Long-Term Care Hospital Nurse's Experience in Coping with End-of-life Care Nursing (요양병원 간호사의 임종간호 대처 경험)

  • Purpose: The purpose of this study is to identify, describe, and understand the meaning of experiences by clarifying the structure of the coping experience of end-of-life care(EOLC) experienced by long-term care hospitals(LTCHs) nurses'. Methods: Data were gathered from 16 nurses who had been working at LTCHs in Korea through one-on-one interviews and the data were analyzed by Colaizzi's phenomenological method. Results: It was structured and identified into 3 categories and 15 theme clusters. The emergent 3 categories were 'Long-term care hospitals nurses' attitudes towards for end-of-life care', 'End-of-life care stress', 'Coping with end-of-life care'. Conclusion: There is a need for practical and systematic education for LTCHs nurses' to positively change end-of-life care attitudes and reduce and prevent end-of-life care stress. In addition, it is suggested to operate programs that can improve coping skills or programs such as expert counseling.

  • Digital Health Care is the convergence of ICT and (non)medical technology, emphasizing the importance of prevent and monitoring health management in terms of new challenging medical paradigm: predictive, preventive, personalized and participatory. Beyond the limited medical industry of long-term care insurance, it is emerging that AI, IoT, Big Data related new services with new technologies in the 4th revolution era. It is also noted that business field based on test bed is emergent; Caring Robot, wearable devices need to be launched in the market. Diverse service is possible with Big Data and AI etc.

  • Coronavirus disease 2019 (COVID-19) is a respiratory tract infection caused by a newly emergent coronavirus, that was first recognized in Wuhan, China, in December 2019. Genetic sequencing of the virus suggests that it is a beta coronavirus closely linked to the SARS virus. This disease has non-specific symptoms such as fever, dry cough, sore throat, and gastrointestinal symptoms. This virus can transmit via aerosol and need to droplet precaution to prevent spreading in public areas. Most people with COVID-19 develop only mild or uncomplicated illness. However, about 20% patients require hospitalization, oxygen supply, and intensive care. There is no currently effective treatment available for COVID-19 unresponsive to supportive care. This is review about the recently published epidemiologic, and clinical features, diagnosis, treatment and prevention of COVID-19.

The Activities of Childhood Cancer Foundation in Taiwan (대만 어린이암재단의 활동)

  • Childhood Cancer Foundation of ROC(CCFROC) has been founded since 1982. Its goal is to provide comprehensive care for cancer children and their families. And in turn, under the provision of good and complete care, they can be back to the school and society. To achieve this goal, it needs to get every body's effort of the society together. The foundation has been founder under this concept and tried to fulfil this task. The first step is to educate the public to know that childhood cancer can be cured and the process of treatment is long and expensive. In order to reach this life-saving procedure, campaigns for raising money is most important thing. Then, pediatric hematologists / oncologists try to organize study group, named Taiwan Pediatric Qncology Goupp(TPOG), to make treatment protocols for childhood cancers. Its primary purpose is to increase the cure rate to prove the earlier the diagnosis and treatment, the better the prognosis. The second step is to provide full support of medical expenses and further to give help for emergent need to living. Through the varieties of active exercises of cancer children or families, foundation provides opportunities for them to share the experience during the different periods of diseases. The third step is to extend the scope of training activities for many kinds of personnel who devote to the care for cancer children. On the other hand, it promotes the cooperation or communication with international organizations and sets up recreation playing ground in the communities. The aim is to provide a comprehensive and integrated care for cancer children during their education, breeding and even accomplishment period. Besides the financial support of medical care, it provides social services to give psychological support for children and their families, to strengthen doctor-patients or doctor-families relationship. The foundation provides the mourning religious activities to cherish the memory of their children who passed away and to comfort their sadness. Since the childhood cancer is curable, its treatment should be active, comprehensive and it needs to provide to all cancer children without exception. Indeed, the work of CCFROC should be changed according to the environment and different time period. But, it needs the full support from the society. We hope the scope and content of the work will be more extended and its achievement will be better than now in the future.

Dementia Strategies using Welfare-technology (복지기술을 활용한 치매전략 방안)

  • South Korea faces elderly care crisis with both low fertility rate and rapid ageing rate. Long-term care insurance in 2008 has taken care burden from family to nation. However, few options, lack of tailored services, and unstable community care plan caused blankness of care in some areas. Especially, increasing dementia rate has become an emergent social issue. Welfare technology could be a useful alternative to fill out the gap between demand and supply. Using FGI method on two groups(high-risk dementia group and MCI group), intimacy, accessibility and preventive way of dementia with welfare technology are analysed in this paper. Despites of a few differences of dementia progress, interests towards technology are expressed. More suggestions using TV and normal telephone to improve daily independence are presented as well. Amongst MCI group, help-service for technology-use at ADS and linkage ways of in-home services are also suggested.

A Comparative Study on the Process of Hospital Visits according to the Characteristics of the Elderly Visiting to Emergency Medical Centers (권역응급의료센터에 내원한 노인의 제 특성에 따른 내원과정 비교)

  • The purpose of this study is to compare and analyze the process of hospital visits according to the characteristics of the elderly visiting to emergency medical centers. The subject of study was 571 patients over the age of 65 who agreed to participate in the study of emergency medical centers from May 1 to 31, 2010. The frequency, percentage and ${\chi}^2$ test of collected data were conducted with SPSS WIN 12.0. As a result, the frequency of the aged with or without urgency revisiting emergency rooms due to chronic degenerative diseases was high. In addition, there were many cases that the aged living only with a spouse or remaining single. It was obscure to classify them into a urgent or non-emergent group which made it longer time to visit a hospital. It may be necessary that the care for urgent elderly patients considering the characteristics of the family environment functions to care the elderly were weakened.

A Study on the Sports Injuries for Middle and High School Football Players in Pusan (부산 시내 중.고등학교 축구선수의 운동손상에 관한 연구)

  • Recently sports injuries are increasing due to the development of sports leisure industries as well as the increase of exercising population, and the study for sports injuries is needed more than before. Then the author studied the reasons for injuries, incidence rate, sites and sorts of sports injuries for the middle and high school football players during one year period from Jan. to Dec. 1994. The study subjects were 319 middle and high school players in Pusan and the method was questionnaire method. The results were as follows; 1. The average injury rate due to football play was 3.3 times per person for the 319 subjects during the last one year, and it was highest in goalkeeper as 4.6 times by position. 2. 68 % of the subjects did not receive periodic health examination, 74.9 % wore body protectors. The sports injuries due to football play occurred most commonly in soil play-ground(79.6 %), during training(52.3 %), and in winter time(33.7 %). 3. The commonly occurred injuries by motion were coalition(39.2 %), tackle(12.9 %), dribble (11.4 %) in order; those by sorts were contusion(34.4 %), sprain (32.9 %), spasm(9.0 %) ; and those by body portions lower extremities(82 %), trunk(17.6 %), upper extremities(12.5 %). 4. The frequently used treatment area was home care as in 42.7 % ; treatment methods were physical therapy, acupuncture/moxacautery, rest, operation in order; choices of treatment method were recommendation by coach/manager, and that by medical professional was the lowest as in 2.2 %. 5. If sports injury occurred during play, coach or manager let the players take rest in 56.2 %, but emergent management by medical professional was done only in 1.9 %. The return to play was decided mostly by the player himself(52.8 %), but that decided by a doctor was only in 1.0 %. In conclusion, to lessen the sports injuries due to football play, annual physical check-up, strengthening of play rules, improvement of playground, conditioning exercise, regulation of body protectors, and preventive training for expected injuries behaviors should be done and the decision to return to play and emergent management should be done by a medical professional as well. Fulminant Headache after Epiduroscopy -A case report- (경막외강 내시경술후 발생한 전격성 두통 -증례 보고-)

  • Epiduroscopy is a new imaging and invasive technique that has become increasingly popular in the diagnosis and therapy of spinal pain syndrome. However, the risk of epiduroscopy is a major concern due to its invasiveness. The complications include hematoma, infection, neural damage, cardiac failure and raising of intracranial pressure. We have experienced a case of severe diffuse headache and dizziness following 1 day after epiduroscopy. Emergent brain MRI finding has no remarkable sign. The patient was admitted for 5 days to be treated with bed rest, adequate hydration and pain control with analgesics. After the treatments, she was recovered without residual sequelae and discharged. We need to take greater care of the undesirable effects detected intra- and post epiduroscopy, especially sign of increased intracranial pressure. Therefore, close monitoring of intracranial pressure is necessary during epiduroscopy.

Application of a Pediatric Advanced Life Support in the Situation of a Dental Treatment (치과진료 시 소아고급생명구조술의 적용)

  • In a dental treatment, a dentist has to know the possibility to happen all kinds of the emergency and to prepare for managing that situation. Especially, the cardiac arrest is the most serious emergent problem. If the accident were happened, most dentists got embarrassed. The American Heart Association (AHA) is offering the Basic Life Support (BLS), Advanced Cardiopulmonary Life Support (ACLS) and Pediatric Advanced Life Support (PALS) programs for the healthcare who need to prepare the life threatening situation. The PALS is specialized to someone who participate in pediatric health-care field. This program is composed of three major emergency problems, such as respiratory emergencies, shock and cardiac arrests. The main concepts of the PALS are early recognition and systemic team approach. The purpose of this study was to introduce about PALS and to prepare response system for emergencies in the dental environment.

  • Immdeiate hypersensitivity reactions to natural rubber and latex products pose a significant threat to patients, healthcare workers, and the general population. The purpose of this study is to summarize the clinical syomptoms of latex rubber allergy and provide guidelines for the management of latex allergy. Contact dermatitis from the nearly constant use of latex gloves during the workday has become a real problem for many health care professionals. Moreover, the patient who come in contact with these gloves or with other latex-containing equipment may show reactons. Far worse than skin problems is life-threatening anaphyaxis, which can be the first indication that a person has latex sensitivity. In conclusion, nurses shold know the latex precautions such as risk factors, adverse reactons to latex, emergent treatment and so on.
