Study on Chungye Relavant to Menstration Starting and Pause Compared by Oriental and Western Medicine (월경(月經)의 개시(開始)와 폐지(閉止)에 관여(關與)하는 천계(天癸)의 동서의학적(東西醫學的) 고찰(考察))

  • This study is to review the oriental medical literatures about Chungye and endocrinologic literatures about menstration, pregnancy, menopause and male-sex cycle. The conclusions obtained are as follows; First, according to Naegyung, Chungye exists in both male and female, it makes human fertile. It comes with starting menstration and is exhausted with menopause. Second, it was said that the name of Chungye originated in Chun of Chunjinjiki(energy of genuine cosmos) and Gye of Imgyejisu(water of Imgye). Third, some authors advocated that Chungye is menstration. But it was criticized by other authors and it is not accepted at the present time. Forth, there was an assertion that Chungye is Eumjung. The assertion has been developed as the hypothesis that Chungye may be a endocrinologic substance relavant to menstration and pregnancy. Fifth, there was an interpretation that Chungye is FSH. But this interpretation might be unreasonable if it is based on the results observed by endocrinologic researches. According to the current endocrinologic researches, Chungye may be GnRH, when Chungye is the necessary endocrinologic substance to menstration and pregnancy, exists in male and female and is exhausted with menopause.

Community Pharmacists' Perception of Barriers to Pharmacy Work (약국업무시 고충에 관한 개국약국 약사의 인식에 관한 연구)

  • The purpose of this study was to evaluate barriers to pharmacists' routine work in community setting. A survey was given to 281 pharmacists who enrolled in 16-week clinical pharmacy educating program in continuous education center for advanced pharmacy at Seoul National University. Three main questions that pharmacists were asked to answer were: (1) difficulties on dispensing prescription, (2) difficulties on communicating with doctors, (3) most difficult disease on patients counseling. The response rates for the survey were more than 60 % for each three questions (62.63 %, 63.7 %, and 64.41 %, respectively). The top three barriers to dispensing prescription were lack of professional knowledge about medications, prescription error and its solving ability and patient counseling. The top three barriers to communicating with doctors were lacking of opportunity to discuss about patients' medication due to unavailability of doctors, doctors' attitude using authoritative manner, and a pharmacist's lack of knowledge. The top 4 most difficult diseases on patients counseling were cardiovascular disorders, dermatologic disorders, endocrinologic disorders, and psychiatric disorders.

  • In this paper, tiangui was considered in it's conception, generation and physiological function according to successive physicians. The following results were obtained : 1. Over the years, there have been many ideas about tiangui. These days, it is proper that tiangui be thought of as some substance which is in shenjing and which promotes generative function. According to current endocrinologic view, tiangui is equivalent to GnRH. 2. Before birth, a parent's energy is inherited to the embryo and after birth, that energy becomes tiangui which is sustained by taking food and nutrition. Tiangui is especially related to shenjing. 3. The most import physiological function of tiangui is the promotion of generative function. So it is related to menstruation and ejaculation. Secondly, tiangui is also concerned with human's growth and development.

  • This study was carried out through the investigation of Oriental and Western medical literature and we was discovered these conclusions about the etiology, treatment, prescription of habitual abortion. The result of this study were as follows : 1. The habitual abortion does agree with 'Hwaltae(滑胎)' and means that spontaneous abortion repeats three times or more. 2. The etiology of habitual abortion is divided 'Deficiency of kidney'(腎虛), 'Deficiency of vital energy and blood'(氣血虛弱), 'Deficiency of yin'(陰虛), 'Clotted blood'(瘀血). 3. By means of cause, the treatment of habitual abortion was mainly made use of 'Tonifying kidney and promoting spleen'(補腎健脾), 'Invigorating vital energy and nourishing blood'(補氣補血), 'Supplementing yin fluid and alleviating fever'(滋陰淸熱), 'Resuscitating blood and removing clotted blood'(活血祛瘀). 4. By means of cause, the prescription of habitual abortion was frequent made use of Bosingochunghwan(補腎固沖丸), Bosingotae$\breve{u}$m(補腎固胎飮), Taesanbans$\breve{o}$ksan(泰山磐石散), $Ch\breve{o}ng\breve{u}mboinghwan$(千金保孕丸), Agyotang(阿膠湯), Gungguibojungtang(芎歸補中湯), Taewon$\breve{u}$m(胎元飮), Gotaej$\breve{o}$n(固胎煎), Baekchulj$\breve{o}$n(白朮煎), Sas$\breve{o}$ngsan(四聖散), Gyegibokryonghwan(桂枝茯苓丸), $Dangguich\breve{o}ng\breve{u}mtang$(當歸千金湯). 5. The Western medical etiology of habitual abortion(recurrent spontaneous early pregnancy loss) is divided Genetic factor(Chromosome aberration), Anatomical deformity, Endocrinologic disorder, Infection, Immunologic factor, Unknown factor(Others).

Radiation Therapy for Pituitary Adenoma -Changes in Endocrine Function after Treatment- (뇌하수체선종의 방사선치료후 혈중 호르몬치의 변화)

  • Seventy four patients with pituitary adenoma received radiation therapy (RT) on the pituitary area using 6 MV linear accelerator during the past 7 years at the Division of Radiation Therapy, Kangnam St. Mary's Hospital, Catholic University Medical College. Thirty nine were men and 35 were women. The age ranged from 7 to 65 years with the mean being 37 years. Sixty five ($88\%$) patients were treated postoperatively and 9 ($12\%$) primary RT, To evaluate the effects of RT, we analyzed the series of endocrinologic studies with prolactin (PRL), growth hormone (GH), adrenocorticotrophic hormone (ACTH), leuteinizing hormone (LH), follicular stimulating hormone (FSH) and thyroid stimulating hormone (TSH) etc after RT. All but one with Nelson's syndrome showed abnormal neuroradiologic changes in the sella turcica with invasive tumor mass around supra- and/or parasella area. The patients were classified as 23 ($29\%$) prolactinomas and 20 ($26\%$) growth hormone (GH) secreting tumors, and 6 ($8\%$ ACTH secreting ones consisting of 4 Cushing's disease and 2 Nelson's syndrome. Twentynine ($37\%$) had nonfunctioning tumor and four ($5\%$) of those secreting pituitary tumors were mixed PRL-GH secreting tumors. The hormonal level in 15 ($65\%$) of 23 PRL and 3 ($15\%$) of 20 GH secreting tumors returned to normal by 2 to 3 years after RT, but five PRL and five GH secreting tumors showed high hormonal level requiring bromocriptine medication. Endocrinologic insufficiency developed by 3 years after RT in 5 of 7 panhypopituitarisms, 4 of seven hypothyroidisms and one of two hypogonadisms, respectively. Fifteen ($20\%$) patients were lost to follow up after RT.

Effectiveness of Korea Red Ginseng in Erectile Dysfunction-Multi-National Approach (발기부전 환자에서의 홍삼의 효능에 관한 연구 -동남아시아의 다국적 연구)

  • Ginseng has been used in maintaining physical vitality throughout the far-eastern countries and recently its metabolism and actions on neurologic, cardiovascular, and endocrinologic systems are studied. Korean red ginseng (KRG) has been used in various ailments, and to prove its efficacy for erectile dysfunction an international study on Asians other than Korean was performed. Patients with borderline organic and psychogenic erectile dysfunction were included. KRG were given daily, and placebo were given as controls. Treatment lasted a total of 3 months. Surveys including libido, erection, ejaculation, sexual activity, and sexual satisfaction were given. Serum testosterone and erectile function study were taken. Among the 64 patients, 37 patients were followed with KRG. Five had diabetes, 5 hypertension, 5 hypercholesterolemia, 6 low testosterone, 6 psychogenic, and 11 idiopathic. The improvement after KRG administration was $70.2\%$ on objective questionnaire and $75.7\%$ on subjective analysis. When KRG were given, all parameters surveyed have shown improvements compared to the placebo. The effects of KRG in Chinese and Singapores were similar to the Koreans. Serum testosterone levels were nonnalized in 6 patients with KRG, who's serum testosterone levels were reduced from pre-study. Two patient reported constipation, and 2 gastric upsets in the KRG group. In conclusion, KRG has beneficiary action on male erectile capabilities with little side effects. KRG is effective in Koreans and also Asians. The exact action mechanism and the active ingredients in KRG need to be studied.

Clinical and Endocrine Characteristics of Patients with McCune-Albright Syndrome (McCune-Albright 증후군의 임상적 및 내분비학적 특징)

  • Purpose: McCune-Albright syndrome (MAS) is caused by activating mutations in the GNAS gene, resulting in peripheral precocious puberty, caf$\acute{e}$-au-lait spots, and polyostotic fibrous dysplasia (POFD). The aim of the present study was to describe the diverse clinical and endocrine characteristics of patients with MAS. Methods: Seven patients with MAS were included in this study and medical charts were reviewed retrospectively for following parameters: patient's sex and age at diagnosis, POFD, ovarian cysts, and precocious puberty. Results: The mean age at diagnosis was $5.8{\pm}4.2$ years. One patient was male (14%) and the other six patients were female (86%). Peripheral precocious puberty was associated with 6 patients (86%). Five patients manifested premature menarche as early as 2 to 5 years of age. Letrozole was administered to 4 patients, tamoxifen to one patient and GnRH agonist to one patient. Five females developed ovarian cysts. Thyroid function tests were performed in all patients and one patient showed hyperthyroidism (14%) and has been treated with methimazole. One patient presented with pseudohypoparathyroisdism, phosphaturia, calciuria suggesting hypophosphatemic rickets. Six patients (86%) revealed POFD. One patient had symptoms of optic nerve compression and secondary esotropia and 2 patients had bone pain. Conclusion: This study described clinical characteristics and endocrine complications of patients with MAS. Careful physical examinations with history taking and serial endocrine function tests should be needed to detect complications such as endocrinologic hyperfunction and POFD.

Surveys including libido, erectile potency, the sexual satisfactions were given. Serum testosterone and erectile function study were taken. Among the 23 patients with KRG, 18 patients were followed. Four had diabetes, 2 hypertension, 3 hyperchole, iterolemia, 1 low testosterone,4 psychogenic, and 4 idiopathic. In 10 patients with placebo, 7 were followed for more than three months. The clinical efficacy of KRG was 66.7% on objective Questionnaire and 72.2% on subjective analysis. When KRG were given, all parameters surveyed have shown improvements compared to the controls. Serum testosterone Bevels were normalized in 2 patients with KRG, whose serum testosterone levels were reduced from prestudy. When the erectile functions after audiovisual stimuli evaluated using Rigiscan in 6 patients with KRG,4 showed rigidity move than 70oyc. One patient reported constipation, and 2 gastric up-sets in the KRG group. In conclusion, KRG has beneficiary action on male erectile capabilities with minimal side effects. Thus KRG has been proven effective in Koreans, and a result on other Asians is pending. The exact action mechanism and the active ingredients in KRC need to be itudied.

Antidiuretic Hormone Levels in Men with Burning Mouth Syndrome: A Pilot Study

  • Purpose: Burning mouth syndrome (BMS) is a disabling pain that mostly occurs in elderly women, but rarely in men. It is characterized by an unremitting oral burning sensation and pain without detectable oral mucosal changes. We investigated the clinical and hematologic features of middle-aged men with BMS, and compared the results to those of men with oral mucositis. Methods: Five men with BMS ($48.60{\pm}6.19years$) and five age-matched controls with oral mucositis ($49.80{\pm}15.26years$) underwent clinical and psychological evaluations and blood tests. Psychological status was evaluated using the Symptom Checklist-90-Revised. Cortisol, estradiol, progesterone, testosterone, follicle-stimulating hormone (FSH), luteinizing hormone (LH), adrenocorticotropic hormone (ACTH), and antidiuretic hormone (ADH) levels and erythrocyte sedimentation rate (ESR) were determined from the blood samples. Results: ADH level was significantly lower in men with BMS than in the controls. ADH levels correlated with testosterone (p<0.01), and ACTH levels strongly correlated with ESR (p<0.05). Progesterone level positively correlated with FSH and LH levels. Pain intensity on a visual analogue scale correlated with estradiol level only in men with BMS. Among psychological factors, the obsessive-compulsive disorder, interpersonal-sensitivity, and anxiety scores were higher in men with BMS than in the controls (p<0.05). However, no correlations were observed between the psychological and hematologic factors in both groups. The BMS symptoms presented only on the tongue, with the lateral border being the most prevalent area. Conclusions: Men with BMS may experience dysregulated endocrinologic or psychoneuroendocrinologic interactions, which might affect oral BMS symptoms, aggravating the severity of the burning sensation.

  • The oriental medicine based on the traditional Chinese medicine has developed characteristically according to the history and racial character respectively; China, Korea and Japan. Japan, among these nations, has accepted western medicine earlier than other nations and has tried to compare western and oriental medicine and combine them. In Japanese traditional medicine, it is characteristic that the old medical classics focusing on Sanghannon (傷寒論) and Geumgyeyoryak(金?要略) has developed The recent tendencies of clinical medicine and researches in Korean oriental medicine are mostly about the study of oriental medicine in view of western medicine and the combination of western and oriental medical treatment like Japan. But the study on the Japanese oriental medicine hasn't so far been tried before in Korea. From now on, we should not overlook that a more interest on Japanese oriental medicine will be very useful. Therefore we have surveyed the background of its origin and the process of development of the theory of ${\ulcorner}$Qi, Blood and Body Fluids${\lrcorner}$. What we wish to show in this paper is to provide a source for the basic understanding by explaining a fundamental theory of physiology and pathology of Japanese oriental medicine. Concepts of ${\ulcorner}$Qi, Blood and Body Fluids${\lrcorner}$ suggested by Nangai Yoshimashi in 1792 is the way of thinking that the circulation of 3 factors- ${\ulcorner}$Qi, Blood and Body Fluids${\lrcorner}$ nourish human body. Among these 3 factors, if Qi does not function smoothly, it causes the condition of a disease like Qi-deficiency, imbalance of Qi-distribution or Qi-depression and stasis; in Blood's case, deficiency of Blood and Blood stasis; and as for Body Fluids, stasis of Body Fluids. In the recent trend of study, there's a try to combining the western and oriental medicine, Qi is considered as psychoneurotic system, Blood as circulatory and endocrinologic system and Body Fluids as immunologic system.
