How to cope with medical emergencies in the Dental Clinic (치과진료실에서 흔한 긴급상황의 처치)

  • Conceptually, the emergency is classified as an urgency and an emergency. The Urgency is not immediately life threatening, but could become so if not resolved promptly. So, it requires prompt patient care same as in the emergency situation. However, the emergency is immediately life threatening and requires immediate action, such as calling 119 and basic life support. Most medical emergencies in the dental clinic cases are urgencies. The incidence of true emergencies is approximately 1/1,000,000. Adequate managing medical urgencies are important because the chance of encountering medical emergencies in the dental clinic is high and higher these days especially because of rapidly aging Korean society. Many dentists indicate that many dentists feel difficulties to recognize and treat medical problems. This paper reviews the concept of medical emergencies and how to cope with commonly occurring urgencies in the dental clinic such as loss of consciousness, hypoglycemia, hyperventilation syndrome. The best treatment for medical urgencies and emergencies in the dental clinic is prevention. Also, it is required to make preparation for emergency situations such as CPR education for dentists and being well-acquainted with equipment and drugs for the emergency care as well as to systemic medical evaluation, patient monitoring, and sedations for controlling patient's anxiety and pain. In this paper, simple algorithms based on guidelines for common urgencies in the dental clinic are suggested. In conclusion, every dentist has competencies to do the urgency care adequately and basic life support. Also, advanced cardiac life support is strongly recommended when sedation is performed in the clinic.

The Work Experiences of Emergency Room Nurses during the COVID-19 Pandemic (COVID-19 대유행 상황에서 응급실 간호사의 근무 경험)

  • Purpose: The purpose of this study was to explore emergency room nurse's work experiences during the COVID-19 pandemic. Methods: Data were collected from 15 registered nurses working in the emergency room between October, 10th and December 15th, 2020, through focus group interviews and in depth interviews. Collected data were analyzed using Colaizzi's phenomenological method. Results: The study derived four categories and eight themes. The four categories are 'sense of cringing with fear', 'nursing in chaotic situation', 'lonely struggles', and 'career growth as a nurse'. Emergency room nurses experienced fear and confusion in the COVID-19 pandemic, but overcame the crisis and had positive experiences of professional growth as a nurse. Conclusion: To protect emergency room nurses from infectious disease and improve their professional quality of life during the COVID-19 pandemic appropriate education and healthcare resource should be provided. It is necessary to reflect clinical worker's opinions and improve the delivery system of guideline to reduce confusion in the field and develop practical infectious disease control guideline. Furthermore, better working conditions, appropriate rewards and social and mental support should be provided for nurses in emergency room.

Comparison of Standards for healthcare Facilities and Environmental Investigation to Analyze Guidelines and Current Status of Healthcare Facilities (의료시설 관련 기준 비교와 환경 조사를 통한 의료시설 지침 및 현황 분석)

  • Purpose: This study aims to analyze and supplement the standards related to healthcare facilities, negative pressure isolation wards, and emergency treatment facilities. In addition, through environmental investigations, analysis of emergency remodeling cases centered on the structural and HVAC characteristics of healthcare facilities is conducted. Methods: Domestic and foreign standards related to healthcare facilities were analyzed. Field investigations and architectural drawing analysis of general and emergency treatment facilities were conducted. Results: Healthcare facilities have different space classifications and air conditioning methods depending on the site situation. Emergency treatment facilities are classified into cases where the HVAC system is remodeled and portable negative pressure unit is installed, and some facilities did not meet the standards for differential pressure and air change rate. Implications: When developing emergency remodeling technology, remodeling and safety evaluation guidelines, it is considered possible to propose clearer guidelines for emergency remodeling treatment facilities for infectious diseases in Korea by referring to the results of this study.

Multiple casualty disaster scene response management: a survey of 119 paramedics (119구급대원의 다수사상자 발생 재난 현장의 대응 역량에 관한 연구)

  • Purpose: The purpose of this study is to understand currently active Korean paramedics' disaster response abilities, including immediate response, severity classification, patient treatment, and patient transfer, in a disaster situation with multiple casualties. Methods: A structured questionnaire consisting of a total of 25 questions was used, including 5 questions on the subject's general characteristics and 20 questions on disaster-related emergency response abilities. Results: Among the disaster response abilities of the participants, the patient transport ability scores were high and the cooperative support ability scores were low. In terms of general characteristics, there was a significant difference in age, and it was high in the 40s, and there was a significant positive correlation between each competency. Conclusion: These results suggest that there is an urgent need to develop a systematic and specialized educational system with components inside and outside fire departments related to multiple casualty disasters to improve overall abilities.

  • The aim of this study was to investigate experience and education needs on medication and emergency situations for young children of child caregivers. The data from 190 caregivers were collected using open-ended questionnaires composed of four items and analysed by content analysis. The categories derived for each theme were as follows: experience of medication of six categories(no referral for medication, young children refused medication, inaccurate referral, a variety of medication, sick children but not having medication), education need of four categories(for child caregivers, for parents, for children, guideline required), experience of emergency situation of six categories(skin damage or bleeding, decreased consciousness due to seizures, high fever persisted, asphyxiation due to foreign body, dislocation or fracture, emergency not knowing how to cope), education need of emergency situation of five categories(contents, methods, cycle, necessity, institutionalization). It is required to improve more practically the education contents and methods related to medication and emergency situation of child caregivers.

  • A flight simulator is an instrument which lets pilots attain proficiency of handling various emergency situations, and the sense of flying. In this thesis, current status of domestic simulators will be stated, and after showing the problems with the present management of these simulators, comparisons of how and what kind of facilities they are managed around the globe will be made. After comparing and analyzing the difference in the way the simulators are ran, measures of improvement in domestically appropriate simulators and increase of operating facilities will be drawn.

Before a lot of information of client are shown, guard expert has to search observation of materialization, extension of reservation, notification of business and causes of danger through personal interviews. Rapid communication system has to be assumed and effective caution, protection and shelter and following prevention are cooperated as well as has to care about safe of guard object by a protective wall. And also has to get out of the ground, keep the large shape when moves, prevent the trials of attack from the assassinators after takeing control over weapons.

Exploration on Disaster Nursing Education in Korea (한국의 재해간호교육 실태)

  • Purpose: The disaster occurs increasingly on the earth and nurses are requested to call out to on-sites or help emergency situation. This descriptive study was done to explore the disaster nursing education in Korea. Method: Data was collected from 116 nursing undergraduate program and 93 colleges responded, and analyzed frequencies and percentiles. Results: There was no institution offering Disaster Nursing titled course. Emergency Nursing was mostly combined with disaster nursing. More half (52.7%) did not deal disaster nursing concept at all, from which half had no plan to open in the near future. The lack of teaching manpower was shown. Recommendation: To meet the nursing need in disaster situation, disaster nursing competencies should be included in nursing major subject at the basic education level to be a registered nurse in Korea.

  • In urban area narrow commuter vehicles have attracted interest as a possible solution to reduce traffic congestion and parking problems. However, a narrow vehicle has an increased to overturn during hard cornering when compared to conventional vehicles. This tendency can be reduced by tilting it toward the inside of the turn. Two types of automatic tilting control systems which are Direct Tilt Control(DTC) and Steering Tilt Control(STC) have been developed. In this paper as one of the technique to improve the handling performance for the unusual vehicle the control system which blends both the DTC and the STC system is considered. It uses the merits of both the DTC and the STC system. As a control strategy for combination the switching control method is used. Finally, the fact that the unusual vehicle is safe under an emergency situation such as slippery road surface is proved by computer simulation.

A phenomenological study of paramedic student's experience on simulation (응급구조학과 학생의 시뮬레이션 경험에 관한 현상학적 연구)

  • Purpose : This study was attempted to explore the paramedic student's experience on simulation education. Methods : Data were collected through in-depth interviews from 15 paramedic students. The study was analyzed by phenomenological method, specifically Colaizzi's method. From the protocol, significant statements were organized into formulated meanings. Results : The formulated meanings were identified 35 themes, 12 theme clusters, and 6 categories. The 6 categories include burden of demonstration, role experience as a paramedic, confidence in field treatment, sense of disjunction caused by difference from real situation, realization and learning by oneself, and sense of stability from hypothetical situation. Conclusion : This study suggested that, even though some negative perceptions were observed, simulation education will fit as a replacement for the field practice of paramedic's students given that more field-like practice environment and repetitive simulation education are present.
