  • Prosthetic treatment using implants in fully edentulous patients includes implant-supported fixed prosthesis, implant hybrid prosthesis, implant retained- or supported-over-denture and implant supported fixed prosthesis has advantages such as psychological stability, pronunciation. If an implant supported fixed prosthesis is planned, the implants should be placed in consideration of pronunciation, esthetics, and oral hygiene. For this, clinical and radiological diagnosis is indispensable. When placing the prosthetic driven implant at the site determined from the diagnosis, a sufficient amount of alveolar bone and soft tissue support are required. If these requirements found to be insufficient, a wide range of bone grafting should be performed in advance. In this case, a fully edentulous patient with severe alveolar bone resorption due to periodontal disease was treated with a full mouth rehabilitation using implant-supported fixed prosthesis preceding maxillary sinus graft and alveolar bone augmentation. We report this patient were satisfied with esthetic and function.

  • In case of treating the functional type of anterior cross bite, removing the cause in early stage prevents from turning to skeletal type, leads to perform normal function, and has improved facial appearance. Functional Regulator III (FR III), one kind of the Functional regulator(FR)s suggested by R$\ddot{o}$lf Fr$\ddot{a}$nkel in 1966, applied to patients with the functional and skeletal anterior crossbite in early mixed, and permanent dentition. This appliance improves unbalanced power condition by blocking abnormal muscle-power effect, so that normal growth can be expected. In this case report, favorable results were obtained by selecting clinical cases of children in their early mixed dentition with functional cross bite. 1. FR IIIs were applied to patients with anterior crossbite for 5~6 months. Anterior crossbite patients were corrected favorably, nevertheless they didn't show any horizontal skeletal-changes by buccal shields. 2. Normal occlusion and esthetic facial contour were achieved from dental movement of maxillary and mandibular anterior teeth while the mandible rotates to posterior and inferior direction.

  • Over the last 30 years, the use of chairside computer-aided design (CAD) and computer-aided manufacturing (CAM) systems has evolved and has become increasingly popular in dentistry. Although CAD/CAM restorations have been used in the anterior dentition, satisfying the esthetic requirements of clinicians and patients, where the restorations are limited to the chairside, remains a challenge. To reproduce multi-shades of CAD/CAM restorations in the clinic, a preliminary experiment to express several shades on A2 lithium disilicate (LS2) blocks using a staining kit was performed. After measurement of the CIE L*a*b* value of specimens, it was compared with that of the commercial shade guide. This report presents two cases with individual customization of shade and surface characterization of the CAD/CAM restorations using predictable methods based on the preliminary experimental data. The anatomical shape of restoration was obtained from 'copy and paste technique' and 'mirror image acquisition technique'. All treatment procedures and fabrication of restorations performed in this report were executed in the clinic itself.

  • In 20th century, that is the times of mass production with mass consumption, a company supplies standard product by the system of mass production line. Therefore, the company, itself, had to introduce its product quality to the customers. It was the buying criterion of consumer against product. In that period, a company utilized its identity in order to give customers product information such as product value, price and quality, etc. However, as digital technology with the wide-spread of informationalization & technical revolution is developed, products have diversification and customers have better incomes. So, buying method tends to thi purchase of self-satisfaction generally. In the buying criterion of consumerbased on personality & sensibility, a company must offer the buying criterion of product which can appeal the special quality & image of product itself to customers, and it must stop appealing the Cl of company under the condition of mass production by product quality and function. This study tried to focus on the method which can create effective brand and its image that are the buying criterion of new product. Also, this study tried to find the effective relation between economical & social paradigm which is the result of social informationalization with intensive knowledge, and buying determination of customer, And, this study tried to present guideline of effective brand image and brand special Quality that is affected to buying determination of customers together. The model of Positive analysis had two types. The first model studied the mutual relation between economical/social change factors and special quality of brand. And, the second model studied the mutual relation between the cause of special quality of brand and formation of brand image through regression analysis. Therefore, the construction of sensitive brand image for forming brand must be requested. The sensitive brand image is highly related to preference of sensibility, and it must be based on mind identity & visual identity. And, mind identity must have creation of value, satisfaction, combination of community, personal preference, etc. Visual identity must have esthetic order, and originality of molding, etc. Namely, brand image must form the accommodation of era change, personality & sensibility satisfaction, the effectiveness of service, etc., in order to create effective brand image.

  • Treacher Collins syndrome (TCS) is autosomal dominant disorder that occurs approximately 1 in 25,000 to 50,000 live births. The main signs of syndrome are hypoplasia of facial bone and microtia. One in third of them is associated with cleft palate and often shows dental hypoplasia. TCS patients need several number of surgery with general anesthesia throughout their life time for recovery of function and esthetic. Endotracheal intubation of TCS patient is very difficult due to microstomia, retrognathia, choanal stenosis, and decreased oropharyngeal airway. Therefore, general anesthesia of adolescent TCS patient with immature incisor roots has high risk of causing dental trauma. This case is regarding TCS patient who was referred to the Department of Pediatric Dentistry, Yonsei University for avulsed upper left central incisor during endotracheal intubation. The purpose of this report is to emphasize the usefulness of mouth guard to prevent dental trauma when endotracheal intubation is needed for TCS patient.

  • Treatment with implants of single tooth missing cases is both functional and esthetic. Although the success rate of single-tooth implant treatments is increasing, sometimes it makes some problems. Problems with single-tooth implant treatments include soft tissue complications, abutment screw fracture, and most commonly, abutment screw loosening, and these involve the instability of the dental implant-superstructure interface. This study investigated and compared dental implant screw joint micromotion of various implant system with external connection or internal connection when tested under simulated clinical loading, Six groups (N=5) were assessed: (1) Branemark AurAdapt (Nobel Biocare, Goteborg, Sweden), (2) Branemark EsthetiCone (Nobel Biocare, Goteborg, Sweden), (3) Neoplant Conical (Neobiotec, Korea), (4) Neoplant UCLA (Neobiotec, Korea), (5) Neoplant 5.5mm Solid (Neobiotec, Korea), and (6) ITI SynOcta (Institute Straumann, Waldenburg, Switzerland). Six identical frameworks were fabricated. Abutment screws were tightened to 32-35 Ncm and occlusal screw were tightened to 15-20 Ncm with an electronic torque controller. A mechanical testing machine applied a compressive cyclic load of 20kg at 10Hz to a contact point on each implant crown. Strain gauge recorded the micromotion of the screw joint interface once a second. Data were selected at 1, 500, 5,000, 10,000, 20,000, 30,000, 40,000 and 50,000 cycle and 2-way ANOVA test was performed to assess the statistical significance. The results of this study were as follows; The micromotion of the implant-superstructure in the interface increased gradually through 50,000 cycles for all implant systems. In the case of the micromotion according to cycle increase, Neoplant Conical and Neoplant UCLA system exhibited significantly increasing micromotion at the implant-superstructure interface (p<0.05), but others not significant. In the case of the micromotion of the implant-superstructure interface at 50,000 cycle, the largest micromotion were recorded in the Branemark EsthetiCone, sequently followed by Neoplant Conical, Neoplant UCLA, Branemark AurAdapt, ITI SynOcta and Neplant Solid. Internal connection system showed smaller micromotion than external connection system. Specially, Neoplant Solid with internal connection system exhibited significantly smaller micromotion than other implant systems except ITI SynOcta with same internal connection system (p<0.05). In the case of external connection, Branemark EsthetiCone and Neoplant Conical system with abutment showed significantly larger micromotion than Branemark AurAdapt without abutment (p<0.05).

  • The purpose of this study was to evaluate the bond strengths between IPS e.max Press and four different types of resin cements that often used for esthetic restoration. Disc shaped IPS e.max Press blocks(N=40, diameter: 12mm, thickness: 3mm) were fabricated according to the manufacture's instruction and cleaned with ultrasonic cleaner. They were embedded into an autopolymerizing acrylic resin. Fourty cylinder shaped resin block(Filtek Z350, diameter: 4.5mm, thickness: 3mm) were fabricated using a plastic tube. Each specimens were randomly divided into 4 experimental group and bonded each other using 4 different resin cements(Variolink II(light-cure), Variolink II(dual-cure), Calibra, Super-Bond C&B) according to the manufactures' recommendations. All the specimens were stored in normal saline at $37^{\circ}C$ for 24 hours before test. Universal testing machine at a crosshead speed of 1mm/min was used to evaluate the shear bond strength. The data were statistically analyzed using one-way ANOVA(P<.01). Multiple comparison was done by the Tukey HSD test. The shear bond strength of Super-Bond C&B to e.max was significantly lower than those of Calibra, Variolink II(light-cure), Variolink II(dual-cure) (P<.01). The shear bond strength of Calibra, Variolink II(light-cure), Variolink II(dual-cure) to e.max were not significantly different. The shear bond strengths of light-cure/dual-cure cement were higher than that of self-cure cement.

  • Implant-fixed prostheses and removable partial dentures are widely used as a traditional treatment method for partially edentulous patients who have lost multiple teeth. Recently, there has been increasing interest in implant-assisted removable partial denture (IARPD), which additionally obtains retention and support using a small number of implants. The IARPDs have higher retention, stability, and aesthetics than conventional removable dentures. It also has the effect of reducing the cost as well as the burden of surgery by placing a small number of implants at the edentulous site. In this case, the patient with a partial edentulous state that has multiple numbers of tooth loss in maxilla initially planned to be treated with implant fixed prosthesis. However, the treatment plan was changed to IARPD using Bar and Locator due to several factors. The clinical results were satisfactory on the aspect of esthetic and masticatory function.

  • Upper and lower cuspids are very important teeth in point of function as well as esthetic. However, failure of eruption of the canine sometimes happens and is frequently encountered clinical problems. Upper cuspids are more frequently impacted as compared with lower cuspis and palatally impaction is more frequent event than labially impaction. Impaction of the mandibular canine is unusual event, especially crossing the midline (transmigration) is very rare. Because of, in most cases, there is no signs and symptoms, impacted teeth can be detected by periodic radiograph examination. The etiology of impaction is unclear and multifactorial. The treatment of impacted canine is influenced by presence of obstacles, location and direction of impaction, space for alignment and development of root. There are several treatment options; (1) Surgical removal and/or observation, (2) Window opening and orthodontic traction, (3) Autotransplantation. The first patient (impaction of upper right cuspid) is treated with window opening and orthodontic traction. The second patient (bilaterally impaction of lower cuspids) is treated with autotransplantation, because of transmigration.
