간호대학생에서 자동화 무선 수면추적기와 주관적 수면도구를 이용한 수면측정 (Measurement of nursing students' sleep using by an automated wireless sleep tracker and subjective sleep scales)

  • Purpose: This study was conducted to investigate sleep patterns in nursing students as measured by an automated wireless sleep tracker and self-reported sleep scales. Methods: Data were collected from October, 2011 to May, 2012. ZQ (a symbolic number that measures the quality and quantity of sleep) was calculated from the automated wireless sleep tracker. Pittsburgh Sleep Quality Index-Korean version, Sleep scale A, and sleep satisfaction were used as self-report sleep scales. Female nursing college students (n=37) were recruited by convenience sampling. Before going to bed, they were asked to put on a head-band type sleeping sensor and to move it the next morning. Every student slept 2~4 nights with the sleep tracker. Data were collected for 86 (91.4%) of 94 nights' sleep. The students were also requested to fill in the PSQI-K, sleep scale A, and sleep satisfaction. Results: The only statistically significant correlation was between ZQ and sleep satisfaction (r=.869, p=.026). The biggest obstacle in using this device was putting on the head-band type sensor. Conclusion: Validity of the wireless sleep tracker was verified in part. The usefulness of this device in nursing research and practice was identified. Further evaluation need to be conducted with various populations.

의료기관인증제도의 국내외 비교 연구 -한국, 미국 및 호주 중심으로- (A Comparative Study on National and International Hospital Accreditation Systems Focusing on Korea, the U.S., and Australia)

  • 본 연구는 국내 의료기관인증제도와 미국과 호주의 의료기관인증제도 및 국제의료질향상학회의 국제인증프로그램에 대해 비교 고찰함으로써 우리나라 의료기관인증제도의 발전을 위한 시사점을 제시하고자 시도되었다. 본 연구에서 도출된 시사점은 다음과 같다. 첫째, 환자 및 지역사회 중심의 관점 강화, 인력배치 기준 강화 등 의료기관 인증기준의 개선이 필요하다. 둘째, 의료기관인증제도를 통한 환자안전의 강화가 필요하다. 셋째, 의료기관인증제도와 임상질지표 및 환자경험평가 연계를 강화해야 한다. 넷째, 조사위원의 역량을 강화해야 한다. 다섯째, 중소병원의 인증제도 참여 활성화가 필요하다. 여섯째, 의료 질 평가 결과 공개의 강화가 요구된다. 본 연구 결과는 우리나라 의료기관인증제도의 개선을 위한 기초자료로 활용될 수 있을 것이다.

Influencing Factors for Sleep Quality among Firefighters: Based on Objective and Subjective Evaluation

  • Purpose: The purpose of this study was to assess insomnia and the quality of sleep, investigate the concordance between objective and self-report sleep patterns, and identify physiological, psychological, and situational factors influencing insomnia and sleep quality among firefighters. Methods: A descriptive, cross-sectional study was conducted with 103 firefighters in Korea. The collected data were analyzed using SPSS 23.0. Descriptive statistics, the independent t-test, and hierarchical logistic regression analysis were performed. Results: Insomnia was found in 66 (64.1 %) of the total subjects, and the average quality of sleep (PSQI) was 5.65 (SD=2.57). Total sleep time (401.00 minutes) and sleep latency (21.60 minutes) measured using self-reported scales were longer than the ones measured using objective measurements by approximately 48.70 and 17.10 minutes, respectively. Factors related to insomnia included the role as a paramedic (OR=4.28, 95% CI: 1.02~17.92), anxiety (OR=1.12, 95% CI: 1.01~1.24), and sedentary lifestyle (OR=0.85, 95% CI: 0.78~0.94), and factors related to sleep quality were physical illness status (OR=5.17, 95% CI: 1.53~17.51) and social support (OR=0.86, 95% CI: 0.78~0.95). Conclusion: The results show a high prevalence of insomnia, poor quality of sleep and the discrepancy between objective and subjective sleep patterns among firefighters. To promote sleep quality and health, early screening and treatment of anxiety and physical illness are required. It is necessary to conduct further studies examining the relationship between physical activity level and sleep.

간호학생을 위한 활력징후 전자교과서 개발과 평가 (Development and Evaluation of a Vital Signs E-book for Undergraduate Student Nurses)

  • Purpose: The purpose of this study was to develop a vital signs e-book for undergraduate student nurses and evaluate the content, system and student satisfaction. Method: This study was done in three stages, the development of a vital signs e-book, implementation and evaluation. The subjects were 73 undergraduate student nurses in Y university. Result: Thirty one learning objectives were used to create the contents. A set of 5 chapters and 18 subsections were defined after validation from nurse educators. The e-book is available at http://123.134.207.23/ebook/vitalsigns. Analysis of the questionnaires showed a mean score for content, system and students satisfaction of $3.17\pm73,\;3.11\pm79,\;and\;2.96\pm.74$ respectively out of a possible 4 points. Conclusion: Nurse educators should provide quality and effective web-based courses that meet undergraduate student nurses' learning needs and they should incorporate web-based learning into traditional teaching to meet the demands of nursing education.

공립요양병원 운영평가 평가지표 개발 방안 (Development of Quality Indicators for Public Convalescent Hospitals)

  • 1990년대 후반부터 공립요양병원이 설립되었고, 2018년 기준, 79개소로 전체 요양병원의 5.1%를 차지하고 있다. 문재인 정부에서 치매 국가책임제를 발표하면서 공립요양병원을 치매안심병원으로 지정하여 중증치매환자에 대한 전문치료를 수행하는 기관으로 육성하고자 다양한 정책을 펼치고 있다. 이와 관련하여 2018년에 치매관리법을 개정하여 공립요양병원 운영평가를 위한 법적 근거가 마련되었으며, 2020년부터 본 평가가 시작되었다. 본 연구는 2020년도 공립요양병원 운영평가를 시행하기 위한 연구의 일환으로 공립요양병원을 치매환자의 전문치료기관 및 회복기 재활환자를 치료할 수 있는 역량 있는 기관으로 육성할 수 있도록 기능을 정립하였으며, 양질의 진료서비스, 공공성, 책무성 등 공공기관으로서의 행태를 평가할 수 있도록 평가체계를 개발하였다. 또한 개발된 평가지표가 적용가능한지 검증하기 위해 공립요양병원 79개소 중 20개소를 대상으로 시범 평가를 시행하였다. 본 평가의 결과를 바탕으로 교육 및 피드백이 진행되고 지속적으로 관리 할 수 있는 모니터링시스템을 구축함으로써 전체 요양병원의 기준(standard)으로서의 기능과 역할을 수행할 수 있을 것으로 기대한다.

간호간병통합서비스 병동 입원 환자의 만족도와 서비스 평가를 위한 ICT 기반 통계분석 (Statistical Analysis Based on ICT for the Satisfaction and Service Evaluation of Patients Admitted to a Nursing Care Integrated Service Ward)

  • 간호간병통합서비스는 적정 간호인력 배치를 통한 팀 간호체계의 총체적인 전문 간호 제공과 병동 환경개선 및 환자 안전관리 등 제반 여건을 갖춘 병동에서 제공하는 입원 서비스이다. 연구 대상은 경기도내 일 종합병원의 해당 병동 입원환자로서 연구 목적을 이해하고 참여하기를 동의한 92명이며, 연구방법은 ICT 기반의 설문 자료를 SPSS/WIN 21.0 프로그램을 활용하여 빈도, 백분율, 평균, 표준편차, t-test, ANOVA을 적용하였으며, 사후검정은 Scheffe test를 하였다. 또한, 간호만족도와 서비스 평가의 상관관계는 Pearson's correlation으로 분석하였다. 본 논문은 만족도의 중요성을 재확인하고 간호간병통합서비스 병동 입원 환자들의 서비스의 질 향상을 위한 기초자료를 제시했다는 점에서 그 의의가 있다.

건강코칭프로그램이 빈곤계층 고혈압대상자의 자기효능감, 건강행위실천 및 삶의 질에 미치는 효과 (The Effect of Health Coaching Programs on Self-Efficacy, Health Behaviors, and Quality of Life in Hypertensive People Living in Poverty)

  • Purpose: This study was designed to determine the effects of health coaching and mediating variables on quantitative aspect of health in low-income hypertensive people. Methods: The experimental group for the current study consisted of 21 clients who received health coaching services, and the control group consisted of 22 clients who received home-visiting nursing services. Two groups received health coaching or home-visiting nursing services once a week for 8 weeks. The evaluation variables were self-efficacy, nutrition management, health behaviors, self-rated health, and quality of life. Results: The results revealed that the level of nutrition management was significantly higher in the experimental group than the control group (F=10.33, p=.005). Conclusion: These results confirm that health coaching is a useful strategy that encourages clients to continuously maintain their own health behaviors. Thus, the findings of the current study provide useful data for establishing measures for the health management of those afflicted with chronic disease, such as hypertension. Furthermore, health coaching may be developed into useful intervention strategies for dealing with chronic diseases and improving home-visiting nursing.

간호서비스 질에 대한 소비자와 제공자의 지각차이에 관한 분석 (A Study of the Perception Gap on Nursing Service between Consumers and Providers)

  • this study was to investigate the perception gap on nursing service between consumers and providers. Method: the questionnaire founded on the SERVQUAL was developed and distributed to 300 patients and 210 nurses at the three subjected general hospitals in three provincial city, Korea during February to March, 2001. For data analysis, Cronbachs' Alpha, frequency, t-test, and paired t-test were used. Result: 1. In the gap analysis on the 20 properties constituting nursing service, providers almost all perceived higher than consumer in quality. Among them, the number of properties being statistically significant was 7 in the expectation, 18 in the important and 7 in the performance. 2. In the comparison analysis of the perception gap on the expectation-performance and the important-performance, it turned out that the subjected hospitals had to improve their equipment and facilities immediately. It was suggested a good strategy to strengthen the responsiveness factor and the assurance factor of nursing service. Conclusion: it could be concluded that nurses have to recognize the blind spot of their perception and endeavor to take away the perception gap between consumers and providers.

국내 질 향상부서 중심의 질 지표 측정 현황, 장애요인과 평가 (The Current State of and Barriers to Quality Measurement, and Quality Managers' Reported Evaluation on Quality Indicators in Korea)

  • The purposes of this study were to Identify the .level of measurement on quality Indicators and evaluate the existing indicators in order to determine the priority of quality indicators' application in Korean general hospitals. A survey was conducted using a questionnaire. The subjects were quality managers working at general hospital having over 300 beds. The criteria were relevance, reliability, precision, impact, application, and preference to evaluate quality indicators. According to these six criteria, each indicator was evaluated on a five point scale(5: excellent, 1: poor). The response rate was $40.4\%$. The hospitals have monitored the average of 3.8 indicators(median 4). The indicators such as return to operating room, unplanned readmission, cancellation of booked operations, death, hospital infection, cesarean section rate, volume per disease or procedure, readmission, re-operation, blood transfusion, and post-procedural complications were frequently measured. The top ten quality indicators in the evaluation by its relevance, validity, reliability, impact, preference and application were decubitus ulcer, clean wound infection, fall, unplanned return to operation room, transfusion reactions, foreign body left In during procedure, unplanned readmission, wound infection after contaminated surgery, postoperative hemorrhage/hematoma, and cesarean section rate in order. The high priority quality indicators frequently measured could be used as primary national indicators. Standardized guidelines about monitoring indicators and the utilization will preliminarily be needed to compare and reuse the data for various purposes and improve the quality of care continuously.

요양보호사 교육기관 교육서비스 질 평가 도구개발 (Development of an Evaluation Instrument for Education Services Quality in Learning Facilities for Care Workers)

  • The purpose of this study is to identify the important factors of education service quality and to develop the evaluation instrument in learning facilities for care workers. This study was processed as four steps. First, the important factors were confirmed through literature reviews and the expert board discussions. Second, the evaluation instrument was developed and validity & reliability of the instrument were checked by factor analysis and Cronbach's ${\alpha}$. Lastly, the instrument was rechecked by the expert board in language usages and importance of the items. The results indicated that the important quality factors were divided with infrastructure and service process. Especially, the process contained providing appropriate education information & counseling, providing appropriate contract & paying fees, keeping specified service principles and so on. The study instrument was developed with 28 items. This instrument will be helpful to improve the education service quality. Besides, the staffs must continuously try to improve and manage their education services using the results of this kind of evaluation instrument.
