산업현장의 최초반응자 필요성에 대한 검토 - 건설업 산업재해를 중심으로 - (Review of the necessity of first responders in industrial sites - Focusing on the industrial accident in the construction site -)

  • Purpose: The purpose of this study is to analyze the current status of disasters and initial first aid in the construction industry, and to suggest the deployment of emergency medical technicians and reinforcement of first aid training in the construction industry. Methods: Among industrial accidents in 2020, the accident characteristics and initial first aid that occurred at construction sites were analyzed, and the emergency dispatch of 119 paramedics was analyzed through the SPSS method. Results: The incidence of serious accidents was high at small and medium-sized construction sites with less than 50 workers, and the incidence of falls was high, and there were few first responders who could provide first aid in the beginning. Therefore, it is necessary to improve first aid response capabilities at small construction sites. Conclusion: In order to improve the ability to respond to first aid at construction sites, it is necessary to deploy first responders and strengthen first aid education.

Does the placement of automated external defibrillators affect first responders' willingness to perform cardiopulmonary resuscitation in high-rise residential buildings?

  • Objective: The increasing number of people living in high-rise apartments may result in a delayed response from emergency medical technicians called out for an out-of-hospital cardiac arrest, making the role of apartment managers as the first responders extremely important. This study investigated whether automated external defibrillator (AED) placement influences the willingness of apartment managers to perform cardiopulmonary resuscitation (CPR) and use an AED. Methods: A cross-sectional target population-based survey was conducted in Daegu, July 2016. Questionnaires were sent to apartment managers working in apartments with more than 500 households. The general characteristics of the respondents, status of CPR education, and knowledge about and willingness to perform CPR and use an AED were investigated. Results: Of the 1,445 respondents, 758 (52.5%) worked in apartments with AEDs, of which 77.8% and 70.8% were willing to perform CPR and use an AED, respectively, compared with 68.1% and 60.0% of respondents who worked in apartments without AEDs. After adjusting for potential confounders, AED placement was associated with the willingness to perform CPR (odds ratio [OR], 1.33; 95% confidence interval [CI], 1.04-1.71) and use an AED (OR, 1.39; 95% CI, 1.10-1.75). Prior CPR training and accurate knowledge of CPR skills were also associated with the willingness to perform CPR and use an AED. Conclusion: Placing AEDs in high-rise apartment buildings and providing refresher CPR education for maintaining CPR skills will be necessary to support apartment managers in their role as first responders.

최초 반응자를 대상으로 한 심폐소생술 교육결과의 분석 (Analysis on the efficacy of CPR training for first responders)

  • This survey is mainly focus on identifying the efficacy of CPR training for 92 first responders who participated in the first aid and emergency rescue program dated on November 22 and November 24, 2000. First responders are trained and evaluated using AHA guideline for 4 hours. Data are analyzed by Percentage, Mean, T-test, F-test using SPSS program. The results for this study are as follows: 1. An average score for CPR evaluation is high as 1.71 of total score 2.0. 2. Between adult CPR and infant CPR, an average score for adult CPR is higher than infant CPR. The highest item of adult CPR and infant CPR is checking for unresponse(1.95 and 1.93), the lowest items are checking for circulation and chest compression position in the adult CPR and location of chest compression in the infant CPR 3. There are statistically significant differences in the evaluation score of CPR according to age, education level, occupation, job experience and emergency accident situation in the past.

국내 응급실에서의 골수강내 주입법 사용 현황 (Current Status of Intraosseous Infusion Technique Use at Emergency Departments in Korea)

  • Purpose: Gaining vascular access is difficult and time-consuming in critically ill children, so nowdays, in many countries, intraosseous vascular access is frequently used for rapid vascular access in critically ill children. Its pharmacokinetics is close to that of the peripheral intravenous route, but its infusion flow rate is faster. The purpose of this study was to determine how widely the intraosseous infusion technique was being used in Korean emergency departments. Methods: We telephoned forty-two (42) randomly selected university-affiliated hospitals. We asked physicians if they use the intraosseous infusion technique. Responders were emergency and pediatric residents and emergency faculty. If they responded that they were not using the intraosseous infusion technique, we asked the reason. Also, we asked about their experiences with the intraosseous infusion technique. Results: Forty-two (42) hospitals were enrolled in this study. No hospital used the intraosseous infusion technique on a regular basis. However, 8 hospitals used the intraosseous infusion technique occasionally. None of the responders had experience with the intraosseous infusion technique. Conclusion: The intraosseous infusion technique is currently underrepresented at emergency departments in Korea.

  • This paper presents the incorporation of still-alive access points (SAPs) and helper access points (HAPs) that can be utilized as anchor nodes for position estimation of a First Responders (FRs) for rapidly deployable Emergency Communication Systems (ECS) in disaster recovery. In addition, the localization environment has is formulated and initially examines the use of a distributed Gauss-Newton algorithm (GNA) as optimization technizue. A simulation has been conducted and compared with the commonly used trilateration approach in position estimation.

Current Status of Infection Prevention and Control Programs for Emergency Medical Personnel in the Republic of Korea

  • Objectives: Emergency medical personnel (EMPs) are pre-hospital emergency responders who are at risk of exposure to infections and may also serve as a source for the transmission of infections. However, few studies of infection control have specifically addressed EMPs in the Republic of Korea (hereafter Korea). The goal of this study was to assess the current status of infection prevention and control programs (IPCPs) for EMPs in Korea. Methods: A cross-sectional survey was conducted to quantitatively assess the resources and activities of IPCPs. A total of 907 EMPs in five metropolitan cities completed a structured questionnaire from September 2014 to January 2015. The data were analyzed using descriptive statistics, multi-response analysis, and the chi-square test. Results: The mean age of the participants was $34.8{\pm}15.1years$. IPCPs were found to have weaknesses with regard to the following resources: the assignment of infection control personnel (ICP) (79.5%), hand hygiene resources such as waterless antiseptics (79.3%), the use of paper towels (38.9%), personal protective equipment such as face shields (46.9%), and safety containers for sharps and a separated space for the disposal of infectious waste (10.1%). Likewise, the following activities were found to be inadequately incorporated into the workflow of EMPs: education about infection control (77.5%), post-exposure management (35.9%), and the decontamination of items and spaces after use (88.4%). ICP were found to have a significant effect on the resources and activities of IPCPs (p<0.001). The resources and activities of IPCPs were found to be significantly different among the five cities (p<0.001). Conclusions: IPCPs for EMPs showed some limitations in their resources and activities. IPCPs should be actively supported, and specific IPCP activities for EMPs should be developed.

119 응급신고에서 수보요원과 신고자의 통화분석을 활용한 머신 러닝 기반의 심정지 탐지 모델 (Machine-learning-based out-of-hospital cardiac arrest (OHCA) detection in emergency calls using speech recognition)

  • 심정지는 초기 대응에 따라 생존율과 예후에 영향을 미치는 중요한 응급 상황이다. 특히 병원밖심정지(out-of-hospital cardiac arrest, OHCA)의 경우, 119 구조대의 초기 조치가 심정지 환자의 생존율을 높이는 데 결정적인 역할을 한다. 그러나 국내에서는 수보요원의 수가 제한적이지만 다량의 신고 전화에 응대해야 하는 현실이다. 이런 상황에서 머신러닝 기반의 OHCA 탐지 프로그램은 수보요원의 보조 역할로 심정지 환자의 생존률을 높일 수 있다. 본 연구에서는 이러한 문제를 해결하기 위해 머신러닝 기반의 심정지(OHCA) 탐지 프로그램을 개발하였다. 이 프로그램은 수보요원과 신고자의 통화 녹취록을 분석하여 심정지 여부를 판단한다. 제안한 모델은 수보요원 및 신고자와의 통화를 자동으로 전사하는 모델, 텍스트 기반의 심정지 탐지 모델, 그리고 프로그램 개발을 위한 서버와 클라이언트로 구성되어 있다. 실험 결과, 본 연구에서 제안한 모델은 F1 점수 기준으로 79.49%의 성능을 보였으며, 수보요원과 비교하여 심정지 감지 시간을 15초 단축하였다. 이 연구는 소규모 데이터셋을 사용하였음에도 불구하고, 심정지 기반의 탐지 프로그램이 수보요원의 보조 역할로 심정지 생존률에 기여할 수 있음을 입증하였다.

  • The purpose of EMS is to provide emergency patients with prompt and suitable emergency treatments therefore it saves their lives and makes them recover fast to return to normal. The purpose of correctional administration is to return inmates to society as sound citizens after their service by providing various correctional education, reformational programs, vocational training and etc. In this way, the aim of correctional administration and EMS is like mutual agreement by their returning to society safely. Therefore this study makes some propositions like the followings for the safety between many inmates who can have physical injury and exposure to diseases caused by particularity of subculture within correctional institutions and the staffs who work for them. It is said about efficient pre-hospital EMS through various causes of emergency situation in the correctional institution, system, manpower, facility, equipments and problems and so on. 1. Recruit the first-rate EMT(emergency medical technician) as central role of pre-hospital EMS according to each correctional institution and working terms. 2. Equip specialized transference system with symptom of patients and purpose of transference. 3. Emergency medical equipments and first-aid medicines should be equipped for field and ambulance. 4. Educate correctional officers as first responders and inmates within correctional institutions about systematic emergency treatment.

Comprehensive Security and Crisis & Emergency Management for Protecting Critical Infrastructure

  • Recently, interest has heightened over 'critical infrastructures' and their reliability in the face of potential terrorist attack. Assault on any of the critical infrastructures as transportation, power, water, telecommunications, and financial services, entails great consequences for their users as well as the other interdependent critical infrastructures. How to protect our vital critical infrastructures is the key question in this paper. The purpose of this article is to suggest the implications for crisis and emergency management to protect the critical infrastructures in our society. For achieving the purpose, we examined the concept of comprehensive security, national crisis, and critical infrastructure and, using the holistic approach, we examined the comprehensive emergency management for suggesting the implications for establishing the critical infrastructure protection system; building up the high reliability organization, organizing and partnering, assessing the risk, preparing first responders, working with private owners of critical infrastructures, working with communities, improving the administrative capacity.
