Teeth impaction, and eruption guidance of impacted teeth by surgical exposure in child and adolescent (임상가를 위한 특집 3 - 소아 청소년에서 발생한 치아의 매복 및 외과적 노출술에 의한 매복치의 맹출유도)

  • Tooth eruption disturbance is seen commonly in mixed dentition or early permanent dentition. During this period of time, children experience the growth and development of craniofacial skeleton and dentition, so the impaction and eruption disturbance of permanent teeth make many problems in oral and maxillofacial areas. Eruption guidance of impacted tooth is not simple because many factors related to impaction are considered. Several factors such as etiology, tooth development stage, location of impacted teeth or eruption pathway, patient cooperation, etc. need to be taken into consideration in deciding the management of the impacted tooth. In the present study, it was assessed the etiology and impaction status of impacted tooth through various cases, and try to explain the treatment method to guide eruption by surgical exposure.

ERUPTION AND AUTOTRANSPLANTATION OF A PERMANENT TOOTH RELATED TO DENTIGEROUS CYST IN MIXED DENTITION (혼합치열기의 함치성 낭종 치료 시 영구치의 맹출과 자가치아이식)

  • Purpose: Dentigerous cyst is the most common intra-osseous lesion of the jaw. Dentigerous cysts can cause delays in eruption of the affected permanent tooth in mixed dentition. It has been suggested that the affected permanent tooth could be erupted spontaneously after the dentigerous cyst was enucleated. But in some cases, orthodontic treatment or autotransplantation technique is known to be required. This study reviews previously performed prognoses of affected permanent teeth, which will lead to a more efficient treatment plan. Patients and Methods: With 28 patients who have undergone cyst enucleation and 10 patients who have undergone autotransplantation, the prognosis of permanent teeth was observed. Results: After cyst enucleation, spontaneous eruption of a permanent tooth was observed in 56.3% patients, orthodontic treatment was performed in 25% patients. The success rate of autotransplantation was 60.0%. Discussion: The first choice for treatment of dentigerous cyst in mixed dentition is to guide spontaneous eruption of permanent teeth. For cases without enough eruption space, the orthodontic treatment should be considered. Autotransplantation should be considered when the spontaneous eruption is not expected. It should be considered that the rate of successful autotransplantation is decreased on maxilla anterior area.

  • A cyst that develops in children's jaw occasionally disturbs the eruption of the succedaneous teeth. These teeth, however, usually have the potential of eruption. So, if the obstacles to eruption are eliminated, it is possible that the teeth erupt spontaneously to their normal position. In those cases, it usually requires the management of the cyst and the eruption guidance of the displaced teeth. Many surgical procedures have been described for the elimination of cysts. When the cyst is large and displaces the permanent teeth, marsupializaion is a surgical technique that may be preferred to enucleation in treatment of cysts. In marsupializaion, if the opening is maintained properly, it may be possible to manage the cyst and guide the displaced teeth into the normal position. In these cases, the cysts were managed with marsupialization in concomittent application of acrylic obturators, and as a result the displaced permanent teeth were guided into normal position. Even though the etiologic factors of the two cases are different, the treatment was the same. And both cases show that the potential for heal ing is remarkable with spontaneous relocation of displaced tooth, provided the opening is maintained during the eruption of the permanent tooth.

  • Patient with alveolar abscess due to dental caries with severe alveolar bone loss, severe tooth mobility, root resorption need extraction of tooth because it is impossible to carry out pulp treatment and restoration by using conventional method. Early loss of primary molar might cause masticatory interference, extrusion of opposing tooth, problem in maintaining space and interference on eruption of permanent tooth. Especially, early loss of primary second molar before the eruption of permanent first molar might cause space closure by mesially erupted permanent first molar and impaction of second premolar. In such a case, distal shoe space maintainer and removable space regaining appliance was the first choice of treatment. But, distal shoe space maintainer need precise adaptation and might cause chronic inflammation if the oral hygiene is poor. In a case using removable space regaining appliance, patient's cooperation is most important. If the distal root of primary second molar is comparably sound and alveolar abscess with alveolar bone loss is localized at mesial root, hemisection should be carried out for precise guide to eruption of the permanent first molar, restoration of masticatory fuction and solution to the discomfort of the patient

  • Dilaceration is most common in maxillary central incisor. Impaction by dilacerated tooth cause orthodontic problem such as tilting of adjacent tooth, midline deviation. Dilaceration is caused by trauma, cyst, other origin and tooth shape, degree of root curvature, location in the alveolar bone, ability of spontaneous eruption should be considered whether treat or not. Labially and lingually dilacerated tooth is difficult to spontaneous eruption. Thus, Prosthetic treatment alternative with extraction, auto transplantation, orthodontic traction with surgical exposure is included within the treatment alternative. These cases are about impacted central incisor with dilacerated root. We use closed eruption technique and guide impacted tooth into normal position by orthodontic traction.

  • The appropriate treatment for eruption guide of impacted teeth necessitates the formulation of a comprehensive treatment plan, which is dependent upon a number of factors such as the condition of the deciduous teeth, dental and skeletal relationship, dental age of the patient, willingness of the patient to undergo extensive dental treatment and financial considerations. If the etiology of the eruption disturbance has been identified, the elimination of the causes and various procedures can be used for eruption guide. Particularly the transplantation is a valuable alternative to extraction of impacted teeth, where surgical exposure and subsequent orthodontic realignment are difficult or impossible. This report present three cases of autotransplantation of impacted maxillary canine. As the result in these cases, atraumatic removal of donor tooth during operation is prerequisite to an optimal clinical result. Due to a high possibility of pulp necrosis, endodontic treatment of fully developed transplanted teeth should be undertaken. In complex case, autotransplantation can save time and less expensive than orthodontic forced eruption. Recipient socket should be prepared to a size that is slightly larger than the root of the donor tooth, and can be prepared with open or close procedure depends on root size of donor tooth and buccolingual width at transplantion site.

Alveolar Cleft Reconstruction Using Chin Bone and Autogenous Tooth Bone Graft Material: Reports of 5 Cases

  • Purpose: To report the successful results of using chin bone graft and autogenous tooth bone graft material (AutoBT) in alveolar cleft patients. Materials and Methods: Five patients with alveolar cleft defects underwent alveolar bone grafting. Three patients were treated using chin bone graft, and the other two patients underwent AutoBT graft. After implant site development using chin bone graft in the fi rst three cases, endosseous implant restorations were placed. In case #4 and 5, AutoBT graft material was placed to guide the normal eruption of partially impacted maxillary right canine and to the upper docking site after distraction osteogenesis. Result: Successful implant restorations with closure of the oronasal fistula were achieved in alveolar cleft defect reconstruction using either chin bone graft (Case #1, 2, 3) or AutoBT graft material (Case #4, 5). Case #4 showed enlarged follicle of the right maxillary canine, indicating a normal eruption guide pattern. Conclusion: Both chin bone graft and AutoBT graft showed favorable outcomes in reconstructing alveolar cleft defects. Autogenous tooth bone graft opens up the possibility of avoiding harvesting autogenous bone graft with complications and morbidities.

Space maintainer using CAD/CAM system without a band: a case report (CAD/CAM 시스템을 이용하여 band 없이 공간유지 장치를 제작한 증례)

  • Early interceptive orthodontic interventions are often started in the developing dentition to guide normal occlusion. The simplest way to prevent eruption failure from deciduous tooth loss is to fix a space maintainer such as a band and loop. Band and loop space maintainer has several problems like cement loss, demineralization, locking of the adjacent tooth, and needs to take a pickup impression of the band. Computer-assisted design/computer-assisted manufacturing (CAD/CAM) space maintainer without band was developed to solve these problems. This case report describes successful eruption of the mandibular second premolar using CAD/CAM without a band.

Retrospective Study of the Characteristics and Treatment of Odontomas (치아종의 임상적 특징과 치료에 대한 후향적 연구)

  • This study was aimed to retrospectively evaluate the clinical findings and treatments of odontomas in the Korean demographic group. The subject of the present study included 75 odontomas in 74 patients who received treatment from Chonbuk National University Dental Hospital between April 2005 and March 2014. The average age of the subjects were 10.3 years old (range 3 - 23 years old). 42 (56.8%) males and 32 (43.2%) females were in the present study. It was found that compound odontomas were about 4 times more common than complex odontomas. Odontomas equally occurred in both maxillary and mandible. The impaction of permanent teeth (73.3%) was the most common complication of odontomas on adjacent teeth. Most odontomas (96.0%) were surgically removed either to induce spontaneous eruption or to guide orthodontically to occlusion. When the impacted tooth could not be naturally or forcefully erupted to occlusion, it was extracted. Early detection and treatment of odontomas would increase the possible preservation of the impacted tooth by eliminating the source of disturbance. Therefore, this study recommends that periodic radiographic examination from birth until the eruption of the permanent third molar to prevent possible hindrance to the teeth eruption.

  • Tooth transposition is an anomaly of eruption characterized by the interchanged positions of two adjacent teeth, which is divided into complete and incomplete transposition. There are three common approaches for treating transposition: aligning the involved teeth in their transposed positions, moving them to their correct anatomic position in the arch and extracting one of the transposed teeth. Considerations in treatment plans are esthetic, function, risk of jeopardizing the roots and damaging the supporting structures, position of the root apex, developmental stages of teeth and expected compliance. The presented case reports described one maxillary canine-the first premolar transposition and two mandibular lateral incisor-canine transpositions. The former transposed teeth were arranged in their transposed position, and the latter transposed teeth were rearranged into their normal position.
