유방암 수술환자의 추후관리 프로그램이 신체, 심리, 사회적 기능에 미치는 효과 (The Effects of a Follow-up Program on Physical, Emotional, and Social Function after Breast Cancer Surgery)

  • Purpose: To evaluate the effects of a follow-up program for breast cancer patients after surgery. Method: A quasi-experimental design was used. The subjects were 56 breast cancer patients (experimental group: 27, control group: 29). The subjects of the experimental group participated in the Follow-up program of 7 sessions once a week, during 4 weeks including one telephone visit. The program consisted of health education of breast cancer, exercise, lymph edema prevention, diet education, theraband rehabilitation exercise, laugh therapy, and breast self examination. The obtained data were analysed by using the ANCOVA of SPSS. Results: There was a significant difference in the scores of physical wellbeing, symptom distress and mood state between the two groups. Conclusion: Considering these research results, short-term intervention could improve physical and emotional functioning abilities by helping better health. Nurses should establish and manage a proper program to provide multipurpose support and it must apply the program after surgery as soon as possible for breast cancer women.

의과대학생의 정서지능이 공감에 미치는 영향 (The Influence of Emotional Intelligence on Empathy in Medical Students)

  • Objective : The purpose of this study was to examine the influence of subscales of emotional intelligence on empathy in first-year medical students. Methods : A total of 215 first-year medical students were eligible for this study. Demographic factors (age, sex, and medical school entrance type) and measures for emotional intelligence and empathy, such as the Wong and Law Emotional Intelligence Scale and the Interpersonal Reactivity Index, were included in the survey. The relationship between emotional intelligence and empathy was analyzed by multiple linear regression, adjusting for age, sex, and entrance type. Results : The female students' level of emotional intelligence was higher than the male students for the self-emotional appraisal (p=0.029) and others' emotional appraisal (p=0.006) subscales. Meanwhile, the graduate-entry students scored higher for the use of emotion (p<0.001) and regulation of emotion (p=0.010) subscales than the direct-entry students. For empathy, the female students revealed a higher score in empathic concern (p=0.009) than the male students, and graduate-entry students scored higher than the direct-entry students in perspective taking (p=0.004) and empathic concern (p<0.001). The multiple linear regression analysis has identified significant influences of subscales of emotional intelligence on empathy: others' emotional appraisal on perspective taking (p<0.001), self-emotional appraisal (p=0.035) and regulation of emotion (p=0.023) on fantasy, others' emotional appraisal (p<0.001) and use of emotion (p=0.020) on empathic concern, and self-emotional appraisal (p=0.033) and regulation of emotion (p<0.001) on personal distress. Conclusion : This study's findings suggest that emotional intelligence may affect empathy in medical students, which can have implications for developing educational strategies to enhance empathetic attitudes in medical students.

리소스 마인드풀니스에 관한 효과성 연구 (Effect of Resource Mindfulness on Emotional State, Focusing on Anxiety and Stress Reduction)

  • Objectives: This study aimed to evaluate the impact of Resource Mindfulness on participants' emotional state, focusing on anxiety and stress reduction and to explore the importance of psychological resources in this process. The effectiveness of Resource Mindfulness was investigated through both quantitative and qualitative analyses, examining its influence on subjective distress, mindfulness, core emotions, and identification of personal resources. Methods: This study involved 27 participants, including Korean medicine students, psychological counselors, clinical psychologists, and Korean medicine specialists, who attended a 2-night, 3-day M&L psychological support education program from July 2 to July 4, 2022. Participants were informed of the study's purpose and procedures. They provided written consent. Quantitative measures included Subjective Units of Disturbance Scale (SUDS), Five Facet Mindfulness Questionnaire (FFMQ), and Core Seven-Emotions Inventory Short Form (CSEI-s). Qualitative analysis was conducted using the "Drawing the Rooms of the Mind" technique. Pre- and post-program assessments were conducted to compare changes in subjective distress, mindfulness, and core emotions. Data were analyzed using paired t-tests and qualitative content analysis methods. Results: Significant reductions in subjective distress and improvements in mindfulness components were observed. Core emotions showed significant decreases in negative emotions. Qualitative analysis revealed increased tranquility, relief, and confidence, with resourceful places often being nature-related. Conclusions: Resource Mindfulness effectively reduced anxiety and stress and enhanced self-awareness and self-efficacy. It is useful for managing various mental health issues. Further research is needed to generalize these findings.

경남소재 일개조선소 근로자의 건강이상소견과 아르곤 용접과의 관련성 (Association between Subjective Distress Symptoms and Argon Welding among Shipyard Workers in Gyeongnam Province)

  • Objective: This study was conducted to investigate the association between subjective distress symptoms and argon welding among workers in Gyeongnam Province shipyard. Method: 31 argon and 29 non-argon welding workers were selected as study subjects in order to measure concentrations of personal dust, welding fumes and other hazardous materials such as ZnO, Pb, Cr, FeO, MnO, Cu, Ni, $TiO_2$, MgO, NO, $NO_2$, $O_3$, $O_2$, $CO_2$, CO and Ar. An interviewer-administered questionnaire survey was also performed on the same subjects. The items queried were as follows: age, height, weight, working duration, welding time, welding rod amounts used, drinking, smoking, and rate of subjective distress symptoms including headache and other symptoms such as fever, vomiting and nausea, metal fume fever, dizziness, tingling sensations, difficulty in breathing, memory loss, sleep disorders, emotional disturbance, hearing loss, hand tremors, visual impairment, neural abnormality, allergic reaction, runny nose and stuffiness, rhinitis, and suffocation. Statistical analysis was performed using SPSS software, version 18. Data are expressed as the mean ${\pm}SD$. An ${\chi}^2$-test and a normality test using a Shapiro wilk test were performed for the above variables. Logistic regression analysis was also conducted to identify the factors that affect the total score for subjective distress symptoms. Result: An association was shown between welding type (argon or non-argon welding) and the total score for subjective distress symptoms. Among the rate of complaining of subjective distress symptoms, vomiting and nausea, difficulty breathing, and allergic reactions were all significantly higher in the argon welding group. Only the concentration of dust and welding fumes was shown to be distributed normally after natural log transformation. According to logistic regression analysis, the correlations of working duration and welding type (argon or non-argon) between the total score of subjective distress symptoms were found to be statistically significant (p=0.041, p=0.049, respectively). Conclusion: Our results suggest that argon welding could cause subjective distress symptoms in shipyard workers.

Effect of Group Therapy on Psychological Symptoms and Quality of Life in Turkish Patients with Breast Cancer

  • Purpose: The aim of this study was to evaluate the effects of the group therapy on psychological symptoms and quality of life of patients with early stage breast cancer. Methods: This study was performed on 16 breast cancer patients who completed treatments. The total group therapy program involved a weekly session of 2-3 hours, for 16 weeks. The group therapy sessions were given to women in the oncology department by a clinical psychologist and also given training sections by the different professional teams. All the required assessments for the study were performed after and before 16 week group therapy intervention. Results: Initially we had taken 21 women but 16 participated in all therapy programs and submitted questionnaires. The mean age was 47.8 years. There were significant differences between before and after group therapy program. Anxiety, depression, and distress showed significant improvements. Hopelessness scale was detected at the border of significance. There was no change in sleep problems and quality of life. According to the analysis of correlation, considering the age factor and year of diagnosis, there was found no statistically significant relationship between anxiety, distress, depression, hopelessness, sleeplessness, and quality of life. Conclusions: This pilot study demonstrated that brief, predominantly group therapy is feasible for patients with breast cancer and, also it may be helpful to cope with emotional and physical distress.

소방공무원의 음주수준과 사건충격, 직무스트레스, 자기노출 간의 관계 연구 (A Study on the relationship among alcohol levels, and impact of event, occupational stress, and distress disclosure among fire officials)

  • Purpose: The aims of this study were to identify the relationships among alcohol level, impact of event, occupational stress, and distress disclosure and to determine the influences on alcohol level among fire officials. Methods: Participants in this cross-sectional, descriptive study were 241 fire officials. Data were collected from June 2018 to July 2018 using self-reported questionnaires and were analyzed using IBM SPSS 22.0 software (IBM, Armonk, NY, USA). Results: The results revealed significant positive relationships between alcohol level and impact of event, and impact of event and occupational stress, and a negative relationships between occupational stress and distress disclosure. Regression analysis revealed that alcohol level accounted for 9.2% of the variance by gender, and impact of event(e.g., sleep problems and emotional numbness). Conclusion: These findings provide information that can be used in the development of mental health promotion programs for fire officials.

Psychological distress and fertility quality of life (FertiQoL) in infertile Korean women: The first validation study of Korean FertiQoL

  • Objective: To investigate psychological distress and fertility quality of life (FertiQoL) in infertile Korean women, and to investigate whether a correlation exists between psychological distress and FertiQoL. Methods: Participants in this study were made up of 141 infertile women and 65 fertile women. We conducted a survey on psychological distress (using the Depression Anxiety Stress Scales [DASS]-42 questionnaire) and administered a FertiQoL questionnaire. The levels of stress hormones (adrenocorticotropic hormone [ACTH] and cortisol) in serum were assessed. Results: The scores for depression ($13.7{\pm}8.4$), anxiety ($10.7{\pm}6.4$), and stress ($18.0{\pm}8.3$) among the infertile women were significantly higher than the scores for depression ($9.4{\pm}7.5$), anxiety ($6.6{\pm}6.0$), and stress ($12.2{\pm}8.3$, p<0.001) among the fertile women. There was no difference in the scores for depression ($13.5{\pm}8.2$, $13.8{\pm}8.6$), anxiety ($10.0{\pm}6.2$, $11.5{\pm}6.6$) and stress ($17.7{\pm}8.4$, $18.4{\pm}8.1$) between younger (${\leq}34$) and older (${\geq}35$) participants. The mind-body (r =-0.495) and emotional (r =-0.590) subscales showed a higher negative correlation with stress compared with other scales of psychological distress. At the same time, the social (r =-0.537) and relational (r =-0.385) subscales showed a higher negative correlation with depression. Levels of cortisol and ACTH in infertile women were $9.1{\mu}g/mL$ and 11.9 pg/mL, respectively, which are within normal ranges. Conclusion: The levels of psychological distress and quality of life in infertile Korean women seem to require psychological intervention. This study provides a baseline measurement of psychological distress and FertiQoL in infertile women in Korea, which will be available for developing psychological interventions for infertile Korean women.

간호⋅간병통합서비스 병동 간호사의 감정노동 파악 및 개선방안: 초점집단인터뷰 적용 (The Emotional Labor Status and Improvement Plans of Nurses Working in the Integrated Nursing Service Ward: Applying Focus Group Interviews)

  • Purpose: The purpose of this study was to investigate the status of emotional labor of nursing personnel working in comprehensive nursing service ward and to suggest the way service improvement can be achieved. Methods: A total of 28 nurses working in comprehensive nursing service ward were divided into four groups to conduct focus group interview. All interviews were recorded and transcribed after the interview to perform data analysis in the order of data classification, topic categorization, and keyword derivation. Results: The five categories of subjects and relating keywords drawn from the focus group interviews are as follows: 1) Emotional labor experience: suppressing emotions, expressing emotions or actions that are different from reality, 2) Situations of emotional labor: verbal abuse and assault, sexual harassment, personal needs and errands, 3) Responses to emotional labor: responding directly, responding directly, receiving senior's help, using the organizational system, persevering, 4) Problems caused by emotional labor: work exhaustion, job change intention, job stress, 5) Protection plan against emotional labor: manual or education for nurses, education for patients and carers, compensation, tough sanctions though system strengthening. Conclusion: This study shows that although nurses working in comprehensive nursing service ward generally experience high levels of emotional labor, the problem solving of them relies mainly on personal response. Therefore, it is necessary to develop various measures to protect nurses in an organizational level response, thus to improve the comprehensive nursing service system.

The art of diabetes care: guidelines for a holistic approach to human and social factors

  • A holistic approach to diabetes considers patient preferences, emotional health, living conditions, and other contextual factors, in addition to medication selection. Human and social factors influence treatment adherence and clinical outcomes. Social issues, cost of care, out-of-pocket expenses, pill burden (number and frequency), and injectable drugs such as insulin, can affect adherence. Clinicians can ask about these contextual factors when discussing treatment options with patients. Patients' emotional health can also affect diabetes self-care. Social stressors such as family issues may impair self-care behaviors. Diabetes can also lead to emotional stress. Diabetes distress correlates with worse glycemic control and lower overall well-being. Patient-centered communication can build the foundation of a trusting relationship with the clinician. Respect for patient preferences and fears can build trust. Relevant communication skills include asking open-ended questions, expressing empathy, active listening, and exploring the patient's perspective. Glycemic goals must be personalized based on frailty, the risk of hypoglycemia, and healthy life expectancy. Lifestyle counseling requires a nonjudgmental approach and tactfulness. The art of diabetes care rests on clinicians perceiving a patient's emotional state. Tailoring the level of advice and diabetes targets based on a patient's personal and contextual factors requires mindfulness by clinicians.

방사선 치료 중인 암환자의 주관적 행복감 향상을 위한 탐색적 연구 (Service improving the subjective happiness in Cancer Patient receiving Radiation Therapy)

  • 본 연구는 방사선 치료 중인 암환자의 주관적 행복감을 향상시키기 위한 서비스를 탐색하기 위한 것이다. 백 한 명의 방사선 치료중인 암환자를 대상으로 설문조사하여 주관적 행복감에 영향을 줄 수 있는 자기의식과 증상 불편감을 파악하여, SPSS 20.0 프로그램을 이용하여 독립표본검정, 일원분산분석, 피어슨 상관분석을 시행하였다. 방사선 치료중인 암환자의 불편감은 피로감, 외모변화, 현기증, 식욕저하, 수면장애의 순으로 높게 나타났으며, 암환자의 주관적 행복감은 증상불편감이 낮을수록, 그리고 개인의 내면적인 의식이 높을수록 향상됨을 나타냈다. 따라서 방사선 치료기간 동안 주관적 행복감을 향상시키기 위해서는 증상불편감은 감소시키고, 개인의 의식수준을 향상시킬 수 있는 신체적 심리적 정서적 측면을 고려한 통합적 서비스가 필요하다. 이를 위해서는 기존의 서비스 프로그램의 보완과 환자의 편의를 고려한 치료병원과 지역암센터나 보건기관을 연계할 수 있는 시스템이 준비되어야 할 것이다.
