Neurochemical Characterization of the TRPV1-Positive Nociceptive Primary Afferents Innervating Skeletal Muscles in the Rats

  • Objective: Transient receptor potential vanilloid subfamily type 1 (TRPV1), a most specific marker of the nociceptive primary afferent, is expressed in peptidergic and non-peptidergic primary afferents innervating skin and viscera. However, its expression in sensory fibers to skeletal muscle is not well known. In this study, we studied the neurochemical characteristics of TRPV1-positive primary afferents to skeletal muscles. Methods: Sprague-Dawley rats were injected with total $20{\mu}l$ of 1% fast blue (FB) into the gastrocnemius and erector spinae muscle and animals were perfused 4 days after injection. FB-positive cells were traced in the L4-L5 (for gastrocnemius muscle) and L2-L4 (for erector spinae muscle) dorsal root ganglia. The neurochemical characteristics of the muscle afferents were studied with multiple immunofluorescence with TRPV1, calcitonin gene-related peptide (CGRP) and $P2X_3$. To identify spinal neurons responding to noxious stimulus to the skeletal muscle, 10% acetic acids were injected into the gastrocnemius and erector spinae muscles and expression of phospho extracellular signal-regulated kinase (pERK) in spinal cords were identified with immunohistochemical method. Results: TRPVl was expressed in about 49% of muscle afferents traced from gastrocnemius and 40% of erector spinae. Sixty-five to 60% of TRPV1-positive muscles afferents also expressed CGRP. In contrast, expression of $P2X_3$ immnoreaction in TRPV1-positive muscle afferents were about 20%. TRPV1-positive primary afferents were contacted with spinal neurons expressing pERK after injection of acetic acid into the muscles. Conclusion: It is consequently suggested that nociception from skeletal muscles are mediated by TRPV1-positive primary afferents and majority of them are also peptidergic.

The Effects of Performing Bridge Exercise and Hip Thrust Exercise using Various Knee Joint Angles on Trunk and Lower Body Muscle Activation in Healthy Subjects

  • Objective: This study aimed to identify the effects of assuming different knee angles and hip abduction during bridge exercise and hip thrust exercise on lower body muscle activity. Design: Cross-sectional study Methods: Thirty-three healthy adults (18 men and 15 women) were instructed to perform the bridge and hip thrust exercises while randomly assuming 120°, 90° and 60° of knee flexion and 0° and 30° of hip abduction. EMG data (%maximum voluntary isometric contraction) were recorded three times from the erector spinae (ES), gluteus maximus (GM) and biceps femoris (BF) muscles of participant's dominant side and the mean values were analyzed. Results: The results showed that, during the hip thrust compared to the bridge exercise, there was significantly greater gluteus maximus muscle activity in all hip conditions while the biceps femoris activity was significantly less, and the erector spinae muscle activity was significantly greater with 30° of hip abduction (p<0.05). With all exercises, the erector spinae and the biceps femoris exhibited significantly greater muscle activity with 60° of knee flexion compared to 90° and 120° of knee flexion (p<0.05), and significantly greater muscle activity with 90° compared to 120° of knee flexion (p<0.05). In the case of the gluteus maximus, greater muscle activity was exhibited with 120° compared to 60° of knee flexion with all hip abduction conditions (p<0.05). Conclusions: It was effective for muscle activation of main agonists such as the gluteus maximus and erector spinae during thrust exercise, and the change in knee flexion angle was effective for muscle activation of the gluteus maximus. Therefore, it is considered that this study can be used as a selective indicator of the target movement angle during hip strengthening exercise for specific muscles.

  • Purpose : The purpose of this study was to examine the effects of the trunk stabilization exercise in the musical tempo on lumbar lordosis angle, muscle activity and pain. Methods : For the 30 people with lumbar lordosis angle legion and back pain, a random selection was made with MLSE (15) and LSE (15) to measure VAS, lumbar lordosis angle and Muscle Activity. Result : There were significant decreases in intra group comparisons to lumbar lordosis angle were seen in MLSE and LSE groups, and significant decreases in inter group comparisons in MLSE groups. significant decreases in intra group comparisons to VAS were seen in MLSE and LSE groups, and significant decreases in inter group comparisons in MLSE groups. Significant intra-group comparison of muscle activity, MLSE groups increases were rectus obdominis(right/left) and erector spinae muscle(right/left), LSE groups increases were erector spinae muscle(right/left), and significant increases in inter group comparisons rectus obdominis(right) and erector spinae muscle(left) in MLSE groups Conclusion : Based on the above findings, a program to restore the lumbar lordosis angle, and increase muscle strength should be developed at by applying the combine existing trunk stabilization physical therapy technique and musical tempo.

Can Abdominal Drawing-In Maneuver Using a Pressure Biofeedback Unit Change Muscle Recruitment Pattern During Prone Hip Extension?

  • This study examined the effects of the abdominal drawing-in (ADI) maneuver using a pressure biofeedback on muscle recruitment pattern of erector spinae and hip extensors and anterior pelvic tilt during hip extension in the prone position. Fourteen able-bodied volunteers, who had no medical history of lower extremity or lumbar spine disease, were recruited for this study. The muscle onset time of erector spinae, gluteus maximus, and medial hamstring and angle of anterior pelvic tilt during hip extension in prone position were measured in two conditions: ADI maneuver condition and non-ADI maneuver condition. Muscle onset time was measured using a surface electromyography (EMG). Kinematic data for angle of anterior pelvic tilt were measured using a motion analysis system. The muscle onset time and angle of anterior pelvic tilt were compared using a paired t-test. The study showed that in ADI maneuver during hip extension in prone position, the muscle onset time for the erector spinae was delayed significantly by a mean of 43.20 ms (SD 43.12), and the onset time for the gluteus maximus preceded significantly by a mean of -4.83 ms (SD 14.10) compared to non-ADI maneuver condition (p<.05). The angle of anterior pelvic tilt was significantly lower in the ADI maneuver condition by a mean of 7.03 degrees (SD 2.59) compared to non-ADI maneuver condition (15.01 degrees) (p<.05). The findings of this study indicated that prone hip extension with the ADI maneuver was an effective method to recruit the gluteus maximus earlier than erector spinae and to decrease anterior pelvic tilting.

Comparison of Trunk Muscles between 3 Different Squats in Normal Adults

  • Objective: This Study aimed to examine the changes in muscle activity of trunk muscles by performing three squat exercises on normal adults. Design: cross-sectional study Methods: Thirty-two adult subjects participated in this study. General squat, overhead squat, and overhead squat combined with abdominal stabilization were randomly performed for 5 seconds, 3 times, to calculate the average muscle activation. Muscle activation was normalized using electrodes on the rectus abdominis, external oblique, internal oblique, and erector spinae muscles to measure maximum voluntary isometric contraction (MVIC) for 5 seconds, repeated 3 times each. Results: There was a significant difference in the mean and maximal muscle activity of the bilateral erector spinae (ES) when comparing the squat to the overhead squat (p<0.05). There was a significant difference (p<0.05) in the mean and maximal muscle activity of the bilateral external oblique (EO) when comparing the overhead squat to the overhead squat combined with the abdominal stabilization technique. When comparing the squat to the overhead squat with abdominal stabilization, there was a significant difference in the mean and maximum muscle activity of the bilateral RA, EO, and left Internal oblique (IO) (p<0.05), and there was a significant difference in the maximum muscle activity of the bilateral erector spinae (ES) (p<0.05). Post hoc tests showed significant differences between squatting methods for the RA, EO, IO, and ES (p<0.017). Conclusion: The results of this study showed that squats combined with abdominal stabilization were more effective at activating core muscles than squats or overhead squats alone.

PNF 하지굴곡패턴운동이 체간근육 활성도에 미치는 영향 (The Effects of PNF Leg Flexion Patterns on EMG Activity of the Trunk)

  • Purpose : The purpose of this study was to analyze the effect of PNF lower extremity flexion pattern on the eletromyographic (EMG) activity in rectus abdominis, internal oblique abdominal, external oblique abdominal, erector spinae. Methods : Twenty-six healthy adults volunteered to participate in this study. Subjects were required complete following two PNF lower extremity patterns; flexion-adduction-external rotation with knee flexion (D1) and flexion-abduction-internal rotation with knee flexion (D2). A paired t-test was used to determine the influence of the PNF two patterns on muscle activity for each muscle and descriptive statistics was used to determine local/global muscle ratio. Results : The D1 pattern was showed significant rectus abdominis (p<.05) and Median of internal oblique/rectus abdominis ratio was 2.23 and internal oblique/external oblique ratio was 1.53. The D2 pattern showed significant erector spinae (p<.05) and Median of internal oblique/rectus abdominis ratio was 3.06 and internal oblique/external oblique ratio was 1.72. Conclusion : The D1 pattern made rectus abdominis activation increase. The D2 pattern made erector spinae activation increase. As compared D1 and D2 pattern on trunk muscle activation, it's will be useful decision making for the trunk muscle strength and stabilization.

다이어트신발(Barkless)이 근육 활성도에 미치는 영향에 관한 연구 (The Study on effect of the Muscle Activities for Dietshoes (Backless))

  • The modern convenient life formed by industrial development becomes lack of exercise and takes an interest in diet. Specially, professional walking shoes is developed as people take an interest in jogging, Those shoes, professional walking shoes or Dietshoes, increase exercise effects by change of heel types. Therefore, this study investigated motility effects by EMG experiment in order to measure Muscle Activities (MA) while wearing diet shoes (backless). Experiment was conducted by EMG measurement, from calf (gastrocnemius muscle), thigh (vastus muscle) and waist (erector spinae muscle), of 12 high school students. Exercise effects between the two shoes were analyzed by EMG (MF; Median Frequency, MPF; Mean Power Frequency, ZCR; Zero Crossing Rate). Results showed that the Dietshoes(MF: 48.21Hz, MPF: 65.0Hz, ZCR: 100.6Hz) had larger EMG value than that of Normal shoes(MF: 40.47Hz, MPF: 58.04Hz, ZCR: 82.09Hz). Also, in MA, the highest activities are showed in the calf, the second one is in waist, and last one is in thigh during gate. ANOVA between shoes in measurement parts showed significant effects in MF (gastrocnemius: p-value=.022, vastus laterals: p-value=.037, erector spinae: p-value=.082), MPF (gastrocnemius: p-value=.032, vastus laterals: p-value=.046, erector spinae: p-value=.090), and ZCR (gastrocnemius: p-value=.000, vastus laterals: p-value=.004, erector spinae: p-value=.134). And MA of Dietshoes is higher than that of Normal shoes, and decreasing rate of MA in Dietshoes is less than that of Normal shoes. Thus, this study validates exercise effects of Dietshoes.

복합 기능 운동 중재 프로그램이 비특이적 만성요통 장년 여성의 통증 변화 및 수축 특성 변화에 미치는 효과 (The Effect of Combined Functional Intervention Program on Changes in Pain and Contractile Properties in Prime-aged Females with Non-specific Chronic Low Back Pain )

  • Objective: This study aimed to investigate the effect of a combined functional intervention program on the pain-related questionnaire, the erector spinae contractile properties, and trunk extensor strength of prime-aged females with non-specific chronic low back pain. Method: 16 (female: 16) prime-aged subjects (age: 60.88 ± 6.09 years, height: 158.16 ± 5.58 cm, weight: 59.05 ± 9.44 kg) participated in this study. Results: Except for Tampa Scale of Kinesiophobia all pain-related questionnaires revealed a significant decrease after the intervention program (p<.001). Tensiomyography of the erector spinae revealed a significant post-intervention program increase in the maximum radial displacement (p<.05) and velocity of contraction (p<.05), however, there wasn't a significant post-intervention program change in the contraction time. Additionally, Trunk extensor strength showed a significant increase post-intervention program (p<.001). Conclusion: Our results indicated that the combined functional intervention program positively changed pain-related questionnaires and reduced erector spinae muscle stiffness, increasing the velocity of erector spinae contraction. Additionally, improved the trunk extensor strength.

프로와 아마추어 골퍼의 미들 아이언 스윙에 관한 근전도 분석 (The Analysis of Electromyography during Professional & Amateur Golfer's Iron Swing)

  • The purpose of this study was to describe and compare the selected electromyographical muscle activities of trunk and hip during Professional and Amateur golfer's Iron swing. Using surface electromyography, we evaluated muscle activities in 6 male professional golfers and 6 male amateur golfers during the golf iron swing. Surface electrodes were used to record the level of muscle activity in the right abdominal oblique, left abdominal oblique, right erector spinae, left erector spinae, right rectus abdominis, left rectus abdominis, right gluteus maximus, left gluteus maximus muscles during the golfer's swing. These signals were compared with IEMG(Integrated EMG) which was normalized by %RVC(Reference voluntary contraction). The golf swing was divided into three phases: take away, forward swing-acceleration, follow-through. We observed patterns of trunk muscle activity throughout three phases of the golf swing. The results can be summarized as follows: LES(Left Erector spinae) had statistically significant difference in take away and forward swing-acceleration phases. It was showed no significant difference in follow-through phase.

Influence of Slashpipe Exercise on Symmetrical Contraction of Trunk Muscle in Normal Adults

  • Purpose: This study examined the effects of slashpipe exercise on reducing the thickness of the left and right external oblique, internal oblique, transverse abdominis, erector spinae, and multifidus muscles. Methods: A total of 29 healthy adult men and women were included in the study. They performed trunk flexion in the supine position and trunk extension in the prone position with a slashpipe and weight bar. The external oblique, internal oblique, and transverse abdominis muscles were measured in the supine position, while the erector spinae and multifidus muscles were measured in the prone position. The data were analyzed using the SPSS ver 21.0 statistical program. The difference in thickness between the right and left sides of the trunk muscle was analyzed by repeated measures analysis. The statistical significance level was set to p<0.05. Results: The results showed that the slashpipe exercise reduced significantly the difference in thickness of the oblique internus and erector spinae muscles compared to the weight bar exercise. Conclusion: The chaotic fluidity of the fluid filled inside the slashpipe could be used as sensory feedback information on body mal-alignment, which would have positively affected the symmetrical contraction of the trunk muscles as a trigger for self-correction. Therefore, it will have a useful effect not only on the health of the general public, but also on low back patients and athletes with muscle asymmetry.
