Various disasters have been continuously occurred in Korea from 1990s to now. However, there is no substantial improvement against damages as compared with the past due to various reasons such as lack of fundamental recognition, ineffective response systems and widespread insensitivity to safety. More worse, new types of disasters have been frequently generated due to rapid changes in social structures and industrial development, unusual changes in weather and changes of international situations. These disasters request comprehensive countermeasures. In particular, while material damages by disasters can be recovered, the losses of precious lives cannot be recuperated in any ways. Thus, it is critical to set effective disaster medical plans. The first way to minimize damages by disasters is the prevention and the next is to set the disaster medical plans focusing on preliminarily activating the emergency medical system to rapidly rescue and take appropriate emergency medical services for casualties in the early stage when any disaster occurs. Nevertheless, no sufficient researches or references do not exist up to now. Even worse, effective emergency medical systems that play critical roles in increasing survival rates of casualties in actual disaster areas is not deployed. For the United States, the consistent countermeasure system is established in FEMA through a close cooperative system with relevant organizations for serious accidents including terrorists' attacks or natural disasters. For the emergency medical services in disasters, the disaster medical plan is set to cope with any disasters in perfect order by special area as operating the National Disaster Medical SystemESF#8 Role by FEMA. Accordingly, we need to set the extensive and integrated disaster prevention system for rapid and flexible operation against various kinds of serious accidents. This study identified overall problems in disaster control plans in Korea and suggested how to improve the emergency medical service system in disaster areas. Furthermore, it aims to prepare the basic data to set the effective emergency medical service plans when substantial casualties break out and more reasonable and systematic disaster control plans to cope with the future occurrence of serious disasters.
Purpose: This study aims to compare 119 emergency medical services (EMS) in South Korea and Japan to provide essential data for EMS improvement in South Korea. Methods: Recent data and regulations on firefighting and EMS in South Korea and Japan were analyzed and compared. Results: South Korea follows a centralized approach to EMS, whereas Japan operates with autonomous bodies that establish their own criteria. Japan considers more regional variables than South Korea. In South Korea, there are shortages in fire station deployment among the 119 emergency medical resources in certain regions, leading to significant regional disparities. South Korea has a larger population served by its 119 emergency medical resources with a higher workload and dispatch numbers than Japan. The percentage of non-transported patients among the total number of dispatches was higher in South Korea. Conclusion: Increasing the number of medical professionals and ambulances per population to the level of Japan to reflect local conditions and include various underlying variables such as daytime population, aging, and emergency dispatch conditions in the deployment of 119 emergency resources, and to reduce the deployment gap between regions, will contribute to improving the performance of the South Korea EMS system.
The purpose of this study was to improve EMS-System in Korea through the research in EMS-System of advanced country. The response time is defined as the interval from the time of call receipt to the time of scene arrival. The important factor was to shorten moving distance of ambulance. It should be considered to accomplish this factor that the vehicle must be increased and the convenient location chosen for optimizing of service area. The transport of emergency patients carried out almost by 119 Emergency Medical Service but out of all the employees at 119 EMS only 11.3% have own qualified EMT degree. They should be employed more and more specially at 119 EMS for a superior level of emergency medical care for civilian. In America, EMT can take care of emergency patients following the order from medical Director at the scene of accident. But in Germany, prehospital care was emphasized from the beginning and, in those days, a medical doctor was sent for treatment of emergency patients at the scene, the so-called a Rendezvous system. Hierby this study makes the suggestion to improve the EMS-System, it is effective to use the medical Director system in America and furthermore a Rendezvous system in Germany. The functional integratin and unification of the report system as well as enough personal and equipmental elements saved together invaluable lives.
Even now, 119 rescue services have dissatisfactory aspects in operation, system and equipments as discussed above, It is the most urgent subject to systemize rescue services so that they can be suitable for our status, for we will make 21C welfare state come true before long. So, this author suggest that the followings have to be raised to activate 119 rescue service. 1) Bring up experts and offer high-quality rescue service 2) Prepare more up-to-date equipments 3) Operate transfer joint organizations 4) Promote the ability to meet with a press at the time of rescue service activities 5) Adjust regulations related to rescue services 6) Make up for a countermeasure to traffic accidents of ambulances 7) Adjust regulations making it mandatory to establish heliport at the target on hospitals more than a defined scale 8) Install more rescue service teams 9) Educate and train officials belonging to briefing rooms, where the officials with long experiences are arranged 10) Minimize the time for rescue team to reach fields 11) Establish legal protection system for rescue the team Nowadays, our country operates the department of fire fighting and rescue services without great difficulty, even though the circumstances are bad - insufficient members and the inferior circumstances. All of the fire fighting officials are given heavy duties in bad circumstances, and so are the team of rescue service. The rescue service team, taking charge of some emergency medical system, do a fire fighting inspection as a non-duty service, though they are scanty of sleep due to prevention and protection services of the fire fighting service team. But, they can not engage in rescue services completely and have to deal with miscellaneous duties. So they can not offer professional emergency medical services. But now, almost every fire fighting organization, belonging to National Emergency Management Agency, are separating rescue services, which shows a lot of good results. People recognize rescue services to get better and better gradually and the demands for this rescue services increase. So, this is the best time when rescue service teams should offer qualitative services rather than quantitative services. The people will recognize this rescue service team to be an organization sacrificing and serving for them. However well institutes and operation systems should be established, the rescue service team can not come true their aim without strong wills that they will serve and sacrifice themselves for people from their hearts. In addition, it is essential for the officials in charge of policies about emergency medical services to have a concernment on and practice the policy without failure.

Purpose: This study was conducted as a direct investigation of the data in the dispatch logbooks and status of patient transportation provided by private emergency transport companies in Busan. Methods: This study was conducted using SPSS 23.0 version for a total of 1,000 processed records of private emergency ambulance services in Busan from September 23, 2017 to November 5, 2019. Results: First, 100% of the emergency patient transfers by private emergency ambulances were carried out between medical institutions; 76.4% of all transfer patients had emergency conditions, and 86.0% had serious diseases. Second, 59.3% of the emergency patients were located at distances less than 10 km and 43.2%, at more than 10 km from the medical institutions. Third, 63.5% of the passengers were accompanied by first-class emergency rescuers according to the severity of the condition. Fourth, 92.7% of the reasons for the selection of medical institutions were transferred to places where professional care was available, accounting for most of the reasons for the selection. Finally, the medical institutions were selected according to the severity of the patient's condition; 76.5% patients were transported to institutions with a large number of doctors, and 42.9% of those were transported to specialized care institutions. Conclusion: This study collected data from 1,000 dispatch records of private emergency transport companies in Busan; these records reflect the government's policies to improve the emergency patient transfer system. The current status of emergency patient transfer offered by private transport companies was analyzed. All of the emergency patient transfers were carried out between medical institutions, and 76% of the transferred patients had emergency conditions.
Purpose: This study looked into the reality of the emergency medical technology department by analyzing the major indicators of university information disclosure systems and finding the competitiveness of the department of emergency medical technology by reviewing various evaluation indices. Methods: This study is a survey of 24 colleges with emergency medical technology and emergency medical technology departments across the country. Quantitative index data from 2017 to 2019 were collected and analyzed through the university information disclosure center web site. Results: Departments of emergency medical technology are generally higher than the target colleges in quantitative indicators, but the indices are somewhat insufficient in terms of "rate of faculty in full service" and "research performance of per one faculty in full service." Conclusion: Based on the results of this study, we recommend increasing the low indicators to enhance the competitiveness of the departments of emergency medical technology.

The purpose of this research which was conducted by surveying the transfer consultation records from 360 medical institutions such as general hospitals, hospitals, clinics to the Emergency Medical Center at E University Hospital for six months(Jan. 1, 2000 - Jun. 30, 2000) are to standardize & complete transfer consultation record of hospitals at the 1st & 2nd referral level and to give patients transferred emergency medical center medical information services on a better quality. The conclusions and suggestions from this study were summarized as follows; (1) Examing the distribution of the referral medical consultation(transfer) sheet type, surgery part local clinic sheet types were 34.4%, medical part local clinic sheet types were 26.7%, undifferentiated local clinic sheet types were 23.9% and hospital level sheet types were 15.0%. (2) The items of the transfer consultation records had been standardized more than 75% in the order of patient's name, date, doctor's name, diagnosis, patient's status, impressions. (3) That the degree of recording completion on these items is in the order of patient's name, date, diagnosis, impressions was revealed. (4) Because the standardization and the degree of recording completion are very low in the patient's gender, age, address, electronic recording system was needed for more perfect input of initial patient informations. (5) This standardizing & complete recording on examination and medication will prevent re-examination and abuse of medication for patients transferred emergency medical center. (6) EMT Transfer System should be fixed in all medical institute for the standardizing & complete recording on care period and departure time will give many emergency patients the proper treatments at the proper time. (7) It was revealed that developing new standardized transfer consultation record & using electronic recording system are needed. (8) The complete recording & Fast Track System were needed for higher rate of bed operation at emergency medical center and more hospital profit.
Purpose: The study was to investigate the feasibility about reorganized the paramedic college into a four-year education system. Methods: Through literature review and analysis, a self-reported questionnaire was created and filled out by the professors of paramedic and analyzed. Results: An analysis of the curriculum indicated that, as paramedic schools involve practice-oriented education, reorganization of the college system is necessary to raise the quality of the actual training. The survey results found that most participants agreed with the need to reorganize the school system. Conclusion: Paramedics have a responsibility regarding the life of the nation. In order to be satisfied with its status as a national job that focuses on humanity and justice, increasing the years needed before graduation is necessary.
In Korea, There are many disasters, like the collapse of Sampung department store, the strike of severe typhoon 'Rusa' and the subway tragedy in Taegu, because of global warming, urbanization, high-density and high-rise of buildings. So, the government made 'The Framework Act' on the safe and management of disaster and 'The National Emergency Management Agency' was established. But emergency medical service systems in Korea is not growing so much. The purpose of this research is to give basic data for the development of emergency medical service systems in Disaster by comparing of disaster management systems and emergency medical systems among the nations of the world, analysing emergency medical systems in disaster in Korea and suggesting some improvement methods. The improvment methods are like this ; First, establishing the National Disaster Medical System in Korea, making the good triage by EMT, expansion of EMT's working area, developing protocols and framing of medical director increasing the working force of EMT, broad inner cavity of ambulance for treatment of patientent, supplement of professional equipments, active using of helicopters are needed in prehospital are. Second, equal establishment of emergency medical center and increase of working force of emergency medical team are needed in hospital area. Finally, enforcement of the dispatcher's qualification, smooth communication among EMSS systems and actualization of medical direction through screen are needed in the Telecommunication system.
