  • Esophageal duplication cysts are quite uncommon benign lesions of the esophagus that have previously been described as occurring only in the thoracic cavity. We experienced a rare case of completely intra-abdominal esophageal duplication cyst in a 10-year-old boy who has suffered from epigastric abdominal pain. Surgical excision is recommended at the time of cyst discovery whether symptoms present or not.

  • Esophagocardiomyotomy [modified Heller procedure] is a widely accepted operation for relief of dysphagia in patient with esophageal achalasia. But patients with advanced achalasia were more likely to get poorer results from a modified Heller myotomy because of the dependent pouch that creates an angulation at the junction of thick-walled dilated esophagus with the thin wailed aganglionic segment and hinders complete emptying. Thorbjarnarson[1975] proposed the method including truncal vagectomy and pyloroplasty. Vagectomy and pyloroplasty should lesson the severity of acid-peptic esophagitis, if reflux should occur postoperatively. Here we presented 4 cases esophageal achalasia treated by modified Heller operation of 3 cases and one case of Thorbjarnarson method. All postoperative results are good.

Congenital esophageal atresia with tracheoesophageal fistula -Report of one case- (선천성 식도폐쇄 및 기관식도루 -1예 보고)

  • Esophageal atresia and tracheoesophageal fistula may occur as separate entities but usually occur in combination. Recently we were experienced a case of esophageal atresia with distal tracheoesophageal fistula in infant patient who presented the symptoms of projectile vomiting and dyspnea. The diagnosis was made by the esophagography and the Haight`s operation was performed transpleurally through 4th intercostal space after gastrostomy. Operative patient tolerated all the operative procedures well in spite of postoperative respiratory complication and recovered uneventfully, permitted feeding on 10th postoperative day. On follow up study after 5th months, Patient reveals good health without other problem.

  • A fistulous communication between an esophageal traction diverticulum and the tracheo-bronchial tree appears to be of rare occurrence. This report reviews the feature of congenital esophagobronchial fistula associated with esophageal traction diverticulum. This 38-year-old male patient suffered from coughing, hemoptysis, fever and chest pain. This patient was taken a diverticulectomy and lobectomy of right lower lobe. Post-operation course was uneventful.

  • The strictured esophagus is not removed usually in corrosive injury. But corrosion carcinoma and the late complications such as esophago-bronchial fistula, chronic mediastinal abscess, diverticulum can occur in case that strictured & inflamed esophagus be remained. Recently transhiatal esophagectomy is being done with low mortality and morbidity. So we are reporting 2 cases of esophageal stricture in young patients which were treated with transhiatal esophagectomy and esophagocologastrostomy. All of 2 cases were successfully treated and recovered. Postoperative esophageal function tests showed the mild reflux but it did not count clinically.

  • Esophageal perforation due to blunt trauma is rare. A 67-year-old male presented to Konyang University Hospital with painful neck swelling. His neck was injured by blunt trauma at work. Esophageal perforation was detected by neck CT and esophagography. We performed primary repair of cervical esophagus through the Lt. neck approach. The postoperative course was uneventful and the patient was discharged at postoperative day 15.

  • Perforation of the esophagus is a rare complication of endotracheal intubation and usually occurs after hasty intubation. A 26-year-old female was transferred from other hospital for further management of empyema of the right lung. During admission, the empyema was found due to esophageal perforation, which had developed during the endotracheal intubation after acute poisoning of carbon monooxide 5 dayes prior to the transfer. The em-pyema and the esophageal perforation were successfully managed by conservative measures including effective drainage.

  • Esophageal perforation, regardless of the etiology, is a catastrophic event. The importances of early diagnosis and an aggressive surgical approach in the management of such a potentially lethal situation are stressed, in fact the mortality rate is directly related to the interval between perforation and initiation of treatment. We experienced a rare case of esophageal rupture caused by compressed air which produce a rupture of the colon not infrequently, which was treated successfully by an aggressive surgical approach consisting of closure of the perforation and adequate drainage.

  • The PTP (Press-Through-Pack) has been widely used as a packing material for tablets and capsules. But esophageal foreign bodies attributed to the PTP may cause injury to esophageal mucous membrane, potentially inducing severe complications such as hemorrhage, perforation, etc. We report three cases of PTP foreign bodies in esophagus with reference to recent literature.
