life, attitude, elderly, anxiety and stress in descending order. Conclusion: It can be concluded that most papers in this Journal during this time period addressed recent women's health issues. Also it showed this Journal and its literature has reached the indexing for Medline.

Trends of Nursing Research in the Journal of Oncology Nursing (종양간호학회지 논문 내용과 경향 분석;창간호에서 2007년까지)

  • Purpose: Purpose of study was to analyze the research articles in the Journal of Korean Oncology Nursing in order to provide an direction for the future research, Methods: This study analyzed 93 studies published in the Journal of Korean Oncology Nursing, from its beginning year to the year 2007, according to the research objectives. The frame of evaluation included years and types of publication, the theoretical frameworks, research design, subject, data collection method, keyword analysis by MeSH. Results: 45.2% of studies was non-degree based studies. 95.7% of studies was not described theoretical framework in the articles. 71.0% was utilized a non-experimental design. 57% of subjects in researches was patients. 76.3% of studies were used the questionnaire for data collection. Concepts as human, nursing, and health were consistently appeared in research. But concepts of environment has been insufficiently conducted. Conclusion: Researches in the Journal of Korean Oncology Nursing has been changed in methodology and the topics of research for the last 7 yr. It progresses in both quantity and quality. But, it is necessary to conduct research founded on theoretical background, various research design, variability of study subjects and topics as supported by scientifically and empirically.

The Perception Degree of Teaching Effectiveness of Nurses and Nursing Students on Clinical Nursing Practicum (임상실습에 대한 간호사와 간호학생의 교수효율성 인지정도)

  • Purpose: This study was conducted to use as a preliminary data to improve the quality of clinical nursing education by comparing the perception of teaching effectiveness in nurses and nursing students. Method: The participants were 165 nurses and 289 nursing students. Data was collected from June 2 to 16, 2008. The research instruments used an effective clinical instructors which was developed by Reeve(1994). Result: The perception of teaching effectiveness in nurses and nursing students were 3.45 and 3.19 respectively and it showed that nurses' perception of teaching effectiveness are higher than that of nursing students. Nurses had strong points in the factors of "interpersonal relationship/ communication", "availability as a supporter", "teaching method and evaluation".In additions, the teaching effectiveness had significant differences to age, years of work, position, educational background and job satisfaction in nurses, otherwise, sex, differences of educational level, satisfaction of clinical practicum and major in students. Conclusion: It is needed a further study as below: to develop an education system which is to improve an interpersonal relationship and to upgrade the quality of educator; to develop a teaching method program of clinical nursing education to improve teaching effectiveness.

Critical Thinking in Nursing Science: A Literature Review (간호학적 비판적 사고능력에 대한 문헌고찰)

  • Purpose: The purposes of this study were as follows; 1) To review the definitions of critical thinking from various perspectives, 2) To examine the critical thinking measurements throughout nursing research, and 3) To review the nursing studies with regard to critical thinking. Methods: This study was a literature review with regard to the critical thinking in nursing in aspects of conceptual meaning, measurements, and research. Results: The definition of critical thinking in nursing included decision making in clinical setting, inference with logical construct to increase nursing quality, interpretation in the context, and evaluation. The critical thinking was a core concept, which meant not only simple nursing process, but included decision making ability. The critical thinking has been conceptualized by both critical thinking disposition and skill. However, there was no nursing specified critical thinking measurement. Critical thinking research has been conducted to describe critical thinking disposition and critical thinking, to determine relationships between critical thinking and clinical competency, and to evaluate the effectiveness of educational programs. Conclusion: The instruments for measuring critical thinking disposition and skill that contain cultural difference and clinical specificity need to be developed to measure critical thinking and increase it.

Satisfaction of Community Practice in Nursing Student (일부 간호대학생의 지역사회간호학 실습에 대한 만족정도)

  • Purpose: Nursing education should be considered the training for professional nurses who can deliver high quality care to meet the needs of health consumers. This study was aimed at evaluation on practice satisfaction of nursing student in the community. Method: The data was collected from 107 students at 4 colleges and 72 students at 3 universities in Daegu city from March 1st to June 30th, 2001. This study was investigated by the questionnaire which was consisted of general characteristics, recognition of nursing and practice satisfaction. The questionnaire of practice satisfaction used in this study was modified from Lee, S.J.(1980)'s and Park(1994)'s questionnaire. The data was analyzed by SAS(ver. 6.12) program and statistical methods used were mean, standard deviation and ANOVA(analysis of variables). Result: The findings of this study were as follows: 1. Students showed that nursing is a kind of activities for promotion of health, prevention of disease, recovery of health and relieving of pain in the recognition of the nursing. 2. Students showed that the community- practice gives an opportunity to learn the relationship with health team for the solution of trouble patients in the recognition for the necessity of the community-practice. 3. In the practice satisfaction, students gained high score in evaluation, but especially low score in instruct of the community-practice. 4. The general characteristics which affected practice satisfaction were grade, motivation of nursing selection and duration of practice in the community-practice. Conclusion: As theses results it was necessary to prepare the programed and developed practice-education in the community.

Concept Analysis of Spiritual Care (영적간호의 개념분석)

  • Purpose: The purpose of this study was to identify attributes of the concept of spiritual care. Method: Walker & Avant's concept analysis framework was employed to review the clinical guidelines, nursing text books, and nursing research articles which were related to spiritual care and published from 1985 to 2005. Result: The attributes of the concept of spiritual care were a three stage process such as spiritual assessment, spiritual intervention, and spiritual evaluation. Spiritual care included three dimensions of relationships such as transpersonal, interpersonal, and intrapersonal. The quality of spiritual care was dependent upon characteristics of care-givers such as perception and knowledge of spiritual care, and the clinical environment. The antecedents of spiritual care was spiritual needs due to the prompt events. The consequence of spiritual care was spiritual well-being. Conclusion: This concept analysis of spiritual care contributed to promote performance of spiritual care in clinical fields by removing conceptual ambiguity and confirming the true meaning of spiritual care.

Identification and delegation of indirect care interventions (간접간호중재의 수행 및 위임에 관한 분석)

  • The purpose of this research was fourfold: (a) to identify the use rate of the indirect care interventions performed by nurses, (b) to estimate the time to perform each intervention, (c) to identify the indirect care interventions to be delegated to others, and (d) to determine the level of provider preparation needed to delegate indirect care interventions. The sample consisted of 199 nurses working in three hospitals. The Indirect Care Survey developed by the Iowa Intervention Project team was used for data collection. The instrument was translated to Korean and validated by nurse experts. Each of the 26 indirect care interventions were used several times a day. Four interventions (i.e.. Documentation, Shift Report, Specimen Management, and Transport) were performed several times a day by 50% or more of the nurses. The most frequently used intervention was Documentation, followed by the interventions Shift report. Environmental Management, Transport, and Examination Assistance. The least used intervention was Quality Monitoring, followed by the interventions Order Transcription, Referral, Health Care Information Exchange, Multidisciplinary Care Conference, and Product Evaluation. The intervention taking the most time to per-form was Technology Management (155.3 minutes), followed by the interventions Documentation, (122.2 minutes), Delegation (84.4 minutes), Supply management (83.4 minutes), and Preceptor: Student (79.9 minutes), Overall, the nurses reported that they would not delegate to others the majority of the interventions. More than 50% of the nurses would not delegate 21 interventions. Shift Report would not be delegated by 95% of the nurses and Documentation would not be delegated by 92% of the nurses. Caregiver Support would be delegated by 68% of the nurses to family. Three interventions (i.e.. Environmental Management, Examination Assistance, and Transport) would be delegated by more than 50% of the nurses to Nursing Assistant. This study will contributes to determining costs of nursing services and enhancing quality of nursing care. Replication study will be needed with large sample.

An Evaluation Study of Dementia Information Providing Web sites in Korea (국내 치매정보에 관한 웹사이트 평가)

  • Purpose: The present study purposed to investigate and analyze domestic websites providing information about dementia and to suggest future directions for the development of dementia-related websites. Mothod: For this purpose, the researcher selected 13 domestic websites that were available in November and December 2004, and evaluated them in terms of construction, operation, accessibility and contents on a scale 4 point (0$\~$3). Result: The construction of dementia-related websites got 6$\~$13 out of 18 points, which suggests that management policies for the operation of dementia-related web sites are inadequate. The operation of dementia-related web sites got 7$\~$15 out of 24 points. In particular, all 13 sites got a low score in the aspect of continuance. With regard to accessibility, the sites were evaluated on how easily users could access the sites and they got 2$\~$8 out of 15 points. In evaluating contents, the sites got 9$\~$18 out of 21 points with regard to the purpose and appropriateness of the contents. Conclusion: This shows that most sites did not provide diverse types of dementia-related information. Because it is highly advantageous to perform primary dementia-preventing management through websites, this study proposes to develop a website evaluation system in order to provide high quality dementia-related information.
