Nutrition Policies in the Republic of Korea : Expert Opinions and Recommendations

  • Considering the increasing global burden of disease attributable to nutrition, the demand for national nutrition policies is growing stronger and stronger. The Republic of Korea is beginning to respond to this growing demand and there have been numerous recent achievements in the area of nutrition. The purpose of this paper is two fold. In the first part of the paper, an overview is provided about existing nutrition action in Korea. In the second part, information is elicited from Korean experts in the field of public health and nutrition. A structured questionnaire was sent directly to renowned Korean experts and was disseminated through the list serve of the Korean Nutrition Society. It asked about existing nutrition related policies, obstacles to nutrition policies in Korea, which professions are 'responsible' for the prevention of nutrition-related diseases and for example about existing nutrition programs within settings. Twenty-two questionnaires were returned. Based on the results, the following actions were considered crucial to develop and implement a nutrition policy in Korea: having a clear advocacy strategy for nutrition to raise awareness about the importance of nutrition, increase transparency of nutrition-related action and make the information easily accessible for all stakeholders, ensure the enforcement of regulations regarding misleading and incorrect food advertisements, strengthening the (public health) nutrition workforce, strengthen the settings-based approach and build and maintain partnerships, and last not least: educate and empower the citizens and ensure that healthy choices are 'easy choices'! Acknowledging the past and ongoing efforts in Korea, it is pivotal that the nutrition workforce grows stronger and the voice of nutritionists even louder, in order to promote and ensure the health and quality of life of the country's citizens. 학생건강증진을 위한 학교보건지표체계 개발에 관한 연구 (Development of School Health Indicator System for the Health Promotion of School Children)

  • Purpose: The purpose of this study was to develop the school health indicator system for the health promotion of school children. Methods: Logic model was adopted for developing conceptual framework of school health indicator system and reviewed school health indicators developed by WHO, CDC, MEST and KCDC. Results: School health contents were classified into five area; school health policy, health promotion of school children, school health education, physical environment and community linkage. School health indicator system was developed for each area based on the logic model. Conclusion: Conceptual framework of school health indicator system was developed and school health indicator system was suggested according to the five school health areas.

서울시 중앙버스전용차로 도입의 부가적인 대기오염 영향성 평가 (Impact of the Exclusive Median Bus Lane System on Air Pollution Concentrations in Seoul, Korea)

  • Since many previous studies reported the health effect of air pollution and indicated traffic as a major pollution source, significant policy efforts have been made to control traffic to reduce air pollution. However, there have been few studies that evaluated such policy implementation. In Seoul, Korea, the exclusive median bus lane system was implemented in 2004, and the metropolitan government applied air pollution reduction policies such as conversion of diesel buses to compressed natural gas buses and installation of emission control devices. This paper aimed to investigate the impact of the exclusive median bus lane system on air pollution reduction. Using hourly concentrations of particulate matter ($PM_{10}$) and nitrogen dioxide ($NO_2$) measured at 131 regulatory monitoring sites in Seoul and Gyeonggi-do for 2001-2014, we calculated annual and daily average concentrations at each site. We assessed the impact of the policy using differences-in-differences analysis by annual and daily average models after adjusting for geographic and/or meteorological variables. This method divides population into treatment and control groups with and without policy application, and compares the difference between the two time periods before and after the policy implementation in the treatment group with the difference in the control group. We classified all monitoring sites into treatment and control groups using two definitions: 1) Seoul vs. Gyeonggi-do; 2) within vs. outside 300 meters from the median bus lane. Pre- and post-policy periods were defined as 2001-2005 and 2006-2014, and 2004 and 2014 in the annual and daily models, respectively. The decrease in $PM_{10}$ concentrations between the two periods across monitoring sites in the treatment group was larger by $1.73-5.88{\mu}g/m^3$ than in the control group. $NO_2$ also showed the decrease without statistical significance. Our findings suggest that an efficient public transport policy combined with pollution abatement policies can contribute to reduction in air pollution.

한국 노인장기요양시설 및 재가 요양보호사들의 노동환경과 노동조건 개선방안 (The Strategy for Improving Work Environment and Working Conditions among Long-term Health Care Workers in Korea)

  • Background: This study aimed to establish a strategy to improve the poor working environment and working conditions among long-term healthcare workers in Korea. Methods: A total of 600 questionnaires with which long-term health care workers participated in the targeted base areas of each city and province nationwide were distributed directly and 525 responses were collected and 506 responses were analyzed. Surveys, on-site field visits, and in-depth interviews were also conducted to understand the working environment as well as conditions and establish a strategy for improving the working environment among long-term healthcare workers to understand the demands of working conditions and working conditions. Results: Korean long-term care workers firstly and mostly enumerated their risk factors for ill-health when lifting or moving elderly recipients directly by hand (69.9%), followed by increased physical workload with old beds, tools, and facilities (42.3%) in the workplaces, shortage of manpower (32%), and source of infection (30%). To improve the working environment as well as conditions, Korean long-term care workers considered improving low-wage structures, ergonomic improvements to solve excessive physical loads, and increasing various bonus payments as well as implementing the salary system, positive social awareness, and increasing resting time. Of 506 responses, 92.3% replied that the long-term care insurance system for the elderly should be developed to expand publicization at the national level. Conclusion: This study proposes to improve the low-wage structure of Korean long-term care workers, automation and improvement of facilities, equipment, and tools to eliminate excessive physical loads (beneficiary elderly lifting), and reduction of night labor.

효과적 검역체계를 위한 검역법 개선방안 (A Study on the Improvement of Quarantine Act for Effective Quarantine System)

  • The development of transport is being easily shared with people all over the world. It is necessary to appropriately and effectively revise the domestic quarantine law because the fatal infectious diseases are at risk of being easily shared. Today, Korea has an advanced quarantine system approved by World Health Organization, but it maintains partnerships with related ministries (Ministry of Foreign Affairs, Ministry of Justice, local medical institutions) and to introduce new medical technology (electronic quarantine) is important. And since the prevention of quarantine infectious diseases and prevention of the spread, in order to maintain international cooperation with the International Health Regulations, the quarantine law and the system should be amended and improved effectively and it is also a way to prepare for the outbreak of new quarantine infectious diseases. In the past, Korea has experienced great confusion during the past outbreak of swine flu and Middle East respiratory syndrome coronavirus. To prevent similar cases from recurring in the past, the revision of the quarantine law and the improvement of the system should be done to cope with the changing environment (new infections, increased number of overseas travelers, etc.).

수도권 대기 중 입자상 물질로 인한 건강부담 추정과 불확실성 (Uncertainty and Estimation of Health Burden from Particulate Matter in Seoul Metropolitan Region)

  • It is well known that exposure to high level of PM (particulate matter) can adversely affect human health. However, little is known about health burden of PM considering the relationship, exposed level of PM, and health level in local communities. And, there is scarcely methodical assessment of uncertainty for application to policies of these assessment results. The scope of this study is divided into two parts: firstly to estimate the death burden of PM10 (particulate matter less then $10{\mu}m$ in diameter) in Seoul metropolitan region, and secondly to evaluate potential uncertainties in these estimates. To estimate the death burden of PM10 in Seoul metropolitan region from 2005~2010, we firstly assessed the relationship between daily mean PM10 and daily death counts in Seoul from 2000~2010, and calculated the death burden of PM10 using BenMAP (Environmental Benefits Mapping and Analysis Program). After that, we identified and characterized uncertainties to substantially influence the results of death burden. The daily mortality risk was increased 1.000227 times (p-value/0.001) associated with $1{\mu}g/m^3$ increase of daily mean PM10 for all ages population, Seoul. And, death burdens of PM10 in Seoul metropolitan region were estimated from 5.51 in 2005 to 5.12 in 2010 per 100,000 people. Finally, we categorized context, model, and input uncertainty and characterized these uncertainties in three dimensions (i.e. location, level, and nature) using uncertainty typology. In our study, we argue that uncertainties need to be identified, assessed, reported and interpreted in order for assessment results to adequately support decision making, such as the establishment of air quality standards based on health burden of air quality.

간호대학생의 대학캠퍼스 환경인식과 건강지각이 스트레스에 미치는 영향 (The Influence of Perceptions of the Campus Environment and Health Perception on Stress of Nursing College Students)

  • 본 연구는 간호대학생의 대학캠퍼스 환경인식과 건강지각이 스트레스에 미치는 영향을 파악하여 스트레스를 감소하기 위한 프로그램 개발의 기초자료를 제공하기 위해 시행되었다. 자료수집은 2016년 12월 01일부터 12월 30일까지 G시의 3개 간호학과 학생을 대상으로 편의 추출하였으며 223명의 수집된 자료를 SPSS/WIN 21.0 프로그램을 이용하여 위계적 다중 회귀분석으로 분석하였다. 연구결과, 스트레스에 차이를 나타내는 일반적 특성은 연령, 실습경험, 성별, 주거형태, 성격과 식습관형태 및 학교보건실 이용경험이었다. 스트레스는 대학캠퍼스 환경인식과 건강지각 간에 부적상관관계를 나타내었다. 스트레스에 영향을 주는 요인은 식습관, 성별, 대학캠퍼스 환경인식과 건강지각이었으며, 스트레스에 대한 이들의 설명력은 23.1%로 나타났다. 따라서 간호대학생의 스트레스를 감소시키기 위해서 대학캠퍼스 환경을 개선하고 건강에 대한 지각을 높일 수 있는 다양한 프로그램을 개발하여 운영해야 할 것이다.

나노물질 안전관리 동향 및 제도 도입에 관한 고찰 (Safety Management System on Nanomaterials with a Regulatory Scheme)

  • 독성, 환경 노출 및 인체에 대한 위해성 유해성 등 안전성에 대한 문제점을 가지고 있는 나노물질의 확인 및 안전성 확보의 요구가 점차 높아지고 있다. 나노물질은 화학물질로 이용될 뿐만 아니라 소비자가 이용하는 제품으로 범 분야에 걸쳐 활용되기 때문에, 보다 효과적인 나노물질의 안전관리 제도화를 위해서는 각국의 정책사례를 규제 강도별, 나노 적용분야별, 규제대상 종류별로 구분하여 정책 동향을 파악하는 것이 중요하다. 나노물질의 잠재적 위험성을 최소화시키기 위해서는 사전예방원칙을 적용한 제도의 구축 또한 필요하다. 더불어 국내 실정에 적합한 정책 수립을 목표로 하고, 국제 흐름에 선제적, 능동적으로 대응할 수 있는 방안도 함께 고려해야 한다.

Changes in Nutrition of Adult's Favorite Foods of High calorie, Low-nutritive Foods

  • This study analyzed in nutrient contents changes of adult's favorite foods between March 2019 and July 2020 after policy implementation nutrient-poor foods based on special act on safety control of adult's dietary life in Korea. Among adult's favorite foods manufactured or sold in 2020 as well as 2019, calories and key nutrients in breads, ice creams and pizzas were improved in comparison to those in the other food groups. However, most of the changes in calories or key nutrient contents exist. The newly introduced candies, breads showed slightly greater improvements in calories and key nutrient contents than in 2019. On the other hand, some negative changes were found in newly introduced chocolates in comparison to previous ones. Overall, policy implementation on foods seemed to induce changes in nutrient contents of adult's favorite foods. In particular, nutrition education is reported to have a positive impact on adult's frequency and preference for processed foods, and more systematic and continuous nutrition education measures should be devised to help adult as consumers selectively purchase healthy foods. This research is meaningful in that it is the first study to analyze the quality changes of adult's favorite foods since the high-calorie and low-nutrient food management policy.

수용체 기반 지역 위해관리를 위한 환경관련 민원 분석: 부산시 사례를 중심으로 (Analysis of Environmental Complaints for Receptor-oriented Risk Management: Busan as a Case Study)

  • Objectives: This study aims to examine receptor-perceived characteristics of environmental and health damage in areas surrounded by industrial workplaces using environmental complaints collected by local governments in Korea. Methods: The environmental complaint data related to workplaces was collected by local governments at each municipality between the years 2015 and 2017. We analyzed the complaint data from the Busan area by the type of business (industry classification), the content of complaints (odor, smoke, noise, and certain health damage), and the frequency of complaints. Results: Among the workplaces examined, industry categories related to retail and service received the highest frequency of complaints related to noise. On the other hand, complaints of dust and odors were raised in higher frequency against manufacturing industries, indicating that air pollution was the main driver of such complaints. Most of the complaints regarding health effects from industries also contained other complaints, such as odor, noise, and dust. Our results indicate that environmental discomfort can lead to more serious health damage, and should be dealt with more seriously. Further management plans for mitigating discomforts such as noise are necessary. Conclusion: This study can be used as background data for identifying the policy priorities related to vulnerable areas polluted by industry and will contribute to improving the environmental health of residents in identified areas.
