Native valve endocarditis (NVE) without preexisting structural valve or congenital cardiac malformation especially in pediatric group is rare. A case of isolated tricuspid valve endocarditis in a 7-year-old child without any cardiac malformation is described. This child had suffered from fever and productive cough for 3 weeks. Blood culture grew Staphylococcus aureus. Fever was not controlled even with proper antibiotic treatment. Transthoracic echocardiogram and lung perfusion scan revealed a large vegetation on the tricuspid valve with multiple embolism Surgical procedures included vegetectomy partial cusps resection and pericardial patch valvuloplasty. Th patient was in NYHA class I during follow up.

15세 남자환자가 갑자기 발생한 흉통과 각혈, 호흡곤란을 주소로 본원 응급실로 내원하였다. 환자는 동맥관 개존증 및 감염성 심내막염띠 합병된 폐색전증으로진단되었다. 3주간 적절한 항생제 치료를 한 후 체외순환하에 동맥관 개존증 패취봉합 및 폐동맥 판막 성형술을 시행하였다. 이후 환자는 순조롭게 회복되어 퇴원 하였다. 동맥관 개존증에 합병된 급성심내막염으로 폐동맥 혈전증이 발생한 환자를 수술 치험하여 좋은 결과를 얻었기 에 보고하는 바이다.

Approximately 5 percent of infective endocarditis are limited to the right side of the heart, the tricuspid valve being the usual site of involvement. Usually there is no underlying cardiac disease, and the vegetations occur on previously normal tricuspid leaflets. This paper reports a case of bacterial endocarditis involving the bio-tricuspid valve in a patient with tetralogy of Fallot, and who required prosthetic valve replacement in addition to surgical therapy for the congenital lesions.

Subannular aortic aneurysm is a word-wide rare disease entity occurring predominantly in young black men. In Korea, there has been no report. We report one patient, 46 years old man, who had been operated urgently because of acute aortic insufficiency and aortic valvular vegetation after antibiotics treatment of Subacute bacterial endocarditis for 6wks. At the operative field, We found the bulging aneurysmal mass between the aorta and superior vena cava above the right pulmonary artery, which has subannular communicating opening into the left ventricular cavity, beneath the anterior commissure of the bicuspid aortic valve. Pathologic findings are consistent with "portion of vascular wall with features of aneurysm.* The patients survived aortic valve replacement and patch closure of subannular aneurysm, with no symptoms at one-year postoperative follow-up.w-up.

A 4-year-old Holstein cow with progressive atrophy and ataxia was submitted for diagnosis. The postmortem examination revealed multifocal yellowish nodules in the pulmonary parenchyma and vegetative masses in the mitral and tricuspid valve of the heart. Both kidneys were severely enlarged, with multiple yellow nodules on the parenchyma. Histopathologically, pulmonary abscesses, vegetative endocarditis, suppurative glomerulonephritis, and fibrino-purulent arthritis were observed. The tiny β-hemolytic bacterial colonies were isolated from the lesions and identified as Arcanobacterium haemolyticum by the VITEK 2 system (bioMérieux, USA). This is the first documented report of an A. haemolyticum infection in a Holstein cow in Korea.

14년령의 중성화한 암컷 시츄가 식욕부진, 침울, 호흡곤란으로 전북대학교 수의과대학 동물병원에 내원하였다. 2주전 유선종양 제거 수술을 받았으며 내원 당시 신체검사에서 구강 내 심한 치은염, 치태 및 치석을 관찰하였다. 청진상 좌측 심기저부에서 이완기 잡음이 들렸으며 우측 심첨부에서는 수축기성 잡음이 들렸다. 심혈관 초음파 검사 결과 삼첨판과 대동맥 판막의 변성과 함께 혈액의 역류를 관찰 할 수 있었다. 세균성 심내막염을 진단하고 그 원인체를 알기 위해 혈액배양을 실시 하였다. 혈액배양 검사 결과가 나오기 전에 항생제를 포함하여 이뇨제와 베나제프릴을 사용하였으며 환자는 잘 관리 되었다. 혈액배양 검사 결과 staphylococcus pseudintermedius가 배양 되었다. 하지만 1주일 후 환자는 급성으로 발생한 폐수종에 의해 사망하였다.

Bacterial endocarditis is a well-recognized complication of prosthetic valve replacement. Postoperative fungal endocarditis of aortitis has been reported with increasing frequency. Aspergillus endocarditis or aortitis following open heart surgery has been reported in several patients. The difficulty in the diagnosis of this type of infection, the problems of therapy of prosthetic valve endocarditis0 and the relative ineffectiveness of antifungal agents account for the high mortality. Recently, we have experienced a patient with aspergillus aortitis after replacement of mitral valve. The diagnosis was finally established by histotogical examination of emboli removed from the femoral artery. The patient died after second open heart surgery for replacement of ascending aorta.

Staphylococcus aureus is an important cause of human infections, and it is also a commensal that colonizes the nose, axillae, vagina, throat, or skin surfaces. S. aureus has increasingly been recognized as a cause of severe invasive illness, and individuals colonized with this pathogen are subsequently at increased risk of its infections. S. aureus infection is a major cause of skin, soft tissue, respiratory, bone, joint, and endovascular disorders, and staphylococcal bacteremia may cause abscess, endocarditis, pneumonia, metastatic infection, foreign body infection, or sepsis. The authors describe a case of a fisherman who died of sepsis on a fishing boat during sailing out for fish. The autopsy shows paravertebral abscess, pus in the pericardial sac, infective endocarditis with vegetation on the aortic valve cusp, myocarditis, pneumonia and nephritis with bacterial colonization, and also liver cirrhosis and multiple gastric ulcerations.

삼첨 판막 심내막염은 감수성이 있는 항생제에 효과적으로 치료될 수 있다. 그러나 패혈증이 지속되거 나, 중증의 심부전, 다발성 폐 색전증 그리고 실 초음파상 증식증 (vegetation)이 있는 경우는 수술이 필요 하다. . 19세횝 남자환자가 감염성심내막염으로 입원하였다. 환자는 9년전에 perimembranous type의 VSD로 수술받은 병 력 이 있다. 심초음파상 삼첨판의 전엽부에 커다란 증식증의 소견이 보였고,과거에 VSD를 봉합했던 부위에 누 출 (leakage)을 통한 좌우단락이 관찰되었다. 항생제치료와 함께 인공판막치 환술을 시행하였다. St. Jude Medical 양엽판막 (size 33 mm으로 삼첨 판막 대 치술을 시 행하였고 잔여 VSD는 단순봉합으 로 폐쇄하였다. 술후 시행한 심초음파검사에서 치환된 삼첨 판막의 기능은 좋았고, 증식증과 좌우단락의 소견은 보이지 않았다. 환자는 합병증없이 술후 25일째에 퇴원하였다. 저자들이 경험한 본 증례는 잔여 VSD로 인해 생긴 감염성심내막염을 초기에 적극적인 수술을 시행 하여 좋은결과를 얻었음을 보여준다. 이 에 문헌고찰과 더불어 보고하는 바이다.

Streptococcus suis (S. suis) is an emerging zoonotic pathogen that causes bacterial meningitis in humans. S. suis is an encapsulated gram-positive facultative anaerobic bacterium and is an important pathogen in pigs. This infectious disease usually manifests in humans as meningitis, endocarditis, septicemia, and arthritis. Most cases originate in Southeast Asia, and human S. suis infections are often reported in countries with a high density of pigs. Meningitis is a common clinical manifestation of S. suis infection. Moreover, hearing loss is a common complication that can be bilateral, profound, and/or permanent. This report presents two cases of bacterial meningitis and hearing loss caused by S. suis in patients without a history of direct exposure to pigs in an intensive pork industry region.
