3D 내시경 영상시컨스의 MPEG-2 코딩 효율에 관한 연구 (A Study on the MPEG-2 Coding Performance of 3D Endoscopic Image Sequence)

  • In this study, for the coding of stereoscopic video sequence, two approaches are presented based on simulcast mathod and sidefield image format. The field sequential method for stereoscopic visualization have been specified. Also, camera parameter and shooting conditions for each test sequence are studied. Coding method based on sidefield format structure revealed better performance over simulcast in PSNR.

  • This paper presents the method of 3D reconstruction of the depth information from the endoscopic stereo scopic images. After camera modeling to find camera parameters, we performed feature-point based stereo matching to find depth information. Acquired some depth information is finally 3D reconstructed using the NURBS(Non Uniform Rational B-Spline) method and OpenGL. The final result image is helpful for the understanding of depth information visually.

내시경 초음파 영상의 점막하 종양 분석 (Submucosal Tumor Analysis of Endoscopic Ultrasonography Images)

  • 내시경 초음파는 초음파 진동자를 내시경 끝에 부착하여 그 주위의 장기를 관찰할 목적으로 개발된 의료기기이다. 내시경 초음파 검사는 점막하 종양을 직접 관찰 할 수 있어 종양의 병리 소견이 예측 가능하지만, 종양의 악성화 여부 등에 대해 주관적인 소견이 개입될 수 있는 문제점이 있다. 따라서 본 논문에서는 주관적인 소견으로 인해 나타나는 문제점을 객관화하여 질병의 정확도와 재현성을 높이기 위해 종양의 각 특징을 분석하는 방법을 제안한다. 제안된 방법을 적용하기 위해서 내시경 초음파 검사로 얻어진 초기 영상에서 분석에 필요한 초음파 영역을 추출한다. 초음파 영역은 여러 요인으로 인하여 명암도 값의 차이가 발생하는데, 이는 객관적인 분석에는 비효율적이다. 따라서 초기 검사 시에 매질로써 주입되는 물 영역의 명암도를 기준으로 하여 초음파 영역의 명암도를 표준화 한다. 표준화된 초음파 영역에서 전문의에 의하여 선택된 종양 영역에 LVQ 알고리즘과 비트 평면 분할 방법을 각각 적용하여 에코가 높은 spot 영역과 칼슘이 침착된 영역을 추출하고 분석한다. 종양 영역의 세밀한 분석을 위하여 명암도 값과, 종양 영역 내에서 전문의가 임의로 선택한 두 지점의 거리에 포함된 명암도 정보를 추출한다. 또한 선택된 종양의 악성도를 구분하기 위하여 종양 영역에서 외곽의 기울기를 계산한다. 내시경 초음파 영상에서 각 질병의 특징을 분석한 결과, 제시된 방법이 종양이 가지는 특징을 분석하는데 도움이 되는 것을 확인할 수 있었다.

Confocal Laser Endomicroscopy in the Diagnosis of Biliary and Pancreatic Disorders: A Systematic Analysis

  • Background/Aims: Endoscopic visualization of the microscopic anatomy can facilitate the real-time diagnosis of pancreatobiliary disorders and provide guidance for treatment. This study aimed to review the technique, image classification, and diagnostic performance of confocal laser endomicroscopy (CLE). Methods: We conducted a systematic review of CLE in pancreatic and biliary ducts of humans, and have provided a narrative of the technique, image classification, diagnostic performance, ongoing research, and limitations. Results: Probe-based CLE differentiates malignant from benign biliary strictures (sensitivity, ≥89%; specificity, ≥61%). Needle-based CLE differentiates mucinous from non-mucinous pancreatic cysts (sensitivity, 59%; specificity, ≥94%) and identifies dysplasia. Pancreatitis may develop in 2-7% of pancreatic cyst cases. Needle-based CLE has potential applications in adenocarcinoma, neuroendocrine tumors, and pancreatitis (chronic or autoimmune). Costs, catheter lifespan, endoscopist training, and interobserver variability are challenges for routine utilization. Conclusions: CLE reveals microscopic pancreatobiliary system anatomy with adequate specificity and sensitivity. Reducing costs and simplifying image interpretation will promote utilization by advanced endoscopists.

  • The diagnosis and treatment of early gastrointestinal cancers is the gastroenterologists' mission because of national cancer screening program in South Korea. The detection of early cancers is emphasized, because these were previously treated with surgical treatment can be currently cured with endoscopic treatment. Gastroenterologists who achieved at least on some level can make an exact diagnosis regardless of what type of endoscopy, but generally, there are some required conditions for an optimal diagnosis. First, clinically important lesions have to be detected easily. Second, the border and morphology of lesions have to be characterized easily. Third, lesions have to be diagnosed exactly. Precancers and early cancers are often subtle and can pose a challenge to gastroenterologists to visualize using standard white light endoscopy. The use of dye solutions aids the diagnosis of early gastrointestinal cancers, however, it is a quite cumbersome to use dye solutions all the time and the solution often bothers the exact observation by pooling into the depression or ulceration of the lesion. To overcome this weakness, newer endoscopes are now developed so called "image enhanced endoscopy" using optical and/or electronic methods such as narrow band imaging (NBI), autofluorescence imaging (AFI), i-scan, flexible spectral imaging color enhancement (FICE) and confocal endomicroscopy (CLE).

Design of an Ultrasmall Flexible-endoscope Illumination Optical System with Bat-wing Light Distribution

  • In this paper, an illumination optical system that can mitigate the saturation phenomenon in the center of an image (caused by the typical flexible-endoscope illumination system using LEDs with Lambertian light distribution) is designed. When an LED with Lambertian light distribution is used as a light source, the amount of light in the center of the endoscopic illumination system is relatively high, compared to the periphery, causing saturation in the image. Since this phenomenon causes difficulty in detecting the patient's lesion, it is necessary to find a lighting-system design that can alleviate the saturation phenomenon. Therefore, in this paper a lighting system with bat-wing light distribution, which can lower the intensity at the center and secure the maximum amount of light at the maximum light distribution angle, is designed. In addition, to check the performance of the designed lighting system, a simulation of illumination and luminance is conducted for a system using a common aspherical lens with otherwise the same components. As a result, it is confirmed that the lighting system designed in this paper effectively reduces the luminance value at the center and secures more luminance values at the periphery than the familiar lighting system.

Application of artificial intelligence for diagnosis of early gastric cancer based on magnifying endoscopy with narrow-band imaging

  • Although magnifying endoscopy with narrow-band imaging is the standard diagnostic test for gastric cancer, diagnosing gastric cancer using this technology requires considerable skill. Artificial intelligence has superior image recognition, and its usefulness in endoscopic image diagnosis has been reported in many cases. The diagnostic performance (accuracy, sensitivity, and specificity) of artificial intelligence using magnifying endoscopy with narrow band still images and videos for gastric cancer was higher than that of expert endoscopists, suggesting the usefulness of artificial intelligence in diagnosing gastric cancer. Histological diagnosis of gastric cancer using artificial intelligence is also promising. However, previous studies on the use of artificial intelligence to diagnose gastric cancer were small-scale; thus, large-scale studies are necessary to examine whether a high diagnostic performance can be achieved. In addition, the diagnosis of gastric cancer using artificial intelligence has not yet become widespread in clinical practice, and further research is necessary. Therefore, in the future, artificial intelligence must be further developed as an instrument, and its diagnostic performance is expected to improve with the accumulation of numerous cases nationwide.

Iatrogenic Intradural Lumbosacral Cyst Following Epiduroscopy

  • We report a rare complication of iatrogenic spinal intradural following minimally invasive extradural endoscopic procedues in the lumbo-sacral spines. To our knowledge, intradural cyst following epiduroscopy has not been reported in the literature. A 65-year-old woman with back pain related with previous lumbar disc surgery underwent endoscopic epidural neuroplasty and nerve block, but her back pain much aggravated after this procedure. Postoperative magnetic resonance imaging revealed a large intradural cyst from S1-2 to L2-3 displacing the nerve roots anteriorly. On T1 and T2-weighted image, the signal within the cyst had the same intensity as cerebrospinal fluid. The patient underwent partial laminectomy of L5 and intradural exploration, and fenestration of the cystic wall was accomplished. During operation, the communication between the cyst and subarachnoid space was not identified, and the content of the cyst was the same as that of cerebrospinal fluid. Postoperatively, the pain attenuated immediately. Incidental durotomy which occurred during advancing the endoscope through epidural space may be the cause of formation of the intradural cyst. Intrdural cyst should be considered, if a patient complains of new symptoms such as aggravation of back pain after epiduroscopy. Surgical treatment, simple fenestration of the cyst may lead to improved outcome. All the procedures using epiduroscopy should be performed with caution.

Peroral Pancreatoscopy with Videoscopy and Narrow-Band Imaging in Intraductal Papillary Mucinous Neoplasms with Dilatation of the Main Pancreatic Duct

  • Background/Aims: Endoscopic evaluation of intraductal papillary mucinous neoplasms (IPMNs) is useful in determining whether the lesions are benign or malignant. This study aimed to examine the usefulness of peroral pancreatoscopy (POPS) in determining the prognosis of IPMNs. Methods: POPS with videoscopy was performed using the mother-baby scope technique. After surgery, computed tomography/magnetic resonance cholangiopancreatography or ultrasonography and blood tests were performed every 6 months during the follow-up. Results: A total of 39 patients with main pancreatic duct (MPD)-type IPMNs underwent POPS using a videoscope, and the protrusions in the MPD were observed in 36 patients. The sensitivity and specificity of cytology/biopsy performed at the time of POPS were 85% and 87.5%, respectively. Of 19 patients who underwent surgery, 18 (95%) patients had negative surgical margins and 1 (5%) patient had a positive margin. Conclusions: In IPMNs with dilatation of the MPD, POPS is considered effective if the lesions can be directly observed. The diagnosis of benign and malignant lesions is possible depending on the degree of lesion elevation. However, in some cases, slightly elevated lesions may increase in size during the follow-up or multiple lesions may be simultaneously present; therefore, careful follow-up is necessary.
