TOTAL ESTHETIC ORTHOGNATHIC SURGERY : I. NEW ESTHETIC LINES AND INTER-ESTHETIC LINE ANGLE (총체적 심미 악안면 성형수술 : I. 상하악 악교정 수술을 위한 새로운 연조직 심미기준선)

  • Improvement of orthognathic surgical techniques make it possible to design esthetic surgical correction for total esthetic face. In order to find the esthetic line which guide esthetic surgical correction in patients of orthognathic surgery, cephalometric soft tissue analysis of esthetic faces were performed. In esthetic Korean young adults, 25 males and 25 females who were within 1 S.D. of E-line, ANB, P/A facial height ratio, were analyzed in natural position keeping their face eye level. 1. Sn position is constant in males and females. The Sn-N'-N' Vertical plane angle is $5.3^{\circ}$ in both sexes. Sn is positioned in front of 5 mm in female 7 mm in male from the N' vertical plane. 2. The Sn-Ls line make constant angle to horizontal plane with $72.5^{\circ}$ in both sexes, which is called "upper esthetic line". The Ls-Pg' line makes constant angle to $72.4^{\circ}$ (range $72.2^{\circ}$ in female to $72.6^{\circ}$ in male), which is called "lower esthetic line". 3. When inter-esthetic line angle (the Sn-Ls line to Ls-Pg' line) has $144.9^{\circ}$, lower third face has esthetic upper and lower lip. 4. In treatment planning, Sn is first corrected in proper position, and then upper and lower esthetic line are established with the angle of 144.9. The maxilla is moved to tangent Ls to the upper esthetic line, and mandible is moved to tangent Li and Pg' to the lower esthetic line, according to the "y"-shaped esthetic lines, then lower third face showes esthetics.

  • Orthognathic surgery(2-jaw or 1-jaw surgery) is very famous one of cosmetic techniques. However, primary purpose of orthognathic surgery is to improve the occlusion of jaw and secondary purpose is to improve the esthetic result. Unfortunately, many patients don't only confuse often primary and secondary purpose of orthognathic surgery but they think the esthetic result is more important than the occlusion. Therefore, oral and maxillofacial surgeon has to fully understand cosmetic needs of patient and reflect that in the treatment plan. Patients with prominent mandibular angle want to have the narrower face of so called 'V-line' shape. Various techniques like the angle shaving, ostectomy of the lateral cortex around the mandibular angle and masseter musclectomy can be used for improving the mandibular angle hypertrophy. These techniques also can be applied in orthognathic surgery at the same time. We operated patients of orthognathic surgery, especially, with wide lower face and post-operative results were satisfactory in all cases. So, we propose mandibular angle management for improving the esthetic result of orthognathic surgery.

Esthetic Implant Prostheses for Anterior Teeth (심미적인 상악 전치부 임플란트 보철물 만들기)

  • Anterior maxillary teeth play an important role in determining a person's first impression and facial profile. Implant surgery in esthetic area requires more careful diagnosis, treatment planning, surgery, and prosthetic restoration than in posterior area. To avoid complications in surgery and prosthetic restoration for implants in esthetic area, accurate diagnosis and appropriate case selection become very important. If you have decided to restore the area with implant prosthesis, you have to know exactly where to place an implant. I will discuss the ideal implant position in terms of mesio-distally, apico-coronally, labio-palatally, and implant angulation. And I would like to point out the selection of fixture diameter & length for anterior implant. Finally, a clinical implant prosthesis case in maxillary central incisor will be shown. In conclusion, for superior esthetic outcome in anterior implant prostheses, we must understand the patient's anatomic condition and know our ability.

  • Esthetic demands for dental treatment are increasing every day. The interdisciplinary relationship of the restorative treatment, periodontal therapy and other treatments such as endodontics, orthodontics and so on is more emphasized nowadays to reconstruct the hard and soft tissue foundation for the esthetic restorative treatment. This article will focus on the periodontal plastic surgery for esthetic restorative treatment. These followings will be discussed. 1. Understand the relationship between teeth and gingival scaffold for esthetics 2. Discuss the classification and treatment of gummy smile 3. Recognize the gingival margin irregularities by gingival recession and how to achieve the harmonic soft tissue margins 4. describe the hard and soft tissue augmentation for ridge augmentation.

Distribution of Basal Cell Carcinoma and Squamous Cell Carcinoma by Facial Esthetic Unit

  • Background The most common cutaneous malignant masses are basal cell carcinoma (BCC) and squamous cell carcinoma. The predominant site of a malignant mass is the face, which has many opportunities to be exposed to ultraviolet radiation. However the predilection sites of malignant masses have been equivocal due to the use of general regions, rather than anatomical landmarks, in surveys. A facial esthetic unit is an anatomical site classified as an area of similar facial contour characteristics that can be distinguished from other areas. The purpose of this study is to determine widely accepted anatomical landmarks using the esthetic unit. Methods We retrospectively analyzed 118 cases of malignant masses in our clinic from January 2005 to October 2012. We evaluated the patients' age, gender, and predilection site of the malignant mass by esthetic unit through pathology, medical records and patient photographs. We mapped the occurrence site of the malignant mass on schematic drawings of the esthetic units. Results Most of the malignant masses were BCC. The ratio of males to females was 1:1.41. The frequent predilection site of a malignant mass was on the nasal unit (33.1%), followed by the buccal unit (11.0%). Primary closure was the most common method of repairing a surgical defect (38.9%), followed by a local flap (35.5%). Conclusions This review described the relationship between clinical characteristics and esthetic units by proposing objective predilection sites for malignant masses, which can be used commonly as a framework in the study of malignant masses by unifying equivocal occurrence sites.

  • The challenges to achieve three dimensional facial proportionality and occusal stability in many patients with complex dentofacial deformity have been met by the development and use of the maxilla, mandible, and chin surgery techniques in combination with efficient orthodontic treatment. There is a clinical, biological, and biomechanical foundation for simultaneous surgical repositioning of the maxilla, mandible, and chin in a significant proportion of adult and adolescent patients. A combination of the surgical and orthodontic approach may provide increased treatment efficiencies and optimal esthetic results. Art and science to determine the treatment objectives, specifically, the desired soft tissue changes are firstly established by using the clinician's "esthetic sense" of the facial beauty and proportion aided to a few cephalometric guidelines. In this sense, the dependence on the clinician's "esthetic eye" by Dr. Bell is more important in analyzing the facial proportion than the satisfaction of rigid cephalometric norms. The purpose of this article was to elucidate the indication for simultaneous surgical repositioning of the maxilla, mandible, and chin, and to describe the clinical cephalometric analysis for orthognathic surgery. Representative 6 case reports were presented and discussed to illustrate the esthetic, orthodontic, and surgical treatment objectives with long-term follow-up.

  • Recently, the number of facial contouring surgery such as angle reduction or malarplasty in Oral and maxillofacial surgery is increasing steadly as the demand for maxillofacial esthetic surgery of the patients is growing. Looking around international trend of oral and maxillofacial surgery, nowadays workshop or symposium about facial soft esthetic surgery such as corrective rhinoplasty, blepharoplasty, face lift are held. This means that maxillofacial esthetic surgery is new scope which oral and maxillofacial surgeons should develope. Therefore the authors present cases of blepharoplasty which got hardly used to oral and maxillofaical surgeon with literature review in order to raise the concern about maxillofacial esthetic surgery.

EFFICACY OF ENaP(ESTHETIC NaP)-LINE FOR ORTHOGNATHIC SURGERY OF KOREAN MANDIBULAR PROGNATHISM (한국인 하악골 전돌증 환자의 악교정수술을 위한 ENaP(Esthetic NaP)선의 유효성에 관한 연구)

  • As the most of dentofacial deformity patients indicated to orthognathic surgery have strong desire for esthetic improvement as well as functional improvement, ideal esthetic evaluation should be made at surgical prediction. Lateral cephalographs has been commonly used for surgical prediction, however, remarkable discrepancy between esthetic viewpoint by simple looking and analysis on lateral cephalographs often found on evaluation of sagittal position of the upper and lower jaws especially in cases of mandibular prognathism of Koreans. In these cases, we have been employed Esthetic NaP(ENaP)-line for corrective evaluation and ideal surgical prediction on lateral cephalographs, but the efficacy of ENaP-line has not been evaluated. This is a study on efficacy of ENaP-line for orthognathic surgery of Korean mandibular prognathism. 170 Korean patients who had been diagnosed as mandibular prognathism and planned for orthognathic surgery at Department of Oral and Maxillofacial Surgery, College of Dentistry, Yonsei University during last 10 years are studied. The obtained results are as followings; among 170 patients of mandibular prognathism, 132 patients(77.6%) had not discrepancy in evaluation of sagittal position of the maxilla between cephalometric and esthetic evaluation(they are classified as Group I), however, 38 patients(22.4%) had definite discrepancy(they are classified as Group II). ENaP-line was employed in all cephalometric analysis of Group II. The proportions of male and female were similar in both Groups. Sixteen vertical reference lines perpendicular to 16 horizontal reference lines were obtained as followings; Each of the representative degree of ${\angle}SN/AFH$, ${\angle}SN/CFH$ and ${\angle}AFH/CFH$ obtained at Group I was applied to SN plane, AFH plane and CFH plane of Group II each other, and so 16 horizontal reference lines could be obtained individually according to each of the applied degree to each plane. And then their reliability to coincide with ENaP-line of Group II was evaluated. A vertical reference line perpendicular to a horizontal line made by application of the representative degree of ${\angle}$AFH/CFH in Group I to AFH line in Group II had the most highest coincidence with ENaP-line of Group II, however, its agreement was 42% in male and 47% in female. Mandibular Reconstruction using Simulation Surgery after Segmental Mandibulectomy

  • Functional and esthetic reconstruction after segmental mandibulectomy is one of the most challenging surgeries in microsurgical reconstruction field. Simulation surgery before free flap reconstruction has been performed for efficient surgery and successful results. Fibula free flap is the flap of the choice for reconstruction of the segmental mandibular defect. Straight nature of the fibula bone requires multiple segmentations to fit into mandible. 3D rapid prototype (RP) model gives a lot of information for mandibular reconstruction. The purpose of this study was to report mandibular reconstruction with free fibular flap using simulation surgery. A total of 30 consecutive patients were included for functional and esthetic evaluation. Among 30 patients, two flaps showed necrosis after radiotherapy. The other flaps were all survived and showed successful reconstruction in both function and esthetics.
