Prolapse Gastropathy Presenting with Hematemesis in a 3-year-old Child - A Case Report (3세 소아에서 토혈을 주소로 내원한 위식도 탈출증 1례)

  • Prolapse gastropathy is not uncommon in adult, but is not reported yet in previously healthy children. A 3-year-old child came to our emergency room after a 1-day history of emesis episodes with coffee-ground hematemesis. During the endoscopic procedure, and the process of retching and vomiting was observed and a tense knuckle of gastric mucosa was seen to be forcefully and repeated prolapsed into the distal esophageal mucosa, and mucosal hematoma was found in the gastric fundus. Upper gastrointestinal study revealed no abnormality and 24 hour pH monitoring revealed no pathologic gastroesophageal reflux. Retching is thought to cause the forceful prolapse and induce subsequent trauma of gastric mucosa. This case illustrates that the episodes of vigorous retching and resultant gastric mucosa are now considered to be the cause of the hematemesis and epigastric pain in children.

  • In this paper, we introduce a smart care surveillance system which can support various CCTV cameras. In order to monitor an emergency requester in case of emergency, the server performs automatical CCTV Pan, Tilt, and Zoom control based on the location coordinates of the emergency requester. Also, a server finds and tracks the emergency requester using image processing and the updated location information. We implement a smart care surveillance system using the Genetec SDK tool to support various CCTV cameras. The efficiency of the rescue operation with the smart care surveillance system can be improved because rescuer can quickly control and monitor the requester's CCTV images.

  • This study is about the development of emergency response guidance according to the fire safety index in the fire safety module, which is one of the on-board remote monitoring system. In this paper divided the zones based on the location of the fire sensor for passenger ship which is one of the service passenger ships and developed on the emergency response guidance by the identified zones.

  • Main Issue of Supreme Court Decision 2005Da16713 Delivered on June 24, 2005 is about the duty of medical care in the interhospital transfer of patients. According to the above Supreme Court Decision, in the interhospital transfer of patients, the decision to transfer should make from the aspect of medical treatment. The hospitals and doctors keep the duty of medical care. In addition to the duty for hospitals/doctors to check the capacity and availability of the hospital to which the patient is transferred, there are also duties to inform about emergency medical service and to sufficiently explain the need for the transfer, the medical conditions of the patient to be transferred and the hospital from which the patient is transferred. The hospital to which the patient is transferred must be thoroughly informed about matters such as the patient's conditions, the treatment the patient was given and reasons for transfer. including information upon referral, completeness of medical records, patient monitoring and so on. The interhospital transfer requires the consent of doctor belonging to the hospital to which the patient is transferred after the consideration of capacity and availability of the hospital and the informed consent of patients or legal representatives.

  • In this research a monitoring system for turning using NC information and the current of feed motor as a monitoring signal was developed. The overall system consists of modules such as learning process, NC data transmission, generation of forecast information, signal acquisition, monitoring and post process. In the learning process, the reference data and the cutting force equation necessary for monitoring are obtained from the accumulated monitoring results. In the generation of forecast information, the information of forecasted cutting forces is acquired from the cutting force equation and NC program and appended to each NC block as a monitor code. Reliability of monitoring is improved by using the monitor code in the real-time monitoring. Monitoring module is divided into two parts : the off-line monitoring where errors of NC program are checked and the on-line monitoring where the level of motor current is monitored during cutting operations. If the actual current level exceeds the limit value provided by the monitor code in the level monitoring, it is recognized as abnormal. In the event of abnormal status, the post processor sends the emergency stop signal to NC controller to stop the operation. Actual experiments have shown that the developed monitoring system works well.

  • Objectives: A study was conducted by analysing the contents and frequency of emergency treatment practiced by emergency medical technicians(EMTs) in emergency situations and contains related factors. Methods: The study subjects included 80 EMTs who participated in ongoing education classes during the period from June 25th to June 26th, 2008. the survey included 53 EMTs employed within 119 first aid teams in fire stations in both a metropolitan city and a medium-sized city. From the survey results relation between frequency of emergency treatment and subjects characteristics explored statistically including t-test, X2 test, Pearson correlation and finally stepwise multiple regression. Results: The most frequent procedure by EMTs checking vital signs. The statistically significant related factors overall of the 23 items of emergency treatment frequency was the working area and the grading of EMT certification. In contrast, significant related factors with frequency of IV linkage were found to be the subjects gender, institution employed and the importance of recognizing the item. The statistically significant related factors of that of ECG monitoring was the gender of respondents. Conclusions: To keep the appropriate frequency of emergency treatment by EMT, active efforts to promote their individual importance and confidence cognition are necessary, especially towards males and persons who have 2nd class certification. Moreover, EMTs who work in small cities must be supported.

The Distribution and Patient Care in Radiography for Emergency Outpatients (응급환자의 방사선영상검사 분포 및 Patient Care)

  • This study was carried out to improve service efficiency and to cope with a emergency situation in emergency radiography, through analysis of the radiographic distribution and literature cited about emergency care. Data collection of radiographic distribution was surveyed for 761 emergency outpatients who visit during JAN, 1994 at ER of the general hospital in Pusan city. The results is as follows: Emergency radiography rate of simple radiography was 61.1 %, special radiography 2.5 %, CT 12.6 %, and ultrasonography 6.7 %. In simple radiography rate, a high rate was distributed on male(63.6 %), thoracicsurgery part(90.0%), admission patient(74.9 %), and long stayed patient at ER. In special raiography rate, a high rate was observed in urologic part(28.6%), and in CT rate, observed neurosurgery part(49.2 %) and neurologic part(36.7%). Ultrasonography rate was high for female(8.8 %) and internal medicine part(15.9 %). There are distributed regional radiography rate in radiographic type that chest(55.3 %) is high in the simple radiography, urinary system(1.2%) in the special study, and brain(40.0 %) in the CT. Regional radiography rate according to diagnostic department also was showed highly for head(64.6%) in neurosurgery, chest(90.0%) in thoracic-surgery, abdomen(58.0%) in general-surgery, spine (40.0% ) in neuro-surgery, and pelvis(15.9%), upper extrimity(20.5%), and lower extrimity(31.8%) in orthopedic-surgery each. Mean radiographic case number per patient of simple radiography was sinificant on sex, age, transfer relation in both total and radiographic patients(p<0.05). Mean radiographic case number was highly distributed on male(2.2 case number) in sex, on thirties(2.7) in age, transfered patient(2.7) in patient type, and on neurosurgery(3.4) in diagnostic charged part. Total radiographic case number in regional part was highly distributed on chest(499 case number). Considering the above results, emergency radiographer should take care of the elder patient in emergency radiography and get hold of injury mechanism to decrease possible secondary injury during radiography. Because of high radiography rate of urinary system in special study, radiographer should know well about dealing with contrastmedia administration and related instrument. All radiographer who take charge emergency patient should cope with a emergency situation during radiography, Because head trauma patients is very important in patient care, especilly in CT at night, charged doctor should be always silted with CT room and monitoring-patient. Radiography was reqested by many diagnostic department in ER. Considering that rate of simple radiography is high, special room for emergency radiography should be established in ER area, and the radiographer of this room should be stationed radiologic technician who is career and can implement emergency patient care.

  • Increasing the number of the solitude old person maintain a healthy lifestyle, the research for the emergency detection and life monitoring is required. In this paper, the wireless sensor network technology is utilized to design a silver-care service model in the area of emergency silver generation activities. The service model proposed in this paper acquired by using mobile devices to access silver-care service with the network information obtained through video monitoring and sensing. Also, Proposed model, based on wide-area network connected with the best wireless network of the PDA and mobile phones for the solitude old person.
