소아청소년의 비만여부에 따른 신체활동수준, 1일 총에너지소비량 및 에너지필요추정량의 평가 (Physical activity level, total daily energy expenditure, and estimated energy expenditure in normal weight and overweight or obese children and adolescents)

  • The purposes of this study were to assess the physical activity level (PAL) and the total daily energy expenditure (TEE) as well as to evaluate the validity of prediction equation for the estimated energy requirement (EER) in normal weight and overweight or obese children and adolescents. The subjects comprised of 100 healthy Korean students aged between 7-18. The anthropometric data was collected. PAL was calculated from the physical activity diary by the 24-hour recall method, and the resting metabolic rate (RMR) was measured by an open-circuit indirect calorimetry using a ventilated hood system. Daily energy expenditure was PAL multiplied by RMR. EER was calculated by using the prediction equation published in KDRIs. There was no significant difference in the means of age and height between the 46 obese subjects and 54 nonobese subjects. The weight and BMI of the obese group (60.2 kg, $25.3kg/m^2$) were significantly higher than those of the nonobese group (42.4 kg, $18.4kg/m^2$). However, PAL was not significantly different between the two groups (nonobese 1.45, obese 1.46). TEE of the obese group (2,212 kcal/day) was significantly higher than that of the nonobese group (1,774 kcal/day). EER (individual PA) and EER (light PA) were significantly higher than TEE (p < 0,001); however, EER (sedentary PA) was not significantly different with TEE in the two groups. These results showed that the levels of physical activity were the same as the sedentary activity both in the nonobese and obese Korean students; moreover, the predictive equation for EER published in KDRI overestimated the TEE of Korean children and adolescents. Therefore, in further research, a new predictive equation for EER should be developed for Korean children and adolescents through the doubly labeled water method.

  • This study was aimed to investigate the nutriational status of Korean female college students who are attending school at a longdistance. Physique calssification by anthropomethic indexes, Body mass index(BMI) & Brocas' index and the body fat distrubition were investigated. The energy balance was also examined comparing caloric intake with caloric expenditure. The result of this study are followings; (1) Average height and weight were 158±17.8 cm, 51.7±7.8 kg. This results were not quite different from '84 Korean Adult Standard of 159.5cm, 52.5kg. (2) By Broca's index, 25.89% of subjects was underweight 67.0% was normal weight and 7.06 was overweight. By BMI, 40% of subjects was underweight, 58.8% was normal weight and 1.2% was overweight. (3) According to % fat measured. By BIA, 7% in subjects was underweight, 64.2% was normal weight, 22% was overweight and 6.1% was obesity. This result differed from BMI's result. It may be measurement error due to the difficulty of removing metal sticking to underwear and tooth correction. (4) The intakes of the nutrients except protein, P, vitamin B₁ complexes, were lower than the intakes indicated by Korean Recommended Allowances. 95) Average caloric intake a day was 1545±394.4 kcal, which was much lower than the RDA. The caloric expenditure was much more than caloric intake, which showed negative energy balance in the subjects of this study.

교사들을 위한 학교 구내식당의 식단에 대한 영양평가 및 교사의 활동량에 관한 연구 - 인천 시내 중.고등학교를 중심으로 - (A Study on Nutritional Evaluation of Foods in School Cafeteria for Teachers and Its Relation to Daily Energy Expenditure of Teachers (In Junior and Senior High School in Incheon))

  • Meals served in school cafeteria for teachers were evaluated in terms of calorie and nutrient level and food composition. Daily energy expenditure of teachers was compared with caloric content of the meal. The followings are results of the study. 1) Twelve different foodstuffs were used in each meal of school cafeteria for teachers. The average amount of food served meal was 396.0g. The ratio of animal food to total food served was 13.2%. The meal was consisted of 44.8% of basic food group 4, 34.7% of basic food group 3, 19.3% of basic food group 1, 0.4% of basic food group 2 and 0.9% of basic food group 5. 2) The average caloric content of the meal was 629.9Kcal. The caloric ratio of carbohydrate, protein and fat to total calorie was 72.4% : 15.3% : 12.3% respectively. The ratio of animal protein to total protein was 43.2%. In meals of school cafeteria, the level o fall nutrients except iron and niacin was lower than RDA for male, whereas the level of all nutrients except protein and niacin was lower than RDA for female. 3) The average daily energy expenditures of teachers were 2,017.2Kcal for male and 1,735.3Kcal for female. The meal in school cafeteria supplied 93.7% of daily energy expenditure of male teachers and 110.0% of daily energy expenditure of female teachers. When planning meals in school cafeteria, recommended allowance, actual energy expenditure and food preference of teachers should be considered to improve the quality of meals. Also it seems desirable to give teachers the nutritional education for taking balanced diet.

Chest compression quality, exercise intensity, and energy expenditure during cardiopulmonary resuscitation using compression-to-ventilation ratios of 15:1 or 30:2 or chest compression only: a randomized, crossover manikin study

  • Objective Our aim was to compare the compression quality, exercise intensity, and energy expenditure in 5-minute single-rescuer cardiopulmonary resuscitation (CPR) using 15:1 or 30:2 compression-to-ventilation (C:V) ratios or chest compression only (CCO). Methods This was a randomized, crossover manikin study. Medical students were randomized to perform either type of CPR and do the others with intervals of at least 1 day. We measured compression quality, ratings of perceived exertion (RPE) score, heart rate, maximal oxygen uptake, and energy expenditure during CPR. Results Forty-seven students were recruited. Mean compression rates did not differ between the 3 groups. However, the mean percentage of adequate compressions in the CCO group was significantly lower than that of the 15:1 or 30:2 group ($31.2{\pm}30.3%$ vs. $55.1{\pm}37.5%$ vs. $54.0{\pm}36.9%$, respectively; P<0.001) and the difference occurred within the first minute. The RPE score in each minute and heart rate change in the CCO group was significantly higher than those of the C:V ratio groups. There was no significant difference in maximal oxygen uptake between the 3 groups. Energy expenditure in the CCO group was relatively lower than that of the 2 C:V ratio groups. Conclusion CPR using a 15:1 C:V ratio may provide a compression quality and exercise intensity comparable to those obtained using a 30:2 C:V ratio. An earlier decrease in compression quality and increase in RPE and heart rate could be produced by CCO CPR compared with 15:1 or 30:2 C:V ratios with relatively lower oxygen uptake and energy expenditure.

3축 가속도계와 신체정보를 이용한 보행 및 주행시 에너지 소비량의 예측 (Estimation of Energy Expenditure of Walking and Running Based on Triaxial Accelerometer and Physical Information)

  • 본 연구의 목적은 신체활동 시에 간단하고 실용적인 방법으로 에너지 소비를 예측하는데 있다. 신체활동은 허리에 부착된 3축 가속도계 센서를 사용하여 가속도의 함으로써 정량화하였다. 에너지 소비량과 가속도계 데이터의 관계를 찾기 위하여 11명(남성 6명, 여성 5명)의 피험자를 상대로 1.5, 3.0, 4.5, 6.0, 6.5, 7.0, 8.5km/hr의 속도로 트레드밀에서 걷거나 달리는 동작을 시행하였다. 각 피험자는 트레드밀 속도가 6.0km/hr 일 경우에는 걷는 동작을 6.5km/hr 이상일 경우께는 달리는 동작을 수행하였다. 실제 에너지 소비량은 가스분석기를 통하여 측정되었다. 예측 식은 피험자의 성, BMI(Body Mass Index), 가속도 데이터로 이루어졌으며 걷기, 달리기로 총 2개의 다중회귀식으로 구현되었다. 실제 측정된 에너지 소비량과 예측된 에너지 소비량 간의 상관계수와 결정계수는 걷기와 달리기에서 각각 R=0.956, R2=0.876와 R=0.881, R2=0.776를 나타내었다. 추후의 연구에서는 보다 많은 피험자의 데이터를 이용하여 보다 신뢰성 있는 에너지 소비량 예측의 회귀식을 구현하고자 한다.

Analysis of Postural Stability and Daily Energy Expenditure to Manage Tumor Patients' Functional Expectation

  • Background: Advances in surgical techniques, implant technology, radiotherapy, and chemotherapy have increased the recovery chances of patients with bone sarcomas. Accordingly, patients' expectations on life quality have also increased, highlighting the importance of objective evaluation of the functional results of reconstruction. Methods: Thirteen patients with distal femoral endoprosthesis, who had been followed for an average of 2.9 years were evaluated. Postural stability, daily energy expenditure, muscle power, and range of motion were the four parameters analyzed in this study. The Musculoskeletal Tumor Society (MSTS) score and Toronto Extremity Salvage Score (TESS) were used to assess postoperative function and examine correlations with other parameters. Results: Patients had sedentary activities in 84% of their daily lives. They exhibited a slower speed in the walk across test and a higher sway velocity in the sit-to-stand test (p = 0.005). MSTS scores were significantly correlated with the daily energy expenditure and walking speed. Conclusions: Objective functional results acquired from various clinics will provide significant data to compare reconstruction techniques, rehabilitation protocols, and surgical techniques. In this way, it will be possible to satisfy the expectations of patients that increase in relation to enhanced recovery.

  • Obesity increases the risk of knee and to a lesser extent hip OA, which combined affect a large percentage of middle-aged and elderly adults and which are major source of disability, and factor of drop a lowering in the physical exercise ability. Energy expenditure from physical activity accounts for up to 30% of total energy expenditure, it can have a significant impact on energy balance. We studied a exercise therapy that improved long-term weight management and produced additional benefits - loss of joint pain, improved joint mobility, and this exercise program will enhance the weight loss and health benefits from physical activity in the treatment of obese patients with osteoarthritis.

단일 3축 가속도센서를 사용한 보행 시 대사에너지 예측 (Prediction of Energy Expenditure by Using a Tri-axial Accelerometer)

  • The purpose of this study was to compare metabolic energy expenditure with the computed kinetic energy for different speeds of walking and running over the treadmill and to find the relevance for individual and group equation by performing a statistical analysis, Bland-Altman plot. Seven male subjects participated, and they were required to walk and run on the treadmill with the gas analyzer and triaxial accelerometer. Walking speeds were 3.0, 4.0, 5.0 and 6.0 km/h and running speeds were 7.0, 8.0 and 9.0 km/h respectively. Kinetic energy was calculated by the integration of acceleration data and compared with the metabolic energy measured by a gas analyzer. Correlation coefficients showed relatively good between the measured metabolic energy and the calculated kinetic energy. In addition, a dramatic increase in kinetic energy was also observed at the transition speed of walking and running, and two standard deviations in Bland-Altman plot, derived from the difference between measured and predicted values, were 1.14, 2.53, 2.93, 1.80, 2.80, 0.60 and 2.48 respectively. It was showed that there is no difference for methods of how to predict the kinetic energy expenditure for individual and group even though people had each different physical characteristic.

Effects of Uphill Exercise on Digestible Energy Intake and Energy Expenditure during Exercise in Yearling Horses

  • The objectives of this study were to evaluate the digestible energy intake and energy expenditure in yearling horses on different training protocols (uphill- and level-track exercise training protocols). Twenty-four thoroughbred yearlings (12 males and 12 females, aged 27.0$\pm$0.9 months) were divided into two groups based on their training on two different tracks: the uphill (with a gradient of about 3%) training group (uphill training) and the level training group (level training). The digestible energy (DE) intake and energy expenditure (EE) during exercise were measured in both the groups. It was found that the DE intake in the uphill training and the level training groups was 5.1$\pm$3.1 and 36.9$\pm$4.8 Mcal/day, respectively. The EE during exercise in the two groups was 3.05$\pm$0.51 and 2.07 $\pm$0.56 Mcal, respectively. Thus, there was a significant difference in the EE (p<0.05), but not in the DE intake between the animals of the two training groups. The EE for a given intensity of exercise was greater in the uphill training group than in the level training group, but the DE intake was not affected by the exercise intensity. The DE intake was not generally affected by the intensity of exercise in this study, but a daily negative gain of body weight was observed in the uphill training group, particularly in the females. Thus, the energy requirement may be higher in yearlings undergoing uphill training than in those undergoing level training.

남자 노인에서 에너지 섭취 조사를 위한 24시간 회상법의 정확도 평가 -이중표식수법을 사용하여- (Accuracy of 24-hour Diet Recalls for Estimating Energy Intake in Elderly Men using the Doubly Labeled Water Method)

  • Objectives: This study assessed the accuracy of the 24-hour diet recall method for estimating the energy intake of elderly men using the doubly labeled water as a reference method. Methods: Seventeen subjects (mean age $72.5{\pm}3.9years$), who maintained the same body weight during the two weeks study period, were included in this study. Three 24-hour diet recalls (two weekdays and one weekend) were obtained over a 14 day period to estimate the mean energy intake. The total energy expenditure was measured over the same 14 days using the doubly labeled water method. The total energy intake and total energy expenditure were compared by paired t-test. Results: The total energy intake from the 24-hour diet recalls method was $2536.7{\pm}350.6kcal/day$, and the total energy expenditure from the doubly labeled water method was $2659.8{\pm}306.8kcal/day$. The total energy intake was slightly under-reported by $-123.2{\pm}260.8kcal/day$ (-4.4%). On the other hand, no significant difference was observed between the total energy intake and total energy expenditure of the subjects (p=0.069). The percentage of accurate predictions was 64.7%. The correlation between the total energy intake and total energy expenditure was statistically significant (r=0.697, p<0.005). Conclusions: The present study supports the use of the 24-hour diet recall method to estimate the mean energy intake in elderly men group. More studies are needed to assess the validity of 24-hour diet recall method in other population groups, including elderly women, adults and children.
